Previous issue date: 2016-12-16 / Introduction: The diagnosis of epilepsy affects the patient's life very broadly. Drug treatment and outpatient follow-up are essential. The unpredictable nature of the epileptic crisis can generate stigma and bring numerous social and psychological consequences. These consequences are often considered more difficult to deal with than the disease itself. The objective of this study was to evaluate the relationship between the perception of stigma and depression, anxiety and other psychological aspects in patients with probably symptomatic focal epilepsy (PSFE) and in patients with temporal lobe epilepsy and hippocampal sclerosis (TLE-HS), and whether there are distinct aspects between them. Casuistry and procedures: 71 subjects (mean age 42.7 years) treated at the Clinical Neurology outpatient clinic of the Celso Pierro Maternity Hospital (PUC - Campinas) of both genders, aged between 18 and 59 years, 29 with probably symptomatic focal epilepsy (PSFE) and 42 with temporal lobe epilepsy and hippocampal sclerosis (TLE-HS). Socio-demographic, clinical and psychological aspects related to epilepsy were evaluated and the Stigma Scale in Epilepsy (EEE), Factorial Neuroticism Scale (EFN) and Symptom Assessment Scale-40 (EAS-40) were applied. The results of the scales were analyzed and related to the clinical and psychological aspects, and the differences between the groups were compared. Statistical tests with significance level p <0.05 were used. Results: the mean EEE score was 42.32 (? 14.35). The results showed that in the comparison between groups, the TLE-HS has older age and lower schooling. There was a significant positive correlation between EEE and all factors evaluated by the EAS-40 and EFN scales, in the dimensions of psychoticism, compulsive obsessiveness, somatization, anxiety of EAS-40; Social vulnerability, psychosocial maladjustment, anxiety and depression of the EFN, in the general sample, but without significant difference between the groups. Conclusion: Stigma is strongly related to the psychological aspects evaluated in patients with PSFE and TLE-HS. There are significant correlations in the EAS and EFN scales in the groups, which suggest that the diagnosis of epilepsy brings many emotional and psychological consequences, regardless of the etiology of the syndrome or the course of the disease. All the experiences of these subjects play a fundamental role in defining behavior and emotional reactions. The diagnosis of epilepsy and its consequences for the life of the subject are independent of the etiology of the disease and attention to psychosocial aspects are as important as the control of the disease. / Introdu??o: O diagn?stico de epilepsia afeta a vida do paciente de maneira muito ampla. O tratamento medicamentoso e acompanhamento ambulatorial s?o fundamentais. O car?ter imprevis?vel da crise epil?ptica pode gerar estigma e trazer in?meras consequ?ncias sociais e psicol?gicas. Essas consequ?ncias s?o consideradas muitas vezes mais dif?ceis de enfrentar do que a pr?pria doen?a. O objetivo desse trabalho foi avaliar a rela??o entre a percep??o do estigma com a depress?o, ansiedade e outros aspectos psicol?gicos em pacientes com epilepsia focal provavelmente sintom?tica (EFPS) e em pacientes com epilepsia de lobo temporal e esclerose do hipocampo (ELT-EH), e se h? aspectos distintos entre eles. Casu?stica e procedimentos: Foram avaliados 71 sujeitos (idade m?dia de 42,7 anos) atendidos no ambulat?rio de Neurologia Cl?nica do Hospital e Maternidade Celso Pierro (PUC ? Campinas) de ambos os g?neros, com idades entre 18 e 59 anos, 29 com epilepsia focal provavelmente sintom?tica (EFPS) e 42 com epilepsia de lobo temporal e esclerose do hipocampo (ELT-EH). Foram avaliados aspectos sociodemogr?ficos, aspectos cl?nicos e psicol?gicos relacionados ? epilepsia e aplicaram-se as Escala de Estigma em Epilepsia (EEE), Escala Fatorial de Neuroticismo (EFN) e Escala de Avalia??o de Sintomas-40 (EAS-40). Foram analisados os resultados das escalas e relacionados com os aspectos cl?nicos e psicol?gicos, e comparadas as diferen?as entre os grupos. Foram utilizados testes estat?sticos com n?vel de signific?ncia p< 0,05. Resultados: o escore m?dio na EEE foi 42,32 (?14,35). Os resultados apontaram que na compara??o entre os grupos, o ELT-EH tem maior idade e menor escolaridade. Houve correla??o positiva significativa entre a EEE e todos os fatores avaliados pelas escalas EAS-40 e EFN, nas dimens?es psicoticismo, obsessividade-compulsividade, somatiza??o, ansiedade da EAS-40; vulnerabilidade social, desajustamento psicossocial, ansiedade e depress?o da EFN, na amostra geral, mas sem diferen?a significativa entre os grupos. Conclus?es: O estigma est? fortemente relacionado aos aspectos psicol?gicos avaliados nos pacientes com EFPS e ELT-EH. H? correla??es significativas nas escalas EAS e EFN nos grupos, o que sugere que o diagn?stico da epilepsia traz in?meras consequ?ncias emocionais e psicol?gicas, independente da etiologia da s?ndrome ou do curso da doen?a. Todas as viv?ncias desses sujeitos t?m papel fundamental para definir comportamentos e rea??es emocionais. O diagn?stico de epilepsia e suas consequ?ncias para a vida do sujeito s?o independentes da etiologia da doen?a e a aten??o aos aspectos psicossociais s?o t?o importantes quanto o controle da doen?a.
