Casuística e Métodos: Foram comparados, em estudo prospectivo randomizado e controlado, dois grupos: grupo intervenção (GI) e grupo controle (GC). Os grupos foram avaliados quanto à função muscular do assoalho pélvico e ao estádio do prolapso genital. Para avaliação da função, utilizaram-se a palpação bidigital, por meio da escala de Oxford, que avalia a potência muscular que é a máxima contração que a paciente consegue realizar, e o endurance muscular, que é o tempo de manutenção da contração em segundos, e ainda, a eletromiografia de superfície, que avalia a atividade elétrica dos MAP. O estadio do prolapso foi avaliado por meio da classificação Pelvic Organ Prolapse Quantification (POPQ) por ginecologista. As pacientes do grupo GI se submeteram a três sessões individuais para o aprendizado dos exercícios e, em seguida, foram encaminhadas para três meses de exercícios domiciliares, com retornos mensais e ligações telefônicas quinzenais. O grupo GC se submeteu a uma sessão individual, em que foram orientados exercícios domiciliares para o assoalho pélvico, sem protocolo definido. Ambos os grupos foram reavaliadas após três meses. Resultados: Foram incluídas 21 pacientes no grupo GI e 16pacientes no grupo GC. O grupo GI apresentou melhora da função muscular, mensurada por Oxford (p < 0,001), endurance (p < 0,001) e contração voluntária máxima (CVM), avaliada por meio de eletromiografia de superfície (p=0,001). Quando comparados os dois grupos, o grupo GI apresentou melhor função muscular. No que se refere ao prolapso genital, 70% das mulheres do grupo GI diminuíram um estádio do prolapso, fato ocorrido em 21% das mulheres do grupo GC. Conclusão: os exercícios hipopressivos associados a contração voluntária dos MAP promoveram melhora da função muscular e diminuição do prolapso genital. / Aim: To evaluate the effects of hypopressive exercises associated with pelvic floor muscles voluntary contraction in pereineal muscles of women with pelvic organ prolapse. Methods: In randomized controlled trial were compared two groups: intervention group (GI) and control group (GC). The groups were evaluated regarding to pelvic floor muscle function and pelvic organ prolapse stage. To evaluate the function, were used bidigital palpation, trough Oxford scale and the muscular endurance, time of contraction maintain in seconds. The surface electromyography was also used. It evaluates the pelvic floor muscle electrical activity. The pelvic organ prolapse stage was classified according to the Pelvic Organ Prolapse Quantification (POP-Q), by a gynecologist. The patients of GI Group underwent three individual sessions for learning the exercises, followed by three months of home exercises, with monthly appointments and fortnightly phone calls. The GC Group underwent to only one session, when received orientation to perform home exercises, without following a defined protocol. Both groups were revaluated after three months. Results: Were included 21 patients in GI group and 16 patients in GC group. The GI Group presented improvement of muscle function, measured by Oxford (p < 0,001), endurance (p < 0,001) and maximum voluntary contraction (MVC), evaluated trough surface electromyography (p=0,001). When compared both groups, GI group presented superior muscle function. With regard to pelvic organ prolapse, 70% of women in GI group decreased on stage of prolapse. This fact occurs in only 21% of women in GC group. Conclusion: hypopressive exercises associated with voluntary pelvic floor muscle contraction improved muscle function and decreased pelvic organ prolapse. / TEDE

As doenças cardiovasculares degenerativas são as principais causas de mortalidade, mas com os avanços nas técnicas cirúrgicas, circulação extracorpórea (CEC), técnicas para proteção do miocárdio, anestesia e cuidados intensivos no pós-operatório, fisioterapia no préoperatório e pós-operatório, houve diminuição da morbimortalidade. A fisioterapia respiratória é freqüentemente utilizada na prevenção e tratamento de tais complicações, podendo ser iniciada no pré-operatório de forma a avaliar, orientar e tratar os pacientes. Demonstrar a importância da atuação da fisioterapia no pré-operatório de cirurgia cardíaca, em relação à redução do tempo de internação hospitalar, alteração de volumes pulmonares e força muscular respiratória. Comparar essas variáveis entre o grupo controle e o grupo intervenção. Foi realizado um estudo clínico prospectivo, com pacientes submetidos à RM, na enfermaria de Cárdio-Tórax, do Hospital das Clínicas da UNESP / Botucatu – SP. Foram avaliados 70 pacientes de ambos os gêneros, com faixa etária entre 40 a 75 anos, que realizaram RM com CEC, subdivididos em dois grupos: GRUPO I - 35 pacientes de ambos os gêneros, que receberam um protocolo de orientação por escrito, exercícios respiratórios e treinamento muscular respiratório com Threshold - IMT®, no pré-operatório e GRUPO II - 35 pacientes de ambos os gêneros, que receberam apenas orientação de rotina da enfermaria no dia da cirurgia, não tendo realizado exercícios respiratórios no pré-operatório. Para a análise dos... / The frequency of surgical procedures has increased steadily in recent decades, including the myocardium revascularization (MR). The degenerative cardiovascular diseases are the leading causes of mortality, but with advances in surgical techniques, cardiopulmonary bypass (CPB) for myocardial protection techniques, anesthesia and intensive care after surgery, physical therapy preoperatively and postoperatively, decreased morbidity and mortality. Respiratory therapy is often used to prevent and treat these complications, can be started preoperatively to assess, advise and treat patients. To demonstrate the importance of physiotherapy in the preoperative period of cardiac surgery in relation to the reduction of hospital stay, changes in lung volumes and respiratory muscle strength. Compare these variables between the control and intervention group. We conducted a prospective clinical study with patients undergoing CABG, the Cardio- Thoracic ward of the Hospital of UNESP / Botucatu - SP. We evaluated 70 patients of both genders, aged between 40 and 75, who performed CABG with CPB, divided into two groups: group I - 35 patients of both genders, who received a written protocol guidance, breathing exercises and respiratory muscle training in Threshold - IMT ®, preoperatively and group II - 35 patients of both genders, who received only orientation of the ward on the day of surgery, not having done breathing exercises before surgery. Assessing the potential of submaximal extended duration exercise as an adjunct treatment for sub-acute schizophrenic in-patients: a pilot study

Research into the therapeutic potential of aerobic exercise has proven fruitful over the past few years; however, no true experimental research undertakings have investigated the psychological benefits of aerobic exercise with schizophrenic semi-acute in-patients. The main objective of this thesis was to seek out evidence for the possibility that aerobic (submaximal long duration) exercise could be considered an adjunct treatment for hospitalised schizophrenic in-patients. In order to accomplish this objective the effects of a 45-minute walking programme, completed three days a week, for five weeks, was investigated. Various areas of mental health were explored in search of evidence of the therapeutic potential of aerobic exercise. These areas included, amongst other things: transfer and discharge rates, improvements in mood levels - Beck Depression Inventory-II (BDI-II; Beck, Steer, and Brown, 1996) Xhosa version; decreasing of anxiety levels (Beck Anxiety Inventory (BAI; Beck and Steer, 1993), Xhosa version); improved Global Assessment of Functioning (GAF; DSM-IV), Scale Scores; and decreases in the number of symptoms patients exhibited. 22 schizophrenic inpatients were randomly selected for this study and randomly assigned to either an aerobic (long duration submaximal) treatment group or (primarily anaerobic) control group. Results revealed that statistical significance could not be found in any of the treatment group's t-test results; despite the treatment group generally bordering on significance more so than the control group. Out of the five variables studied (Positive Symptoms, Negative Symptoms, BAI, BDI-II, and GAF Scale) three variables (Negative Symptoms, BDI-II, and GAF Scale) in the treatment group bordered more on significance than did the control group. Thus three (60 %) out of the five areas studied revealed that the treatment group had more significant results. This suggests an overall impression that the treatment group responded slightly better. The Researcher recommended that aerobic exercise therapy be considered a treatment protocol in psychiatric institutions and offered further suggestions pertaining to the effective implementation of these programmes. Included in these recommendations were motivational strategies and warnings about the possible negative effects of exercise therapy. A supplemental goal of this thesis was to explore all of the previously offered theoretical psychological mechanisms of positive mental change; and to seek out evidence, for or against these forces. Participants were given pre- and post- treatment quantitative interviews; as well as, qualitative posttreatment interviews where their phenomenological responses were analysed to seek out evidence of these mechanisms. Evidence of various causative factors was found and a new theoretical mechanism was proposed in this thesis.
