Bakgrund: I Sverige har kvinnor enligt lag kunnat välja att genomgå en abort sedan 1975. Utmaningar och svårigheter kan upplevas av både sjukvårdspersonal och kvinnor då det finns en emotionell dimension vid abort. Sjuksköterskor inom abortvård har beskrivit etiskt svåra situationer i mötet med kvinnor som genomgår abort. Inom sjuksköterskans ansvarsområde beskrivs ett professionellt förhållningssätt präglat av respekt för individens autonomi och integritet. Syfte: Syftet var att beskriva kvinnors upplevelser vid en inducerad abort. Metod: En litteraturöversikt har gjorts enligt Fribergs metod. Tio vetenskapliga artiklar, åtta kvalitativa, en kvantitativ och en mixad metod har använts för att tematiseras till ett resultat. Resultat: Analysarbetet resulterade i två huvudteman och sex subteman. Första huvudtemat är Beslutsprocessen, att göra eller att inte göra abort? med följande underteman: Blandade känslor vid ett svårt beslut och Autonomi och det egna valet. Andra huvudtemat är: Att genomgå abort med följande underteman: En emotionell handling, Att genomgå en abort fysiskt, Bemötande och stöd och Delaktighet och trygghet Diskussion: I diskussionen behandlas kvinnans upplevelse av delaktighetsamt sjukvårdspersonalens bemötande vid inducerad abort. Författarna diskuterade dessa ämnen i relation till litteraturöversiktens bakgrund, vetenskaplig forskning inom personcentrerad vård, egna reflektioner samt Phil Barkers tidvattenmodell. / Background: In Sweden, women have legally been able to choose to undergo an abortion since 1975. Challenges and difficulties can be experienced by both healthcare professionals and women since there is an emotional dimension to abortion. Nurses in abortion care have described ethically difficult situations in their encounter with women who undergo an abortion. A professional approach characterized with respect for the individual's autonomy and integrity are described within the nurse's area of responsibility. Aim: The purpose of the study was to illustrate women´s experience of induced abortion. Method: A literature review were performed based on Friberg's method. Ten articles, nine qualitative, one quantitative type and one mixed method have been used to be thematized into the result. Results: The analysis resulted in two main themes and six sub-themes. The first main theme is the decision making process, to perform or not to perform an abortion? with the following sub-themes: Mixed feelings in a difficult decision and Autonomy and your own choice. The second main theme is: To undergo and abortion with the following sub-themes: An emotional act, To undergo an abortion physically, Patient treatment and support and Participation and security. Discussion: The discussion raises the women's experience of participation and its impact on integrity as well as the care staff's patient treatment in relation to induced abortion. The authors discussed these subjects in relation to the literature review background, scientific research within person-centered care, their own reflections and Phil Barker's tidal model.

SAMMANFATTNING Bakgrund: Trots att abort är ett vanligt förekommande fenomen, är antalet utförda aborter väldigt varierande världen över. Det redovisas att i genomsnitt ett av fem havandeskap avbryts samt att infektioner av varierande grad kan uppstå vid indicerad abort. Syfte: Syftet med denna litteraturöversikt var att beskriva förekomsten av postoperativa infektioner samt nyttan av antibiotikaprofylax i samband med en kirurgisk abort. Metod: Litteraturöversikten baserades på 16 vetenskapliga originalartiklar av kvantitativ ansats (randomiserade kontrollerade studier). Artiklarna analyserades, redovisades, granskades och sammanfogades till denna studies resultat. Resultat: De vanligaste förekommande postoperativa infektionerna efter kirurgisk abort var klamydia, gonorré och bakteriell vaginos vilka förekom i genomsnitt till 9,2 %. Doxycyklin visade sig vara den bästa kostnadseffektiva och profylaktiska behandlingen i jämförelse med andra antibiotikaläkemedel som ingår i studierna. Slutsats: Denna litteraturöversikt bekräftar resultatet från tidigare forskning att profylaktisk antibiotikabehandling vid kirurgisk abort eventuellt kan minska förekomsten av postoperativa infektioner. Nyckelord: kirurgisk abort, antibiotikaprofylax, infektion, omvårdnad, vårdlidande. / ABSTRACT Background: Abortion is a recurring phenomenon; the total number of performed procedures varies greatly around the world. The statistics show that about one in five pregnancies is terminated. It is reported that infections of different degree will occur with induced abortion. Purpose: The purpose of this review is to study the frequency of postoperative infections and the usefulness of antibiotic prophylaxis when surgical abortion is performed. Methods: The literature review is based on 16 original scientific articles of randomized controlled trials in quantitative studies. The articles were analysed, reported, reviewed and merged into this study result. Results: The most common post-operative infections after surgical abortion were chlamydia, gonorrhoea and bacterial vaginosis, which averaged to 9,2 %. Doxycycline proved to be the best cost-effective and prophylactic treatment in comparison to other antibiotic drugs included in the studies. Conclusion: This review confirms the results of earlier research showing that antibiotic prophylaxis can possibly reduce the number of postoperative infections. Induced Abortions and Posttraumatic Stress - Is there any relation? : A Swedish multi-centre studyWallin Lundell, Inger January 2014 (has links)
Introduction: Induced abortion is a common medical intervention. Whether psychological sequelae might follow induced abortion has long been a subject of concern among researchers, and there is lack of knowledge about the relationship between posttraumatic disorder (PTSD) and induced abortion. Aims: To study and compare PTSD, posttraumatic stress symptoms (PTSS) and anxiety- and depressive symptoms among women seeking abortion, allowing for demographic variables. Further aims were to assess risk factors and to assess PTSD and PTSS following induced abortion in relation to experienced care at the clinic. Methods: This was a multi-centre cohort study targeting women who requested an induced abortion at the outpatient clinics of the gynaecology and obstetrics departments of six public hospitals in Sweden. All women who requested an induced abortion before the end of gestational week 12 were approached for participation. PTSD, PTSS, anxiety- and depressive symptoms, personality traits and women’s perceptions of abortion care were measured by means of questionnaires. Measurements were made at the first visit before the abortion as well as three- and six-months thereafter. Data collection was performed from September 2009 to January 2011. Results: 1,514 women filled out the questionnaire before the abortion. Abortion-seeking women did not suffer from PTSD to a greater extent than the general Swedish female population. Few women (51/720) developed PTSD or PTSS after the abortion, 11 did so due to trauma experience related to the abortion. Women at risk of posttraumatic stress were more likely to be young, having anxiety- or depressive symptoms and personality traits related to neuroticism. Furthermore, women with PTSD or PTSS were more likely to perceive certain aspects of the abortion care as deficient. Conclusions: The vast majority of women coped well with the induced abortion. Few developed posttraumatic stress post abortion. The majority did so because of trauma experiences unrelated to the induced abortion. Young women and women with mental distress are vulnerable groups that need to be paid attention to in abortion care.
