This is an exploratory-descriptive study, in a qualitative approach, whose data were collected through the application of a questionnaire based on the nursing diagnoses of NANDA International, Definitions and Classifications, included in the book Nursing Diagnostics (2012-2014). This questionnaire was completed by seven nurses who work in a private health care company that performs Chronic Disease Management for those aged 60 or over in their homes. The application of the questionnaire with 13 questions occurred twice: the first one, before the training stage, and the second, after that stage, during the first half of 2017, in the city of São Paulo. As results, it was verified the need for a more competent multidisciplinary training of nurses for health guidance to the elderly in their home, especially in the light of gerontological knowledge. The sample of this study seemed to show, through the initial frustration of its procedures, that, according to our hypothesis, a reflexive-critical reasoning about one's own actions of care was lacking, as well as a more affectionate and concentrated look at each elderly person. It would not suffice, in our view, to apply techniques with rigid protocols, which provide only punctuations to pre-defined answers, insufficient to capture the long-lived singularity; the protocols should be seen in what they are: indicators for more effective health actions. The results, in fact, attest that, affected by the gerontological training, nurses began to assume different procedures in the reception and orientation to the elderly, with these being alerted to the necessary adherence to daily habits of sustainable life / Este estudo teve como objetivo geral compreender como um enfermeiro-visitador pode proceder, de forma adequada, em sua orientação a uma pessoa idosa em residência, a ponto de esta introjetar tais orientações em saúde, e incorporá-las em seu cotidiano de vida, o que significa que se possa atestar a eficácia dos resultados de tais procedimentos. Trata-se de uma pesquisa exploratório-descritiva, em abordagem qualitativa, cujos dados foram coletados por meio da aplicação de um questionário baseado nos diagnósticos de enfermagem da NANDA Internacional, Definições e Classificações, constantes do livro Diagnósticos de Enfermagem (2012-2014). Este questionário foi preenchido por sete enfermeiras que atuam em uma empresa privada de Saúde Suplementar a qual realiza Gerenciamento de Doenças Crônicas àqueles com 60 ou mais anos em suas residências. A aplicação do questionário com 13 perguntas se deu por duas vezes: a primeira, antes da etapa de capacitação e, a segunda, após essa etapa, durante o primeiro semestre de 2017, na capital paulistana. Como resultados, verificou-se a necessidade de uma mais proficiente formação multidisciplinar do enfermeiro para a orientação de saúde à pessoa idosa no seu lar, especialmente sob a luz de conhecimentos gerontológicos. A amostra deste estudo pareceu evidenciar, pela frustração inicial de seus procedimentos, que faltaria, segundo nossa hipótese, um raciocínio reflexivo-crítico sobre as próprias ações do cuidar, além de um olhar com mais afeto e concentração a cada idoso. Não bastaria, a nosso ver, aplicar técnicas com protocolos rígidos, que fornecem apenas pontuações a respostas pré-definidas, insuficientes para capturar a singularidade longeva; os protocolos devem ser vistos no que são: indicadores para ações de saúde mais efetivas. Os resultados atestaram que, afetados pela capacitação gerontológica, os enfermeiros passaram a assumir procedimentos diferenciados no acolhimento e orientação aos idosos, com estes se alertando para a necessária adesão a hábitos cotidianos de vida sustentável
