Previous issue date: 2014 / O interesse pelo trabalho em saúde em situações extremas é o fio condutor desta tese, que investiga a atividade de trabalhadores, buscando compreender os modos de gestão dos usos de si no confronto com a realidade do trabalho de cuidado. A Organização Não Governamental Médicos Sem Fronteiras e a Emergência de um hospital público no município do Rio de Janeiro constituíram-se em campos de pesquisa distintos, porém afinados no objetivo de prestar socorro e salvar vidas. Os desafios impostos por contextos críticos e complexos, onde limites e riscos são permanentemente confrontados, tornam o trabalho mais exigente, expondo a vulnerabilidade dos pacientes, dos trabalhadores e das organizações, mas também sua capacidade de superação, enfrentamento e criação. A perspectiva ergológica de Yves Schwartz e a concepção de saúde de Georges Canguilhem dão o norte teórico-metodológico necessário à compreensão das dramáticas vividas pelo corpo-si, como articulador e mediador da atividade, renormatizando e reinventando modos de viver o trabalho. A pesquisa de base qualitativa fez uso de observação participante, elaboração e leitura de diários de bordo, e entrevistas apoiadas na abordagem dialógica, visando a compreensão da atividade a partir das vivências dos trabalhadores. Os resultados encontrados apontam para aspectos diferenciados entre os dois contextos, como a diferença entre trabalho e emprego, a liberdade de escolha, a relação com pacientes e familiares, o reconhecimento e a valorização profissional, a proteção institucional e a questão da carreira e dos projetos de vida; assim como ressaltam pontos convergentes como o desgaste físico/mental e o tempo de trabalho, os conflitos com as estruturas organizacionais e suas concepções de gestão, a relevância da parceria e da cooperação no coletivo e a confirmação de vocações e da mobilização dos trabalhadores, preservando o sentido do trabalho na assistência e no cuidado em saúde. / The concern for the health work in extreme situations is the central thread of this thesis that investigates the activity of professionals, in order to realize the ways of management of uses of self in the comparison with the reality of care. No-governmental organization Médicos Sem Fronteiras and a public hospital emergence of the city of Rio de Janeiro become fields of investigation distinct, but joined in the aim of help and save lives. The imposed challenges by critical and complexes contexts, wherein limits and risks are permanently confronted, make the work more demanding, showing the vulnerability of patients, professionals and organizations, but also their capacity of overcoming, tackling and creating. Yves Schwartz‟s ergological perspective and Georges Canguillem‟s conception of health give the theoretical-methodological north necessary to the understanding of the dramatics experienced by the body-self, as articulator and mediator of the activity, renormalizing and reinventing ways of life and work. The qualitative basis investigation has used participant observation, elaboration and reading of logbooks, and interviews backed in the dialogical approach, aiming to understand the activity from the professionals‟ experiences. The outcomes show different aspects between the two contexts, as the difference between work and job, freedom of choice, the relation with patients and relatives, the acknowledgment and professional valorization, the institutional protection and the issue of career and life projects; as well as highlight converging points with the physical-mental wearing and the working time, the conflicts with the organizational structures and their conceptions of management, the relevance of partnership and of collective cooperation, and a confirmation of vocations and of the mobilization of professionals preserving the sense of the nursing work and of the health care.
