In the past decade wireless devices have invaded the medical area with a wide range of capability as components of a wireless personal area network (WPAN) and Wireless Body Area Network (WBAN). These applications in medical telemetry are not only improving the quality of life of patients and doctor-patient efficiency, but also enabling medical personnel to monitor patients remotely and give them timely health information, reminders, and support-potentially extending the reach of health care by making it available anywhere, anytime. This paper exploits the pairing vulnerability in Bluetooth Low energy (Bluetooth Smart) for HealthCare devices used in medical telemetry applications and demonstrates the key role security plays in telemetry.

L'introduction des technologies de l'information et de la communication (TIC) dans la relation de soins est une déclinaison d'un mouvement plus global, tendant à la construction d'une véritable société de l'information. Si le secteur de la santé n'est pas épargné par ces usages nouveaux, il mérite néanmoins une étude particulière tant le colloque dit singulier, entre le médecin et son patient, est empreint de singularité. La télémédecine, en tant que pratique médicale à distance, s'illustre par son ancienneté, et la nouveauté de son cadre normatif. D'abord, offrant une mutation spatiale et temporelle de l'exercice médical, les TIC déplacent les frontières traditionnelles, et celles notamment reconnues à l'auteur et à la finalité de l'acte médical. Le colloque devient pluriel et dématérialisé. Ensuite, les enjeux du recours aux TIC, nouvel écran technique, ont rendu impérieux l'intervention du droit, afin qu'il encadre et en régule l'usage. Le droit de la relation de soins recourant aux TIC est protéiforme. Les droits et devoirs des médecins et des patients s'appliquent invariablement à cette nouvelle forme de pratique médicale. Mais, le législateur a tenté d'étoffer ce cadre en initiant des règles dédiées, parcellaires et lacunaires. Le recours aux autres branches du droit est nécessaire au dessein d'un encadrement global. Il démontre pourtant son insuffisance ou son inadaptation à une activité si singulière. / The introduction of information and communication technologies (ICT) in the caring relationship is part of a wider movement, aiming at building a real society of information. Although public health is not spared from those new customs, it nevertheless deserves its own specific study, given the singularity imprinted by patient-doctor relationships. Telemedicine, as a distant medical practice, is characterized by its seniority and the novelty of its normative frame. First of all, offering a spatio-temporal mutation of the medical practice, TIC shift traditional boundaries, including those recognized to the author and finality of the medical act. The doctor-patient bond becomes plural and dematerialized. Then, challenges implied by the resort to TIC, this new technical screen, made very imperious the intervention of the law to supervise and control its use. Caring relationship law resorting to TIC is protean. Doctors and patients' rights and duties invariably apply to this new form of medical practice. However, the lawmaker tried to fill this frame out by initiating dedicated rules, yet remaining piecemeal and lacunar. Resorting to the other fields of law is needed with view to a wide supervision. It however demonstrates its failure or unsuitability for such singular activity.

Telemedicine is the use of information and communication technology to provide clinical health care at a distance. This approach is usually adopted to bridge the distance in rural settings, but it is also a method to meet demographical and economical challenges. Västerbotten, a sparsely populated county with an ageing population, is in leading edge when it comes to telemedicine. The purpose of this report was to examine the diffusion and the inventory of telemedicine in the county council of Västerbotten from a geographical perspective. To gain a deeper understanding of diffusion and inventory of telemedicine, my purpose was also to examine these processes from an international and national perspective. Telemedicine and diffusion as concepts, and how these have been portrayed in the literature were studied, as well as the driving forces behind, and the barriers to further diffusion of telemedicine. The main focus in the report was telemedicine in the county council of Västerbotten and their ongoing inventory of telemedicine. The background of the inventory, how it is being performed and what the results have been up to this date were investigated. All these aspects were performed through interviews. An important factor to successful implementation of telemedicine, is the use of comprehensive and systematic performed evaluations of telemedicine activities. The inventory performed by the county council of Västerbotten is an important contribution to the lack of evidence in this area. Although, a major barrier to successful implementation and further diffusion of telemedicine, appears to be attitudes among health care staff.

Σχεδιασμός και ανάπτυξη ιστοσελίδων δικτυακού τόπου τηλεϊατρικής για ανάκτηση ιατρικών δεδομένων

Η καλύτερη χρήση των πόρων υγειονομικής περίθαλψης, η πρόσβαση στη σωστή πληροφόρηση από οπουδήποτε ανά πάσα στιγμή, οι απαιτήσεις για αυξημένη ασφάλεια και η κοινή χρήση των ιατρικών δεδομένων είναι βασικοί στόχοι της ψηφιακής ιατρικής απεικόνισης. Έτσι δημιουργήθηκε μια στροφή σε πλαίσια και πρότυπα που να υποστηρίζονται σε διεθνές επίπεδο, με σκοπό την τυποποίηση-ενωποίηση των ιατρικών πληροφοριών. Η υλοποίηση ιστοσελίδων δικτυακού τόπου τηλεϊατρικής είναι η αναγκαία διεπαφή, ώστε οι χρήστες να αποκτούν πρόσβαση στα ιατρικά δεδομένα. Αυτές διαρθρώνονται από πολλές λειτουργικές μονάδες και κατά τον σχεδιασμό τους απαιτείται σαφής ορισμός και συντονισμός των ενεργειών που εκτελεί η κάθε μονάδα στο παρασκήνιο. Για τον λόγο αυτό δομούνται τα πρωτόκολλα που θα καθοδηγούν τις εργασίες κάθε λογισμικού και διαμορφώνουν τον τρόπο επικοινωνίας μεταξύ των εμπλεκομένων μονάδων. Από το σύνολο των τεχνολογιών που χρησιμοποιούνται για την υλοποίηση μιας ιστοσελίδας τηλεϊατρικής, σε αυτήν τη διπλωματική εξετάζουμε το κομμάτι που συμβάλει στην ανάκτηση ιατρικών δεδομένων. Πιο συγκεκριμένα εξετάζουμε συνολικά τον τρόπο διαχείρισης ιατρικών εικόνων. Το έργο αυτό το αναλαμβάνει το πρωτόκολλο απεικόνισης ιατρικών δεδομένων XDS-i, και η παρουσίασή του είναι ουσιαστικά το αντικείμενο της διπλωματικής αυτής. Στην αρχή γίνεται μια γενική εισαγωγή του αναγνώστη στον κόσμο της διαχείρισης ψηφιακής ιατρικής πληροφορίας και τις ανάγκες χρήσης διαφόρων πρωτόκολλων ενοποίησης. Στο δεύτερο κεφάλαιο γίνεται μια γενική επισκόπηση του πρωτοκόλλου. Στο τρίτο κεφάλαιο εξετάζουμε διεξοδικότερα την υλοποίηση του XDS-i και τους τελεστές/διαβιβάσεις που χρησιμοποιεί για να πετύχει τον σκοπό του. Επιπλέον περιγράφεται παραστατικά η λειτουργία και η χρησιμότητα του σε κάποια πραγματικά σενάρια. Στο τέταρτο κεφάλαιο αναλύονται βαθύτερα τεχνικά θέματα που αφορούν τη χρήση των παραμέτρων και επιλογών στη διαβίβαση για ανάκτηση εικόνων. Στο πέμπτο κεφάλαιο γίνεται αναφορά σε κάποιες χώρες και κοινότητες που έχουν οργανώσει τον τομέα της υγείας τους υιοθετώντας συστήματα που κάνουν χρήση του πρωτόκολλο XDS-I, καθώς και ορισμένες εμπορικές εφαρμογές. Τέλος παρουσιάζεται το στάδιο που βρίσκεται η εξέλιξη του πρωτοκόλλου αυτήν την στιγμή. Στο παράρτημα παρουσιάζονται κάποια ζητήματα που εμφανίζονται γενικά στο πρωτόκολλο XDS και το πλαίσιο μέσα στο οποίο αναπτύσσεται το πρωτόκολλο XDS-i, ως κομμάτι του XDS. Στην συνέχεια δίνεται μια περιγραφή του τρόπου διαχείρισης - διευθέτησης των πληροφοριών των ασθενών. Τέλος συζητούνται κάποια θέματα ασφαλείας και με τον τρόπο που τα διαχειρίζεται το πρωτόκολλο. / The best use of healthcare resources, access to valid information from anywhere at any time, demands for increased security and sharing of medical data are key objectives of digital medical imaging. This created a shift in frameworks and standards that are supported at the international level to standardize - integrate medical information. The implementation of telemedicine web site is the necessary interface, so users can gain access to medical data. This kind of page is structured in several modules and their design requires a clear definition and coordination of actions performed by each unit in the background. For this reason, structured protocols guide the functioning of any software and configure the way the involved units communicate. Of all the technologies used to implement a telemedicine site, we examine the recovery of medical data, specifically looking at the overall management of medical images. This work is undertaken by the medical data imaging protocol XDS-i, and its presentation is the subject of this thesis. In the beginning there is a general introduction to the reader in the world of digital medical information management needs and use different integrate protocols. The second chapter is an overview of the protocol. In the third chapter we examine more closely the implementation of XDS-i and the actors / transactions uses to achieve its purpose. Moreover, it is described how it is functioning in some real scenarios. The fourth chapter analyzes the deeper technical issues concerning the use of parameters and options for image transmission and recovery. The fifth chapter refers to some countries and communities that have organized the health sector by adopting systems that use the protocol XDS-I, and some commercial applications. At the end we discuss the evolution and development of the protocol at this time. The Annex presents some issues that generally occur in the XDS protocol and framework within which developed the Protocol XDS-i, as part of the XDS. Then, given a description of how management - arrangement of information for patients. Finally discuss some security issues and the way they are managed by the protocol.

Effect of wireless glucose meter on hyperglycemia and prenatal visits

Gestational diabetes mellitus can have devastating effects in the health of the mother and child. While pregnancy rates are decreasing, prevalence of GDM is increasing, and it is estimated that up to 9% of pregnancies are complicated by diabetes in the United States. Traditional treatment and monitoring of gestational diabetes mellitus relies on patient’s compliance to document glycemic levels. This proposed study will evaluate the effectiveness of telemedicine using a wireless glucose meter that transmits information to the providers in real time. The prospective open cohort randomized clinical trial will take place in medical centers around Boston. Two hundred participants diagnosed with gestational diabetes will be recruited over a period of 24 months from these centers and randomly placed into two groups. One group will follow traditional treatment, and the intervention group will be asked to use iGlucose meter system. Glycemic levels and frequency of prenatal visits will be evaluated and analyzed. If telemedicine proves to be efficacious in treating GDM, this would give providers a new treatment plan to consider to effectively manage blood glucose levels and reduce poor perinatal outcomes related to gestational diabetes mellitus.

The idea is that both patients and doctors will use this prototype for exchanging useful information and thus reduce some of the multiple inconvenient that this disease has. The solution proposed in this project can be generalized to cover other aspects of the communication between the patient and the doctor.
