Do South African mothers shake their babies? incidence and risk factors for infant abuse in Cape Town

Abusive head trauma from shaking is a recognised common cause of fatal head injury in young children globally, although there is little evidence of its occurrence in South Africa. This is perplexing given that the country has amongst the highest reported under-five child mortality and infanticide rates worldwide. To determine whether infants under one-year are violently shaken, a cross-sectional study was conducted with 385 mothers and other primary female caregivers (ages 18 to 60 years; mean age = 27 years) from three high-risk communities in Cape Town. Semi-structured interviews were used to examine: (1) the incidence of shaking, (2) the triggers for shaking, (3) the risk factors for shaking, thoughts of shaking, and knowledge of the dangers of shaking, and (4) the methods used to console crying infants. Results showed that 13.2% (n = 51) of all participants self-reported violent shaking, and 20% (n = 77) had thoughts of shaking their infants. Following a content analysis, three primary triggers for shaking were identified, these were: inconsolable infant crying, feeling angry or frustrated, and being stressed. Findings from a thematic analysis also showed that shaking occurred during a momentary loss of control, and participants seemed to have limited support at the time. The results from three hierarchical logistic regression analyses showed that (1) alcohol use, infant age, a lower knowledge of the dangers of shaking, inconsolable crying, and having thoughts of shaking, predicted shaking, (2) caregiver age, infant age, knowledge of the dangers of shaking, and caregiver responses to infant crying, predicted having thoughts of shaking, and (3) social support, caregiver history of childhood abuse, and having thoughts of shaking, predicted knowledge of the dangers of shaking. Finally, a content analysis revealed three protective factors for infant crying, these were: (1) having easy, contented children, (2) not feeling stressed in response to infant crying, and (3) leaving an infant alone to self-soothe. Taken together, the current findings have programmatic implications that may help prevent the violent shaking of young children in South Africa.

The reproductive choices made by South African mothers who have children with down syndrome

Student Number : 9703129J - MSc(Med) research report - School of Pathology - Faculty of Health Sciences / Down syndrome is the commonest cause of congenital developmental disability in industrialized countries, where it occurs in approximately 1.4 per 1000 live births. In South Africa, the birth prevalence of Down syndrome was documented as 1.8 and 2.09 per 1000 live births in urban and rural populations, respectively. The physical, psychosocial and emotional burden of Down syndrome on affected families is significant. The aim of this study was to determine the reproductive choices of women with a child with Down syndrome, aged 1 year or older. The survey was conducted using a structured questionnaire. The sample consisted of fifty women; 36 African, 4 Asian and 10 Caucasian. The questionnaire assessed the mothers’ knowledge of Down syndrome prior to diagnosis, what counselling was received and how this knowledge was utilised. Information was also obtained on the mothers’ use of family planning, the knowledge and use of prenatal medical genetic screening and diagnosis, and what decisions would be made in future pregnancies. None of the sample group of mothers had prenatal diagnosis in their pregnancy with their Down syndrome child, but 76% (38) said that they would want prenatal diagnosis in any future pregnancies. Of the 50 mothers, 21 (42%) said they would terminate a pregnancy if Down syndrome had been detected, 26 (52%) said they would not, and 3 (6%) said they were unsure what they would have done if faced with this decision. Of the Caucasian women, 40% (4) said they would opt for termination of pregnancy, 40% (4) said they would not and 20% (2) were unsure. Of the African and Asian women, 52.8% (19) and 75% (3) respectively said they would not terminate an affected fetus. The information from this study can be used to improve the understanding of how women and their families cope with their children with Down syndrome and give insight for the provision of more effective and comprehensive genetic counselling.

Women's Access to Political Power in Ancient Egypt and Igboland: A Critical Study

This is an Afrocentric examination of women's use of agency in Ancient Egypt and Igboland. Most histories written on Kemetic women not only disconnect them from Africa but also fail to fully address the significance of their position within the political spiritual structure of the state. Additionally, the presence of matriarchy in Ancient Egypt is dismissed on the basis that patriarchy is the most visible and seemingly the most dominant form of governance. Diop contended that matriarchy was one of the key factors that connected Ancient Egypt with other parts of Africa which is best understood as the Africa cultural continuity theory. My research analyzes the validity of his theory by comparing how Kemetic women exercised agency in their political structure to how Igbo women exercised political agency. I identified Igbo women as a cultural group to be compared to Kemet because of their historical political resistance in their state during the colonial period. However, it is their traditional roles prior to British invasion that is most relevant to my study. I define matriarchy as the central role of the mother in the social and political function of societal structures, the political positions occupied by women that inform the decisions of the state and the inclusion of female principles within the religious-political order of the nation. Matriarchy as a critical framework was used to identify how Kemetic women and Igbo women accessed political power by means of motherhood, political leadership, and spiritual authority. The findings of this study show that Igbo women and Ancient Egyptian women were integral to the political operation of their states. Furthermore, the results indicate that Ancient Egypt and Igboland shared cultural commonalities as it relates to the roles that women occupied as spiritual specialists, political leaders and mothers. / African American Studies

Saving Africa’s Children: Transnational Adoption and The New Humanitarian Order

This PhD Dissertation was completed through 2011 to 2016 and was nominated for a CAGS-UMI Distinguished Dissertation Award. / My dissertation explores transnational adoptions of black African children by white Western parents as a site through which to think about global affective relationality and transnational histories within intimate proximities. The image of an interracial, transnational family can seem to be a fulfillment of the potential for transcendent love symbolized by humanitarian fundraisers such as Live Aid— a love that collapses borders and brings together races in multicultural bliss. Furthermore, adoptions of African children can potentially challenge discursive systems of categorization that frame the black body as existing outside the body politic. At the same time, however, we cannot understand transnational adoption without taking into account the histories of power that make possible and potentially limit the contours of these affective orientations. Indeed, representations of a transnational family consisting particularly of black African children and white Western parents not only invoke the logic of white moral motherhood within the context of contemporary globalization; they also point to European philosophical traditions that presuppose the colonizer’s right to the black body. In this project, thus, I ask: what are the sociopolitical and cultural motivations behind the desire to express humanitarian love towards African children through the act of adoption? How might these motivations create avenues for exclusion and exploitation even as they create new geographies of belonging? To answer these questions, this project brings the affective domain of contemporary transnational adoption between African children and white American parents into conversation with histories of colonial transnational intimacies and the precarious lived experiences of classed and racialized individuals in the African postcolony. In challenging popular celebratory fictions of the transnational family, it critically examines not only the utopian aspirations and social costs of transnational adoption as a humanitarian project, but also the very affect produced and channeled through adoption as a humanitarian act. / Thesis / Doctor of Philosophy (PhD) / My dissertation takes a multidisciplinary approach to analyze transnational adoptions of black African children by white Western parents. It offers answers to the following questions: 1. How do the ghosts of colonialism, along with the violent realities of globalization, expose the inequities hidden within idealized humanitarian narratives of rescue underlying global adoptions while at the same time revealing their transformative potential? 2. How can we account for the experiences and psychic struggles of the African adoptee, and what do their contradictions of idealized Western narratives tell us about the fantasies and anxieties of their Western parents? Ultimately, I argue that while the transnational family suggests transformative transnational connections, Western humanitarian frameworks have also sought to manage the messiness of these connections, to fix white and black bodies into old colonial roles, and to exclude certain bodies, namely those of the African birth mothers, out of the affective realm of transnational adoption. At the same time, these attempts at management, I argue, only speak to the productive potential of these messy relations to transform and exceed colonial limitations.
