Effets périphériques et centraux de l'exercice excentrique aigu sur bicyclette ergométrique chez le sujet sain et le patient atteint de BPCO / Peripheral and central effects of acute and chronic eccentric exercise on cycle ergometer in healthy subjects and patients

De nouvelles modalités de réentrainement sont nécessaires pour les patients présentant une sévère limitation à l’exercice tels que les patients souffrant de broncho-pneumopathie chronique obstructive (BPCO). Le pédalage « excentrique », caractérisé par une meilleure efficience, pourrait permettre à ces patients de se réentrainer plus efficacement. Toutefois, les modalités de prescription et les effets du pédalage excentrique méritent d’être étudiés.Chez le sujet sain, nos résultats ont montré un retard de réactivation parasympathique au décours du pédalage excentrique par rapport au concentrique réalisés à même puissance mécanique, avec une influence de la fréquence de pédalage sur les réponses cardio-respiratoires et autonomes. À même puissance métabolique, le pédalage excentrique se caractérise par un stress cardio-vasculaire et respiratoire plus important que lors du pédalage concentrique, et des réponses neuro-végétatives en faveur d’une activité sympathique plus élevée. La fonction vasculaire est altérée à l’issue du pédalage excentrique. Chez le patient atteint de BPCO, comme chez le sujet sain, le pédalage excentrique a un effet tachypnéisant.Nous pensons que la tension musculaire accrue nécessaire en pédalage excentrique à l’atteinte d’une puissance métabolique équivalente à l’exercice concentrique serait à l’origine d’une plus grande activité sympathique et d’une contrainte hémodynamique périphérique, expliquant l’augmentation de la contrainte cardio-vasculaire en pédalage excentrique. / New exercise modalities are required for patients severely limited during exercise such as chronic obstructive pulmonary disease patients (COPD). Eccentric “cycling”, characterized by better efficiency, could allow COPD patients to train more effectively. Nonetheless, prescription modalities and effects of eccentric cycling deserve to be more studied.In healthy subjects, our results showed a delayed parasympathetic reactivation after eccentric compared to concentric cycling matched for mechanical power, together with an influence of pedalling frequency on autonomic and cardiorespiratory responses. At similar metabolic power, eccentric cycling is characterized by a greater cardiovascular and respiratory stress than during concentric cycling, and altered autonomic nervous systems responses in favour to greater sympathetic activity. Vascular function is altered after eccentric cycling. In COPD patients, as in healthy subjects, eccentric cycling impose tachypneic breathing pattern.We believe that the higher muscle tension during eccentric cycling necessary to reach metabolic power similar to concentric leads to greater sympathetic activity and peripheral hemodynamic constraint, likely explaining the increased constraints on cardiovascular system during eccentric cycling.

Promuovere l'aderenza alla Ventilazione Non Invasiva (NIV) nella BroncoPneumopatia Cronica Ostruttiva (BPCO) con un breve intervento psicologico / PROMOTING ADHERENCE TO NON INVASIVE VENTILATION (NIV) IN CHRONIC OBSTRUCTIVE PULMONARY DISEASE (COPD) WITH A BRIEF PSYCHOLOGICAL INTERVENTION

Persone con broncopneumopatia cronica ostruttiva (BPCO) possono avere diversi atteggiamenti verso la ventilazione non invasiva (NIV), inclusi il rifiuto o l’utilizzo inappropriato. Questo costituisce uno spreco di risorse pubbliche, comportando costi significativi per l'assistenza sanitaria. Lo studio mirava ad analizzare l'impatto di un breve intervento di supporto psicologico sull'aderenza alla NIV in pazienti con BPCO. E’stato condotto uno studio clinico controllato e randomizzato a due rami su 84 pazienti con BPCO e con indicazione per NIV. Il gruppo sperimentale ha ricevuto un breve supporto psicologico, che include colloqui clinici, esercizi di rilassamento e di mindfulness. I controlli hanno ricevuto cure standard e hanno guardato video educativi relativi alla gestione della malattia. Il percorso era strutturato in 4-8 sessioni in ospedale, a casa e/o in telemedicina. Sono stati valutati valori, credenze ed esperienze di NIV attraverso interviste semi-strutturate e colloqui clinici. L'intervento psicologico è risultato correlato a miglioramenti della qualità della vita e dell'aderenza. Ha permesso di delineare un modello teorico del processo di adattamento alla NIV in pazienti con BPCO. I risultati suggeriscono che questo intervento psicologico può incrementare l'accettazione e l'aderenza alla NIV nella BPCO nella pratica clinica e sottolinea l'importanza di determinare le ragioni sottostanti l’uso della NIV. / People with chronic obstructive pulmonary disease (COPD) may have different attitudes toward non-invasive ventilation (NIV), including rejection and misuse. That is a waste of public resources and results in significant health care costs. The study aimed to analyze the impact of a brief psychological intervention on adherence to NIV among COPD patients. A two-branch randomized controlled trial was conducted with 84 COPD patients using NIV. The experimental group received psychological support, including counseling, relaxation and mindfulness-based exercises. Controls received standard care and were exposed to educational videos. The intervention was structured over 4 to 8 meetings at the hospital, at home and/or via telemedicine. Values, beliefs and experiences of NIV were assessed with semi-structured interviews. The psychological intervention was related to improvements in both adherence and quality of life. A theoretical model of the adaptation’s process to NIV in COPD patients emerged. The findings suggest that this psychological intervention could increase acceptance and adherence to NIV in COPD in clinical practice.

Contribution à l'étude par TDM du retentissement sur le coeur gauche de la pathologie pulmonaire. / Contribution in computed tomography of impact on the left heart of pulmonary diseases.

La pathologie pulmonaire et la pathologie cardiaque interagissent et le bloc “Cœur- poumon” est à considérer comme une seule et même entité en imagerie: la lecture du cœur, des vaisseaux, des poumons et du médiastin doit être globale et intégrée, quelle que soit la modalité d’imagerie. Nous avons pu ainsi illustrer la contribution de la TDM dans l’évaluation de la prévalence de la pathologie coronarienne asymptomatique au cours de la fibrose pulmonaire idiopathique, l’analyse du volume de l’oreillette gauche au cours des BPCO, la corrélation entre volume pulmonaire hypoperfusé et le rapport VD/VG, et l’évaluation de l’envahissement cardiaque au cours des néoplasies pulmonaires. Les avancées techniques en scannographie nous permettent désormais d’approcher outre la morphologie le versant fonctionnel, notamment par l’évaluation de la fonction cardiaque, de la perfusion pulmonaire, en attendant le développement en routine clinique de l’approche par TDM de la fonction de ventilation. Notre thèse avait pour ambition de contribuer modestement à la promotion d’une approche morphologique et fonctionnelle intégrée de l’imagerie du cœur, des vaisseaux thoraciques et du poumon, dont les bénéfices cliniques nous apparaissent quotidiens et dont l’enseignement nous apparait indissociable. / Pulmonary and cardiac pathology interact and the « heart-lung » bloc is to consider as one and same entity in imaging: heart, vessels, lungs and mediastinum analysis must be global and integrated, whatever imaging modality.In this work, we have illustrated computed tomography contribution in: evaluating asymptomatic coronary disease in cystic fibrosis pathology, left atrial volume in COPD patients, correlation of hypo perfused lung volume and RV/LV ratio, and cardiac invasion evaluation in broncho-pulmonary neoplasm.Computed tomography technological advances allow us to evaluate morphology and functional side, in evaluating cardiac function, pulmonary perfusion, waiting for the development of pulmonary ventilation in routine. Our work contributes to the development of a morphological and functional approach in heart, great vessels and lung imaging.
