Requisito para optar al Título de
Cirujano Dentista / Introducción: Determinar el riesgo cariogénico del paciente es un requisito fundamental para efectuar un adecuado diagnóstico de salud bucal. Establecer el recuento de Streptococcus mutans (S.mutans) sobre piezas dentarias puede permitir identificar el nivel de riesgo microbiológico en desarrollar caries, y en el caso de piezas restauradas, caries secundarias, principal causa de fallas de restauraciones, evitando el futuro recambio de ellas. Conocer la colonización de microbiota cariogénica en restauraciones dentarias y dientes sanos, podría ser un aspecto a considerar en las decisiones de tratamiento, posibilitando la selección de un material de obturación y medidas preventivas, ajustada con el riesgo cariogénico local y propio de cada paciente.
Material y Método: Se seleccionaron 69 pacientes de la clínica de Operatoria 4to año de la Facultad de Odontología de la Universidad de Chile durante el período de septiembre a diciembre del año 2011. En cada uno de ellos se tomó una muestra de placa bacteriana dental de una pieza posterior sana y una restaurada por oclusal con resina compuesta utilizando la técnica de cubeta. Este método consiste en una impresión directa sobre las superficies oclusales de restauraciones, mediante una cubetilla de flúor gel modificada cargada con agar TYCSB. Las cubetas se incubaron en estufa a 37°C por 48 horas, para posteriormente proceder al recuento bacteriano.

Resultados: Mediante el método de la cubeta se logró aislar Unidades Formadoras de Colonias (UFC) de S. mutans en dientes con resina compuesta oclusal y en piezas sanas en un 95,6% de las muestras. Se observó una diferencia estadísticamente significativa (p <0,05) donde las muestras de biofilm de placa bacteriana depositada sobre las restauraciones de resina presentaban mayor cantidad de UFC/cm2 que las superficie de piezas sanas.
Conclusiones: A partir de muestras de placa bacteriana dental obtenidas mediante la técnica de cubeta existen diferencias significativas en el recuento de S. mutans entre dientes con resina compuesta oclusal y dientes permanentes sanos, siendo mayor en las que presentaban resina compuesta.

estudios que evalúen las decisiones que toman los odontólogos frente al manejo
de la caries dental; por lo que, el presente trabajo tiene como propósito, establecer
diferencias en la toma de decisiones de tratamiento en cariología, de acuerdo a las
variables: edad, sexo, años de experiencia clínica, lugar de ejercicio profesional,
atención exclusiva de niños, universidad de egreso y tener especialidad; atribuidas
al odontólogo-docente de la Región Metropolitana (RM).
Material y métodos: Se realizó un estudio observacional, analítico, de corte
transversal. Previa autorización para participar del estudio, se aplicó el
“Cuestionario sobre decisiones de tratamiento de caries oclusales y proximales”,
validado en Chile por Ruiz y cols., 2012; a los odontólogos-docentes de
asignaturas relacionadas con el manejo de la caries (cariología, operatoria
restaurativa, prótesis removible, odontopediatría, odontología integral)
pertenecientes a todas las universidades de la RM que imparten la carrera de
Odontología, al año 2011. El instrumento evalúa conocimientos y creencias
respecto de la caries, estrategias de tratamiento y elección de materiales dentales;
y cuenta con cuatro dominios que al ser ponderados califican al individuo como
“conservador” o “invasivo” al momento de tomar decisiones en cariología.
Resultados: La población de estudio fue de 234 docentes quienes consintieron
participar del estudio, correspondiente al 77,2% del universo. El 57,7% de ellos
resultó ser clasificado como “conservador”, mientras que el resto fue clasificado
como “invasivo”. Ninguna de las variables exploradas logró explicar los resultados
(p>0,05).
Conclusión: No existe suficiente evidencia para afirmar que hay diferencia
estadísticamente significativa entre la toma de decisiones de tratamiento en
cariología y las variables: edad, sexo, años de experiencia clínica, lugar de
ejercicio profesional, atención exclusiva de niños, universidad de egreso y tener
especialidad; atribuidas al odontólogo-docente de la RM. Se requieren nuevos
estudios para determinar las variables que explican la distribución de odontólogos
conservadores entre los docentes universitarios.

Efecto del tratamiento rehabilitador integral de caries temprana de la infancia en los niveles de Streptococcus mutans salivales de niños atendidos en la Clínica de Odontopediatría de la Escuela de Graduados de la Universidad de ChileCiampi Díaz, Nicole January 2013 (has links)
Trabajo de Investigación Requisito para optar al Título de Cirujano Dentista / Introducción: La caries temprana de la infancia (CTI) es una enfermedad dietobacteriana,
siendo Streptococcus mutans (S.mutans) el principal patógeno asociado.
Afecta a niños menores de 6 años, generando destrucción y dolor, siendo necesario un
tratamiento que controle los factores de riesgo relacionados y recupere el tejido dañado.
En Chile, no existen estudios que evalúen el impacto de un tratamiento rehabilitador
integral en la microbiota oral de niños con CTI.
Objetivo: Determinar el efecto del tratamiento rehabilitador integral de CTI en los niveles
de Streptococcus mutans en niños tratados en la Clínica de Odontopediatría de la Escuela
de Graduados de la Universidad de Chile
Materiales y Métodos: Estudio retrospectivo observacional. Se analizaron 89 fichas
clínicas de pacientes diagnosticados con CTI rehabilitados integralmente en el posgrado
de Odontopediatría de la Universidad de Chile, las cuales registraban el recuento de
S.mutans salival antes de iniciar el tratamiento, después del tratamiento preventivo y
después del tratamiento rehabilitador. Se analizaron las diferencias entre los niveles de
S.mutans de manera cuantitativa y según categorías (riesgo bajo, moderado o alto),
durante las distintas etapas del tratamiento. Se analizaron también otras variables como
tipo de material restaurador, edad e índice ceod.
Resultados: Durante el análisis cuantitativo, se encontraron diferencias estadísticamente
significativas al comparar las concentraciones de S.mutans salival inicial con las
concentraciones luego del tratamiento preventivo (4,7±1,03 log UFC/ml vs 4,2 ± 0,96 log
UFC/ml p<0,05) y luego del tratamiento rehabilitador (4,7±1,03 log UFC/ml vs 3,74 ± 0,88
log UFC/ml p<0,05), así como al comparar las concentraciones después del tratamiento
preventivo con las posteriores al tratamiento rehabilitador (4,2 ± 0,96 log UFC/ml vs 3,74 ±
0,88 log UFC/ml p<0,05). Durante el análisis por categorías, sólo se encontraron
diferencias significativas entre las concentraciones previas y después de ambos
tratamientos, sin encontrar diferencias entre ambos tratamientos. No hubo diferencias al
analizar las variaciones según categorías con el tipo de material restaurador, edad ni
índice ceod.
Conclusión: El tratamiento rehabilitador integral produce una reducción significativa en la
concentración de S.mutans salival tanto en la etapa preventiva como en la etapa
rehabilitadora.

Producción de álcali por actividad de ureasa y arginina deiminasa en saliva y biofilm dental en niños de 8 años con distinta historia de caries dentalMaureira Chicahual, Jéssica January 2013 (has links)

La generación de álcali en saliva y biofilm dental (biopelícula) es producto de la
actividad de dos vías enzimáticas productoras de amonio: a) hidrólisis de urea por
la enzima ureasa y b) hidrólisis de arginina a través de enzimas del sistema
arginina deiminasa (SAD), ambas provenientes de bacterias presentes en la
cavidad oral. El potencial alcalinogénico de estos mecanismos enzimáticos podría
estar asociado positivamente con un menor índice de caries. Por lo tanto, es
primordial, a través de métodos establecidos, cuantificar la actividad de ambos
mecanismos enzimáticos y correlacionarlos con índice de caries en una población
infantil.
Materiales y Métodos
Fueron examinados, mediantes métodos visual y táctil, por examinadores
calibrados (índice de Kappa= 0,77) según criterios OMS, 65 niños de 8 años de
edad de tres colegios seleccionados. Se recolectaron muestras de saliva y biofilm
dental de todos los niños. Se determinó la suma del índice COPD y ceod de cada
niño y se agruparon en; Bajo, Moderado, Alto. En muestras de saliva y biofilm
dental se determinó concentración de amonio, proteínas y actividad específica de
ureasa y SAD.
Resultados
Con curvas de calibración de amonio y proteínas con coeficiente de correlación
cercano a 1, se determinó la actividad enzimática medida a través de la
cuantificación de amonio. Al medir actividad de ureasa en saliva se encontraron
diferencias estadísticamente significativas entre los grupos (p<0,05). Al medir
actividad de ureasa en biopelícula y SAD en saliva y biopelícula no se encontraron
diferencias estadísticamente significativas entre los grupos (p>0,05). Al comparar
índice de caries entre colegios de zona urbana y rural, existe tendencia al aumento
en ambos índices en niños de colegio rural, pero estos no son estadísticamente

significativos (p>0,05).
Conclusión
El aumento de actividad de ureasa en saliva se asocia con la disminución de
índices de COPD-ceod en niños de 8 años provenientes de la R.M.
La actividad de ureasa en biopelícula no se asocia con la disminución de índices
de COPD y ceod en niños de 8 años.
Existe unan clara tendencia al aumento de la producción de álcali de ambos
sistemas enzimáticos en saliva, en niños con menores índices de caries, que
pueden influir en el equilibrio ácido-base del medio ambiente oral.
Existe tendencia a mayores índices de caries en niños de 8 años que habitan en
zona rural de la Región Metropolitana.
Son necesarios nuevos estudios que controlen factores externos que puedan
influir considerablemente en los resultados

Buffering capacity of saliva, salivary flow rates and cortisol levels in patients with active cariesHira, Priyesh Gunvant 18 March 2013 (has links)
Introduction: Dental caries is caused by the interaction of the host, oral flora and diet. Stress is
one of the host factors implicated. Studies have shown that there is an association between
stress and salivary cortisol levels. However, no studies have investigated the relationship
between stress, salivary cortisol levels and caries susceptibility. Aims and objectives: The aim
of the study was to determine whether there is a correlation between active dental caries, resting
and stimulated flow of saliva, salivary buffering capacity, saliva cortisol levels and stress in
patients attending a general dental practice in Lenasia South. Methods and materials: Sixty
subjects between the ages of 18 and 60 were included in the study. Thirty controls with no active
caries, a minimum of 28 teeth and a mean decayed, missing filled surfaces (DMFS) score of 4 or
less, and 30 subjects with active caries were included in the study and formed the experimental
group. Patients with Sjögren’s Syndrome or connective tissue diseases, on medication that may
cause xerostomia, or a history of previous or current irradiation were excluded from the study. At
the initial visit resting and stimulated saliva samples were collected and the volume was
measured. The buffering capacity and cortisol levels of the resting saliva samples were
measured. In addition the Depression Anxiety Stress Scales (DASS) questionnaire was used to
determine the stress levels of the participants. The teeth of the subjects with active caries, i.e.
the experimental group, were restored. They returned after 4 weeks for a follow up visit and their
resting and stimulated salivary flow, buffering capacity of saliva, salivary cortisol and the stress
levels were measured. The results were compared using the two sample t test, chi – squared test
and a generalized logistic regression analysis. Results: The DMFS of the control group,
0.40 + 0.97, was significantly lower (p < 0.001) than 29.27 + 21.94, in the experimental group. No
significant differences were found between the controls and caries prone subjects when the
resting flow rates, 0.37 + 0.30 ml/min and 0.32 + 0.19 ml/min; stimulated flow rates,
0.99 + 0.56 ml/min and 0.84 + 0.35 ml/min; buffering capacity of saliva,
19.16 + 4.68 ml 0.01N lactic acid and 21.73 + 9.77 ml 0.01N lactic acid, were compared and the
salivary cortisol levels of the controls 17.71 + 22.51 ng/ml, were higher than 11.80 + 14.61 ng/ml
in the the caries prone subjects. The DASS scores of the two groups were similar,
i.e. 11.33 + 8.48 and 11.2 + 9.6, respectively. After the carious teeth of the caries prone subjects
were restored, the flow rate of resting saliva increased from 0.32 + 0.19 ml/min to
0.37 + 0.16 ml/min, the stimulated saliva from 0.84 + 0.35 ml/min to 0.88 + 0.32 ml/min and the
buffering capacity of saliva from 21.73 + 9.77 ml 0.01N lactic acid to 22.25 + 7.55 ml 0.01N lactic
acid and the salivary cortisol levels decrease from 11.80 + 14.61 ng/ml to 10.00 + 12.12 ng/ml.
Again none of these differences were significant. Conclusion: These results suggest that stress
levels measured by the DASS questionnaire may not be related to caries. A less subjective
questionnaire may find a relationship between salivary cortisol levels, stress and dental caries.

Although the prevalence of dental caries experience among the children in the world has decreased significantly in the last fifty years, dental caries is one of the most predominant chronic childhood diseases worldwide, and it is a public health problem, both for individual children and families who choose to deal with young children suffering from a toothache.
Aims and objectives
The purpose of this study was to determine caries experience and associated factors contributing to the condition among preschool children in Johannesburg. The objectives were to determine the dmft status, the pufa index of the children and to compare the prevalence of caries between age-groups. Furthermore, the association between demographic characteristics and the caries experience was assessed.
Methodology
A cross-sectional study was carried out at six sites in Johannesburg on children aged 2-5 years. Data was collected by means of a parental questionnaire covering demographic information and oral dietary habits. This was followed by an oral examination using two indices: Decayed Missing Filled Teeth (dmft) and Pulp Exposure Ulceration Fistula Abscess (pufa).
Results
Two hundred and twenty-two preschool children aged 2- 5 years participated in this study (n= 222): girls (n= 125) (56%), slightly outnumbering boys (n= 97) (44%). The mean dmft at 2, 3, 4 and 5 years was 0.96, 1.93, 3.44 and 3.38 respectively. The prevalence of dental caries
of the children was 47.74% with a mean dmft of 2.41 (SD ± 3.60). The mean age of the children in the study was 3.5 years (SD ± 1.01 years).
The results showed statistically significant difference among the age groups. The 4 and 5-year olds had higher levels of dmft than the 2 and 3 year olds (P < 0.001). There were no significant differences (P = 0.3566) in the mean dmft between the boys and girls in the study. There were negligible pufa scores, only 2.2% of the children had pufa scores. The largest contribution was the abscesses.
The average dmft value for the children in Berea/Yeoville area was significantly higher (3.8; SD± 4.2) than the one for the children in OR Tambo Diepsloot immunization clinic (1.5; SD± 2.8), (P < 0.001). Household size and parental education had no significant influence on the dmft.
Conclusion
The prevalence of dental caries was found to be high among 2 to 5-year-old preschool children in Johannesburg, and the mean dmft was directly proportional to the age. / MT2018

Purpose: Pediatric dentists could serve a role in care of children with leukemia. Oral sequelae of cancer therapies are well documented. The purpose of this study is to assess the feasibility and effectiveness of the pediatric dentist in the care of patients with leukemia. Methods: Pediatric Hematology and Oncology at Virginia Commonwealth University was educated on the proposed protocol and administered a questionnaire to assess feasibility of implementing prevention education by the pediatric dentist. Patients were randomized into two groups at diagnosis: one receiving current oral health protocol and those receiving one-on-one prevention education with the pediatric dentist at three points during treatment. Data was collected through clinical intraoral examination and salivary sample. Results: All respondents reported this would address a known problem for patients. They agreed it is feasible and would be a valuable addition to care of these patients. Subject recruitment is ongoing for the effectiveness portion of the study. Conclusion: The addition of the pediatric dentist to the pediatric oncology care team is warranted and feasible.

In the mid 1970s American paediatric dentists started publishing claims that breast feeding an infant on demand and/or during the night was as dangerous to the infant's dental health, in terms of the production of caries, as the previously established dangers of Baby Bottle Tooth Decay. It was argued by some dentists that this danger increased if the infant was breast fed for an extended period of time. Such claims have gained support from many dentists, and their professional bodies, into the twenty-first century. This thesis examines the historical, social, commercial and cultural influences that supported the development of such assertions. It analyses the ways in which expert reinforcement was gained, and scrutinises the scientific controversy as publicly fought in professional journals and other outlets. The thesis also examines the growing status of the dental profession, especially in connection with its claim to specialised scientific and medical knowledge in the production of dental caries. Further, consideration is given to the medicalisation of breast feeding and the associated commercial infant feeding dispute. The thesis attempts to redress the current lack of theoretical analysis of the construction of dental knowledge. To date there has been minimal academic contribution to the history of dentistry; however, the absence of extensive analysis has been advantageous in enabling an original approach to the material.

GLUT2 and TAS1R2 Genotype and Risk of Dental CariesChng, Tabitha 21 November 2012 (has links)
To determine whether polymorphisms in the GLUT2 and TAS1R2 genes are associated with dental caries, 80 Caucasian individuals were recruited. A clinical and radiographic examination determined 3 caries scores: DMFT (decayed, missing and filled teeth), DMFT+radiographs and ICDAS (International Caries Detection and Assessment System). Associations between genotype(s) and caries scores were analysed. A significant increase in DMFT scores was shown in GLUT2 polymorphism carriers (i.e. GLUT2 risk). Carriers of the TAS1R2 gene polymorphism (i.e. TAS1R2 resistant) consistently demonstrated lower caries scores. Caries scores were significantly decreased in the GLUT 2 Resistant/ TAS1R2 Resistant combined group as compared to the GLUT2 Risk/ TAS1R2 Risk combined groups. Variations in GLUT2 and TAS1R2 genes are associated with risk of dental caries. A possible compounded effect of having both genetic polymorphisms is suggested.
