Abordagem psicológica das arritmias cardíacas : uma análise das emoções relatadas em exame de Holter

Conseqüentemente, não esclarecem quanto aos efeitos cardíacos de situações em que as pessoas vivenciam alegria, satisfação ou segurança. Considerando, então, a necessidade de ampliar o conhecimento sobre tais aspectos psicológicos, realizou-se uma investigação com o objetivo de identificar e associar emoções e episódios de vida diária que ocorrem simultaneamente a arritmias cardíacas. A pesquisa foi conduzida em uma clínica do Distrito Federal e teve a participação de 13 homens (média de idade = 58,5) e 17 mulheres (média de idade = 57,8), que procuraram o serviço para a realização do exame de Holter. Inicialmente, de acordo com a rotina de atendimento prevista, reuniram-se os dados do Relatório Tabular, que indica o total de arritmias ocorridas a cada intervalo de hora, e as informações lançadas no Relatório Diário pelo participante, acrescidas do registro das emoções vivenciadas ao longo das 24 horas. Em seguida, por ocasião da retirada do aparelho, realizaram-se entrevistas semi-estruturadas, as quais foram gravadas em áudio, transcritas e submetidas ao software ALCESTE. O conteúdo dos Relatórios Diários foi emparelhado, hora a hora, com os registros do Relatório Tabular, adotandose como critério contabilizar apenas um registro, a cada intervalo de uma hora para cada paciente, nos intervalos em que houvesse tanto relato de atividade quanto de estado emocional, durante o período de vigília. Esses dados foram analisados por método de estatística descritiva, optando-se pela média aritmética dos registros. Os resultados obtidos mostraram diferenças acentuadas entre os gêneros. As mulheres apresentaram mais arritmias que os homens. Todavia, a distribuição ao longo das 24 horas foi mais irregular entre os participantes do sexo masculino. As atividades registradas foram organizadas em três categorias: mentais, físicas e no trânsito. Na primeira, as arritmias distribuíram-se de forma homogênea nos dois gêneros. Já na categoria atividades físicas, bem como nas situações relacionadas ao trânsito verificaram-se resultados opostos em homens e mulheres. Em razão do tamanho da amostra, procedeu-se à análise individual dos casos, elaborando-se um sistema com três categorias principais (emoções positivas, negativas e neutras) subdivididas em dez subcategorias. Relatos de alegria apresentaram-se pouco simultâneos a arritmias em ambos os sexos. Entre as mulheres, as arritmias ventriculares ocorreram mais simultaneamente à preocupação do que à ansiedade e não se mostraram relacionadas à raiva. Entre os homens, ocorreu o inverso. A tristeza não foi associada a arritmias ventriculares por nenhum dos gêneros, mas foi relatada mais freqüentemente durante as arritmias supraventriculares em mulheres. A ansiedade foi mais vinculada aos dois tipos de arritmias pelos homens. A análise das entrevistas por meio do ALCESTE discriminou três classes (situações relacionais, vivências pessoais e comportamentos) subdivididas em cinco categorias. Também foi possível verificar que os participantes não atribuem a doença às relações interpessoais, mas relacionam seu estado de saúde a comportamentos e emoções. Ao final da dissertação, discutem-se as limitações do estudo desenvolvido e sugerem-se mais pesquisas no campo da Psicologia Aplicada à Cardiologia, voltadas especificamente para essa temática. __________________________________________________________________________________ ABSTRACT / In general way, studies that aim to detect risk factors associated to the illnesses of heart give emphasis to negative emotions. Consequently, they do not clarify the cardiac effects of situations where people feel joy, satisfaction or insecurity. Therefore, considering the need to extend the knowledge on such psychological aspects, it has been done an inquiry with the purpose to identify and to associate emotions and episodes of daily life with simultaneous cardiac arrhythmias. The research was lead in a clinic of Distrito Federal and had the participation of 13 men (average= 58,5 years) and 17 women (average= 57,8 years). All volunteers have gone for an accomplishment of Holter examination. Initially, in accordance with the foreseen routine of attendance, the data of the Tabular Summary, which indicates the total of arrhythmias occurred to each interval of hour, and the information launched in the Report of Activities for the participant had been congregated. After, it has been added the register of the emotions felt throughout the 24 hours. In occasion of the withdrawal of the device, halfstructuralized interviews had been done, which had been recorded in audio, transcribed and submitted to ALCESTE software. The content of Daily Reports was pair uped, hour the hour, with Tabular Summary registers, adopting itself as criterion to consider only one register for each interval of one hour where it had been there story of activity or emotional state, during the period of vigil, for each patient. These data had been analyzed by method of descriptive statistics considering the arithmetic mean of the registers. The gotten results have shown great differences between sorts. Women had presented more arrhythmias than men. However, the distribution throughout the 24 hours was more irregular between participants of masculine sex. The registered activities had been organized in three categories: mental, physical and in traffic. In the first one, the arrhythmias had been distributed homogeneously in both sorts. The category of physical activities has shown opposite results between men and women, as well as the situations related to traffic. In reason of the size of the sample, it had been analyzed the cases individually, elaborating itself a system with three main categories (positive, negative and neutral emotions) subdivided into ten subcategories. Stories of joy had presented very few simultaneous arrhythmias in both sorts. Between women, the ventricular arrhythmias had occurred more simultaneously to the concern of anxiety and had not revealed arrhythmias related to anger. However between men, the inverse occurred. The sadness was not associated with ventricular arrhythmias for none of the sorts, but it was pointed more frequently during the supraventricular arrhythmias in women. The anxiety had been revealed in the two types of arrhythmias for men. The analysis of the interviews by means of the ALCESTE has given three classes (relationary situations, personal experiences and behaviors) subdivided into five categories. It was also possible to verify that the participants do not attribute the illness to the interpersonal relations, but relate its state of health to behaviors and emotions. In the end of the developed study, its limitations are argued and it is suggested more research in the field of the Psychology Applied to Cardiology, specifically toward this thematic one.
