Previous issue date: 2009-11-25 / Introdução: a higienização das mãos (HM) é uma das medidas mais importantes na prevenção das infecções relacionadas à saúde. A aderência à higienização das mãos pode ser influenciada por diversas variáveis que devem ser identificadas para que medidas corretivas ou de incentivo sejam implementadas. Objetivos: Avaliar a aderência da higienização das mãos pelos profissionais de saúde em um hospital de oncologia pediátrica, analisar fatores que interferem na aderência a higienização das mãos, avaliar a adesão ao uso adequado das luvas, mensurar o conhecimento dos profissionais de saúde quanto à importância da higienização das mãos e analisar o impacto das intervenções. Material e Métodos: Foi realizado um estudo prospectivo, conduzido entre julho de 2006 e dezembro de 2008, constituído por 3 fases. A primeira correspondeu à observação da prática de higienização das mãos no ambiente do IOP. A segunda fase correspondeu à intervenção para fortalecimento da higienização das mãos no IOP, compreendendo a aplicação de um questionário e o desenvolvimento de atividades para estimular esta prática, desenhadas a partir da análise dos dados da primeira fase e das deficiências verificadas nas respostas ao questionário. Por fim, a terceira fase correspondeu a um segundo período de observação. Resultados: a higienização das mãos no IOP foi considerada baixa, havendo melhora na aderência após o programa de incentivo à HM, passando de 39,01% na fase 1 para 52,17% na fase 3. Os profissionais de saúde demonstraram preferêcia pelo uso da lavagem das mãos com água e sabão, principalmente após a realização dos procedimentos com os pacientes, com melhora da utilização do álcool gel na fase 3. A aderência à HM foi maior entre os médicos, profissionais da unidade de Transplante de Medula Óssea, quando houve escolha pela lavagem das mãos com água e sabão anti-séptico, quando o paciente estava em precaução de contato e o profissional utilizando luvas. O questionário aplicado mostrou que os profissionais de saúde possuem bom conhecimento sobre a importância e adequada higienização das mãos. / Introduction: hand hygiene is regarded as the most important method of prevention of healthcare-associated infections. The hand hygiene adherence can be influenced by several factors, which must be identified in order to implement rewarding or corrective measures. Objectives: our objectives were: to evaluate the hand hygiene adherence in a pediatric oncology hospital; to analyze the factors related to hand hygiene compliance; to evaluate the appropriateness of glove use; to measure knowledge of healthcare workers regarding the importance of hand hygiene; to examine the impact of the interventions. Material and Methods: we conducted a prospective study in three phases between July, 2006 and December, 2008. The first phase consisted in an observational study of hand hygiene adherence. In the second period we implemented some interventions with the aim of improving the hand hygiene adherence, including a survey distributed to healthcare workers, and the development of activities for encouraging this attitude, based on the analysis of the data from the first phase and the survey. Finally, the third phase corresponded to an observational period. Results: the hand hygiene adherence was considered low, however we observed improvement after the intervention (39,01% in phase 1 to 52,17% in phase 3). The healthcare workers preferred to wash their hands with water and soap, especially after the patient care, and the use of the alcohol hand gel has been improved in phase 3. The highest hand hygiene adherence rate has been observed among the physicians, in the bone marrow transplantation unit, when the healthcare worker washed their hands with water and anti-septic soap, during the patient care in contact precautions and when the healthcare workers worn gloves. The results of survey showed that healthcare workers possessed good knowledge concerning the importance of the adequate hand hygiene. / TEDE / BV UNIFESP: Teses e dissertações
