Sobrecarga en la experiencia del cuidador familiar primario del adulto mayor con enfermedad de Alzheimer / Overload in the experience of the primary family caregiver of the elderly adult with Alzheimer's disease

La presente investigación se enfocó en describir el fenómeno de la sobrecarga y su impacto en el cuidador familiar primario de adultos mayores con enfermedad de Alzheimer. Para este fin, se desarrolló un estudio cualitativo aplicando la técnica de la entrevista a 10 cuidadores primarios. La información recabada fue analizada empleando la técnica del análisis de contenido. Los principales hallazgos revelan que la mayor parte de los cuidadores presentan sobrecarga debido a que consideran el rol de cuidador como demandante, carecen de información acerca de la enfermedad y perciben la ausencia de apoyo en el cuidado de los otros miembros de la familia. Lo descrito tiene un impacto en el bienestar y las actividades del cuidador, quien suele expresar emociones como la frustración y tristeza profunda, así como, señala un estancamiento en el desarrollo de sus metas profesionales. / This investigation wants to describe the overload experience of the primary family caregiver of the elderly adult with Alzheimer's disease. Considering this, the present qualitative study has applied an interview technique to 10 primary family caregivers. The present information has been analyzed by the content analysis technique. The main findings reveal that most of the caregivers have overload. This is attributed not only because they consider this a demanding roll, they also have a lack of knowledge about the disease, and this caregivers experience feelings like frustration and deep sadness. In most of the cases, presence of family issues has been caused by the lack of support associated to an avoiding behavior from the family members. Finally, a change in general well-being of the caregiver is detected, also professional stagnation and consequences to physical and mental health. / Tesis

Perspectives of minority ethnic caregivers of people with dementia interviewed as part of the IDEAL programme

Yes / Postwar migrants from the Caribbean and Indian subcontinent (Bangladesh, India, and Pakistan) to the UK are now experiencing the onset of age-related diseases such as dementia. Our evidence base, both quantitative and qualitative, documenting the experiences of family caregivers of people with dementia is largely drawn from studies undertaken with white European, North American, and Australasian populations. Consequently, there is a need for research in the field of dementia caregiving to reflect the increasing diversity in ethnic identities of the older adult population of the UK. Using semistructured interviews, we investigated the experiences of 18 caregivers of people with dementia in Black Caribbean, Black African, and South Asian (Indian, Pakistani, and Bangladeshi) communities in England. Participants were recruited from the Join Dementia Research platform and were predominantly female intergenerational carers. We identified the following three themes: motivation to care (spending time with the care recipient and reciprocity), positive and negative consequences of caregiving (rewards and consequences), and the cultural context of caregiving (cultural norms and values supporting caregiving and negative attitudes towards dementia). Our findings develop existing literature by identifying (a) the importance of spending time with the person they care for, (b) the absence of faith as a caregiving driver, and (c) the challenge of watching the declining health of a parent. We highlight how the different motivations to care are intertwined and dynamic. This diploma thesis examines the impact of nutrition as a vital variable for human health and analyses risk factors involved in unhealthy diet. When incorporated into framework of general principals and broader social context, it is focused on an age group of adults. The thesis is divided into theoretical and practical part. The theoretical part outlines the term nutrition, describes functions of different components of nutrition and energy balance in organism. Furthermore, the theoretical part deals with methods of assessment of nutritional status, description of diet recommendations and evaluation of risks to the health, which can be caused by diet. The practical segment focuses on establishment of dietary habits and spread of knowledge about benefits of healthy and costs of unhealthy nutrition among students attending selected school in Čelákovice. By the use of survey then followed by comparison of surveyed data, this thesis finds insufficient degree of dietary knowledge amongst students of all three fields of study - Graduate pharmaceutical assistant, Graduate social worker and Pre-school and After-school Education. Nutritrional intake and physical activity of an adult population with diabetes mellitus using the continuous subcutaneous insulin infusion pump

This study described three days of dietary intake, physical activity, and glycemic control in 14 female and eight male adult diabetics using Continuos Subcutaneous Insulin Infusion (CSII). Changes in weight after initiation of CSII were also described. Dietary intakes were analyzed using the NUTREDFO computer program then compared to established standards. Both sexes had mean intakes of zinc, folate, vitamin B6, and magnesium below the Recommended Dietary Allowances (RDA, 1980) for age and sex. Female consumption of iron and total calories consumed were also below recommendations. When dietary intakes were analyzed on a nutrient per 1,000 kcal basis men had intakes of zinc and folate below recommended levels, but met suggested allowances for other nutrients. Women still had suboptimal intakes of zinc, folate, iron, and, to a lesser extent, vitamin B6 and magnesium. Mean protein intake was within the recommendation range outlined by the American Diabetes Association (ADA, 1979), but above levels suggested in a 1987 (ADA) update. Intake of total fat, saturated fat, monounsaturated fat, and cholesterol were above and carbohydrate was below recommended levels (ADA, 1987). No significant differences between the mean dietary intake of an age and sex matched group of females from this study and the second National Health and Nutrition Examination Survey (NHANES II, 1983) were noted. When variability in caloric and carbohydrate intake was examined no significant differences between days were found. Mean energy expenditures were higher than estimated levels for the general population. Activities were often reported in fairly large blocks of time, thus changes in activity on a minute to minute basis nay not have been accurately described. Mean blood glucose was near normal on all three days of the study. However, glycosylated hemoglobin levels, done within two months of the study period, were elevated in seven out of 18 subjects. Weight gain is often reported after initiation of CSII. The women in this study showed a net increase of 3.13 lbs after changing to CSII. However, men lost an average of 1.79 lbs after starting pump therapy. To the extent that this population is indicative of CSII users as a whole it was concluded that education, directed at increasing dietary intake of foods rich in zinc, folate, magnesium, and vitamin B6 for both sexes and iron intake for females, would be beneficial. Information on appropriate ways to comply with the dietary recommendations of the ADA (1987) and to offset the weight gain often associated with CSII may also be useful to the pump wearer.

This research was based on the production of dissertations and post-graduation theses entered in the database of the Coordination for the Improvement of Higher Education Personnel (CAPES in Portuguese), from the period between 1993 and 2010. There could be found 117 dissertations and 22 doctor theses, totaling 139 papers on the topic adult population living on the streets. Out of these, 27 studies (19.5%) came from the Social Service branch, produced in the period from 1993 to 2010, which were divided into 24 dissertations and three doctor theses. This set of works reveals the predominance of studies aiming to characterize this phenomenon and to give more visibility to the process of exclusion, as well as services offered to this population in the areas surveyed. This study reveals the concern of the authors to register the bonds between people created by this phenomenon, the changes in the labor force and the globalization process undergoing today. The most productions on this subject were done in universities, which can motivate a movement of exchange to generate an epistemic community on that subject / Essa dissertação procurou identificar iniciativas de investigação e produção de conhecimentos sobre a população adulta em situação de rua como indicação para constituição de uma comunidade epistêmica sobre a temática. Tomou-se por base a produção de teses e dissertações de Pós-Graduação inscritas no Banco de Dados da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES, no período de 1993 a 2010. Nele foram encontradas 117 dissertações de mestrado e 22 teses de doutorado, totalizando 139 trabalhos sobre o tema população adulta de rua. Destes, 27 trabalhos (19,5%) são da área de Serviço Social, produzidos no período de 1993 a 2010, sendo 24 dissertações de mestrado e três teses de doutorado. O conjunto das produções revela o predomínio de estudos voltados para a preocupação em caracterizar o fenômeno, dando visibilidade ao processo de exclusão, bem como os serviços oferecidos a essa população nos espaços pesquisados. O estudo revela a preocupação dos autores em registrar os vínculos do fenômeno, as mudanças ocorridas no mundo do trabalho e o processo de globalização vigente. Merging Task-Centered Social Work and Motivational Interviewing in Outpatient Medication Assisted Substance Abuse Treatment: Model Development for Social Work Practice

To advance social work practice and decrease the research practice gap, this dissertation followed a model development paradigm consisting of several phases. Based on the task-centered model of social work practice and motivational interviewing, a new combined model was construed. The two underlying models were analyzed and synthesized, using technical eclecticism as the integrative approach. The resulting combined model was described by guidelines and manualized. To test the combined model in an applied setting, a study was designed in collaboration with social workers at a substance abuse counseling center. There, the combined model intervention was implemented in an outpatient medication assisted treatment program dispensing methadone and buprenorphine to a mainly African-American population. The agency program aimed at detoxification, but also provided methadone maintenance. It offered additional groups and acupuncture. Ten clients and four social work practitioners participated in the intervention study. The study used a mixed-method approach in data collection and analysis. Client practitioner verbal interaction was recorded using digital audio recording. The digital audio files were loaded directly into Atlas.ti software to be used for analysis. Qualitative data analysis with Atlas.ti was performed for two research tasks, a) assessing implementation fidelity of the manual based intervention and b) exploring model development aspects to improve model guidelines. Treatment fidelity was analyzed through deductive coding and frequency counts. Model development analysis was performed similar to a grounded theory model and used content analysis and constant comparison methodologies. Addiction Severity Index and Readiness Ruler, urine drug screens, problem change, and task accomplishment ratings were used as quantitative outcome measures to produce time series data in order to chart individual case progress in a single system design. After testing the intervention, a focus group with participating practitioners was conducted. Overall beneficence with clients improving and progressing successfully in the program was found. The integration of the underlying models was deemed successful. Their elements were found to be complementary and intricately linked. Crucial for successful implementation is that the program environment supports and accepts client choices. Model guidelines were reviewed and improved for further field testing.
