Familjemedlem till person med psykossjukdom : bemötande och utanförskap i psykiatrisk vård

This thesis focused on the situation of family members of persons with psychotic illness, particular on their experience of the approach of the healthcare professionals and of their feelings of alienation regarding the professional care of their family member. A further aim was to explore how siblings who have participated in a support group for siblings experienced their situation. A questionnaire was developed that enabled the aims of this thesis to be investigated (I). Seventy family members from various parts of Sweden participated, and data were collected via the questionnaire developed in study I (II-III). Thirteen siblings who previously had participated in a support group participated in follow-up focus groups interviews (IV). The questionnaire developed was shown to be reliable and valid in these studies (I). In many cases, the participants had experienced an approach from professionals that indicated that they did not experience openness, confirmation and cooperation, and that they felt powerless and socially isolated in relation to the care. There was also found to be a certain degree of association between how the participants experienced the approach and whether they felt alienation (II). The majority of the participants considered openness, confirmation, and cooperation to be important aspects of professional’s approach. The result also identified a low level of agreement between the participants’ experience and what they considered to be significant in the professional’s approach (III). The findings revealed the complexity of being a sibling of an individual with psychotic illness. Participating in a support group for siblings can be of importance in gaining knowledge and minimizing feelings of being alone (IV). Although the psychiatric care services in Sweden have been aware of the importance of cooperating with family members, the results indicated that there is a need for further research in this area.

Using collaborative action research to support adoptive parents in their communications with school staff

To address a gap in the empirical literature on Educational Psychology (EP) practice and on adoption, and to gain perspectives from the Local Authority (LA), this thesis describes collaborative action research conducted by an Educational Psychologist (EP) with adoptive parents to support their communications with school staff. Participants were members of an Adoption Support Group and colleagues in Social Care. Information was gathered via focus groups and semi-structured questionnaires to ascertain the types of difficulties expressed by adoptive parents and inform the processes of communication systems. Participatory action research empowered participants to engage in meaningful and purposeful actions of planning, designing and evaluating information. The research narrative reports on first person inquiry through personal reflection and learning. Second person inquiry was generated in the course of research interactions with participants and the data that emerged from their realities to inform practical learning in action. Third person inquiry moved towards thinking around explanations for issues and the generation of knowledge. Knowledge was developed about parents’ perspectives on the barriers and enabling factors involved in their communications with school staff. The action research approach captured the potential of the insider position to generate rich data in situ while promoting a collaborative response to the social situation faced by the adopters when communicating with school staff. My insider position as researcher, holding multiple roles as an adoptive parent, as an EP and LA officer, is actively acknowledged as influencing understanding and the conceptualisations of the findings. The participants collaborated in the generation of a resource that provided opportunities for insight into issues to improve working practice and may provide a tool to allow parents to communicate effectively with school staff. The resource supported two main recognised functions: practical structure and emotional support. Empowering approaches were those that respected their knowledge, used their language and meanings in an emancipatory way that removed barriers, and were inclusive of them and their children. Communication is enhanced by and depends on systems that are empowering for parents and staff to co-construct shared understandings.

Bullying at work is a form of violence which consists in prolonged and repeated exposure of workers to offensive and humiliating situations and may be a trigger of psychological distress in many individuals and groups. Literature in the field identifies support groups as an emergent treatment alternative with positive results. This study aimed to qualitatively describe the development of a support group for people who suffered bullying at work in a public university in the South of Brazil. The group was composed of 5 women who took part in 8 meetings during 2014 and 2015. The main results of this intervention provide information about bullying at work, developing new coping strategies and the development of a framework of mutual support among the participants. / O assédio moral no trabalho remete à exposição prolongada e repetitiva de trabalhadores a situações ofensivas, humilhantes e constrangedoras, que podem desencadear intenso sofri­ mento psicológico às vítimas. A literatura identifica os grupos de apoio aos sujeitos assediados moralmente como alternativas de intervenção emergentes e com resultados positivos aos participantes. Dados os casos identificados em uma universidade pública do sul do Brasil, foi realizado um grupo de apoio psicológico e o artigo descreve qualitativamente o desenvolvi­ mento do mesmo. O grupo foi composto por 5 mulheres, que participaram de 8 encontros nos anos 2014 e 2015. Dentre os principais resultados dessa intervenção, destaca-se um maior esclarecimento sobre o assédio moral, elaboração de novas estratégias de enfrenta­ mento e o desenvolvimento de um contexto de apoio mútuo entre as participantes. Este trabajo tuvo como objetivo describir cualitativamente el desarrollo de un grupo de apoyo que ocurrió en una universidad pública en el sur de Brasil. El grupo estaba compuesto por 5 mujeres que participaron en 8 reuniones en los años 2014 y 2015. Los resultados de esta intervención brindan información sobre el acoso laboral, el desarrollo de nuevas estrategias de afrontamiento y el desarrollo de un contexto con apoyo mutuo entre los participantes.

"Educação sexual de grupos de adultos portadores de estomas intestinais definitivos: processo da implantação e implementação" / "Sexual Education of adults with definitive intestinal stomas: creation and implementation process"

Trata-se de uma pesquisa de natureza qualitativa, do tipo descritivo exploratório, que teve como proposta descrever o processo de implantação e implementação de um grupo de pacientes portadores de estomas intestinais definitivos e identificar a percepção desses quanto à sexualidade como uma dimensão importante de suas vidas. Para a coleta de dados, foram utilizadas as falas dos sujeitos participantes do grupo durante os encontros realizados, transcritas na íntegra pela pesquisadora. As falas, posteriormente foram analisadas segundo a análise de prosa de André (1983). Foram realizados 14 encontros com 3 horas de duração cada, participaram do grupo 19 portadores de estomas definitivos de São José do Rio Preto e região, uma coordenadora (pesquisadora), um coloproctologista, uma psicóloga e uma observadora não-participante. Toda dinâmica do grupo obedeceu a uma programação feita previamente pela pesquisadora. Os dados obtidos permitiram concluir que essa atividade grupal proporcionou aos portadores um espaço no qual puderam trocar experiências, adquirir conhecimentos, criar vínculos, encontrar cumplicidade, companheirismo e aceitar a sua condição, pois ouviram as vivências dos outros e não se sentiram únicos a terem problemas, facilitando o sentimento de universalidade. Quanto à percepção da sexualidade como uma dimensão importante em suas vidas identificou-se ser esse um tema pouco abordado pelos profissionais de saúde e necessário para o reajustamento sexual satisfatório. Evidenciou-se que a saúde sexual não foi inibida pela doença, pois quanto mais orientados adequadamente, e mais estimulados para a prática sexual, mais satisfeitos e interessados se mostraram para buscar caminhos, superando obstáculos como: a recusa do parceiro, disfunção erétil, alteração imagem e o próprio estoma. Alguns caminhos apontados para atingir a satisfação sexual foram a troca de parceiro, busca de profissionais especializados, criatividade no ato sexual e alguns cuidados com o dispositivo. Nesse espaço evidenciou-se que a prática sexual seja ela qual for, é importante e necessária para a vida de cada um. A experiência de vida de cada integrante permitiu o desenvolvimento da autonomia sexual e fez com que os portadores reconhecessem a necessidade de mantê-la ou buscar auxílio, para o alcance do prazer, satisfação e retorno à vida. / This is a descriptive exploratory qualitative research, with the goal to describe the creation and implementation process of a group of patients with definitive intestinal stomas and to identify their perceptions with respect to sexuality as an important dimension of their lives. In order to collect data, the author used the speech of the subjects who participated in the group during their meetings that were transcribed by the researcher. Their speeches were analyzed according to André’s prose analysis. The group met 14 times, with meetings of about 3 hours each. 19 patients from the city of São José do Rio Preto and region with definitive stomas participated in the meetings with the researcher, one physician, one psychologist and one non-participant observer. The dynamics of the group followed the researcher’s program. Data showed that this activity gave to patients a space to exchange experiences, to learn and create links, to find complicity and colleagues as well as to accept their condition as they listen the others experiences and feel that they are not the only ones who have problems, facilitating the feeling of “universality”. With respect to their sexuality perception as an important dimension of their lives, author identified that this theme was not often mentioned by the health professionals and that this is necessary to a satisfactory sexual readjustment.. The author found out that when sexual health was not inhibited by the disease, they were adequately oriented and stimulated to sexual practice and were satisfied and interested to find ways and overcome some problems such as: the partner refusal, erectile dysfunction, image alteration and the stoma. Some ways used to have sexual satisfaction were to change partners, to search for the care of specialized professionals, creativity during sexual intercourse and to pay attention to the device. In this space, the author found that the sexual practice is important and necessary to their lives. The life experience of each one of the members enabled the development of sexual autonomy and the need to maintain it or to search for health in order to have pleasure, satisfaction and to feel alive.

O presente estudo tem por finalidade apreender a construção dos sentidos que são produzidos e negociados no processo de um grupo de apoio a familiares de pessoas diagnosticadas com Anorexia Nervosa e Bulimia Nervosa atendidos em contexto ambulatorial. Mais especificamente, objetiva delinear os sentidos construídos pelos familiares sobre a sua participação no grupo. Acreditamos que o grupo de apoio oferece um contexto fecundo para investigar (e desconstruir) a maneira como as idéias e os valores são constituídos pela tradição da comunidade discursiva e que adquirem uma aparência de ?realidade? para as pessoas que pertencem àquela comunidade. Espera-se que, além de permitir a sistematização de conhecimento na área, os dados oriundos do presente estudo possam trazer benefícios para os usuários desse e de outros serviços com características semelhantes. Nosso objeto de estudo, o grupo de apoio psicológico aos familiares, da maneira como se configura hoje tem cerca de cinco anos de funcionamento. Desde o início é um grupo de ?portas abertas?, isto é, aberto aos parentes e acompanhantes dos pacientes atendidos. Não tem número definido de vagas e todos os familiares dos pacientes atualmente atendidos estão convidados a participar dos encontros. A freqüência é semanal, com uma hora de duração. Foram audio-gravadas e transcritas 10 sessões consecutivas do grupo, no qual 37 familiares estiveram presentes. Após a leitura exaustiva das sessões foram realçadas as falas dos participantes relacionadas à temática da participação grupal. Percebendo-se a pregnância dessa temática e a riqueza na presença de múltiplos sentidos para a participação grupal nas 3 primeiras sessões consecutivas gravadas, optou-se pelo seu recorte para a análise, sendo que a primeira sessão foi analisada em toda sua extensão e as outras 2 sessões foram recortadas em seus trechos mais significativos. A análise dessas sessões foi empreendida dentro do referencial téorico do construcionismo social e utilizando-se do recurso metodológico de delimitações temático-seqüenciais. Os diferentes momentos da sessão delimitados tematicamente foram: os diferentes sentidos para o estar no grupo; a construção da diferença no grupo; o reconhecimento das semelhanças e desigualdades entre os participantes; a construção dos diferentes lugares no grupo; a compreensão dos sentidos para o espaço grupal; a construção da possibilidade de continuarem juntos; as indicações para a participação grupal e a possibilidade de um novo sentido para a diferença. Ao negociarem esses sentidos, os participantes constroem a si-mesmos, a doença e o grupo. Os julgamentos construídos no grupo podem ser tomados como verdades, cristalizando determinadas maneiras descrições e valores, que podem promover movimentos de segregação e afastamento no grupo. Todavia, esses sentidos podem ser revisitados e reconfigurados, em uma constante teia que enlaça novos significados a cada nova interação (CAPES). / The aim of this study is the comprehension of the meanings produced and negotiated in the process of a familiar support group of people that has been diagnosed with anorexia nervosa and bulimia nervosa taken care in an ambulatory context. More specifically, this study aims to delineate the meanings that these familiars give about their participation at the group. We believe that the support group offers a valorous context for the investigation (deconstruction) of the ways that the discursive traditional communities construct its ideas and values and how those ideas gain the status of reality for the people in those communities. We hope that the results of this study can help the users of this and others services with similar characteristics. Our study object, the familiar support group, in its actual configuration, has five years of functioning. Since its beginning the group is open to the familiars and companions of the people taken care of at the service. It does not have a definitive number of participants that can be in, and all the families are invited to participate. We taped and transcript 10 sessions of this group. 37 participants were present in those sessions. After the exhaustive reading of this material, we enlighten the participant\'s sayings that were referred to the familiar participation. Once we realized the presence of this thematic in the first tree sessions, those were chosen to constitute the corpus of our analysis. The analysis was based in the social constructionist perspective and in the use of the methodological and theoretical recourse of analysis called sequential and thematic delimitations. The different moments of these session delimited by these themes were: the different meanings for the group participation; the construction of the difference in the group; the recognition of the similarities and differences between the participants; the different places taken in the group; the comprehension for the different group\'s meanings; the construction of the possibilities for being together; the indications for the group\'s participation; and the possibilities for the appearance of a new meaning for the difference. When negotiating these meanings, the participants construct themselves, the disease and the group. The judges that emerge at the group can be taken as true, being crystallized and promoting segregation movements in the group. However, these meanings can be revisited and be reconfigured in a constant art of enlacing new meanings as new relation and interaction begins (CAPES).

La prise en charge des Personnes Placées Sous Main de Justice (PPSMJ) et plus particulièrement des auteurs d’infraction à caractère sexuel suscitent des interrogations dès lors qu’il s’agit de questionner leur pertinence quant à la prévention de la récidive et la qualité de la réinsertion. Organisé autour d’une double composante, thérapeutique et pénitentiaire, l’accompagnement socio-pénal des PPSMJ tend aujourd’hui à s’inspirer de certaines pratiques étrangères. Centrées sur l’identification de facteurs de risque, de nouvelles approches voient le jour. Cette recherche doctorale vise à mettre en évidence les effets de pratiques de groupes de parole dans les champs thérapeutique (Groupe Thérapeutique Structuré) et pénitentiaire (Groupe de Parole de Prévention de la Récidive). L’hypothèse de ce travail suppose que le produit de la rencontre entre soin thérapeutique et accompagnement pénitentiaire favorise l’empowerment, c’est-à-dire l’autonomisation et la responsabilisation de la PPSMJ. En déconstruisant les objectifs et méthodes et en observant les effets des pratiques émergentes, il s’agit de repérer les évolutions de leurs participants quant à leur rapport à soi, à autrui, à la norme et à l’acte. Analysant distinctement puis conjointement les pratiques, la méthodologie employée décrit leur cohérence interne, leur différenciation et leur éventuelle complémentarité dans le cadre législatif instituant la prise en charge des PPSMJ. La mise en commun des effets de ces pratiques présume alors un lieu de visibilité des changements à l’œuvre lors du parcours socio-pénal. Cet espace, nous l’avons décrit comme un dispositif socio-sanitaire nécessaire à la PPSMJ pour s’inscrire pleinement dans son parcours pénal. / Offenders treatment, and particularly sexual offenders treatment, gives rise to debates when effects on recidivism and reintegration into society is questioned. Split into penitentiary and therapeutic practices, French probationary system takes model on foreign practices. Based on risk factors identification, some new approaches emerge. This research aims to show the effects of new therapeutic and penitentiary practices. Hypothesis is made that therapeutic and penitentiary programs, when both are applied, are favoring empowerment. Analyses of methods and goals, observation of practices, provide a vision of the offenders’ evolution concerning its relation to oneself, others, law and criminal behavior. First, effects practices are analyzed separately and, in a second time, their joint effects are described. Pooling the effects of new practices assume the identification of a specified place to accord to the offender the means to participate wholly into his sentence.

Devido à relevância da doença faz-se necessário um conjunto de cuidados junto às pacientes que lhes permita situarem-se em sua nova condição. A proposta desta pesquisa é avaliar o impacto do Grupo de Suporte sobre as repercussões psicossociais do adoecimento de mulheres com câncer de mama. Método: o estudo é qualitativo e longitudinal. Nove mulheres com diagnóstico de câncer de mama receberam assistência, semanal, no Grupo de Suporte, por 4 meses, no Hospital Universitário. Antes e depois da intervenção grupal foram realizadas entrevistas com roteiro semiestruturado com o intuito de comparar possíveis mudanças pré- e pós-grupo. A análise do material levou em conta dois níveis de focalização: o individual e o grupal. A Análise de Discurso, a clínica psicanalítica e as práticas de grupo sustentam as interpretações deste material. Resultados: os resultados encontrados apontaram que o medo e os fantasmas associados à doença e à morte foram amenizados; as angústias e sentimentos depressivos se evidenciaram, sobretudo pela perda dos cabelos e pelo medo da recidiva e da morte; por fim, o trabalho possibilitou o afrouxamento de certas defesas psíquicas, dificultadoras do trabalho de elaboração. / Breast cancer diagnosis generally leads to emotional impact due to fear of bodily changes, fear of death or due to multiple losses in the emotional, social and material spheres. Given the relevance of the disease there is the need of a set of care measures to be taken with the patients which allows them to be situated in their new condition. The purpose of this research is to assess the impact of the Support Group on the psychosocial repercussions of the breast cancer diagnosis. Method: nine women with a breast cancer diagnosis received weekly assistance at the Support Group, for 4 months, at the University Hospital. The study is qualitative and longitudinal. Before and after the group intervention there were interviews following a semi-structured script aiming to compare the possible changes pre and post group. The analysis of the material took into consideration two focus levels: individual and group. The Discourse Analysis, the psychoanalytical clinic and the group practices are the basis of the interpretation. Results: the results found that the fear and ghosts associated with the disease and with death were lightened; the anguish and depressive feelings were shown, mainly by the hair loss and by the fear of recurrence and of death; finally, the work made it possible to lighten certain psychological defenses, which make the elaboration work more difficult.

Development of a Sexual Assault Support Group for Female Rape Victims

Survivors of sexual assault struggle to cope with subsequent psychological disturbances. The project facility was a rural hospital in the southwestern United States in which postasault care was initially provided, but survivors later returned to the emergency department. Support groups can be a powerful tool to alleviate long-term consequences of assault by helping individuals cope and improving socialization. This project used the social-ecological theory to explore whether a sexual assault support group would impact the progress of survivors toward improved social interaction, improved socialization, and decreased psychological disturbance. The purpose of the quality improvement (QI) project was the development of a sexual assault support group, based on the International Association of Forensic Nurses guideline to improve and expand health care services for survivors of sexual assault. The QI development was accomplished in conjunction with a team of local experts consisting of law enforcement, social worker, nurse, victims advocate and, a victim of sexual assault, who provided process evaluation regarding their satisfaction with the planning process through the completion of an anonymous, 10-question, Likert-type survey. A descriptive analysis of the data provided information that positively supported the development of the project. Project deliverables included the developed QI, a plan for later implementation, and plans for outcome evaluation through measurement of socialization, psychological disturbances, and emergency department visits. This project has the potential to achieve positive social change through improved quality of life for survivors of sexual assault

This diploma thesis is an empirical ethnographic study of forms of support for LGBTQ+ students in all types of schools from primary school to university. The monitored topic is Institutional provision of the LGBTQ+ support network and the establishment of extra- curricular support groups for LGBTQ+ pupils. The aim of the work is to verify the provision of LGBTQ+ support and to specify the functioning of extracurricular support groups. The study then focuses focus on the potential problems students identify when seeking support in school and on the problems of articulation of identity at critical moments in school education. Using the qualitative Grounded Theory method revealed the conditions for providing support in both schools and LGBTQ+ support groups. Due to the COVID19 pandemic, data collection has moved into virtual space. Using online semi-structured interviews, LGBTQ+ students described the problems of accessibility of support in schools and problems surrounding the articulation of identity at critical moments in school education.and the problems surrounding the articulation of identity at critical moments in school education.
