Window to the outside world : designing a new technology to supplement opportunities for community engagement of older adults in rural NE Scotland

Rural areas are particularly affected by demographic ageing. In later life, the likelihood of living with chronic, manageable, physical conditions increases, affecting the ability to get out and about and be involved with the community. New digital technologies offer novel opportunities for overcoming physical barriers associated with engagement with others, which, amongst other benefits, can enhance wellbeing. These technologies may benefit all age groups in all types of community but could be of particular value in rural areas, which are characterised with dispersed settlement structures, accessibility restrictions and out-migration of family members and in-migration of new residents. The aim of this PhD project was to explore the potential benefits of digital technology, specifically designed for largely housebound older adults in rural North East Scotland, to enhance the sense of involvement they have with the local community. In this multi-methods project, sixteen interviews were conducted with largely housebound older adults living across North East Scotland, the majority in remote and accessible rural areas, to investigate how technology could enhance the sense of involvement older adults could have with their local community. These interviews, along with findings from two focus groups carried out with older adults, were used iteratively to inform the overall concept of the technology, followed by the design and development of a technology prototype. The prototype, entitled Window to the Outside World, was then evaluated with ten interviewees to explore whether live-streaming of local events could potentially enhance the sense of involvement one had with their local community, for those who were largely housebound in rural areas. The findings indicated that digital technologies, such as Window to the Outside World, carry much potential to provide supplementary avenues for increasing one's sense of involvement in the community for largely housebound older adults. There is scope to investigate whether use of such a technology could bring both individual and community-wide benefits for wellbeing, through increased opportunities for identity continuity, imaginative mobility as well as those gained from adopting a purely spectator role, and potential therapeutic benefits from the content of the footage. There is potential to develop the technology further to make it fit to their needs even more effectively.

nÃo hà / As pessoas atingidas pela hansenÃase podem experimentar restriÃÃes em sua participaÃÃo em situaÃÃes da vida diÃria, mesmo na ausÃncia de deficiÃncias. A qualidade de vida dessas pessoas tambÃm pode sofrer influÃncia da doenÃa, afetando em vÃrias dimensÃes. O estudo objetivou caracterizar a participaÃÃo social e os padrÃes de qualidade de vida de pessoas atingidas pela hansenÃase atendidas no Hospital UniversitÃrio Walter CantÃdio no municÃpio de Fortaleza, no perÃodo de janeiro de 2010 a julho de 2014. Trata-se de um estudo transversal populacional. A coleta de dados ocorreu no perÃodo de maio a novembro de 2014. A populaÃÃo do estudo foi composta por 102 pessoas atingidas pela hansenÃase acompanhadas no AmbulatÃrio de Dermatologia do referido hospital. As pessoas eram convidas por contato telefÃnico ou quando compareciam ao ambulatÃrio. Os dados foram coletados atravÃs da aplicaÃÃo das escalas de participaÃÃo e de qualidade de vida, WHOQOL-bref (na populaÃÃo geral) e WHOQOL-OLD (em maiores de 60 anos), como preconizado pela OMS. Os aspectos Ãticos foram respeitados conforme as exigÃncias da ResoluÃÃo 466/12 do Conselho Nacional de SaÃde, sendo aprovada pelo Comità de Ãtica em Pesquisa da UFC sob o parecer 662.240, e ainda pelo CEP do Hospital UniversitÃrio Walter CantÃdio da UFC sob o parecer 721.299. Predominou a populaÃÃo economicamente ativa, com ensino fundamental incompleto e residente em Fortaleza. O grau de incapacidade fÃsica no diagnÃstico diferente de zero, bem como a reaÃÃo hansÃnica, estiveram presente em 29,4% e 48,1%, da populaÃÃo do estudo, respectivamente. Observou-se que 84,6% dos participantes nÃo apresentavam restriÃÃo significativa à participaÃÃo social. A restriÃÃo leve esteve presente em 8,8% da populaÃÃo do estudo. 53,7% dos participantes do estudo classificou sua qualidade de vida como boa e mostrou satisfaÃÃo com a sua saÃde. De acordo com o WHOQOL-bref, a qualidade de vida das pessoas atingidas pela hansenÃase foi considerada boa em todos os domÃnios: fÃsico, psicolÃgico, relaÃÃes sociais e meio ambiente. Para a WHOQOL-OLD, a qualidade de vida das pessoas atingidas pela hansenÃase maiores de 60 anos foi considerada boa nas seguintes facetas: habilidades sensoriais, autonomia, participaÃÃo social, morte e morrer e intimidade. A faceta atividades passadas, presentes e futuras caracterizou-se como Ãtima qualidade de vida. Diante disso, percebe-se a necessidade de investigar a participaÃÃo social e a qualidade de vida das pessoas atingidas pela hansenÃase para o planejamento e implantaÃÃo de aÃÃes que proporcionem um cuidado mais efetivo a fim de amenizar os agravos decorrentes da doenÃa. / People affected by leprosy may experience restrictions on their participation in daily life situations, even without disabilities. The quality of life of these people can also be influenced by the disease, affecting in various dimensions. The study aimed to characterize the social participation and the quality standards of life of people affected by leprosy treated at University Hospital Walter CantÃdio in Fortaleza, from January 2010 to July 2014. It is a cross-sectional population study. Data collection occurred from May to November 2014. The study population consisted of 102 people affected by leprosy accompanied at the Dermatology Clinic of the hospital. People were invite by telephone or when they came to the clinic. Data were collected through the application of the scales of participation and quality of life, WHOQOL-BREF (in the general population) and WHOQOL-OLD (from 60 years), as recommended by WHO. The ethical aspects were respected as required by the National Health Council Resolution 466/12 and was approved by the Ethics Committee in the UFC Research under the opinion 662,240, and also by the University Hospital of the CEP Walter CantÃdio the UFC in the opinion 721,299. Predominated the economically active population, with incomplete primary education and living in Fortaleza. The degree of disability in the diagnostic different from zero, and the leprosy reaction, were present in 29,4 and 48,15, of the study population, respectively. It was observed that a 84,6% of the participants presented significant restriction on social participation.The mild restriction was present in 8.8% of the study population. 53,7% of the study participants rated their quality of life as good and showed satisfaction with their health. According to the WHOQOL-bref, the quality of life of people affected by leprosy was considered good in all areas: physical, psychological, social relationships and environment. For the WHOQOL-OLD, the quality of life of those affected by leprosy greater than 60 years was considered good in the following facets: sensory abilities, autonomy, social participation, death and dying and intimacy. The facet past, present and future activities was characterized as excellent quality of life. Thus, we see the need to investigate social participation and quality of life of people affected by leprosy to the planning and implementation of actions that provide a more effective care to ease the hardships caused by disease.
