Side Effect Information and the Influence on Patient Medicine-Taking Behaviour

Lack of patient adherence with medication results in health care costs and adverse clinical outcomes. Although fear of side effects can contribute to non-adherence, being informed about them can actually improve matters. Nonetheless, questions persist as to the most efficient way to convey that type of information to patients for a given medication. Information on side effects is largely limited to a simple list in medication leaflets, often without frequency data (that is, lacking detail as to how often they might occur). The decision-making literature suggests that the interpretation of information varies depending on the presentation format or the frame used. This study examined the impact of providing numerical information for side effect frequency, levels of illness severity, and side effect framing on the likelihood of taking an OTC medicine.<p> Participants received a headache scenario with three drug options (X, Y and Z) to consider for use. These painkillers had three levels of potency (defined as 50, 75, and 100% effective) and were accompanied with three levels of side effects (two, four, and six items). When considering their drug choice for the headaches, participants received drug information written without side effect frequency data, then again with side effect frequency data. Subjects rated their likelihood of taking Drug X, Y and Z on a scale of 1 (very unlikely) to 100 (very likely). Participants were also asked to show their likelihood of taking a different set of two medications for headaches (coined Drug N and P) based on positively-slanted or negatively-slanted wording in relation to chances of experiencing a side effect (heartburn).<p> Thirty subjects from Saskatoon over 50 years of age participated. The average age was 66.6 years and 63.3 percent of participants were female. Less than half of participants (n=11) had previous experience with side effects. Most participants were using at least one medicine (whether OTC or prescribed) and described themselves as knowledgeable or somewhat knowledgeable.<p> Participants were more likely to take the hypothetical drugs in the situations described when they received frequency data for side effects (p<0.05). Also, there was a significant higher mean likelihood of use when the drug was framed positively (p<0.01).<p> When considering decisions involving drug effectiveness and their side effects, the provision of frequency data increased patient likelihood of use. Framing the context in positive format also increased patient likelihood to use a medicine. This information could be important for pharmacists counseling on medication side effects, especially for those patients with medication adherence problems.

Bulk electric system reliability evaluation incorporating wind power and demand side management

Electric power systems are experiencing dramatic changes with respect to structure, operation and regulation and are facing increasing pressure due to environmental and societal constraints. Bulk electric system reliability is an important consideration in power system planning, design and operation particularly in the new competitive environment. A wide range of methods have been developed to perform bulk electric system reliability evaluation. Theoretically, sequential Monte Carlo simulation can include all aspects and contingencies in a power system and can be used to produce an informative set of reliability indices. It has become a practical and viable tool for large system reliability assessment technique due to the development of computing power and is used in the studies described in this thesis. The well-being approach used in this research provides the opportunity to integrate an accepted deterministic criterion into a probabilistic framework. This research work includes the investigation of important factors that impact bulk electric system adequacy evaluation and security constrained adequacy assessment using the well-being analysis framework.<p> Load forecast uncertainty is an important consideration in an electrical power system. This research includes load forecast uncertainty considerations in bulk electric system reliability assessment and the effects on system, load point and well-being indices and reliability index probability distributions are examined. There has been increasing worldwide interest in the utilization of wind power as a renewable energy source over the last two decades due to enhanced public awareness of the environment. Increasing penetration of wind power has significant impacts on power system reliability, and security analyses become more uncertain due to the unpredictable nature of wind power. The effects of wind power additions in generating and bulk electric system reliability assessment considering site wind speed correlations and the interactive effects of wind power and load forecast uncertainty on system reliability are examined. The concept of the security cost associated with operating in the marginal state in the well-being framework is incorporated in the economic analyses associated with system expansion planning including wind power and load forecast uncertainty. Overall reliability cost/worth analyses including security cost concepts are applied to select an optimal wind power injection strategy in a bulk electric system. The effects of the various demand side management measures on system reliability are illustrated using the system, load point, and well-being indices, and the reliability index probability distributions. The reliability effects of demand side management procedures in a bulk electric system including wind power and load forecast uncertainty considerations are also investigated. The system reliability effects due to specific demand side management programs are quantified and examined in terms of their reliability benefits.

Design and Characterisation of A SynchronousCo-Axuak Double Magnetron Sputtering System

High power impulse magnetron sputtering (HiPIMS) is a novel pulsed power technique. In HiPIMS, high power pulses are applied to the target for short duration with a low duty factor. It provides a high degree of ionization of the sputtered material (in some cases up to 90%) and a high plasma density (1019 m-3) which results in densification of the grown films. Recently a large side-transport of the sputtered material has been discovered, meaning that the sputtered material is transported radially outwards, parallel to the cathode surface. In this research, we use this effect and study the side-ways deposition of thin films. We designed a new magnetron sputtering system, consisting of two opposing magnetrons with similar polarity. Ti films were grown on Si using the side-ways transport of the sputtered material. Scanning electron microscope was employed to investigate the microstructure of the grown films. Optical emission spectroscopy (OES) measurements were made for investigating the ionized fraction of the sputtered material while Langmuir probe measurements were made for evaluating the plasma parameters such as electron density. The conclusion is that the system works well for side-ways deposition and it can be useful for coating the interior of cylindrically shaped objects. It is a promising technique that should be used in industry.

Strategic Brand Analysis in Destination Identity Context : A Case Study of Albania

Tourism industry is one of the most prominent industries at the moment. Globalization has been playing a big role by mainly diminishing traveling barriers between countries. This trend had brought also an increase in competition between countries to identify themselves and to define how they want to be perceived by the tourists. In this case, developing a clear destination brand identity is crucial for success. Brand identity and brand image are two concepts that are related to each other but their meanings are different. There are several studies from the demand side concerning with destination brand image. Conversely, there is a lack of studies conducted in destination brand identity by considering the supplier side. Thus,the main purpose of this study is to investigate the brand identity creation from the supplier side. A conceptual model of “Strategic Brand Analysis in Destination Identity Context” was developed from the reviewed literature. This model guided the empirical investigation of regulators, academicians, civil society and business enterprises in Albanian identity context. In order to collect diverse perspectives, ten qualitative interviews from different fields were conducted. By conduction of interviews and qualitative content analysis with the support of “QSR NVivo 7” software, the strategic brand analysis of destination identity in Albanian context was examined. Additionally, presented theories about brand identity and collected empirical data were analyzed together. Based on this analysis, revised model was recommended for the “Strategic Brand Analysis in Destination Identity Context”. The results suggested that a comprehensive analysis including “Stakeholders Analysis” and “TourismCompetition Analysis” is required as the initial phase of destination identity formation. Each part of analysis has combining aspects, which need to be considered. This study contributes to the literature on brand identity and personality. In addition, the study also has implications for policy-makers of tourism destinations and managers of tourism companies. While the theoretical findings can be regarded as an exploration in the literature,this study fulfilled first research concerning destination brand identity of Albania in tourism.

A PAPR Reduction Scheme Without Side Information in Pilot-Aided OFDM Systems

High peak to average power ratio (PAPR) is one of the major drawbacks in orthogonal frequency division multiplexing (OFDM) systems. In recently years, various methods have been proposed to reduce the PAPR performance. The selected mapping (SLM) scheme is perhaps the most popular one because it provides outstanding PAPR reduction performance. In addition, the subcarrier magnitude remains the same in the SLM scheme. However, there are two major shortcomings in the SLM scheme. First of all, it requires a number of inverse fast Fourier transforms (IFFTs) to produce candidate signals, dramatically increasing the computational complexity. In addition, side information has to be transmitted to the receiver to indicate the candidate signal that results in the best PAPR, leading to the decrease in bandwidth utilization. To overcome these two drawbacks, this thesis proposes a novel SLM scheme that does not need side information. The proposed scheme is based on a low complexity SLM scheme [C.-P. Li, S.-H. Wang, and C.-L. Wang, ¡§Novel low-complexity SLM schemes for PAPR reduction in OFDM systems,¡¨ IEEE Trans. Signal Process., vol. 58, no. 5, pp. 2916¡V2921, May 2010] in pilot-aided OFDM system. Simulation experiments are conducted to verify the performance of the proposed scheme. It is shown that the bit error rate (BER) performance of the proposed scheme is very similar to that of the traditional SLM scheme with perfect knowledge of the side information. Therefore, the proposed scheme not only has the advantages of low complexity and high bandwidth utilization, but also has a superior BER performance.

Automatic Recognition of Artificial Objects in Side-scan Sonar Imerage

Abstract The interpretation and identification of information from the side-scan sonar imagery are mainly depended on visual observation and personal experiences. Recent studies tended to increase the identification efficiency by using numerical analysis methods. This can reduce the error that cause by the differences of observer¡¦s experience as well as by extended time observation. The position around the center line of the slant range corrected side-scan sonar imagery might result in the degradation of the ability of numerical methods to successfully detect artificial objects. Theoretically, this problem could be solved by using a specific characteristic function to identify the existence of concrete reefs, and then filtering the noise of the central line area with a threshold value. This study was intended to develop fully automatic sonar imagery processing system for the identification of cubic concrete and cross-type protective artificial reefs in Taiwan offshore area. The procedures of the automatic sonar imagery processing system are as follows: (1) Image Acquisition¡G500kHz with slant range of 75m. (2) Feature Extraction¡Ggrey level co-occurrence matrix (i.e., Entropy, Homogeneity and Mean) (3) Classification¡Gunsupervised Bayesian classifier. (4) Object Identification¡Gby characteristic feature (i.e., Entropy). (5) Object¡¦s Status Analysis¡Gobject¡¦s circumference¡Barea¡Bcenter of mass and quantity. This study used the sonar images collected at Chey-Ding artificial reef site in Kaohsiung City as a case study, aiming to verify the automatic sonar imagery processing system and find out the optimum window size. The image characteristic functions include one set of first order parameter (i.e., mean) and two sets of second order parameter (i.e., entropy and homogeneity). Eight(8) sonar images with 1-8 sets of cubic concrete and cross-type protective artificial reefs where used in this step. The identification efficiency of the system, in terms of the produce¡¦s accuracy, is 79.41%. The results illustrated that there were 16~28 sets of artificial reefs being detected in this case which is comparable with the actual amount of 17 sets. Based on this investigation, the optimum window size was concluded to be 12¡Ñ12 pixels with sliding size of 4 pixel. Imagery collected at Fang-Liau artificial reef site of Pingtung County was tested. For the purpose of applicability, the original imagery (2048¡Ñ2800 pixels) was divided into 8 consecutive smaller sized imagery frames(2048¡Ñ350 pixels). The influence of using a two-fold classification procedure and a central filtering method to reduce the noise that caused by slant range correction were discussed. The results showed that central line filtering method is applicable. The results of object¡¦s status analysis showed that there are 156-236 sets of reefs existed. Automatic determination of the target using the characteristic function of entropy is feasible. If the value is larger than 1.45, it represents positive identification of concrete artificial reefs. It can be classified as muddy sand seabed type if the value is smaller than 1.35. If the value is between 1.35~1.45, it illustrates the existence of a transition zone where objects of smaller in dimensions might exist. To achieve the purpose of automatic operation, firstly, we have to identify the existence of the concrete reefs by using the specific characteristic function. Based on the result of existing concrete reefs, suture line filtering method will hence be used to filter the noise from the image information. For that all of the procedures are automatically operated without human intervention. The Study of Cr4+ Fluorescence Enhancement in Crystal Fiber Using Side Deposition

Due to the fast expansion and development in the optical communication industry, the demand for the broad-band laser light source as used to the optical transmission network system has correspondingly increased. Cr4+ doped YAG crystal fibers, with its broad-band spectral property, is thus becoming more and more indispensable to the growth of the industry. However, the Cr4+:YAG crystal fiber in its own repetitive growth process brings with itself one problem: after each re-growth, the concentration of Cr2O3 and Cr4+ ions would reduce appreciatively. Hence, finding sound solutions for effectively raising the concentration of Cr4+ is now becoming an essential issue in the field. The thesis mainly focuses on the development of using Cr4+:YAG as the laser gain medium. Thin layers of CaO, MgO, and Cr2O3 was coated on the circumference of the Cr:YAG crystal fiber. The LHPG method was then employed to re-grow the coated samples, during which the doped concentration of CaO, MgO and Cr2O3 can in-diffuse. And the effect of charge compensation would go further to simultaneously raise the concentration of Cr4+ ions. Now we have successfully enhance the concentration of Cr4+ ions to 4.86x10-3 wt.%. This study, with the use of the E-Gun coating machine and the IAD (ion-beam assisted deposition) system, also probes the technical side of how to better improve the quality of the crystal fiber laser. Both end faces of the Cr4+:YAG crystal fiber were coated with optical thin films by TiO2 and SiO2 targets. In addition to raise the quality of the thin films, the IAD system also functions to create a laser cavity in which both the anti-reflectance (AR) effect (for pumping the light source) and the high reflectance (HR) effect (for stimulating the light source) are achieved. The accompanied benefits would be the reduction of transmission loss, the increased laser efficiency, and thus a more successful and more stable crystal fiber laser.

Thermal behavior and molecular packing of alkylated poly(ethylene- alt- maleic anhydride) and poly(styrene-alt-maleic anhydride) were studied by means of differential scanning calorimetry, thermogravimetry, polarizing optical microscopy, and X-ray diffraction. Effects of the length and graft density of alkyl side chains and backbone rigidity on the thermal behavior and on the structure of the mesophases are discussed.

High Voltage Customer Electric Energy Management Strategies Research

Abstract This thesis proposes a PC based electric energy management system as well as load control strategies for demand side management in high voltage customer. Besides, this thesis proposes a sequential search method for the decision of optimal demand contract. By the proposed approach, We expect to decrease the basic demand charge and the total electrical cost. The load survey and load characteristics of selected high voltage customers are first fulfilled to derive the load composition and statistic data for large air conditioner. Furthermore,digital power meters are installed at each substation and they are connected in star configuration with telephone network to form automatic meter reading system. Power parameters such as V, I, P, Q, P.F. etc. are periodically collected via telephone network. By inspecting the trend of peak load as well as the load composition, the specification and structure of electric energy management system and their application functions are difined. The proposed PC based electric energy management system is integrated programmable logic controller (PLC) with power meters to form basic Supervisory Control And Data Acquisition ¡]SCADA¡^functions. Besides, advance functions such as demand monitoring/load shedding, periodical load control, clock load control, direct load control, alarm, and real time/historical trending are embedded to enhance the capability of proposed system. By the Visual Function Block in Diamond Control View, automatic meter reading system can be simulated and demonstrated. The academic power system in National Sun Yat-Sen University(NSYSU) are selected for testing to demonstrate the effectiveness of proposed system. Finally,the effect of peak load cutting will not only save energy consumption of the customer but also increase the power capacity of substations for Taiwan power system.
