Erfarenheter av bemötande från vårdpersonal - Perspektiv från människor med beroendesjukdom : En litteraturöversikt / Experiences of treatment by healthcare professionals – Perspectives from people with addiction disorder : A literature review

Bakgrund: Beroende av narkotika och alkohol är ett stort samhällsproblem världen över som bidrar till negativa konsekvenser för den utsatte samt dess anhöriga. Människor med beroendesjukdom har stor samsjuklighet både fysiskt och psykiskt vilket gör att sjuksköterskor och övrig vårdpersonal möter denna grupp frekvent. Studier visar att vårdpersonal ser på människor med beroende som svåra att behandla och att tillfredsställa i vården. Enligt hälso- och sjukvårdslagen ska vårdgivare vårda alla människor på lika villkor och sjuksköterskan har ansvar att bedriva jämlik vård till alla människor som är i behov av omvårdnad. Syfte: Syftet var att beskriva vilka erfarenheter människor med beroendesjukdom har av bemötande från hälso- och sjukvårdspersonal. Metod: Elva kvalitativa vetenskapliga artiklar har hämtats från databaserna CINAHL samt PubMed och sammanfattats i en litteraturöversikt. Artikelanalys har gjorts utifrån Fribergs analysmodell. Resultat: Resultatet visade att majoriteten av studiedeltagarna någon gång upplevt ett stigmatiserande bemötande från vårdpersonal. Bristande kunskap ansågs bidra till ett stigmatiserande bemötande. Däremot hade deltagare även erfarenhet av att bli bemötta med respekt och värdighet, framförallt från personal som arbetade inom specialiserad beroendevård. Sammanfattning: Den stigmatisering som deltagare vittnade om handlade dels om att bli ovänligt bemötta, dömda på förhand, inte blivit tagna på allvar men även att de var oönskade patienter i vårdsammanhang. Deltagare berättade även att vårdpersonal ofta kunde för lite om beroendesjukdom och kunde betrakta beroende som ett val snarare än en sjukdom. Flera deltagare i studierna berättade även om vårdmöten som kantats av respekt och öppenhet från vårdpersonal. Detta genom att deltagare har blivit involverade i vården, att de kände sig lyssnade på och att vårdpersonalen bemötte dem med medkänsla och empati. / Background Dependency on drugs and alcohol is a major social problem worldwide, contributing to negative consequences for both the afflicted individuals and their loved ones. People with addiction have a higher prevalence of both physical and mental co-morbidities, making this group more frequently encountered by nurses and other healthcare professionals. Studies show that healthcare professionals view individuals with addiction as difficult to treat and satisfy in their care. According to the Health and Medical Services Act, healthcare providers are required to provide care to all individuals equally, and nurses have a responsibility to deliver equitable care to all those in need of nursing care. Aim The purpose was to describe the experience of people with addiction disorders regarding the treatment they rave eceived from health care professionals. Method Eleven qualitative scientific articles retrieved from the databases CINAHL and PubMed were summarized in a literature review. The article analysis was based on Friberg's analysis model. Results The results indicated that the majority of the study participants had experienced stigmatizing treatment from healthcare professionals at some point. Insufficient knowledge was considered a contributing factor to stigmatizing encounters. However, participants also had experienced respectful and inclusive treatment, especially from staff working in spicialized addiction care. Summary The stigmatization that participants witnessed was partly about being treated unfriendly, judged beforehand, not taken seriously but also being unwanted patients in health care settings. Participants also reported that health care professionals often knew too little about addiction disorder and could see addiction as a choice rather than a disease. Several participants also reported health care meetings characterized by respect and openness from health care professionals. This was achieved by involving participants in their care, feeling listened to, and being treated with compassion and empathy by healthcare professionals.

Theses Title: Cognitive construct of attachment by addicted people Autor: Ivana Křížová ABSTRACT This dissertation thesis aims to the theme of attachment as a cognitive construct. The purpose is to explore if is a relation between attachment style and the cognitive constructs that are associated with it by addicted people. The thesis is based on on the theory of personal constructs and personal meaning of events in the life of people. The theoretical part describes theory of attachment, cognitive constructs and addiction. The empirical part describes research design and data analysis. To the research was used a research battery that measured attachment style, early maladaptive schemas and cognitive constructs related to attachment. The research battery consisted of Adult Attachment Projective (AAP), Experiences in Close Relationships (ECR), Young Schemas Questionnaire (YSQ) and non-standardized Cognitive Construct Questionnaire (DKK). The research group consisted of 91 probands, probands with addicted disorder, probands with psychiatric disorder and non-clinical population. Results are partly confirmed relation between cognitive constructs and attachment style. Studies show that some individuals are losing control of time, neglecting their basic hygiene care, social interaction, causing significant losses in their relationships, academic and professional activities. Objectives: The objective of the study was to verify the prevalence of Internet addiction in undergraduate students from two public and private universities and a Federal Institute in the city of Pelotas and to investigate associated factors. Methods: A crosssectional and randomized study was performed using the following instruments: International Addiction Test, International Physical Activity Questionnaire, Beck Depression Inventory, Epworth Sleepiness Scale and Games Dependency Questionnaire. Results: The prevalence of internet addiction was 41.7%, being inversely proportional to age. There was a significant correlation with depression, those who presented changes in screening tests for depression had a prevalence of internet addiction 83% higher. Three out of ten university students presented daytime somnolence and 3% of the sample were classified as at risk for gambling dependence. There is a correlation between consumption of hypercaloric foods, alcohol use and sedentary lifestyle, and dependence on the internet. Conclusion: The dependence of internet on university students is a reality that needs to be approached in an interdisciplinary way, since it compromises the physical and mental health of these young people. / Introdução: A internet apresenta inúmeras funcionalidades e está totalmente integrada à sociedade moderna. Estudos evidenciam que alguns indivíduos estariam perdendo o controle do tempo, negligenciando seus cuidados básicos de higiene, convívio social, acarretando prejuízos significativos em seus relacionamentos, atividades acadêmicas e profissionais. Objetivos: O objetivo do estudo foi verificar a prevalência de dependência de internet em estudantes universitários dos cursos de graduação de duas universidades, pública e privada, e de um Instituto Federal na cidade de Pelotas e investigar fatores associados. Métodos: Foi realizado estudo transversal e randomizado, sendo utilizados os seguintes instrumentos: International Addiction Test, International Physical Activity Questionnaire, Beck Depression Inventory, Epworth sleepness scale e Game Addiction Scale. Resultados: A prevalência de dependência de internet foi de 41,7%, sendo inversamente proporcional à idade. Verificou-se uma correlação significativa com depressão, aqueles que apresentaram alterações nos testes de triagem para depressão tiveram uma prevalência de dependência de internet 83% maior. Três em cada dez universitários apresentaram sonolência diurna e 3% da amostra foram classificados como em risco para dependência de jogos. Existe uma correlação entre o consumo de alimentos hipercalóricos, uso de álcool e sedentarismo e dependência de internet. Conclusão: A dependência de internet em universitários é uma realidade que precisa ser abordada de uma maneira interdisciplinar, visto que compromete a saúde física e mental desses jovens.
