Novas perspectivas na adaptação de AASI em idosos / New perspectives in the adaptation of hearing aids to the elderly

A terceira idade é considerada uma etapa da vida em que são observadas muitas barreiras, mas também muitos avanços nas tecnologias assistivas a essa população, que ao passar dos anos, têm aumentado não somente sua expectativa de vida, mas também sua atuação com relação às questões sociais e políticas, com engajamento inclusive no mercado de trabalho. No entanto, nesse momento do envelhecimento ocorrem degenerações funcionais e estruturais, ocasionando prejuízos observados nas situações de interação do idoso com o meio em que vive. O processo de envelhecimento das funções auditivas traz consigo a necessidade de novos estudos para a quebra dos paradigmas em relação ao processo de seleção, verificação e adaptação dos AASIs em idosos. Assim, o objetivo dessa pesquisa foi estudar o impacto das medidas de prescrição na adaptação do indivíduo deficiente auditivo idoso, analisando comparativamente os valores dos alvos prescritos pelos métodos DSL m[i/o] v5.0 e NAL-NL2, nas curvas de respostas em função da frequência, na mensuração da percepção de fala no silencio e no ruído, como também, nas habilidades auditivas finais com o uso da amplificação. As programações foram mensuradas e comparadas em relação ao seu desempenho nas avaliações da Real Ear Aided Response (REAR), do Hearing in Noise Test (HINT), na utilização do algoritmo de datalogging e também por meio do questionário Client Oriented Scale of Improvement (COSI). O estudo foi realizado com 28 idosos submetidos à etapa de pré-adaptação, que consistiu na realização de testes domiciliares em dois momentos distintos, utilizando-se alternadamente os métodos DSL m[i/o] v5.0 e NAL-NL2 para programação dos AASIs, que foram testados individualmente por 15 dias, confrontando os resultados em busca da melhor programação para o prosseguimento em etapa posterior. Na etapa pós-adaptação, foram realizados três retornos (30, 60 e 90 dias), reavaliando a cada retorno, a otimização da programação escolhida, obtendo-se ao final, os resultados da melhora do desempenho auditivo individual objetivo e subjetivo. As avaliações mostram que na etapa 1 (pré-adaptação), a melhor programação para toda a amostra foi proposta pelo método DSL m[i/o] v5.0, prosseguindo-se para a etapa 2, onde foi verificada melhora do seu desempenho a cada retorno realizado após a adaptação dos aparelhos de amplificação sonora individuais (AASIs). Dessa forma, o estudo concluiu que a programação proposta pelo método DSL m[i/o] v5.0 demonstrou-se superior a NAL-NL2 para todos os participantes do estudo em todas as avaliações realizadas, confirmando a hipótese de que a programação NAL-NL2 suprime o ganho necessário às perdas auditivas mais incidentes nos idosos decorrentes da presbiacusia. / Old age is considered a stage of life in which many barriers are observed, but also many advances in assistive technologies for this population, which over the years have increased not only their life expectancy but also their performance in relation to the issues social and political, with an engagement in the labor market. However, at this time of aging, functional and structural degenerations occur, causing damages observed in situations of interaction of the elderly with the environment in which they live. The process of aging of the auditory functions brings with it the need of new studies for the breakdown of the paradigms in relation to the process of selection, verification and adaptation of hearing aids in the elderly. Thus, the objective of this research was to study the impact of prescription measures on the adaptation of the elderly hearing impaired individual, comparing the values of the targets prescribed by the DSL m[i/o] v5.0 and NAL-NL2 methods in the response curves as a function of frequency, in the measurement of speech perception in silence and noise, as well as in the final auditory abilities with the use of amplification. The schedules were measured and compared in relation to their performance in the Real Ear Aided Response (REAR), Hearing in Noise Test (HINT), the use of the datalogging algorithm and also in the application of the Client Oriented Scale of Improvement (COSI). The study was carried out with 28 elderly subjects undergoing preadaptation, two home tests of 15 days were applied, one with each schedule, confronting their results in search of the best programming for the continuation in a later stage. In the post-adaptation stage, three returns (30, 60 and 90 days) were performed, re-evaluating the optimization of the chosen program at each return, obtaining at the end the results of improvement of individual and objective auditory performance. The evaluations show that in step 1 (pre-adaptation), the best programming for the entire sample was proposed by the DSL m[i/o]v5.0, proceeding to step 2, where it was verified improvement of its performance to each return made after the adaptation of the individual sound amplifiers (AASIs). Thus, the study concluded that the programming proposed by the DSL m[i/o]v5.0 was shown to be superior to NAL-NL2 in every sample for all the evaluations performed, confirming the hypothesis that NAL-NL2 programming suppresses the gain needed for hearing losses more incidental in the elderly due to presbycusis.

Plataforma virtual sobre gerenciamento do zumbido: uma ferramenta de acesso técnico científico para fonoaudiólogos / Virtual platform on tinnitus management: a scientific technical access tool for speech therapists

O zumbido é um sintoma relatado frequentemente na rotina clínica fonoaudiológica, a qual está inserida desde a avaliação e diagnóstico até a intervenção deste sintoma. Diversos estudos são desenvolvidos com objetivos direcionados à promoção de saúde auditiva, sendo a Telessaúde uma alternativa para este cenário. O objetivo deste estudo foi desenvolver e avaliar uma plataforma virtual de transmissão de informações sobre o gerenciamento do zumbido para fonoaudiólogos utilizando a Teleducação. Visando oferecer atualização e conhecimento aos profissionais de todas as regiões do país, foi proposta a realização do curso à distância, por meio do Portal do Zumbido, disponibilizado no endereço eletrônico zumbido.fob.usp.br/. A plataforma é composta por módulos que contemplam a introdução ao sintoma, avaliação e intervenção do zumbido, sendo que todos os materiais elaborados são em forma de Infográficos disponibilizados para download. Foram convidados a avaliar a plataforma, fonoaudiólogos das diferentes regiões do Brasil por meio de correio eletrônico, havendo cadastramento de 46 profissionais. Deste total, 38 responderam a avaliação da plataforma virtual de aprendizagem, por meio de um questionário sobre o conteúdo teórico e uma Ficha de Pesquisa Motivacional. Os profissionais responderam corretamente entre 90% e 100% das questões, demonstrando que o conteúdo foi absorvido de forma positiva, além disto, o material foi considerado impressionante após a realização da análise dos dados da Ficha de Pesquisa Motivacional. O Portal do Zumbido foi elaborado e disponibilizado no endereço eletrônico zumbido.fob.usp.br/, com avaliação satisfatória pelos profissionais participantes do estudo. / Tinnitus is a prevalent symptom in the audiological clinics and the speech therapist participate in the assessment, diagnose and intervention of tinnitus. A range of studies are developed with the aim to promote auditory health, thus telemedicine is an option for this purpose. According to the purpose of telemedicine, the aim of the present study was to develop and to evaluate a virtual platform on tinnitus management for speech therapists. The purpose was to offer distance learning course, by means of Portal do Zumbido, available in the following eletronic address: zumbido.fob.usp.br/. The platform was composed for modules and comprised the introduction to the symptom, assessment and intervention for tinnitus and the elaborated information is presented by means of infographics, available to download. Speech therapists of different regions of Brazil, receveid an e-mail invitation to participate and avaluate the study and 46 were registered. From all the 46 speech therapists registered, 38 evaluated the virtual platform, by means of a questionnaire about the theoric content and a Motivational Research. The professionals correctly answered 90% and 100% of the questions, which means the content was learned, in addittion, the content was considered to be impressive when the Motivational Research was analysed. The Portal do Zumbido was elaborated and provided in the following eletronic address: zumbido.fob.usp.br/, furthermore, the speech therapists made satisfactory evaluation.

Gerador de som: análise da eficácia dos ruídos mascaradores no alívio do zumbido / Sound generator: analysis of the effectiveness of masking noise in tinnitus relief

Introdução: o zumbido é uma desordem que acomete 10 a 15% da população mundial e por se tratar de um sintoma subjetivo que apresenta grande repercussão na vida do indivíduo, pesquisas científicas na área são de grande importância. A terapia sonora realizada por meio do aparelho de amplificação sonora individual (AASI) com gerador de som (GS) associado é uma das formas de tratamento do zumbido e vem ganhando espaço nos serviços de saúde auditiva. Proposição: analisar a eficácia de quatro ruídos mascaradores no alívio do zumbido de indivíduos com perda auditiva sensorioneural bilateral de grau leve e moderado e a influência dos mesmos na percepção da fala. Casuística e Métodos: o estudo foi desenvolvido na Clínica de Fonoaudiologia da Faculdade de Odontologia de Bauru (FOB) - USP após a aprovação do Comitê de Ética e anuência do paciente. Participaram 35 indivíduos com zumbido e perda auditiva sensorioneural leve e moderada bilateral, divididos em quatro grupos, sendo três grupos compostos por nove indivíduos e um grupo por oito. Todos os grupos passaram pela adaptação do AASI com GS, sendo regulados no modo combinado (GS + amplificação). No grupo 1 (G1) foi aplicado o estímulo White Noise, no grupo 2 (G2) o Pink Noise, o grupo 3 (G3), o Speech Noise e o grupo 4 (G4) o High Tone Noise. Todos os pacientes passaram pelos seguintes procedimentos: anamnese, diagnóstico audiológico composto por audiometria de alta frequência, imitanciometria e acufenometria. Foram aplicados os questionários Tinnitus Handicap Inventory (THI), a Escala Análoga Visual (EAV) e avaliação da percepção da fala por meio do Hearing in Noise Test (HINT) na situação de fala no silêncio (FS) e ruído frontal (RF). Os procedimentos foram realizados pré e pós-intervenção auditiva, após três meses de uso do AASI. Os resultados foram tabulados e descritos de acordo com a análise estatística descritiva de todas as variáveis estudadas. Resultados: todos os grupos apresentaram diferença estatisticamente significativa para o questionário THI, EAV e HINT na situação de FS, pré e pós-intervenção auditiva. Na situação de RF, somente o G2 apresentou diferença significativa. Porém, na comparação entre grupos não houve diferença significativa. Conclusão: os quatro ruídos mascaradores demonstraram ser igualmente eficazes na melhora do zumbido e na percepção de fala na situação de silêncio, porém no RF, o G2 apresentou melhora significativa. / Introduction: tinnitus is a disorder that affects 10 to 15% of the world\'s population and since it is a subjective symptom which presents great impact on the individual\'s life, all scientific contribution becomes significant and important. The sound therapy performed through the individual sound amplification apparatus (AASI) with associated sound generator (GS) is one of the forms of tinnitus treatment and has been obtaining space in the hearing health services. Proposition: analyzing the effectiveness of four masking noises in relieving tinnitus of individuals with mild and moderate bilateral sensorineural hearing loss and influence thereof on speech perception. Casuistry and Methods: the study was developed at the Speech Therapy Clinic of the Faculty of Dentistry of Bauru (FOB) - USP after approval by the Ethics Committee and patient consent. Participants were 35 individuals with tinnitus and bilateral mild and moderate sensorineural hearing loss, divided into four groups, three groups composed of nine subjects and one group comprising eight subjects. All groups underwent hearing loss adaptation with GS, being regulated in the combined mode (GS + Amplification). In Group 1 (G1) the White Noise stimulus was applied, in group 2 (G2) Pink Noise was applied, in group 3 (G3), Speech Noise and in group 4 (G4) the High Tone Noise was applied. All patients were subjected to the following procedures: anamnesis, audiological diagnosis composed of high frequency audiometry, immittanciometry and acuphenometry. The Tinnitus Handicap Inventory (THI), Visual Analogue Scale (EAV) and Speech Perception Assessment questionnaires were applied through the Hearing in Noise Test (HINT) in the speech silence (FS) and frontal noise (RF) situation. The procedures were performed prior to and after hearing intervention, after three months of use of AASI. The results were tabulated and described according to the descriptive statistical analysis of all variables studied. Results: all groups presented a statistically significant difference for the THI, EAV and HINT questionnaires in the FS, pre- and post-hearing intervention. In the RF situation, only G2 presented a significant difference. However, in the comparison among groups there was no significant difference. Conclusion: the four masking noises proved to be equally effective in improving tinnitus and in speech perception in the silent situation, but in RF situation, G2 presented significant improvement.

Sistema de frequência modulada e malformação de orelha: benefício e efeito do uso no ambiente educacional / Frequency modulation (FM) System and ear malformation: benefit and effects of the use of this device in the educational environment

As malformações congênitas de orelha causam dificuldades auditivas, interferindo no desenvolvimento das habilidades de fala e linguagem e, consequentemente, prejudicando o desenvolvimento social, emocional, cognitivo e acadêmico da criança. Para minimizar estas dificuldades, o aparelho de amplificação sonora individual (AASI) por condução óssea é indicado. Com a possibilidade de adaptação do sistema de frequência modulada (FM) pelo SUS, de acordo com a portaria n° 1.274 do Ministério da Saúde, de 25 de junho de 2013, o objetivo deste trabalho foi avaliar o benefício deste dispositivo em crianças e adolescentes com malformação de orelha externa e/ou média, usuários de AASI por condução óssea, sob a ótica dos próprios usuários, seus pais e professores. Participaram da pesquisa vinte e quatro crianças e adolescentes, doze pais ou responsáveis e doze professores. Foram utilizados os instrumentos: Listas de Sentenças em Português, para avaliar a percepção de fala no ruído e Classroom Participation Questionnaire (CPQ), para a auto avaliação da participação em sala de aula (usuários); Questionário Avaliação do Sistema FM, para avaliar o desempenho em diferentes situações auditivas (pais) e Screening Instrument For Targeting Educational Risk in Secondary Students (SIFTER), para a avaliação do desempenho acadêmico (professor). Todas as avaliações foram realizadas nas condições com e sem o sistema de FM e para a comparação dos resultados foi utilizado o teste t pareado. Em todos os procedimentos estatísticos foi adotado nível de significância de 5% (p<0,05). Foi observada diferença significante com o uso do sistema de FM para todos os aspectos avaliados, evidenciando o benefício da adaptação deste dispositivo para esta população. / Congenital ear malformations cause hearing impairment, interfering with speech and language development, and, consequently, impairing the social, emotional, cognitive and academic development of the child. Bone conduction hearing aids are indicated to minimize these difficulties. With the possibility of fitting the FM System by the Brazilian Health Public System, according to the ordinance no. 1,274 from the Ministry of Health, dated June 25, 2013, this study aimed to evaluate the benefit of the FM System in children and adolescents with external ear and/or middle ear malformation, users of bone conduction hearing aids, through the perspective of the users, their parents and teachers. Twenty-four children and adolescents, twelve parents or guardians, and twelve teachers participated in this study. The instruments used were: Sentence Lists in Brazilian Portuguese, to evaluate speech perception in noise, and Classroom Participation Questionnaire (CPQ), for self-assessment of classroom participation (users); FM System Evaluation Questionnaire, to evaluate performance in different auditory situations (parents); and Screening Instrument for Targeting Educational Risk in Secondary Students (SIFTER), to evaluate academic performance (teachers). All evaluations were performed with and without the FM System, and the paired t-test was used to compare the results. A significance level of 5% was adopted (p<0.05) in all statistical tests. A significant difference was observed with the use of the FM system in all aspects evaluated, demonstrating the benefit of fitting this device in this population.

Estudo comparativo dos métodos prescritivos NAL-NL2 e DSL v5.0a na programação individualizada do AASI para o idoso / Comparative study of the prescriptive methods NAL-NL2 and DSL v5.0a in the individualized programming of hearing aids for the elderly

Introdução: O uso de métodos prescritivos na programação do aparelho de amplificação sonora individual (AASI) tem como principal objetivo a aplicação da amplificação recomendada aos usuários, melhorando a audibilidade de sons suaves e fala conversacional e garantindo conforto para sons fortes. Diferentes métodos, no entanto, podem ter diferentes objetivos de ajuste. Considerando as modificações nas estruturas do meato acústico externo do indivíduo idoso, se faz necessário um direcionamento do processo de seleção e adaptação do AASI nessa população, propondo um protocolo individualizado e que leve em conta os ajustes recomendados pelos métodos de prescrição. Objetivo: Analisar comparativamente os métodos prescritivos NAL-NL2 e DSL v5.0a de acordo com programação individualizada do AASI para o indivíduo idoso com deficiência auditiva. Metodologia: Participaram do estudo 60 indivíduos idosos com deficiência auditiva. Após a assinatura do Termo de Consentimento Livre e Esclarecido, os participantes do estudo foram submetidos à realização da meatoscopia, mensuração da RECD (Real Ear to Coupler Difference) e programação individualizada do AASI com os métodos prescritivos NAL-NL2 e DSL v5.0a. A verificação do desempenho com cada prescrição foi realizada por meio das medidas da REAR (Real Ear Aided Response), cálculo do SII (Speech Intelligibility Index) e teste HINT (Hearing In Noise Test). A análise estatística comparativa dos dados obtidos nas duas situações foi realizada por meio dos testes t pareado e de Wilcoxon. Resultados: Houve diferença estatisticamente significante com melhor desempenho do método NAL-NL2 na avaliação da REAR nas faixas de frequências baixas e altas para sons de entrada de média e forte intensidade, na faixa de frequência alta para sons de entrada de fraca intensidade, e no cálculo do índice de inteligibilidade de fala para sons de entrada fracos. O método DSL v5.0a apresentou melhores resultados com diferença estatisticamente significante na avaliação da REAR em frequências médias para sons de entrada médios, em frequências baixas e médias para sons de entrada fracos, no cálculo do índice de inteligibilidade de fala para sons de entrada médios e fortes, e no teste HINT nas situações de silêncio e ruído. Conclusões: Não foi possível estabelecer um método mais adequado em todos os parâmetros avaliados. Na avaliação da percepção de fala houve uma diferença importante com melhor resultado para o método DSL v5.0a, o que oferece uma alternativa satisfatória em relação à percepção de fala no silêncio e no ruído e viabiliza a utilização deste método com a população idosa. A continuidade de estudos envolvendo o indivíduo idoso é de extrema relevância, considerando as características individuais desta população e a necessidade de estabelecer uma prática clínica específica para a mesma. O estabelecimento de diferentes perfis audiológicos dentre esta população é importante para determinar qual a melhor conduta para cada um no contexto da programação e adaptação do AASI, conforme todas as etapas e parâmetros a serem levados em consideração para um melhor resultado. / Introduction: The use of prescriptive methods in the hearing aids programming has as main objective the application of the recommended amplification to the users, improving the audibility of soft sounds and conversational speech and guaranteeing comfort for loud sounds. Different methods, however, may have different adjustment goals. Considering the modifications in the structures of the external acoustic meatus of the elderly patient, an orientation of the hearing aids selection and adaptation process in this population is necessary, proposing an individualized protocol that take into account the adjustments recommended by the prescription methods. Objective: To comparatively analyze the NAL-NL2 and DSL v5.0a prescriptive methods according to the hearing aids individualized programming for the elderly with hearing impairment. Methodology: The study included 60 elderly individuals with hearing impairment. After signing the Free Informed Consent Form, the participants of the study underwent a meatoscopy, RECD (Real Ear to Coupler Difference) measurement and hearing aids individualized programming with the prescriptive methods NAL-NL2 and DSL v5.0a. Performance verification for each prescription was performed using REAR measurements (Real Ear Aided Response), SII calculation (Speech Intelligibility Index) and HINT (Hearing In Noise Test). Comparative statistical analysis of the data obtained in both situations was performed using the paired t test and the Wilcoxon test. Results: There were statistically significant differences with a better performance of the NAL-NL2 method in the REAR evaluation in low and high frequency bands for medium and loud intensity input sounds, in the high frequency range for low intensity input sounds, and in the speech intelligibility index calculation for soft input sounds. DSL v5.0a presented better results with statistically significant difference in the REAR evaluation in medium frequencies for medium input sounds, in low and medium frequencies for soft input sounds, in the speech intelligibility index calculation for medium and loud input sound, and in the HINT test in silence and noise situations. Conclusions: It was not possible to determine a most suitable method for all evaluated parameters. On the speech perception evaluation there was an important difference having the DSL v5.0a method achieved better results, which offers a satisfactory alternative regarding speech perception in silence and noise conditions and allows the use of this method with the elderly population. The continuity of studies involving the elderly is extremely relevant considering the individual characteristics of this population and the need to establish a specific clinical practice for them. The establishment of different audiological profiles among this population is important to determine the best conduct for each one in the context of hearing aids programming and adaptation, according to all the steps and parameters to be taken into account for a better result.

Mapeamento visível da fala amplificada na verificação do aparelho de amplificação sonora individual digital / Hearing aid verification using visible speech mapping

Introdução: Na etapa de verificação da adaptação, o aparelho de amplificação sonora individual vem sendo estudado nos últimos anos por diferentes protocolos e equipamentos, a fim de aumentar a satisfação do usuário. Objetivo: Elaborar um protocolo de verificação de aparelho de amplificação sonora individual utilizando o mapeamento visível da fala amplificada na prática clínica e verificar a satisfação do usuário. Método: Em estudo transversal randomizado duplo-cego, foram avaliados 60 indivíduos adultos separados em dois grupos, sem experiência prévia ao uso do aparelho de amplificação sonora individual modelo retroauricular de tecnologia digital. O Grupo A foi submetido ao Protocolo IIIA, contendo os testes de ganho de inserção, reconhecimento de palavras, mapeamento visível da fala amplificada com verificação sem aconselhamento e os questionários: Client Oriented Scale of Improvement e Satisfaction with amplification in daily life. O Grupo B respondeu ao protocolo IIIB composto por: teste de reconhecimento de palavras, Mapeamento visível da fala amplificada com aconselhamento e os mesmo questionários Client Oriented Scale of Improvement e Satisfaction with amplification in daily life. Resultados: Demonstraram moderada correlação entre o ganho de inserção e o mapeamento visível da fala amplificada. Quando este último registrou os estímulos: suave, médio e forte dentro do campo dinâmico auditivo para 10% de aumento na resposta do teste de reconhecimento de palavras, significou 0,4 a 0,5 pontos de aumento na satisfação do usuário para a situação de silêncio e 0,2 a 0,3 para a situação com ruído competitivo. Conclusões: O protocolo para a verificação do aparelho de amplificação sonora individual elaborado demonstrou ser efetivo para a prática clínica e auxiliar no aumento de satisfação do usuário. / Introduction: The hearing aid verification stage has been studied over many years by different protocols and equipments to increase de user satisfaction. Objective: design a hearing aid protocol using Visible Speech Mapping applied to clinical practice in order to analyze the user satisfaction. Method: The cross-sectional, randomized study was done with 60 adults, separated into two groups without prior amplification experience binaurally fitted with digital behind-the-ear hearing aid technology. Four weeks later, the group A answered Protocol IIIA covering insertion gain, Word recognition test, Visible Seech Mapping verification tool without counseling, Client Oriented Scale of Improvement and Satisfation with Amplification in Daily Life questionnaires. The group B was evaluated using the Protocol IIIB containing Word Recognition Test, Visible Speech Mapping verification and counseling tool, Client Oriented Scale of Improvement and Satisfation with Amplification in Daily Life questionnaries. Results: The results showed moderated correlation between insertion gain and Visible Speech Mapping analyzing the procedures when applied to the group A. When Visible Speech Mapping register a soft, medium and strong stimulus within de dynamic range, each 10% improvement at the Word Recognition Test´s result means a 0.4 to 0.5 points, at silence, and 0.2 a 0.3 points increase at speech-in- noise on the satisfaction global score. Conclusions: digital hearing aid verification protocol using Visible Speech Mapping has demonstrated to be effective to the clinical practice.

A deficiência auditiva pode ter impacto negativo na qualidade de vida do indivíduo interferindo nas esferas das atividades e participação. O Aparelho de Amplificação Sonora Individual (AASI) é uma alternativa viável de tratamento para grande parte desta população. Entretanto, a simples aquisição de AASI não garante a resolução da deficiência. Estudos vêm apontando benefícios do aconselhamento de ajuste pessoal ao usuário de AASI desde a etapa de diagnóstico audiológico, favorecendo o processo de adaptação. Neste sentido, o objetivo deste estudo foi analisar o modelo transteórico de aconselhamento (MTT) que tem permitido uma abordagem mais ampla do indivíduo de maneira a englobar o sistema de crenças e aspectos motivacionais em candidatos a uso de AASI. Participaram deste estudo 20 candidatos (10 homens e 10 mulheres), com idades entre 25 e 64 anos (média = 52,95 anos) e deficiência auditiva neurossensorial bilateral de grau variando entre leve e severa. As estratégias utilizadas para o aconselhamento foram: a Escala de Prontidão (EP) e a Balança Decisória (BD), ambas propostas pelo Ida Institute, e as Proposições de Babeu et al. (2004). O aconselhamento foi realizado em três sessões: após o diagnóstico audiológico; no retorno para o teste com AASI, mas antes do mesmo; e após, ao menos, uma semana de utilização do AASI. Em cada sessão eram aplicadas as três ferramentas. As atitudes dos participantes foram avaliadas nos três momentos por meio do Questionário de Atitudes frente à Deficiência Auditiva (ALHQ). Como resultado, os participantes de modo geral mostraram por meio da EP, nos três momentos, que consideravam ser muito importante a melhora da audição e percebiam-se preparados para o uso do AASI. A BD indicou os elementos a serem trabalhados e que levaram os participantes a atribuírem valores na EP e a escolha nas Proposições de Babeu et al. (2004) por meio de reflexões quanto às vantagens e desvantagens com relação à audição prejudicada e ao uso de AASI. Tanto a BD quanto as Proposições de Babeu et al. (2004) possibilitaram a discriminação do estágio de mudança a que cada participante se encontrava. A maioria dos participantes indicou o estágio de ação para a sua condição motivacional nos três momentos. Comparando as atitudes dos participantes nesses três momentos, foram evidenciadas diferenças estatisticamente significantes somente com a influência da experiência com AASI (subescalas de Negação da perda auditiva e Estratégias negativas de enfrentamento). Embora não tenham sido evidenciadas diferenças estatisticamente significantes sem esta influência, a diminuição na amplitude do desvio padrão nas subescalas de Estima Relacionada à Audição e Associações Negativas sugere uma tendência maior à disposição para mudanças a qual pode ser atribuída ao aconselhamento uma vez que nesta etapa o participante ainda não utilizava AASI. Conclui-se que, o MTT aplicado às etapas iniciais do processo de adaptação do AASI constitui em uma proposta objetiva e factível para que este processo englobe aspectos importantes tais como o sistema de crenças, ideias, sentimentos e a condição motivacional do candidato, permitindo identificar as diferenças individuais bem como organizar as próximas etapas do processo de adaptação. / Hearing loss can negatively impact the quality of life of individuals, interfering their activities and participation. Hearing aids (HA) can be a viable treatment alternative for most of this population. However, the mere acquisition of HA does not guarantee the resolution of hearing loss. Researchers have emphasized the benefits of personal adjustment counseling for hearing aid users from audiology diagnostic step , facilitating the fitting process. This study, in particular, analyzed the transtheoretical model of counseling (MTT), which allows a broad approach to each individual, encompassing a system of beliefs and motivational aspects of the candidates who will begin using HA. This study included 20 volunteers10 men and 10 women) aged between 25 and 64 years (mean = 53 years) with bilateral sensorineural hearing loss varying between mild and severe. Three strategies were employed for counseling the volunteers, (1) the Readiness Scale (RS) strategy and (2) the Decisional Balance (DB) strategy, both from the Ida Institute, and (3) the Propositions of Babeu et al. (2004) strategy. Counseling was conducted during three sessions, (1) after the audiological diagnosis, (2) just prior to the first test with HA, and (3) after at least one week of using HA. During each session, the three strategies were applied. The attitudes of the participants were also evaluated over these three time periods using the Attitudes towards Loss of Hearing Questionnaire (ALHQ). Under the context of the RS strategy, the subjects demonstrated during the three sessions that was very important to improve ones hearing and perceived themselves prepared for hearing aid use. The BD strategy indicated elements to be improved, allowing participants to assign the values of the RS and chose the propositions of Babeu et al. (2004) by analyzing the advantages and disadvantages in relation to impaired hearing and the use of HA. Both the BD strategy and the propositions of Babeu et al. (2004) strategy allowed the discrimination of the stages of change that each participant experienced. Most participants indicated the action stage for their motivational condition during the three sessions. When comparing the attitudes of participants during these three sessions, significant differences were found only after using HA (subscales of \"denial of hearing loss\" and \"negative coping strategies\"). Although no statistically significant differences without this influence were found, the decrease in the amplitude of the standard deviation in the subscales of hearing-related esteem and negative associations might suggest that patients with out HA are more willing to change after counseling . It is concluded that when the MTT is applied to the initial steps of the hearing aid fitting process that an objective and feasible proposal can incorporate important aspects such as beliefs, ideas, feelings and motivational conditions of candidates, to identify their individual differences and organize the next steps of the fitting process, facilitating this process.

A Teleducação interativa na capacitação de profissionais em saúde auditiva / The Interactive Tele education in the training of professionals in hearing health

Introdução: A Telessaúde caracteriza-se pelo uso da tecnologia de informação e comunicação na área da saúde. Na Fonoaudiologia, o uso desta ferramenta por meio da Teleducação e Teleassistência se caracteriza como uma importante estratégia que leva a informação para alunos, profissionais, pacientes e comunidade em geral. Esse fator, aliado à grande extensão geográfica do Brasil e a distribuição heterogênea dos profissionais da saúde, vem ao encontro da necessidade de atendimento especializado para pacientes e descentralização do conhecimento para profissionais. Neste contexto, o uso da Teleducação emerge como uma proposta para minimizar as dificuldades encontradas pelo paciente, no processo de adaptação do aparelho de amplificação sonora individual (AASI), na qual muitos materiais educacionais são produzidos com o intuito de orientar e facilitar a adaptação do indivíduo deficiente auditivo, à sua nova realidade. Objetivo: Este projeto tem por objetivo elaborar um programa de capacitação por meio da Teleducação Interativa, sobre o aparelho de amplificação sonora individual, e analisar a eficácia do mesmo, quanto ao aprendizado e motivação dos profissionais. Resultados: A primeira etapa compreendeu a elaboração do programa de capacitação, na qual foi desenvolvido um cybertutor sobre o tema AASI e as ferramentas fórum de discussão e web conferência, com o intuito de complementar o aprendizado. Na segunda etapa, referente à análise do programa, verificou-se que não houve significância estatística no aspecto relacionado ao aprendizado do conteúdo, na avaliação pré e pós teste. Em relação à motivação dos profissionais, o estudo obteve resultado positivo, sendo avaliado como curso impressionante. Discussão: O programa de capacitação foi desenvolvido, respaldado em estudos voltados para elaboração de materiais didáticos à distância. No que se referem aos resultados da avaliação, todas as dificuldades encontradas neste aspecto corroboraram com a literatura. Conclusão: A elaboração do Programa de Capacitação por meio da Teleducação Interativa sobre o tema aparelho de amplificação sonora individual (AASI) foi realizada satisfatoriamente, alcançando um índice de motivação em relação à participação altamente positivo, porém os resultados da comparação da avaliação da situação problema pré e pós teste não foram estatisticamente significantes. / Introduction: Telehealth is characterized by the use of information technology and communication in healthcare. In audiology, the use of this tool through Telecare and Tele-education is characterized as an important strategy that leads the information for students, professionals, patients and the general community. This factor, combined with the large geographical extent of Brazil and the heterogeneous distribution of health professionals, meets the need of specialized care for patients and decentralization of knowledge for professionals. In this context, the use of Tele-education has emerged as a proposal to minimize the difficulties encountered by the patient in the process of adaptation of a hearing aids (HA), in which many educational materials are produced in order to guide and facilitate the adaptation HA to their new reality. Objective: This project aims to develop a training program through the Interactive Tele-education on the individual sound amplification devices, and analyze the effectiveness of the same as learning and motivation. Results: The first stage of the preparation of the training program, which was developed on the theme Cybertutor hearing aids and tools discussion forum and web conferencing in order to supplement learning. In the second stage, regarding the analysis of the program, it was found that there was no statistical significance in the learning aspect of the content, in the pre and post test. Regarding the motivation of staff, the study was positive, which was assessed as \"impressive progress\". Discussion: The training program was developed, backed up by studies aimed at developing distance learning materials. In that refer to results of the evaluation, all difficulties found in this aspect corroborated with the literature. Conclusion: The development of the Training Program through the Interactive Tele-education on the subject of HA was performed satisfactorily, reaching a level of motivation in relation to participation highly positive, but the comparison results of the evaluation of the problem situation pre and post-test were not statistically significant.

Mapeamento visível da fala amplificada na verificação do aparelho de amplificação sonora individual digital / Hearing aid verification using visible speech mapping

Introdução: Na etapa de verificação da adaptação, o aparelho de amplificação sonora individual vem sendo estudado nos últimos anos por diferentes protocolos e equipamentos, a fim de aumentar a satisfação do usuário. Objetivo: Elaborar um protocolo de verificação de aparelho de amplificação sonora individual utilizando o mapeamento visível da fala amplificada na prática clínica e verificar a satisfação do usuário. Método: Em estudo transversal randomizado duplo-cego, foram avaliados 60 indivíduos adultos separados em dois grupos, sem experiência prévia ao uso do aparelho de amplificação sonora individual modelo retroauricular de tecnologia digital. O Grupo A foi submetido ao Protocolo IIIA, contendo os testes de ganho de inserção, reconhecimento de palavras, mapeamento visível da fala amplificada com verificação sem aconselhamento e os questionários: Client Oriented Scale of Improvement e Satisfaction with amplification in daily life. O Grupo B respondeu ao protocolo IIIB composto por: teste de reconhecimento de palavras, Mapeamento visível da fala amplificada com aconselhamento e os mesmo questionários Client Oriented Scale of Improvement e Satisfaction with amplification in daily life. Resultados: Demonstraram moderada correlação entre o ganho de inserção e o mapeamento visível da fala amplificada. Quando este último registrou os estímulos: suave, médio e forte dentro do campo dinâmico auditivo para 10% de aumento na resposta do teste de reconhecimento de palavras, significou 0,4 a 0,5 pontos de aumento na satisfação do usuário para a situação de silêncio e 0,2 a 0,3 para a situação com ruído competitivo. Conclusões: O protocolo para a verificação do aparelho de amplificação sonora individual elaborado demonstrou ser efetivo para a prática clínica e auxiliar no aumento de satisfação do usuário. / Introduction: The hearing aid verification stage has been studied over many years by different protocols and equipments to increase de user satisfaction. Objective: design a hearing aid protocol using Visible Speech Mapping applied to clinical practice in order to analyze the user satisfaction. Method: The cross-sectional, randomized study was done with 60 adults, separated into two groups without prior amplification experience binaurally fitted with digital behind-the-ear hearing aid technology. Four weeks later, the group A answered Protocol IIIA covering insertion gain, Word recognition test, Visible Seech Mapping verification tool without counseling, Client Oriented Scale of Improvement and Satisfation with Amplification in Daily Life questionnaires. The group B was evaluated using the Protocol IIIB containing Word Recognition Test, Visible Speech Mapping verification and counseling tool, Client Oriented Scale of Improvement and Satisfation with Amplification in Daily Life questionnaries. Results: The results showed moderated correlation between insertion gain and Visible Speech Mapping analyzing the procedures when applied to the group A. When Visible Speech Mapping register a soft, medium and strong stimulus within de dynamic range, each 10% improvement at the Word Recognition Test´s result means a 0.4 to 0.5 points, at silence, and 0.2 a 0.3 points increase at speech-in- noise on the satisfaction global score. Conclusions: digital hearing aid verification protocol using Visible Speech Mapping has demonstrated to be effective to the clinical practice.

Gerador de som: análise da eficácia dos ruídos mascaradores no alívio do zumbido / Sound generator: analysis of the effectiveness of masking noise in tinnitus relief

Introdução: o zumbido é uma desordem que acomete 10 a 15% da população mundial e por se tratar de um sintoma subjetivo que apresenta grande repercussão na vida do indivíduo, pesquisas científicas na área são de grande importância. A terapia sonora realizada por meio do aparelho de amplificação sonora individual (AASI) com gerador de som (GS) associado é uma das formas de tratamento do zumbido e vem ganhando espaço nos serviços de saúde auditiva. Proposição: analisar a eficácia de quatro ruídos mascaradores no alívio do zumbido de indivíduos com perda auditiva sensorioneural bilateral de grau leve e moderado e a influência dos mesmos na percepção da fala. Casuística e Métodos: o estudo foi desenvolvido na Clínica de Fonoaudiologia da Faculdade de Odontologia de Bauru (FOB) - USP após a aprovação do Comitê de Ética e anuência do paciente. Participaram 35 indivíduos com zumbido e perda auditiva sensorioneural leve e moderada bilateral, divididos em quatro grupos, sendo três grupos compostos por nove indivíduos e um grupo por oito. Todos os grupos passaram pela adaptação do AASI com GS, sendo regulados no modo combinado (GS + amplificação). No grupo 1 (G1) foi aplicado o estímulo White Noise, no grupo 2 (G2) o Pink Noise, o grupo 3 (G3), o Speech Noise e o grupo 4 (G4) o High Tone Noise. Todos os pacientes passaram pelos seguintes procedimentos: anamnese, diagnóstico audiológico composto por audiometria de alta frequência, imitanciometria e acufenometria. Foram aplicados os questionários Tinnitus Handicap Inventory (THI), a Escala Análoga Visual (EAV) e avaliação da percepção da fala por meio do Hearing in Noise Test (HINT) na situação de fala no silêncio (FS) e ruído frontal (RF). Os procedimentos foram realizados pré e pós-intervenção auditiva, após três meses de uso do AASI. Os resultados foram tabulados e descritos de acordo com a análise estatística descritiva de todas as variáveis estudadas. Resultados: todos os grupos apresentaram diferença estatisticamente significativa para o questionário THI, EAV e HINT na situação de FS, pré e pós-intervenção auditiva. Na situação de RF, somente o G2 apresentou diferença significativa. Porém, na comparação entre grupos não houve diferença significativa. Conclusão: os quatro ruídos mascaradores demonstraram ser igualmente eficazes na melhora do zumbido e na percepção de fala na situação de silêncio, porém no RF, o G2 apresentou melhora significativa. / Introduction: tinnitus is a disorder that affects 10 to 15% of the world\'s population and since it is a subjective symptom which presents great impact on the individual\'s life, all scientific contribution becomes significant and important. The sound therapy performed through the individual sound amplification apparatus (AASI) with associated sound generator (GS) is one of the forms of tinnitus treatment and has been obtaining space in the hearing health services. Proposition: analyzing the effectiveness of four masking noises in relieving tinnitus of individuals with mild and moderate bilateral sensorineural hearing loss and influence thereof on speech perception. Casuistry and Methods: the study was developed at the Speech Therapy Clinic of the Faculty of Dentistry of Bauru (FOB) - USP after approval by the Ethics Committee and patient consent. Participants were 35 individuals with tinnitus and bilateral mild and moderate sensorineural hearing loss, divided into four groups, three groups composed of nine subjects and one group comprising eight subjects. All groups underwent hearing loss adaptation with GS, being regulated in the combined mode (GS + Amplification). In Group 1 (G1) the White Noise stimulus was applied, in group 2 (G2) Pink Noise was applied, in group 3 (G3), Speech Noise and in group 4 (G4) the High Tone Noise was applied. All patients were subjected to the following procedures: anamnesis, audiological diagnosis composed of high frequency audiometry, immittanciometry and acuphenometry. The Tinnitus Handicap Inventory (THI), Visual Analogue Scale (EAV) and Speech Perception Assessment questionnaires were applied through the Hearing in Noise Test (HINT) in the speech silence (FS) and frontal noise (RF) situation. The procedures were performed prior to and after hearing intervention, after three months of use of AASI. The results were tabulated and described according to the descriptive statistical analysis of all variables studied. Results: all groups presented a statistically significant difference for the THI, EAV and HINT questionnaires in the FS, pre- and post-hearing intervention. In the RF situation, only G2 presented a significant difference. However, in the comparison among groups there was no significant difference. Conclusion: the four masking noises proved to be equally effective in improving tinnitus and in speech perception in the silent situation, but in RF situation, G2 presented significant improvement.
