Aneurisma da aorta abdominal infra-renal: avaliação ultra-sonográfica em homens acima de 50 anos

Foram avaliados 240 homens por meio da ultrasonografia abdominal (USAb), com média de idade de 65,1 anos (±9,8 anos). A aorta abdominal foi medida no sentido ânteroposterior (AP) e látero-lateral (LL) aproximadamente a 2cm abaixo da artéria mesentérica superior (AMS) e 2cm acima de sua bifurcação. O critério utilizado para considerar aneurisma foi o maior diâmetro encontrado igual ou maior que 3,1cm. Também por questionário, foram avaliados os fatores de risco (tabagismo, sedentarismo, alimentação) e as doenças associadas (HAS, DPOC, IM, DM, AOP ou hiperlipidemia). Nos 240 homens, foram encontrados 11 aneurismas, sendo, portanto, a freqüência de 4,6%. Desses 11 aneurismas, 8 mediam entre 3,1 e 4cm (72,7%) e 3, entre 4,1 e 5cm (27,3%). O maior diâmetro da aorta aneurismática foi de 5 cm (sentido AP a 2cm abaixo da AMS). Foi encontrada uma associação significativa entre aneurisma e AOP e DM, não ocorrendo o mesmo com os demais fatores de risco ou outras doenças associadas. A freqüência de aneurisma encontrada em nossa amostra não foi diferente da referida nos estudos populacionais publicados na literatura, o que mostra a importância da doença em nosso meio, e os indivíduos com AOP e DM têm risco maior de desenvolver a doença. / In order to evaluate the occurrence of Infra-Renal Abdominal Aortic Aneurysm (AAAIR), a sample of the male population in the city of Marília aged 50 years or older was studied from 2000 to 2002. A group of 240 men with mean age of 65,1 years (±9,8 years) was evaluated through abdominal ultra-sonography examination. The abdominal aorta was measured in the anteroposterior (AP) and in the latero-lateral directions (LL) approximately 2cm below the superior mesenteric artery and 2cm above its bifurcation. The largest diameter equal or larger than 3.1cm found was the criterion used for aneurysm. Risk factors such as smoking, eating, and exercise habits and associated diseases (systemic arterial hypertension, chronic obstructive pulmonary disease, myocardial infarction, diabetes mellitus, occlusive peripheral arterial disease, or hyperlipidemia) were also evaluated through questionnaires. Eleven aneurysms were found in the 240 men, which meant a frequency of 4,6%. Out of these 11 aneurysms, 8 measured from 3.1 to 4cm (72,7%) and 3 measured from 4.1 to 5cm (27,3%). The largest diameter of the aneurysmatic aorta was 5cm (AP direction approximately 2cm of the superior mesenteric artery). A significant association between aneurysm and peripheral vascular disease and diabetes mellitus was found. The same did not occur with the other risk factors or other associated diseases. The frequency of aneurysm found in our sample was not different from the frequency mentioned in population studies published in the literature, which shows the importance of the disease in our environment and that patients with peripheral vascular disease and diabetes mellitus have a higher risk to develop the disease.

Sua gênese estaria relacionada com migração, para a circulação cerebral, de trombos e cristais de colesterol que se desprenderiam de placas complexas, localizadas na aorta torácica proximal. Existem várias semelhanças entre a fisiopatologia do desenvolvimento da placa de aterosclerose e a remodelação miocárdica. Por causa disso, formulou-se a hipótese de que a avaliação de pacientes com AVC e AIT, por meio do ecocardiograma transtorácico ((ETT), pode identificar características associadas com risco aumentado de placa de aterosclerose em aorta. It would be caused by migration of thrombi and cholesterol cristals released from complex plaques, located at the proximalis thoracic aorta, to the cerebral circulation. A aterosclerose é um componente ativo no processo de dilatação da aorta e no desenvolvimento quanto nos aneurismas, parece ser promovido pelos mesmos fatores de risco, como o tabagismo, hipertensão e hipercolesterolemia. O objetivo deste estudo foi fazer uma análise comparativa dos principais aspectos epidemiológicos e histopatológicos entre o aneurisma aórtico e a aterosclerose em humanos. Foram estudados 67 casos necropsiados no Serviço de Verificação de Óbitos da Secretaria Estadual de Saúde localizado na Universidade Federal de Pernambuco na cidade do Recife – Brasil durante o período de 2010 a 2013. Os resultados das coletas destes 47 indivíduos (70%) tinham comprometimento aneurismático e 20 aortas necropsiadas com alto grau de aterosclerose aórtica. Também foram estudados lesões de acordo com a porção da aorta identificando-se casos de aneurisma de aorta torácico (n=31) e aneurismas de aorta abdominal (n=16). O sexo predominante dentro das lesões aneurismáticas foram o masculino (55%), contudo, na lesão aterosclerótica a predominância esteve no sexo feminino com 11 casos (55%). As principais co-morbidades associadas foram a hipertensão 32 (76%) e o tabagismo28 (59%), tanto para os casos de aneurismas quanto para os casos com aterosclerose de aorta. Nas análises histopatológicas das três lesões deste estudo, observamos que o processo inflamatório esteve presente nos três casos, sendo de maior relevância nos casos de aterosclerose com 23% (n=14), quando comparados com aneurismas dissecantes da aorta torácico com 19% (n=12) e aneurismas dissecantes da aorta abdominal com 17% dos casos (N=10). O processo hemorrágico foi mais relevante em aneurismas dissecantes da aorta abdominal com 23% dos casos (n=14), em aneurismas dissecantes da aorta torácica foi de 23% (n=13) e nos casos de aterosclerose o percentual foi de 20% (n=12). Quanto a avaliação das fibras elásticas e colágenas, a degradação destas proteínas foi mais relevante nos casos de aneurismas dissecante da aorta abdominal, quando comparados com os casos de aneurismas da região torácica e aortas ateroscleróticas. A partir dos resultados obtidos pode-se concluir que existem diferenças morfo-estruturais e epidemiológicas importantes entre os diferentes tipos de lesões da aorta, o que deve favorecer o desenvolvimento de mais estudos considerando-se estas lesões também como entidades distintas embora ocorram no mesmo vaso.

The experiment we carried out the classical in vitro method on the isolated rat aorta, and the results evaluated by the computer program GraphPad. Only glycitin did not show statistically significant relaxation of blood vessels, which can be attributed to ineffective glycosidic form. Other izoflavonoids- glycitein, daidzein and tectorigenin seem to be promising in terms of potential vasodilatation. Further studies would be needed to verify also the in vivo effects. Likely structural features include the hydroxy group at 7-position and a methoxy group at position 8 of the core od isoflavonoids. The mechanism of action is not yet known, but most likely appears endothelium-dependent mechanisms, alpha 1 receptor antagonism and agonism of muscarinic M3 receptor. Effects of occlusion of the thoracic aorta on habituation of the flexor withdrawal reflex in the rat

Experiments were carried out to investigate the extent to which habituation of the flexor reflex depended on mechanisms operating at spinal interneurones. An attempt was made to cause selective degeneration of interneurones in the spinal cord of the rat by subjecting the cord to a period of ischaemia. Ischaemia was produced by temporary occlusion of the thoracic aorta. The flexor withdrawal reflex was tested 3, 7 or 14 days after occlusion. When compared with data from control animals it was found that ischaemia had resulted in both a qualitative change and a quantitative diminution in the amount of habituation which occurred during the presentation of 400 uniform stimuli. The mechanical properties of the aorta of the cephalopod mollusc, Octopus dofleini (Wulker)

In any circulatory system, the mechanical properties of the walls of the arteries will have significant effects on the dynamics of blood flow. The distensibi1ity of the artery wall is important in determining the wave propagation characteristics of the system, as well as contributing a pulse smoothing effect on the intermittant flow of blood from the heart. These properties are well studied in mammalian circulatory systems, but have never been directly investigated in any invertebrate animal. This study examines the structure and mechanical properties of the major artery in a cephalopod mollusc, Octopus dofleini, and relates these properties to cardiovascular dynamics in the living animal. The dorsal aorta of cephalopods is the large blood vessel through which blood flows from the systemic heart. The walls of this artery are comprised of thick layers of circularly and longitudinally oriented muscle cells. Outside the muscle layers is a loose collagenous adventitia. Innermost, adjacent to the lumen is a layer of elastic fibres, analogous to vertebrate elastin fibres. In addition, these fibres extend throughout the artery wall as a network of extracellular connective tissue. The elastic fibres have been isolated from the octopus aorta and subjected to direct mechanical and chemical tests. These fibres are composed of a protein which has rubber-like properties, that is, the material has high extensibility and a modulus of elasticity, G (= 4.65 X 10⁵ N/m² ), which is similar to a rubber. Chemical and optical properties of this unusual protein suggest that the molecular structure is one of covalently crosslinked, random coil, kinetically free chains. This interpretation is consistent with the thermoelastic data for these protein fibres which fit closely the predictions of the kinetic theory of entropic elasticity in rubbers. In vitro mechanical tests on the octopus aorta showed that this vessel is a highly distensible, resilient elastic tube which, like mammalian arteries, becomes much less compliant as it is extended. The tangential elastic modulus in the circumferential direction (Ec) ranged from 9X10³ to 2X10⁵ N/m² over the normal resting range of physiological pressures (20 to 50 cm of water), while the elastic modulus in the longitudinal direction (El) was always about one-half of Ec. Hysteresis in quasi-static inflations was about 25%. The dynamic modulus of elasticity increased continuously with frequencies from 0.1 Hz to 10Hz. Tan d varied from about 0.10 to 0.15 over the physiologically relevant range of frequencies in this animal (0.1 to 1.0 Hz). These results show that this artery can function as an elastic, pulse smoothing component in the octopus circulation. Further, it appears that the elastic properties of the intact artery can be attributed largely to the rubber-like protein found in the wall. In vivo measurements of blood pressure and flow in the octopus show an impedance spectrum which is similar to what has been observed in turtles and frogs, that is, the arterial circulation of this invertebrate is adequately described by a simple Windkessel model. Pulse wave velocity measured in vivo appeared to be greater than 10 m/sec, although the velocity predicted from the elastic properties of the aorta was less than 5 m/sec. These anomalies arise because, due to the low heart rate of the octopus, the length of the aorta is less than 5% of the wavelength of the significant frequencies in the pressure pulse. The arterial system of the octopus is one which is dominated by strong wave reflections, but does not exhibit other transmission effects such as peaking and distortion of the travelling pressure wave. Para executar o que nos propomos, foram u-tilizados 24 corações oriundos de cadáveres adultos, b feito uso de métodos de dissecção fina, com auxílio de lupa e este-reomicroscópio, de técnicas histológicas e de cortes espessos e microscopia de luz polarizada. Com base nos resultados obtidos, julgamos poder emitir as seguintes conclusões alusivas a este trabalho: a parede do s.c, em toda sua extensão, é constituída por três estratos: adventícia, média e íntima; a túnica média da parede do s.c. é constituída por feixes de fibras musculares semelhantes aquelas do miocárdio, e que originam-se, na sua maioria, da parede do átrio esquerdo; d 5 feixes musculares dispo em -se em forma de espirais alongadas que se agrupam em dois planos; os feixes musculares ao nível do óstio de desembocadura do s.c, são predominantemente semicirculares, formando uma espécie de esfíncter; os feixes de fibras colágenas formam redes de malhas alongadas nas túnicas adventícia e íntima, na túnica média elas dispõem-se entre os feixes musculares; as fibras elásticas também formam redes de malhas alongadas e na túnica média constituem camadas aproximadamente concêntricas que envolvem os feixes musculares; o s.c. encontra-se rodeado, em toda sua extensão, por uma quantidade variável de tecido adiposo. Portanto, baseados em nossos resultados, podemos suge rir que durante a sístole atrial ocorreria a fechamento do óstio do s.c, através da contração das fibras musculares que o envolve, complementado pela atuação da válvula de Thebesius, o que impediria o refluxo sanguíneo. Em seguida, pela contração das fibras espirais da sua parede, haveria um aumento da luz e um encurtamento do órgão e posteriormente, através da contração destas fibras, agora com disposição anular, ocorreria uma diminuição da luz do s.c. As fibras colágenas presentes na parede do s.c teriam por função evitar a sua distensão excessiva, enquanto que o componente elástico seria responsável por manter um certo grau de distensibilidade contra aumentos da pressão sanguínea, e auxiliar na fase esfinctérica da musculatura / Abstract: The mechanisms yhich contribute to the closing of the ostium of the coronary sinus and that impede the venous reflux during the atrial systole are not fully understood. The arrangement of the fibrous elements which make up the wall of the sinus is also unknown. The purpose of this paper is to present a morpho-functional analysis at a mesoscopic level, of the coronary sinus, considering, in an integrated way, the arrangement of the muscular, collagen and elastic elements which compose the wall of the vessel, as well as the participation of of these elements in the opening and closing mechanism of the ostium in the right atrial cavity. To carry out our proposal 24 adult cadaver hearts were analyzed fay means of fine dissection, with the aid of a magnifying glass and of stereo microscopy, of histological technics and of thick cuttings as yell as polarized light microscopy. Based on the results, we believe we are able to draw the following conclusions from our study; the tunica media of the coronary sinus wall is entirely composed of bundles of muscle fibers similar to those of the myocardium; these bundles originate mainly in the wall of the left atrium; the muscular bundles are found in the form of elongated spirals that come together on two planes; the muscular bundles of the ostium of the coronary sinus are predominantly semicircular creating a type of sphincter; the collagenous fiber bundles form a network of elongated mesh in both the tunica intima and the external coat; in the tunica media, the fibers are found between the muscular bundles; the elastic fibers also form an elongated meshwork and, in the media, constitute almost concentric layers which envelop the muscular bundles; the coronary sinus is almost completely surrounded in a varying quantity of adipose tissue. Consequently, supported by our results, we can suggest that during an atrial systole the ostium of the coronary sinus closes by the contraction of its surrounding muscular bundles, complemented by the action of the Thebesius valve thus impeding the reflux of blood. Afterward, by the contraction of the spiral fibers in its wall, an expansion of the lumen and a shortening of the organ occurs after which another contraction of the fibers, now with an annulate disposition, occurs resulting in a decrease in the size of the lumen of the coronary sinus. The collagenous fibers present in the wall of the coronary sinus prevent excessive distension, while the elastic component is responsible for maintaining a certain degree of expandability during increase of blood pressure as well as helping in the sphincteric phase of the musculature / Mestrado / Mestre em Odontologia

Neste cenário, os programas multimodais de aceleração da recuperação pós-operatória (PMARP) podem contribuir para a otimização dos cuidados ao paciente e diminuir a utilização de recursos financeiros, mantendo-se a qualidade e a segurança. O objetivo desta revisão sistemática foi avaliar a eficácia e a segurança destes programas no reparo cirúrgico eletivo do AAA. Realizou-se revisão sistemática de estudos clínicos envolvendo os PMARP no reparo cirúrgico do AAA, avaliando a eficácia e segurança, quando comparados com os cuidados perioperatórios convencionais. Estudos clínicos foram recuperados eletronicamente das bases de dados MEDLINE, EMBASE, LILACS, Cochrane Trials Register (CENTRAL), por meio de estratégia de busca com termos apropriados. Identificaram-se os estudos aleatorizados controlados e os não aleatorizados (série de casos). Formulários padronizados foram utlizados para extração de dados. A análise estatística dos estudos aleatorizados foi executada com o programa REVMAN 5.0.2 e para os estudos não aleatórios foi utilizado o StatsDirect. Foram recuperados 13 estudos de série de casos e um ensaio clínico aleatorizado controlado unicêntrico. No ensaio clínico aleatorizado e controlado verificou-se redução significativa no tempo de hospitalização com o PMARP. A metanálise proporcional de série de casos não resultou em nenhuma diferença significativa entre pacientes tratados pelo PMARP e o tratamento convencional. O PMARP em cirurgias eletivas de AAA reduz o tempo de internação hospitalar, sem aumento de mortalidade. Devido aos níveis baixos de evidências, existe a necessidade de realização de mais ensaios aleatórios controlados para gerar grau de recomendação mais sólido / The open surgical repair of abdominal aortic aneurysm (AAA) is a procedure with high morbidity and mortality rates, prolonged hospital stay and high cost. In this scenario, the multimodal program for accelerated postoperative recovery (MPAPR) can contribute to the optimization of patient care and reduce the use of financial resources, maintaining quality and safety. The aim of this systematic review was to evaluate the efficacy and safety of these programs in elective surgical repair of AAA. We conducted a systematic review of clinical studies involving MPAPR in the surgical repair of AAA, evaluating the efficacy and safety compared with conventional perioperative care. Clinical studies were retrieved electronically from MEDLINE, EMBASE, LILACS, Cochrane Trials Register (CENTRAL), through search strategy with the appropriate terms. Identified the randomized controlled trials and non-randomized (number of cases). Standardized forms were utilized for data extraction. Statistical analysis of randomized trials were performed with the program RevMan 5.0.2 and the non-randomized studies was used StatsDirect. Retrieved 13 studies were case series and a randomized controlled single-center. In randomized controlled clinical trial found a significant reduction in hospitalization time with MPAPR. The meta-proportional number of cases resulted in no significant difference between patients treated by MPAPR and conventional treatment. The MPAPR in elective AAA reduces length of hospital stay without increasing mortality. Due to the low levels of evidence, there is a need for more randomized controlled trials to generate any degree of recommendation

Neste cenário, os programas multimodais de aceleração da recuperação pós-operatória (PMARP) podem contribuir para a otimização dos cuidados ao paciente e diminuir a utilização de recursos financeiros, mantendo-se a qualidade e a segurança. O objetivo desta revisão sistemática foi avaliar a eficácia e a segurança destes programas no reparo cirúrgico eletivo do AAA. Realizou-se revisão sistemática de estudos clínicos envolvendo os PMARP no reparo cirúrgico do AAA, avaliando a eficácia e segurança, quando comparados com os cuidados perioperatórios convencionais. Estudos clínicos foram recuperados eletronicamente das bases de dados MEDLINE, EMBASE, LILACS, Cochrane Trials Register (CENTRAL), por meio de estratégia de busca com termos apropriados. Identificaram-se os estudos aleatorizados controlados e os não aleatorizados (série de casos). Formulários padronizados foram utilizados para extração de dados. A análise estatística dos estudos aleatorizados foi executada com o programa REVMAN 5.0.2 e para os estudos não aleatórios foi utilizado o StatsDirect. Foram recuperados 13 estudos de série de casos e um ensaio clínico aleatorizado controlado unicêntrico. No ensaio clínico aleatorizado e controlado verificou-se redução significativa no tempo de hospitalização com o PMARP. A metanálise proporcional de série de casos não resultou em nenhuma diferença significativa entre pacientes tratados pelo PMARP e o tratamento convencional. O PMARP em cirurgias eletivas de AAA reduz o tempo de internação hospitalar, sem aumento de mortalidade. Devido aos níveis baixos de evidências, existe a necessidade de realização de mais ensaios aleatórios controlados para gerar grau de recomendação mais sólido / Abstract: The open surgical repair of abdominal aortic aneurysm (AAA) is a procedure with high morbidity and mortality rates, prolonged hospital stay and high cost. In this scenario, the multimodal program for accelerated postoperative recovery (MPAPR) can contribute to the optimization of patient care and reduce the use of financial resources, maintaining quality and safety. The aim of this systematic review was to evaluate the efficacy and safety of these programs in elective surgical repair of AAA. We conducted a systematic review of clinical studies involving MPAPR in the surgical repair of AAA, evaluating the efficacy and safety compared with conventional perioperative care. Clinical studies were retrieved electronically from MEDLINE, EMBASE, LILACS, Cochrane Trials Register (CENTRAL), through search strategy with the appropriate terms. Identified the randomized controlled trials and non-randomized (number of cases). Standardized forms were utilized for data extraction. Statistical analysis of randomized trials were performed with the program RevMan 5.0.2 and the non-randomized studies was used StatsDirect. Retrieved 13 studies were case series and a randomized controlled single-center. In randomized controlled clinical trial found a significant reduction in hospitalization time with MPAPR. The meta-proportional number of cases resulted in no significant difference between patients treated by MPAPR and conventional treatment. The MPAPR in elective AAA reduces length of hospital stay without increasing mortality.
