We seek to understand the speeches that have produced the clinic with severe cases in a Psychosocial Day-Care Centers in the central region of Rio Grande do Sul State. In this research were investigated the discourses that make up and determine serious cases, debating how the conceptions of health and mental health impact in the clinic with these cases, analyzing the conceptions of clinic-watch produced from the logic asylum and investigating the institutional speeches about the clinic of psychosis. To achieve the goals we conducted a case study on a CAPS I, building a Board Diary. The method to address the data was the Institutional Analysis of the speech, Marlene Guirado. The theoretical framework for understanding the theme was directed by the construction of two articles, the first: Speeches about Serious Case in Psychosocial Day-Care Centers: The health-disease process; And the second: Speeches from the Clinic with Severe Cases in Psychosocial Day-Care Centers. The materials of the two articles synthesize the understanding of the research on how the cases are being considered serious and the clinical model offered in these cases. / Buscamos compreender os discursos que têm produzido a clínica com casos graves em Centros de Atenção Psicossocial na região central estado do Rio Grande do Sul. Nesta pesquisa foram investigados os discursos que compõem e determinam os casos graves, debatendo de que modo as concepções de saúde e de saúde mental repercutem na clínica com esses casos, analisando as concepções de clínica-cuidado que se produzem a partir da lógica antimanicomial e investigando os discursos institucionais sobre a clínica da psicose. Para alcançar os objetivos foi realizado um estudo de caso em um CAPS I, construindo um diário de bordo. A forma de abordar os dados foi a Análise Institucional do Discurso, de Marlene Guirado. O referencial teórico para compreensão do tema foi direcionado pela construção de dois artigos, o primeiro: Discursos sobre os casos graves em Centros de Atenção Psicossocial: O processo saúde-doença; E o segundo: Discursos da clínica com casos graves em Centros de Atenção Psicossocial. Os materiais dos dois artigos sintetizam a compreensão da pesquisa sobre como os casos estão sendo considerados graves e o modelo de clínica ofertado nesses casos.
