La visée de notre recherche est, concernant des adolescents ayant subi un à trois ans auparavant une atteinte cérébrale d’origine somatique ou accidentelle (consécutive à une tumeur cérébrale, un AVC ou un traumatisme crânien suivis d’un coma), de préciser les conditions d’un travail culturel d’élaboration psychique en groupe, au sein d’une unité hospitalière. Notre recherche s’appuie sur des travaux étayant trois axes : celui des représentations sociales (à quoi est affronté l’adolescent handicapé ou malade, dans son rapport au socius) ; celui se rapportant aux effets psychologiques de la maladie grave ou du traumatisme crânien sur le jeune sujet ; enfin des travaux permettant de faire le point sur le travail groupal de parole à visée thérapeutique et élaborative, pour des adolescents malades ou handicapés. Notre méthodologie principale consiste, à partir du paradigme de la psychanalyse appliquée aux groupes, à analyser les transcriptions des groupes de parole conduits en milieu hospitalier pendant deux années, à raison d’une heure une fois tous les quinze jours. Les adolescents concernés sont âgés de 14 à 21 ans, garçons ou filles présents dans le groupe observé durant le temps de leur hospitalisation. Nous précisons et discutons des conditions requises et nécessaires (méthodologiques, cliniques) à l’émergence du travail d’élaboration, dans le dispositif à médiation groupale de parole, conçu pour aussi bien qu'avec les adolescents cérébrolésés, hospitalisés. Nous croyons pouvoir vérifier que le dispositif soutient ou engage certains adolescents dans un travail d’objectalisation de l’atteinte, de mise en récit de l’événement et/ou d’expression de leurs vécus d’étrangeté. Par ailleurs, les difficultés induites par le contexte (atteintes graves, traitements lourds, spécificités du fonctionnement hospitalier) nécessitent de véritables « transactions » psychiques groupales. / The aiming of our research is, concerning young people having undergone one to three years before a cerebral attack, following a somatic or accidental injury (consecutive with a disease or a cranial traumatism follow-ups of coma), to specify the conditions of a cultural work of psychical working - over in group, within a hospital unit. It based on work supporting three axes: that of the social representations (with what is faced the handicapped or sick adolescent in his report with the socius ; that referring to the psychological effects of the grave disease or the cranial traumatism on the adolescent ; finally work allowing to give a progress reports on work groupal of word to therapeutic and elaborative aiming, for sick or handicapped teenagers. Our principal methodological axis consists to analyse transcriptions of the groups led in hospital unit during two years, at a rate of one hour once every fifteen days. The adolescents concerned are old from 14 to 21 years, boys or girls present in the group observed during the time of their hospitalisation. We specify and discuss the requirements and necessary (methodological, clinical) for emergence of the work of development, in the device with therapeutic group, conceived for and also with the injuried teenagers, hospitalized. We believe capacity to check that the device supports or engages some teenagers in a work of objectalisation of the injury, transposition of brain injury in “psychic event” and/or expression of their lived of strangeness. In addition, the difficulties induced by the context (heavy gravely hurts, treatments, specificities of hospital operation) require true psychic groupal “transactions”.
