Este trabalho propõe padroniza-lo e validar o programa STEPanizer © stereology tool version 1.0 como método de classificação complementar para o teste. Foram utilizados 31 cães (62 olhos), sem predominância de sexo ou raça, com idade média de 18,8 ± 18.4 meses. Dentre estes, 14 apresentavam a superfície ocular hígida e 17 apresentavam protrusão unilateral da glândula da terceira pálpebra. Foram realizados em todos os cães, testes de rotina para avaliar a higidez da superfície ocular. O TCL foi classificado de acordo com Rolando (1984) e pelo programa STEPanizer. Neste, testou-se a avaliação com 16, 25, 36, 49, 64, 81 e 100 pontos equidistantes de contagem, no sistema teste que fica sobreposto à imagem, correlacionando-os com a classificação segundo Rolando (1984). Os dados foram analisados com o software Statistical Analysis System (SAS®). Valores de maior correlação (r), p < 0.05, menor erro mean square (MSE) e maior coeficiente de determinação (R2 ajustado) foram utilizados para definir o melhor número de pontos no STEPanizer. O TCL classificado segundo Rolando (1984) resultou, em média, tipo II para os animais hígidos e tipo III para os animais com protrusão da glândula da terceira pálpebra. A classificação do TCL utilizando o STEPanizer mostrou melhor correlação com a classificação de Rolando quando feita por meio da contagem com 100 pontos equidistantes (r - 0.9; MSE 0.38 e R2 ajustado 0.81). A classificação para o TCL utilizando o STEPanizer mostrou-se objetiva e factível de ser empregada na rotina clínica, sendo útil como ferramenta de padronização para a classificação do TCL, feita segundo Rolando (1984). ______________________________________________________________________________ RESUMO DO ARTIGO 2 / Objetivo Avaliar a superfície ocular de cães com protrusão da glândula da terceira pálpebra, antes e após a cirurgia de reposicionamento da glândula, comparativamente a animais hígidos. Animais vinte e sete cães, sendo 14 sem alterações de superfície ocular (grupo controle – GC) e 13 acometidos unilateralmente pela protrusão da glândula da terceira pálpebra (grupo afetado – GA), machos ou fêmeas, com idade media de 20,8 ± 19 meses. Procedimentos Foram realizados, em todos os cães, o teste lacrimal de Schirmer (TLS); o tempo de ruptura do filme lacrimal (TRFL); a coloração com fluoresceína (FLUO), a análise histopatológica do fórnix conjuntival (BIO) e o teste de cristalização da lagrima (TCL), classificado como o proposto por Rolando (1984) e por meio do programa STEPanizer © Stereology tool version 1.0. Sendo que no GA, os testes também foram realizados nos dias 7, 15, 30 e 60 após a reposição da glândula protruída. Os dados foram analisados com o software Statistical Analysis System (SAS ®). Valores de maior correlação (r), p < 0.05, menor erro mean square (MSE) e maior coeficiente de determinação (R2 ajustado) foram considerados significativos. Resultados Não se verificou diferença significativa para os testes TLS, TRFL e FLUO nem antes, nem após o procedimento de reposição da glândula. O TCL classificado de acordo com o STEPanizer mostrou piora significativa após o procedimento cirúrgico (p < 0,001), piora não significativa quando classificado de acordo com Rolando (1984). A classificação do TCL 20 pelo STEPanizer é altamente correlacionada com a de Rolando, tanto nos olhos dos animais hígidos (r – 0,94), como nos olhos afetados pela protrusão (r – 0,91). A concentração de células caliciformes por campo de grande aumento (400x) para os olhos do GC (22,28 ± 5,78) foi significativamente maior do que no GA (18,45 ± 10,6). Conclusão O diagnóstico e a terapêutica cirúrgica precoce provavelmente evitaram os sinais característicos da cronicidade desta afecção sobre a superfície ocular dos animais deste estudo. A classificação do TCL utilizando o STEPanizer, mostrou-se mais sensível do que a de Rolando (1984). As alterações visualizadas no olho sem a protrusão foram as mesmas visualizadas no olho afetado pela protrusão, o que gera a hipótese de que a etiopatogenia da doença pode ter início com alterações de superfície ocular ainda não descritas em literatura. ______________________________________________________________________________ ABSTRACT OF THE ARTICLE 1 / The tear ferning test (FT) has no standard in dogs. This paper proposes this standardization it and validation using the STEPanizer © Stereology tool version 1.0 program (STEPanizer) as a complementary classification method for the test. Thirty one dogs (62 eyes) of no predominant sex or bread, with an average age of 18.8 ± 18.4 months were used. Among these, 14 had a healthy ocular surface and 17 had a unilateral third eyelid gland protrusion. Routine tests for evaluating the health of the ocular surface were performed on all dogs. The FT was classified according to Rolando (1984) and by the STEPanizer program. The STEPanizer uses a test system with equidistant points overlaid on the analyzed image. A total of 16, 25, 36, 49, 64, 81 and 100 equidistant points were evaluated, correlating them with the Rolando (1984) classification. Data were analyzed with the Statistical Analysis System (SAS®) software. Higher correlation (r) coefficients, p < 0.05, lower mean square error (MSE) and higher adjusted R2 were used to define the best number of points. Healthy animals showed, on average, FT classification according Rolando (1984) of Type II in healthy animals and Type III in animals with third eyelid gland protrusion. The FT regression on STEPanizer using 100 equidistant points (r - 0.9; MSE 0.38 and adjusted R2 0.81) showed the highest correlation with Rolando (1984) classification. The FT classification using STEPanizer proved to be objective and feasible for use in clinical routine, being useful as a standardized tool for the FT Rolando (1984) classification. INDEX TERMS: eye, ocular surface, validation, Rolando (1984), test system, third eyelid gland protrusion. ______________________________________________________________________________ ABSTRACT OF THE ARTICLE 2 / Objective Evaluate third eyelid gland protrusion dogs ocular surface both before and after surgical repositioning of the gland, compared to healthy animals. Animals Twenty-seven dogs, 14 with healthy ocular surface (control group - CG) and 13 unilaterally affected by third eyelid gland protrusion (affected group - AG), male or female, with a mean age of 20.8 ± 19 months. Procedures In all dogs were performed the Schirmer tear test (STT); the breakup time of the tear film (BUT); the fluorescein staining (FLUO), the histopathological analysis of the conjunctival fornix (BIO) and the tear Ferning test (FT), classified as proposed by Rolando (1984) and with STEPanizer © Stereology tool version 1.0 program. On AG, the tests were also performed on 7, 15, 30 and 60 days after the gland replacement. Data were analyzed with the Statistical Analysis System software. Higher correlation values, p < 0.05, lower mean square error and higher determination coefficient were significant. Results No statistical difference was found for STT, BUT and FLUO tests. FT classified using STEPanizer showed significant worse after surgery (p <0.001), but no significant when classified according to Rolando (1984). On AG, changes in the eyes without protrusion were the same displayed in the affected eyes. Conclusion Early diagnosis and surgical therapy of affected dogs prevented chronic changes on their ocular surface. The FT rating using the STEPanizer was more sensitive than Rolando (1984) classification. It’s assumed that the disease pathogenesis may begin with changes in ocular surface not previously described in the literature.

Efeitos clínicos e histopatológicos da aplicação autóloga da fração de células mononucleares da medula óssea sobre a glândula da terceira pálpebra de coelhos e de cães

O estudo demonstrou predisposição da raça cocker spaniel (P<0,0001), sinais clínicos mais evidentes em animais com valores do Teste da Lágrima de Schirmer realizado sem colírio anestésico (TLS-1) < 5mm/minuto (P<0,0001) e maior incidência da CCS em cães com idade superior a seis anos. Exceção foi observada em cães da raça Yorkshire terrier, acometidos mais precocemente. Os sinais clínicos observados com maior frequência foram hiperemia conjuntival, secreção mucosa, neovascularização e pigmentação de corneanas. A ciclosporina A foi o principal medicamento prescrito. A fim de investigar o potencial da terapia com células-tronco, foi criado um protocolo aplicado ao tratamento da CCS, de colheita de medula óssea (MO), separação de fração de células mononucleares (FCM) e administração autóloga por via subconjuntival palpebral sobre a glândula da terceira pálpebra de coelhos e cães. Foram utilizados dez coelhos e 20 cães, que, após passarem por exames clínicos e laboratoriais e ambientação, foram divididos em grupo controle e grupo tratado. Para obtenção da FCM, foi realizada colheita, centrifugação e isolamento da medula óssea. O material, ressuspenso em solução salina, foi administrado no grupo tratado, enquanto o grupo controle recebeu apenas solução salina. Os animais passaram por avaliações clínicas e TLS-1 até 60 dias da aplicação, e avaliação histológica da glândula da terceira pálpebra, que demonstraram não haver alterações que contraindicassem o tratamento. Tanto a administração de solução salina quanto a de solução salina + FCM aumentaram significativamente os valores do TLS, mas o aumento foi mais pronunciado nos cães que receberam células mononucleares. Aos 60 dias da aplicação da FCM, os cães tratados apresentaram aumento de 3,4mm;min ao TLS-1. _______________________________________________________________________________ ABSTRACT / A retrospective study of 157 dogs with keratoconjunctivitis sicca (KCS) examined at the Veterinary Hospital at the University of Brasilia was undertaken in order to establish the patterns of disease in Distrito Federal. The study demonstrated predisposition of cocker spaniels (P <0.0001), more clinical signs in patients with Schirmer Tear Test (STT) values less than 5 mm/minutes (P <0.0001) and higher incidence of KCS on dogs over six years (except on Yorkshire terriers, which were stricken younger). Clinical signs more frequently observed were conjunctival hyperaemia, mucus discharge, neovascularization and pigmentation. Cyclosporin A was the main drug prescribed, regardless of the amount of clinical signs and values of the STT presented by each animal. In order to investigate the potential of stem cell therapy, an experimental model of harvest of bone marrow (BM), separation of mononuclear cell fraction (MCF) and administration by subconjunctival autologous eyelid on the third eyelid gland in rabbits and dogs was created and applied to the treatment of KCS. Ten rabbits and 20 dogs were were divided into control and treated group, after undergoing clinical and laboratorial exams and quarantine. To obtain MCF, BM was harvested, centrifuged and isolated. Treated group was submitted to the autotransplant of MCF + saline , while the control group received only saline. The animals underwent clinical and histological evaluation and STT, which showed no contraindicative changes. Both the administration of saline as that of saline + FCM significantly increased the values of the TLS, but the increase was more pronounced in dogs that were inoculated mononuclear cells.

Embora se verifiquem resultados satisfatórios no emprego de imunomoduladores, alguns pacientes não respondem positivamente ao uso desses fármacos, e a terapia celular pode se tornar uma alternativa ao tratamento convencional. Objetivou-se avaliar os efeitos da aplicação de fração autógena de células mononucleares (FCM) da medula óssea na região periglandular da terceira pálpebra, sobre a superfície ocular de cães com CCS. Foram selecionados 15 pacientes (25 olhos acometidos) provenientes do Serviço de Oftalmologia Veterinária da FAV/UnB, sendo nove machos e seis fêmeas, de dez raças distintas. Para inclusão neste estudo, os indivíduos apresentaram valores inferiores à 10mm/min ao teste lacrimal de Schirmer (TLS). Eles foram submetidos ao TLS, à tonometria de aplanação, à biomicroscopia com lâmpada em fenda, ao tempo de ruptura do filme lacrimal (TRFL), ao teste da lissamina verde e ao teste da fluoresceína, semanalmente, durante 16 semanas consecutivas. Realizou-se, neste mesmo período, avaliação quanto à hiperemia e à secreção ocular, à quemose e à transparência corneal. A medula óssea foi coletada mediante punção pelo esterno, a FCM foi isolada por centrifugação e o pellet contendo a FCM foi aplicado na região periglandular da terceira pálpebra no dia inicial do estudo. Realizou-se biopsia da conjuntiva bulbar temporal superior para avaliação do grau de inflamação e quantificação das células caliciformes no dia da aplicação da FCM e 16 semanas após a mesma. Os resultados obtidos foram avaliados empregando-se teste t-Student e descritos em um gráfico de pontos acompanhado de regressão linear. Verificou-se aumento dos valores do TLS até a sexta semana após o tratamento, seguindo-se diminuição do TLS, mas nos dois momentos não ocorreu diferença estatisticamente significativa dos valores iniciais. Quadro semelhante foi observado com a avaliação dos parâmetros clínicos. Entretanto, o TRFL aumentou ao término das avaliações, bem como a impregnação por lissamina verde foi menor do que a verificada previamente ao tratamento. A inflamação conjuntival foi classificada como inferior, estável e superior aos valores iniciais em 30%, 40% e 30% dos olhos, respectivamente. A média no número de células caliciformes aumentou de 78,3 células/campo para 161 células/campo, verificando-se melhora em 50% dos olhos avaliados, estabilidade em 20% e piora em 30%. Diante das condições experimentais descritas, conclui-se que o uso da fração de células mononucleares da medula óssea aumentou a quantidade de células caliciformes conjuntivais, além de melhorar parâmetros de avaliação da superfície ocular. Desta forma tornando-se uma opção promissora para o tratamento da CCS canina, conferindo melhora à superfície ocular dos pacientes acometidos pela ceratoconjuntivite seca. O efeito de plasticidade celular e a ação anti-inflamatória da FCM sobre a glândula da terceira pálpebra merecem maiores investigações. ______________________________________________________________________________ ABSTRACT / The keratoconjunctivitis sicca in dogs is a common condition in clinical practice. While there are satisfactory results in the use of immunomodulators, some patients do not respond positively to these drugs, and cell therapy may become an alternative to conventional treatment. This study aimed to evaluate the effects of the application of autologous mononuclear cell fraction (MCF) bone marrow periglandular in the region of the third eyelid, on the ocular surface of dogs with CCS. We selected 15 patients (25 affected eyes) from the Department of Veterinary Ophthalmology FAV/UNB, nine males and six females, ten distinct breed. For inclusion in this study, subjects had values below the 10mm/min the Schirmer tear test (STT). They were submitted to TLS, the applanation tonometry, slit lamp examination with the slit lamp, the breakup time of the tear film (BUT), the test of lissamine green and fluorescein test weekly for 16 consecutive weeks. Took place in the same period, evaluation as to hyperemia and ocular discharge, the chemosis and corneal transparency. Bone marrow was collected by puncture of the sternum, MCF was isolated by centrifugation and the pellet containing the MCF was applied in the region of the third eyelid periglandular the initial day of the study. Held biopsy superior temporal bulbar conjunctiva for assessing the degree of inflammation and quantification of goblet cells on the day of application of MCF and 16 weeks thereafter. The results were analyzed employing Student's t test and described in a scatterplot accompanied by linear regression. An increase in the values of the TLS until six weeks after treatment, followed by reduction of the TLS, but the two moments not statistically significant difference from baseline. A similar picture was observed with assessment of clinical parameters. However, BUT increased at the end of the evaluations, as well as lissamine green impregnation was lower than that determined prior to treatment. The conjunctival inflammation was ranked lower, stable and superior to the initial values by 30%, 40% and 30% of eyes, respectively. The mean number of goblet cells increased from 78.3 cells / field to 161 cells / field, verifying an improvement in 50% of eyes evaluated, stability in 20% and worsened in 30%. Given the experimental conditions described, it is concluded that the use of the fraction of bone marrow mononuclear cells increased the amount of conjunctival goblet cells, and improve evaluation parameters of the ocular surface. Thus making it a promising option for the treatment of canine CCS, giving improves the ocular surface of patients suffering from keratoconjunctivitis sicca. The effect of cellular plasticity and anti-inflammatory action of MCF on the gland of the third eyelid deserve further investigation.
