[pt] O processo de doença, tratamento e hospitalização na criança suscita inúmeros afetos, tais como, medo, desamparo, sofrimento, frustração, ansiedade e angústia. O presente trabalho tem como objetivo investigar as implicações envolvidas no adoecimento infantil a partir do referencial teórico da psicologia
hospitalar e da psicanálise, ilustrado por vinhetas clínicas e falas de crianças doentes que se submeteram ou ainda irão se submeter ao transplante de medula óssea. As narrativas das crianças que participaram deste estudo foram colhidas durante a internação e acompanhamento ambulatorial. Não foi utilizado nenhum tipo de roteiro ou entrevista, portanto, foram privilegiadas as falas espontâneas e informais das crianças, durante os atendimentos psicológicos ou em conversas informais com a equipe de saúde. No decorrer desta dissertação identificou-se que a fragilidade egóica e o sofrimento psíquico fazem parte do processo de adoecimento infantil, onde as representações afetivas são carregadas de intensidade. Constatou-se que por mais frágil que a criança possa apresentar-se, muitas delas conseguem superar as adversidades e dores que a doença e o tratamento impõe às suas vidas. Através deste estudo, portanto, encontram-se indicativos que demonstram que a criança tem a dimensão da representação da doença, tratamento e agravamento do seu estado clínico a partir do reconhecimento das transformações ocorridas em sua vida e sintomas que se apresentam em seu corpo. / [en] In Children, the process that occurs during a disease its treatment and hospitalization raise many emotions such as fear, helplessness, suffering, frustration, anxiety and distress. Specifically, children undergoing bone marrow transplant experience physical discomfort not only due to the aggressive nature of the treatment but also the imminent risk of death. This study aims to investigate the implications involved in the childs sickness from a psychoanalytical and health psychology point of view as illustrated by clinical vignettes and narratives of sick children who have undergone or will undergo bone marrow transplantation. Children were interviewed during hospitalization and also outpatients. No script of any type was used during the interview, so we focused on the spontaneous responses of children that occurred during the psychological or informal conversations with the health team. Throughout this thesis it was found that the fragile ego and psychic suffering are part of the process of infant illness, and that the children s responses are laden with emotional intensity. It was found that although the child is fragile, many of them can overcome the adversities and pain that the disease and treatment impose on their lives. Through this study we find indications that the child has an understanding of the seriousness of the disease, its treatment and worsening clinical status based on the recognition of the changes occurring in his life and symptoms that are present in his body.

[pt] O objetivo deste trabalho é investigar as repercussões psíquicas da cirurgia bariátrica em pacientes obesos. Buscaremos articular a prática, a pesquisa e a teoria psicanalítica com os depoimentos coletados pela autora, psicóloga membro da equipe de cirurgia bariátrica de um hospital público no Rio de Janeiro. Destacaremos o sofrimento psíquico causado pelo estigma social da obesidade, que estimula a procura por métodos radicais, como a cirurgia de redução de estômago. Em seguida, abordaremos algumas contribuições trazidas pela psicanálise ao atendimento clínico de pacientes obesos. Por último, analisaremos o grupo operativo realizado com os pacientes do ambulatório de psicologia do hospital, buscando promover sua saúde mental. / [en] The purpose of this work is to investigate the psychological repercussions of bariatric surgery on obese patients. We will articulate psychoanalytical practice, research and theory with patient testimonials collected by the author, a psychologist member of the bariatric surgery staff at a public hospital in Rio de Janeiro. We will highlight the psychological suffering caused by the social stigma of obesity, which stimulates the search for radical methods, such as gastric surgery. Then, we will examine the contributions made by psychoanalysis to the clinical treatment of obese patients. Finally, we will analyze the operative group carried out with psychological outpatients of the hospital, in order to promote their mental health.

[pt] Em UTI Neonatal estão internados bebês prematuros, sindrômicos ou com alguma malformação, havendo sempre um risco de morte. Diante das fragilidades de seus bebês e precipitados em um ambiente hostil onde há excesso de barulho, de luminosidade e de estímulos, os pais muitas vezes sentem-se angustiados, tristes e com raiva. Com os recursos psíquicos abalados e com sua capacidade de pensamento afetada, os pais podem lançar mão de mecanismos primitivos, como a identificação projetiva (Klein, 1946/1991a), para se defender. A equipe de saúde da UTI Neonatal, além de ser receptora das projeções parentais, necessita fazer face à constante ameaça de morte dos bebês. Acreditamos que diante desta ameça o ambiente da UTI Neonatal seria potencialmente traumático, inclusive para a equipe de saúde. O presente trabalho pretende, a partir da nossa experiência em UTI Neonatal, abordar o sofrimento psíquico a que está sujeito a equipe de saúde. Em razão do trabalho específico que desenvolvem, os membros da equipe de saúde podem chegar a um esgotamento emocional (Burnout). Pretendemos discutir como a equipe de saúde, para estar diante do sofrimento dos pais e dos bebês internados, necessita ela própria receber holding (Winnicott, 1975) e contenção (Bion, 1962) por parte da instituição de saúde. Acreditamos que os
grupos de fala possuem importante função antitraumática para a manutenção da capacidade de pensar da equipe de saúde, para que possam continuar exercendo criativamente as suas atividades. / [en] In Neonatal ICU, premature, syndromic or malformed babies are hospitalized and there is always a risk to life. Faced with the vulnerabilities of their babies and precipitated in a hostile environment where there is too much noise, light and stimulation, parents often feel distressed, sad and angry. With unsettled psychological resources and impaired thinking ability, parents can use primitive mechanisms, such as projective identification (Klein, 1946/1991a), to defend themselves. Neonatal ICU health staff, in addition to receiving parental projections, needs to address the constant threat of infant death. Due to these circumstances, we believe that the environment of the Neonatal ICU would be potentially traumatic, including for the health staff. The present work intends, based on our experience in Neonatal ICU, to address the psychological suffering to which the health team is subjected. Because of their specific work, health staff members can experience emotional burnout. We intend to discuss how the health staff, in order to face the suffering of parents and hospitalized babies, needs to receive holding (Winnicott, 1975) and containment (Bion, 1962) by the health institution. We believe that conversation groups have an important anti-traumatic function to maintain the health team s ability to think so that they can continue to perform their activities creatively.

[pt] Em uma pesquisa descritiva e de estudo de campo, buscou-se
avaliar quais indicadores de carga mental (CM) de trabalho
são predominantes para a produção de acidentes em uma
tarefa de microtomia. A carga mental depende das
exigências da tarefa e do grau de mobilização do sujeito,
da fração de sua capacidade de trabalho que ele investiu na
tarefa, considerando-se que o grau de prazer e satisfação
no trabalho pode variar em função da natureza da tarefa
executada. Partiu-se da hipótese que os fatores de carga
mental de trabalho associados ao risco de acidentes a que
está exposto o operador no manuseio do micrótomo rotativo
representam fonte de sofrimento psíquico. Os sinais de
sofrimento psíquico podem ser vinculados à realização de
tarefas consideradas perigosas e, muitas atividades de
trabalho podem ocasionar desgastes e custos para o
indivíduo de ordem física, mental, emocional e afetiva. Foi
relevante examinar quais foram os fatores organizacionais
referentes às condições ambientais e aos horários de
trabalho, para uma ação preventiva, em conjunto aos
fatores predominantes de carga mental. O suporte
metodológico da pesquisa envolveu a elaboração de um
questionário adaptado do método L.E.S.T. (Laboratório de
Economia e Sociologia do Trabalho) que avalia a carga
mental a partir de quatro indicadores, a saber,
constrangimento de tempo; complexidade/rapidez; atenção, e
minúcia. Os resultados obtidos mostraram que a variável
complexidade-rapidez, minúcia e atenção podem ser
consideradas como efetores para carga mental. A variável
denominada fatores psicossociais não mostrou eficácia
durante este processo de análise. Um aspecto relatado está
associado aos acidentes produzidos durante a operação do
equipamento e sua manutenção. / [en] In a descriptive research and in a field study, we
evaluated which indicators of
mental workload are dominant factors in accident production
in a microtomy task.
The mental workload depends on the task demands and on the
degree of mobilization
of the worker, which is the fraction of his/her work
capacity invested in the task
considering itself that the degree of pleasure and
satisfaction in work could vary as a
function of the nature of executed task.From the hypothesis
that the factors of mental
workload associated to accident risks to which operators
are exposed during handling
of a rotative microtome represent a source of psychic
suffering. The signals of
psychic suffering could be related to the accomplishment of
tasks considered as
dangerous and many work activities could cause physical,
mental, emotional and
affective wearing and costs to individuals. Examining which
organizational factors
are related to environmental conditions and to working
hours was relevant to a
preventive action, as well as the dominant factors of
mental workload. The research
methodology involved the preparation of a questionnaire
adapting the L.E.S.T.
(Laboratoire d`Economie et Sociologie du Travail) method
that evaluates the mental
workload by four indicators, which are time constraint;
complexity/ swiftness;
attention; and detail. The results showed that
complexity/swiftness, detail and
attention indicators could be considered as effective
indicators of mental workload.
The indicator psychosocials factors did not presented
efficacy in this analytical
procedure. A reported feature is associated to accidents
happening during the
handling of equipment and its maintenance.
