CoordenaÃÃo de AperfeiÃoamento de NÃvel Superior / Fecal incontinence has a major impact on quality of life and affects up to 24% of women and vaginal delivery is one of the main risk factors. Patients with this condition require a specialized evaluation for proper patient management. The aim of this study was evaluate the anatomical and functional aspects of the anal canal and pelvic floor using anorectal three-dimensional ultrasonography combined with transvaginal and anorectal manometry in the evaluation of anal sphincters and pubovisceral muscle (MPV) disruptions in incontinent women with vaginal delivery. As well to correlate the severity of symptoms of fecal incontinence with the findings of ultrasound and manometry and establish a new sonographic scoring system for patients with fecal incontinence with muscle injury. Clinical evaluation was performed with the Cleveland Clinic Florida incontinence score (CCF), functional evaluation with anorectal manometry and anatomical assessment with three-dimensional ultrasound. In this study 63 patients were included with a mean age of 61 years. Most patients (35/56%) had intact MPV and sphincter injury. Of the 28 patients who had injury in MPV, 9 (32.1%) had no sphincter injury. The area of the levator hiatus increased significantly in the Valsalva maneuver in patients with lesions in the MPV. The median incontinence score and ultrasonographic score were significantly higher in patients with lesions in the MPV. The average of contraction pressure in the patients with injury MPV was significantly lower than those with intact MPV. A significant correlation between CCF incontinence scores and sonographic scores was found. There was also a significant correlation between incontinence scores and the length of the sphincter as well as in relation to the hiatus area. It was concluded that there is a high prevalence of sphincter and MPV lesions in incontinent women with previous vaginal delivery. In addition, the severity of the fecal incontinence symptoms is significantly related to the defects of these musculature. A new ultrasound score can improve the management of these patients / A incontinÃncia fecal (IF) tem grande impacto na qualidade de vida e afeta atà 24% da populaÃÃo feminina, sendo o parto vaginal um dos principais fatores de risco. As pacientes com esta condiÃÃo necessitam de avaliaÃÃo especializada para seu adequado manejo. O objetivo deste estudo foi avaliar aspectos anatÃmicos e funcionais do canal anal e mÃsculo pubovisceral(MPV) utilizando a ultrassonografia tridimensional (US3D) anorretal combinada com a endovaginal e com a manometria anorretal para a avaliaÃÃo das lesÃes dos esfÃncteres anais e MPV em mulheres incontinentes com parto vaginal prÃvio. AlÃm disto, correlacionar a intensidade dos sintomas de incontinÃncia fecal com os achados ultrassonogrÃficos e estabelecer um novo escore ultrassonogrÃfico para pacientes com IF. A avaliaÃÃo clÃnica foi realizada com o escore de incontinÃncia da Cleveland Clinic Florida(CCF), a avaliaÃÃo funcional com manometria anorretal e a avaliaÃÃo anatÃmica com US3D. Foi realizado um estudo transversal em que foram incluÃdas 63 pacientes com mÃdia da idade de 61 anos. A maioria das pacientes (56%) tinham MPV Ãntegro e lesÃo esfincteriana. Das 28 (44%) pacientes que possuÃam lesÃo no MPV, 9(32,1%) nÃo possuÃam lesÃo esfincteriana. A Ãrea do hiato dos levantadores aumentou significantemente na manobra de Valsalva nas pacientes com lesÃo no MPV. A mediana do escore de incontinÃncia e do escore ultrassonogrÃfico foram significantemente maiores no grupo de pacientes com lesÃo no MPV. A mÃdia da pressÃo de contraÃÃo das pacientes com lesÃo no MPV foi significantemente mais baixa do que as das pacientes com MPV Ãntegro. Foi encontrada correlaÃÃo significante entre os escores de incontinÃncia da CCF e os escores ultrassonogrÃficos. Houve tambÃm correlaÃÃo significante entre os escores de IF e o comprimento dos esfÃncteres assim como em relaÃÃo a Ãrea do hiato. Conclui-se que hà uma alta prevalÃncia de lesÃes esfincterianas e no MPV em mulheres incontinentes com parto vaginal prÃvio. AlÃm disso, a gravidade dos sintomas de IF à significativamente relacionada com os defeitos destas musculaturas. Um novo escore ultrassonogrÃfico pode melhorar o manejo dessas pacientes.

CoordenaÃÃo de AperfeÃoamento de Pessoal de NÃvel Superior / Abdominoplasty is a lower abdominal lipectomy combined with muscle fascia repair by plication of the aponeuroses of the rectus abdominis muscle. The procedure is traumatic and associated with stress-related inflammation. Preoperative administration of nutraceuticals is believed to reduce postoperative complications such as inflammation and improve surgical results. Once a day for 7 days prior to surgery, patients were supplemented with two diets: SNO1 (negative control) or SNO2. The former was a dairy drink containing 0% fat, 64% carbohydrate (100% maltodextrin) and 36% protein (100% calcium caseinate), corresponding to 37.4 kcal/100mL. The latter was a dairy drink containing 14% carbohydrate (25% maltodextrin, 75% fructose), 25% protein (60% L-alanyl-glutamine, 40% L-arginine) and 61% lipids in the form of a mixture of oleic acid-rich sunflower oil, canola oil, fish oil and medium-chain triglycerides containing ω-9, ω-6 and ω-3 fatty acids, the latter with α-linolenic acid, eicosapetaenoic acid and docosahexaenoic acid, with a ω-9/ω-6 ratio of 3.2:1 (antioxidant action) and a ω-6/ω-3 ratio of 1.4:1 (anti-inflammatory action), corresponding to 200 kcal/100mL. Each patient ingested 200mL SNO1 or SNO2 blended with 50 g diet ice-cream. The sample consisted of 25 adult patients randomly assigned to SNO1 (n=11) or SNO2 (n=14) and submitted to total abdominoplasty. The study variables included HSP-27, HSP-70, IL-1β, IL-6, PCR and TNF-α. Clinical and lab tests were performed on four occasions: T0=on the 1st day of supplementation following a 12-hour fast; T1=on the 8th day, upon induction of anesthesia, after confirming ingestion of study diet for 7 consecutive days; T2=on the 9th day (POD 1), 24 hours after surgery; and T3=on the 11th day (POD 3), 72 hours after surgery. No significant difference was observed between the groups at any of the four moments in time. In conclusion, oral nutraceutical supplementation with L-alanyl-glutamine, arginine and mixtures of oils with high ω-9/ω-6 ratio and low ω-6/ω-3 ratio has no significant preconditioning effect on inflammatory mediators in surgical trauma. / A abdominoplastia consiste de lipectomia abdominal baixa, associada à reparaÃÃo ou âplÃsticaâ mÃsculo-fascial, atravÃs da plicatura das aponeuroses dos mÃsculos retos abdominais. Caracteriza-se como um trauma cirÃrgico, promovendo uma inflamaÃÃo inserida no conceito mais amplo de âresposta ao estresseâ. A nutriÃÃo prÃ-operatÃria pode minimizar as complicaÃÃes pÃs-operatÃrias e melhorar os resultados cirÃrgicos. No presente estudo foi ofertada uma vez ao dia Ãs pacientes suplementaÃÃo oral 1 (SNO1) ou suplementaÃÃo oral 2 (SNO2), no perÃodo prÃ-operatÃrio, durantes 7 dias. A SNO1 à considerada como controle negativo, constitui uma bebida lÃctea com 0% de gordura, composta por 64% de carboidrato (100% maltodextrina) e 36% de proteÃna (100% caseinato de cÃlcio) com calorias totais igual a 37,4kcal/100ml. A SNO2 considerada como o grupo teste foi composta por 14% de carboidrato (25% maltodextrina e 75% frutose), 25% de proteÃna (60%L-alanil-glutamina e 40%L-arginina) e 61% de lipÃdios (mistura de Ãleos de girassol de alto teor olÃico, canola, peixe e triglicerÃdeo de cadeia mÃdia (TCM) contendo Ãmega-9 (ω9), Ãmega-6 (ω6) e Ãmega-3 (ω3), deste Ãltimo na forma de Ãcido α-linolÃnico ALA, Ãcido eicosapetaenÃico EPA e Ãcido docosahexaenÃico DHA com relaÃÃes ω-9/ω-6 de 3,2:1 e ω-6/ω-3 de 1,4:1, com calorias totais de 200kcal/100ml. Cada paciente ingeriu 200 ml de SNO1 ou SNO2, misturado a 50 gramas de sorvete diet. Foram selecionadas e acompanhadas um grupo de 25 pacientes adultas submetidas à abdominoplastia total que foram agrupadas, aleatoriamente, nos grupos SNO1 (N=11) e SNO2 (N=14). As variÃveis avaliadas foram: HSP-27, HSP-70, IL-1β, IL-6, PCR e TNF-α. As avaliaÃÃes clÃnicas e laboratoriais foram realizadas em quatro momentos: no prÃ-cirÃrgico 1 dia (T0) de inÃcio da suplementaÃÃo, com as pacientes em jejum por 12 horas; no 8 dia (T1) na induÃÃo anestÃsica da cirurgia, apÃs confirmar que a paciente ingeriu por sete dias seguidos a suplementaÃÃo; no 9 dia ou 1 PO (T2), 24h apÃs o procedimento de abdominoplastia total; e no 11 dia ou 3 PO (T3), 72h apÃs a cirurgia. NÃo foram evidenciadas diferenÃas estatisticamente significantes entre os grupos e os tempos. Portanto, a suplementaÃÃo nutricional oral utilizando l-alanil-glutamina, arginina e misturas de Ãleos com elevada relaÃÃo ω-9/ ω-6 e baixa relaÃÃo ω-6/ω-3 contendo os Ãcidos ω-3 ALA, EPA e DHA, nÃo possui efeito prÃ-condicionante nutracÃutico sobre proteÃnas de choque e nos mediadores inflamatÃrios aqui estudados no trauma cirÃrgico.

Postoperative Ursodeoxycholic Acid Administration in Patients Undergoing Bypass Gastric in Roux Y Surgery on the Prevention of Cholelithiasis / Uso do Ãcido ursodesoxicÃlico no pÃs-operatÃrio imediato para a prevenÃÃo de litÃase biliar em pacientes submetidos à derivaÃÃo gÃstrica em Y de ROUX

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / The aim of the present study was to evaluate prospectively the influence of ursodeoxycholic acid (UDCA) administration in the prevention were cholelithiasis in the first six months after Roux-en-Y gastric bypass surgery. The data was collected through questionnaires which investigate demographic and anthropometric characteristics. Medical records were analysed to evaluate comorbidities as hepatic steatosis, gastroesophageal reflux disease, gastritis, Hypertension, cardiovascular disease, dyslipidemia, osteoarthritis and infertility. Therefore, 137 patients, with whom 69 were from Monsenhor Bruno Clinic (GROUP A) and 68 were from the Obesity Nucleus of Cearà (GROUP B), were sellected. The patients from GROUP A group received UDCA, while the patients from GROUP B, group did not received UDCA. The quantitative categorical results were presented as percentages and counts, and the numerical results were measured as central trend measures. Normality tests were performed for the numerical variables. According on the variables normality, ANOVA or Mann-Whitney tests were performed. For categorical variables, the chi-square test was performed. For paired variables, the McNemar test was applied. The odds ratio were calculated to measure the strength of association between UCDA exposure and the disease. P values up to 0.05 were considered significant. The data were analyzed using Statistical Package for the Social Sciences (SPSS), v23, SPSS, Inc.. There was no statistical significance in the formation or not of the cholelithiasis (p = 0.110) between GROUP A or GROUP B patients according to the naturality.The gender (p = 0.090) and marital status (p = 0.663) did not presented statistical significance either. In the GROUP A group, 68 (57,6%) did not developed cholelithiasis and 1 (5,3%) developed cholelithiasis. In the GROUP B group, 50 (42,4%) did not developed cholelithiasis, while 18 (94,5%) developed this disorder (OR= 24,4, IC95%= 3,1-189,4, p< 0,001). In the GROUP A group, 64 (92,6%) patients did not presented hepatic steatosis and 5 (7,4%) persisted with hepatic steatosis. In the GROUP B group, hepatic steatosis was not evidenced in 68 (100%) patients, however, it was significant to form cholelithiasis (p< 0,024). The others cormobities evalueted presented significative improvement. The results demonstrate that patients from NOC group, who had not received UDCA, had a higher chance of developing cholelithiasis when comparing with the patients from GROUP A group, who had received UDCA (OR= 24,4, IC95%= 3,1-189,4, p< 0,001). / Objetivo: No presente trabalho, objetivou-se avaliar prospectivamente a contribuiÃÃo do uso do Ãcido ursodesoxicÃlico (AUDC), nos primeiros 6 meses de pÃs-operatorio, na fase rÃpida de perda de peso, com o escopo de prevenir colelitÃase nos pacientes que se submetem à Bypass GÃstrico em Y de Roux. MÃtodo: Os dados foram coletados por meio de tÃcnicas de entrevistas e consultas aos prontuÃrio mediante a utilizaÃÃo de formulÃrios para investigaÃÃo de indicadores sÃcios demogrÃfico, antropomÃtricos e, as principais comorbidezes, esteatose hepÃtica, doenÃa do refluxo gastro-esfÃgico, gastrite, hipertensÃo arterial, doenÃa cardiovascular, dislipidemia, osteoartrite e infertilidade. Deste modo, fizeram parte, inscritas no protocolo, 137 pacientes, sendo 69 da ClÃnica Monsenhor Bruno (GRUPO A) e 68 pacientes do NÃcleo do Obeso do Cearà (GRUPO B-GRUPO CONTRÃLE). AnÃlise de Dados EstatÃsticos, foram apresentados os resultados quantitativos categÃricos em forma de percentuais e contagens e os numÃricos em forma de medidas de tendÃncia central. Foram realizados testes de normalidade para as variÃveis numÃricas. Dependendo da normalidade das variÃveis, foram feitos os testes de ANOVA ou Mann-Whitney, conforme adequado. Para categÃricas, foi utilizado o teste do qui-quadrado. Para variÃveis pareadas, do tipo antes e depois, foi utilizado o teste de McNemar. Foram calculadas razÃes de chances (odds ratio) para mensurar associaÃÃo dos determinantes com o desfecho principal. Foram considerados significativas as comparaÃÃes com valor de p atà 0,05.Os dados foram tabulados e analisados pelo software SPSS (Statistical Package for the Social Sciences), v23, SPSS, Inc. para anÃlise e avaliaÃÃo dos dados obtidos na coleta. Resultados: Segundo a amostragem sÃcio-demogrÃficas, comparando-se os pacientes GRUPO A e os do GRUPO B, quando se refere à naturalidade, observa-se que nÃo houve significÃncia estatÃstica na formaÃÃo ou nÃo da colelitÃase (p= 0,110), o mesmo observando-se quanto ao gÃnero, (p= 0,090) e tambÃm ao estado civil (p=0,663). Ressalta-se os resultados dos 69 pacientes do protocolo da GRUPO A que fizeram uso do AUDC, 68 (57,6%) pacientes nÃo desenvolveram colelitÃase, e sÃmente 1 (5,3%) paciente formou colelitÃase, jà os pacientes que compuseram o protocolo do GRUPO B 50 (42,4%) pacientes nÃo formaram colelitÃase, e 18 (94,5%) pacientes formaram colelitÃase (OR= 24,4, IC95%= 3,1-189,4, p< 0,001). Na apreciaÃÃo da esteatose hepÃtica no pÃs-operatÃrio os pacientes que usaram AUDC da GRUPO A registrou-se que 64 (92,6%) destes pacientes, nÃo apresentavam esteatose hepÃtica e 5 (7,4%) dos pacientes permaneciam com esteatose hepÃtica, jà no grupo do GRUPO B que nÃo fizeram uso 68 (100%) pacientes, nÃo evidenciou-se esteatose hepÃtica, mostrou significÃncia estatÃstica na formaÃÃo de colelitÃase (p< 0,024). As demais comorbidezes avaliadas, tiveram melhora com suas doenÃas controladas. ConclusÃo: Os resultados demonstrados deram suporte para concluir que os pacientes do protocolo do GRUPO B, que nÃo usaram o AUDC, evidenciaram uma probabilidade, confirmada pelo dado estatÃstico oddis ration de 24,4 vezes maior de desenvolverm colelitÃase, quando comparado com os pacientes que fizeram uso do AUDC do GRUPO A, com Ãndice de confianÃa de 95%, e p< 0,001, com relevÃncia significante (OR= 24,4, IC95%= 3,1-189,4, p< 0,001).

AÃÃes do Ãcido caurenÃico e da l-arginina sobre o estresse oxidativo e a inflamaÃÃo em retalhos cutÃneos randomizados em ratos / Effects of kaurenoic acid and l-arginine on the oxidative stress and inflammation in randomized skin flaps in rats

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / The protective effect of kaurenoic acid (KA) and of L-arginine (LA) on the oxidative stress and inflammation was evaluated using an experimental model of ischemia-reperfusion of randomized skin flaps on the back of rats. Fifty-four young Wistar rats, average weight 267,2 g, were distributed randomly in three groups (GC-saline control, AK-kaurenoic acid group, GA â L-arginine group), numerically equal and subsequently redistributed into three subgroups (T-0, T-1 and T-2) with six animals each. All surgical procedures were performed under general anesthesia, using a combination of ketamine hydrochloride (90 mgkg) with xylazine (10mgkg), administered intraperitoneally.(ip). Groups GC and KA rats received the drugs used in this study by gavage; GA rats received LA ip. Treatments were provided 2: hours before the experiment (T0) and 24h (T1). T3 rats (168 h) received daily (6) doses of KA, until sacrifice. Skin concentrations of IL-1 &#946;, TNF-&#945;, MDA, GSH, MPO, Nitrite/Nitrate (Nox) ratio and MPO activity ere assayed.. The Kolmorogov-Smirnov test was used for determination of the distribution pattern of the samples. The data are presented as mean  SE. ANOVA followed by the Dunnett test, were used for statistical evaluation. Values of p 0.05 were considered statistically significant KA promoted a significant reduction of IL-1, TNF&#945; and MDA concentrations, after 24/48 h of randomized cutaneous flap construction demonstrating anti-inflammatory and antiperoxdative activities. LA promoted significant decrease of MDA concentration at T1 timepoint, and a significant increase in GSH concentration in 24/48 h timespoints, demonstrating its antiperoxidative and antioxidant activities. KA also promoted a sigbnificant increase in GSH concentrations after 24/48h. KA promoted significant decrease of MPO activity after 24h48h, showing anti-inflammatory activity. KA and LA promoted a significant decrease of the nitrite/nitrate ratio at T1, showing antioxidant activity. However, at 168 h timepoint the use of LA promoted significant increase of nitrite/nitrate ratio, demonstration a pro-oxidant activity. It is concluded that KA presents antiperoxidative action and intense antioxidant and anti-inflammatory activity while LA presents antiperoxidative and antioxidant action, during ischemia and reperfusion of randomized cutaneous flap rats / O efeito protetor do Ãcido caurenoico e da L-Arginina sobre o estresse oxidativo e a inflamaÃÃo foi avaliado, utilizando um modelo experimental de isquemia-reperfusÃo de retalho cutÃneo randomizado no dorso de ratos. Cinquenta e quatro ratos, Wistar, jovens, com peso mÃdio de peso mÃdio 267,2 g, foram distribuÃdos ao acaso, em trÃs grupos (GC - controle salina, AK â grupo Ãcido caurenoico, GA â grupo L-Arginina), numericamente iguais e posteriormente redistribuÃdos em trÃs subgrupos (T-0, T-1 e T-2), com seis animais cada. Todos os procedimentos cirÃrgicos foram realizados sob os efeitos de anestesia geral, utilizando-se de uma associaÃÃo do cloridrato de ketamina (90 mg/kg) com xilazina (10mg/kg), administrados por via intra-peritoneal. Os animais pertencentes aos grupos controle e Ãcido caurenoico receberam as drogas utilizadas nesse estudo por via oral (gavagem); enquanto que, o grupo L-arginina, por via intraperitoneal, 2 horas antes do experimento , no tempo T-1, uma dose pÃs-operatÃria e no tempo T-2, seis doses pÃs-operatÃrias. Foram determinadas as concentraÃÃes de IL-1&#946;, TNF-&#945;, MDA, GSH, , a relaÃÃo Nitrito/Nitrato (Nox) e a atividade da MPO no tecido. Para determinaÃÃo do padrÃo de distribuiÃÃo das amostras utilizou-se o teste de Kolmorogov-Smirnov. Os dados foram apresentados como mÃdia  SEM. ANOVA seguida pelo teste de Dunnett, foi utilizada para avaliaÃÃo estatÃstica. Valores de p<0,05 foram considerados significantes. O Ãcido caurenoico reduziu significantemente as concentraÃÃes IL-1, decorridas 24h e 48h da construÃÃo do retalho cutÃneo randomizado; bem como, de TNF&#945;, decorridas 24h e 48h, demonstrando atividade anti-inflamatÃria. O Ãcido caurenoico promoveu diminuiÃÃo significante da concentraÃÃo de MDA decorridas 24/48h, demonstranto atividade antilipoperoxidativa. O Ãcido caurenÃico promoveu diminuiÃÃo significante da atividade de MPO, decorridas 24h/48h, evidenciando atividade anti-inflamatÃria. O Ãcido caurenoico e a L-arginina promoveram a diminuiÃÃo significante da relaÃÃo nitrito/nitrato, decorridas 24h, demonstrando atividade antioxidante. No entanto, a L-arginina promoveu aumento significante da relaÃÃo nitrito/nitrato decorridas 168h, evidenciando atividade prÃ-oxidante. Conclui-se que o Ãcido caurenÃico apresenta aÃÃo antilipoperoxidativa, intensa aÃÃo antioxidante e atividade anti-inflamatÃria; enquanto a L-arginina apresenta aÃÃo antilipoperoxidativa e antioxidante, durante a isquemia e reperfusÃo de retalho cutÃneo randomizado em dorso de ratos.

O tratamento do IFA (Impacto FÃmoro-Acetabular) e das lesÃes labrais à crucial para a preservaÃÃo do quadril em atletas jovens e ativos. Objetivo: Relatar as afecÃÃes do quadril vistas em atletas de alta performance, descrever a tÃcnica artroscÃpica utilizada para tratar essas doenÃas e determinar o retorno dos jogadores profissionais ao esporte apÃs artroscopia do quadril. MÃtodos: Os atletas incluÃdos no trabalho eram jogadores de futebol vinculados à FederaÃÃo Internacional de Futebol e Associado (FIFA), tenistas ranqueados na AssociaÃÃo dos Tenistas Profissionais (ATP). Todos foram submetidos a artroscopia por patologias intra-articulares do quadril. Nove tenistas (6 homens, 3 mulheres), cinquenta e quatro jogadores de futebol (39 homens, 14 mulheres) foram operados entre Maio de 2005 e Julho de 2010. A mÃdia de idade dos atletas foi de 25 anos (variando de 18 a 31). Todos atletas procuraram tratamento apÃs incapacidade para jogar. Resultados: A mÃdia prÃ-operatÃria do Ãngulo CE de Wiberg foi de 37 graus (variando de 33 a 42) e a mÃdia do Ãngulo alfa foi de 72 graus (variando de 65 a 83). Todos os pacientes tiveram o diagnÃstico de impacto fÃmoro-acetabular (IFA) no prÃ-operatÃrio. Durante a artroscopia, todos os jogadores foram submetidos ao reparo labral e tratamento do IFA (sendo 6 do tipo pincer, 57 do tipo combinado). O tamanho mÃdio da lesÃo labral foi de 25mm (variando de 5 a 40). Em mÃdia trÃs (variando de 2 a 5) Ãncoras de sutura foram utilizadas para reparar a lesÃo. Seis jogadores precisaram de microfratura em uma lesÃo condral no acetÃbulo. Em quarenta e um quadris havia lesÃo do ligamento redondo que foi tratada com desbridamento. Dois jogadores foram submetidos a liberaÃÃo do tendÃo do mÃsculo iliopsoas. O escore modificado de quadril chamado Harris (MHHS) melhorou de 61 a 90 (p = 0,001). A satisfaÃÃo mÃdia dos pacientes apÃs 1 ano da cirurgia era de 10 pontos (escala de 1 a 10). Todos os atletas voltaram a jogar em nÃvel profissional. ConclusÃo: Este estudo demonstrou que o tratamento artroscÃpico do IFA e de lesÃes labrais em atletas de alta performance proporciona melhora clÃnica significativa. O tratamento artroscÃpico para IFA à uma maneira segura e eficaz para promover o retorno ao esporte de jogadores profissionais. / Background: Lesions of the acetabular labrum are known causes of hip pain and represent the beginning of a cascade of events that can lead to osteoarthritis. The treatment of FAI (femoroacetabular impingement) and labral tears is crucial for hip preservation in young and active athletes. Purpose: To define hip pathologies seen in high performance athletes, to describe the arthroscopic technique used to treat these diseases and to determine the return to sport of the professional players following hip arthroscopy. Methodology: The athletes included in the study are soccer players tied to International Federation of Football Associated (FIFA), unranked tennis players in the Association of Tennis Professionals (ATP). All patients underwent arthroscopy for intra-articular hip pathologies. Nine tennis players (6 males, 3 females), fifty-four soccer players (39 males, 14 females) were operated between May 2005 and July 2010. The mean age of the athletes was 25 years old (range 18-31). All athletes sought treatment after failure to play. Result: The average preoperative Wibergâs CE angle was 37 degrees (range 33 to 42) and the average alpha angle was 72 degrees (range 65 to 83). All patients were diagnosed with femoroacetabular impingement (FAI) preoperatively. During arthroscopy, all players underwent labral repair and treatment of FAI (6 isolated pincer type, 57 combined type). The average size of the labral tear was 25 mm (range 5 to 40). On average three (range 2 to 5) suture anchors were used to repair the tear. Six players required microfracture of a chondral lesion on the acetabulum. In forty-one hips there was tear of the ligamentum teres which were treated with debridement. Two players underwent an iliopsoas release. The Modified Harris Hip Score (MHHS) improved from 61 to 90 (p=0.001). The median patient satisfaction was 10 (score from 1 to 10) after one year of surgery. All athletes returned to play professional level. Conclusion: This study demonstrated that arthroscopic treatment of FAI and chondro-labral dysfunction in high performance athletes provides a significant clinical improvement. Arthroscopic treatment of FAI is a safe and effective way for professional players to return to play.

CoordenaÃÃo de AperfeiÃoamento de NÃvel Superior / CÃncer colorretal tem alta incidÃncia, sendo influenciado por fatores genÃticos e ambientais. Estudos sugerem que Ãcidos graxos Ãmegas 3, 6 e 9 estÃo envolvidos na sua gÃnese e prevenÃÃo. Objetivo foi verificar efeitos de dietas hiperlipÃdica, normolipÃdica e hipolipÃdica, com baixa relaÃÃo &#61559;-6:&#61559;-3 e alta &#61559;-9:&#61559;-6 na carcinogÃnese colorretal induzida por azoximetano (AOM). Foram usados 60 ratos Wistar, 3 semanas de idade, pesando 40 a 50g e distribuÃdos em 5 grupos de 12 animais: GI- Dieta PadrÃo sem AOM; GII- Dieta PadrÃo e AOM; GIII- Dieta HiperlipÃdica e AOM; GIV- Dieta NormolipÃdica e AOM; GV- Dieta HipolipÃdica e AOM. Avaliados peso e ingesta 4 vezes por semana atà a 36Â. Os colos foram analisados quanto à presenÃa de tumores, focos de criptas aberrantes, nÃmero de criptas por foco, incidÃncia, invasÃo, atipias e multiplicidade dos carcinomas. Determinaram-se colesterol total e fraÃÃes, triglicerÃdeos, glicemia, ALT e AST, glutationa, TBARS, interleucinas no plasma. Ãndice apoptÃtico e mitÃtico, NFkB, COX-2 e K-Ras foram medidos em tecido tumora. GI e GII apresentaram aumento da massa corporal em relaÃÃo aos GIII, GIV e GV. Ingestas de GI e GII foram maiores que em GIII, GIV e GV. Houve reduÃÃo do nÃmero de criptas aberrantes e do total de focos com menos e mais de 5 criptas em GIV e GV comparados com GII. Houve diminuiÃÃo na incidÃncia dos tumores em GV. O Ãndice de carcinogÃnese foi superior no GII em relaÃÃo ao GV. Glutationa foi maior e TBARS menor em GV. IL-10 no GIII foi maior que em GI. IFN&#947; elevou-se em GII em relaÃÃo aos GIII e GV. TNF-&#945; aumentou em GIV e GV em relaÃÃo ao GI. Apoptose aumentou nos grupos V, IV, III, em relaÃÃo aos GII e GI. Ãndice mitÃtico foi menor em GV em relaÃÃo aos GII, GIII e GIV. Conclui-se que dietas com baixa razÃo &#61559;-6:&#61559;-3 e alta &#61559;-9:&#61559;-6 reduzem a ingesta, massa corporal e peroxidaÃÃo lipÃdica; elevam a capacidade antioxidante e protegem contra a carcinogÃnese cÃlica. / Colorectal cancer has a high incidence and is known to be influenced by dietary factors. The fatty acids &#61559;-3, &#61559;-6 and &#61559;-9 are believed to be involved in its genesis and prevention. The objective was to evaluate the effect of hyperlipidic, normolipidic and hypolipidic diets with low &#61559;-6:&#61559;-3 ratio and high &#61559;-9:&#61559;-6 ratio on azoxymethane (AOM) induced colorectal carcinogenesis in rats. Sixty 3-week-old male Wistar rats weighing 40-50g were assigned to 5 groups of 12 each: GI=Standard diet without AOM; GII=Standard diet and AOM; GIII=Hyperlipidic diet and AOM; GIV=Normolipidic diet and AOM; GV=Hypolipidic diet and AOM. Weight and ingestion were registered 4 times/week until the 36th week. Colons were analyzed for tumors, aberrant crypt foci, number of crypts per focus, incidence, differentiation, invasion and carcinoma multiplicity. The levels of total and fractionated cholesterol, triglycerides, glycemia, ALT, AST, glutathione, TBARS, interleukins were determined in plasma. Apoptosis, mitosis, NFkB, COX-2 and K-Ras were measured in tumor tissue. Body mass was greater in GI and GII than in GIII, GIV and GV. Ingestion was greater in GI and GII than in GIII, GIV and GV. The number of aberrant crypts and the total number of foci with <5 and >5 crypts decreased in GIV and GV compared to GII. In GV, the incidence of tumors was reduced. The index of carcinogenesis was higher in GII than in GV. Glutathione was higher and TBARS was lower in GV. GIII was higher than in GI with regard to IL-10. IFN&#947; was higher in GII than in GIII and GV. TNF-&#945; was higher in GIV and GV than in GI. Apoptosis increased in GV, GIV and GIII in relation to GII and GI. The mitotic index was lower in GV than in GII, GIII and GIV. In conclusion, hyperlipidic, normolipidic and hypolipidic diets with low &#61559;-6:&#61559;-3 ratio and high &#61559;-9:&#61559;-6 ratio reduce ingestion, body mass and lipid peroxidation; increase the antioxidant capacity and offer protection against colorectal carcinogenesis.

FundaÃÃo Cearense de Apoio ao Desenvolvimento Cientifico e TecnolÃgico / A deficiÃncia de Ãxido nÃtrico (NO) tem sido implicada como um dos principais mecanismos de disfunÃÃo endotelial e de DisfunÃÃo erÃtil. A busca de novas medicaÃÃes deve ser importante para diminuir a peso dessa doenÃa. O estudo avaliou o relaxamento, in vitro, induzido por um novo fÃrmaco doador de NO do complexo Nitrosil-RutÃnio (RUT-BPY-IMI) em tiras de corpos cavernosos de humanos, retirados de doadores de ÃrgÃos para transplante nÃo-vivos. Os tecidos, imersos em sistemas de banhos isolados em soluÃÃo de Krebs (pH7,4, 37ÂC), foram contraÃdos em soluÃÃo de K+ 80mM, e posteriormente novamente prÃ-contraÃdos com Fenilefrina (Phe 10ÂM) e curvas de concentraÃÃo-resposta ( 1012 a 10-4) foram obtidas. Inicialmente o fÃrmaco era comparado com o VeÃculo (DMSO) e com substÃncias jà utilizadas em outros estudos, que demonstraram bons efeitos relaxantes ( SNP, BAY 41-2272 e Vardenafila). Para esclarecer o mecanismo pela qual esse fÃrmaco promove seu relaxamento, foram realizados os seguintes experimentos: avaliaÃÃo do relaxamento alcanÃado com a contraÃÃo com K+80mM, adiÃÃo de um inibidos da Ãxido NÃtrico sintase (NOS), L-NAME (100ÂM); adiÃÃo de ODQ (30ÂM), um inibidor da GuanilatoCiclase solÃvel; um removedor de NO extracelular, Hemoglobina bovina (30ÂM); Glibenclamida ( 10ÂM), um bloqueador de canais de Ãons potÃssio ATP-dependente (Katp). Os tecidos expostos ao RUT-BPY-IMI, SNP e DMSO foram congelados em nitrogÃnio lÃquido para mensurar a quantidade de GMPc. A substÃncia provocou um relaxamento mÃximo (Emax) na musculatura lisa de corpo cavernoso importante ( Emax= 112,92% Â10,03% e pEC50=4,991Â0,1916), 60% a mais que o veÃculo. Comparando RUT-BPy-IMI com o SNP, a droga estudada mostrou tÃo eficiente como o SNP, nÃo mostrando diferenÃa entre os dois Emax (p=0,3437). Jà com o BAY 41-2272, nÃo houve diferenÃa estatÃstica em nenhum dos parÃmetros estudados (Emax e pEC50)..O Emax alcanÃado na curva concentraÃÃo- resposta com K+ 80mM foi cerca de 20% menor que a curva com Phe (10ÂM) (p=0,0208). NÃo houve inibiÃÃo do efeito relaxante mÃximo da droga com L-NAME (p>0,05), ou desvio da curva para a direita. A adiÃÃo ODQ (30ÂM) ao banho diminuiu em cerca de 50% o Emax da substÃncia (p<0,05). Jà a adiÃÃo da hemoglobina bovina (30ÂM) nÃo alterou a capacidade relaxante mÃxima da substÃncia (p<0,05). NÃo houve bloqueio de efeito mÃximo do fÃrmaco com a Glibenclamida (10ÂM) (p>0,05). O RUT-BPY produziu uma concentraÃÃo intracelular de GMPc 74% maior que o veÃculo, e menor que o SNP O RUT-BPY-IMI à uma substÃncia doadora NO, potente relaxadora da musculatura lisa do corpo cavernoso humano. A substÃncia possivelmente nÃo induz NOS, atua, provavelmente ativando a guanilatociclase solÃvel, produzindo GMPc, liberando NO intracelular. NÃo parece atuar nos canais de potÃssio, e nÃo atuam no Katp / Deficiency of nitric oxide (NO) has been implicated as one of the main mechanisms of Endothelial dysfunction and erectile dysfunction. The search for new medications must be important to lessen the burden of this disease. The study evaluated the relaxation, in vitro, induced by a new drug of the Nitrosyl-Ruthenium complex (RUT-BPY-IMI) into strips of corpora cavernosa, taken from non-living human organ donor for transplantation. The strips were immersed in tissue baths in Krebs solution (pH7 .4, 37 C). They were contracted in 80 mm K + solution, and later again with Phenylephrine (PHE 10 &#956;m) and concentration-response curves (10-12 to 10-4&#956;M) were obtained. Initially the drug was compared with the vehicle (DMSO) and then with substances which have already been used in other studies. These drugs showed very good relaxing effects (SNP, BAY 41-2272 and Vardenafil). To clarify the mechanism by which this drug promotes relaxation, the following experiments were carried out: evaluation of relaxation achieved with the contraction with 80 mm K +, effect of an inhibitor of nitric oxide syntase (NOS), L-NAME (100 &#956;m); addition of ODQ (30 &#956;m), an inhibitor of the soluble GuanylateCyclase; a remover of the extracellular NO, bovine Hemoglobin (30 &#956;m); Glibenclamide (10 &#956;m), a potassium ion channel blocker ATP-dependent (KATP). The tissues exposed to the RUT-BPY-IMI, SNP and DMSO were frozen in liquid nitrogen to measure the amount of GMPc.The substance caused an EMAX relaxation in smooth muscle of the corpus cavernosum (EMAX = 112.92%  10.03% and pEC50 = 4.991  0.1916), 70% more than the vehicle. Comparing RUT-BPY-IMI with the SNP, the drug studied showed as efficient as the SNP, there were no difference between both EMAX (p = 0,3437),. Compared with the BAY 41-2272, there was no statistical difference in any of the parameters studied (pEC50 and EMAX).. The EMAX achieved in concentration-response curve with 80 mm K + was about 20% lower than the curve with PHE (10 &#956;m) (p = 0.0208). There was no inhibition of the relaxing effect of drug with L-NAME (p 0.05 >), or deviation from the curve to the right. The addition of ODQ (30 &#956;m) to the bath, decreased 50% of the EMAX of the substance (p<0,05). The addition of bovine hemoglobin (30 &#956;m) did not alter the capacity maximum relaxing substance (p<0,05). There was no inhibition or blockade of the maximum effect of the drug with Glibenclamide (10 &#956;m) (p 0.05 >). The RUT-BPY-IMI tissue concentration of cGMP produced was 74% over the vehicle, and less than the SNP (p<0,05).RUT-BPY-IMI is a NO-donor, it produces a powerful relaxation of the smooth muscle of the corpus cavernosum. Probably, the substance does not induce NOS, acts by activating the soluble guanylateCyclase, which produces cGMP, releasing the intracellular NO. It seems that it does not activate potassium channels, and not act throw KATP.

Análise estereológica de núcleos hipotalâmicos envolvidos com a regulação da ingestão alimentar em ratos Wistar submetidos a tratamento imunossupressor com tacrolimus e micofenolato mofetil

Defesa: Curitiba, 18/12/2017 / Inclui referências: f. 55-60 / Resumo: Drogas imunossupressoras são indispensáveis para pacientes pós-transplante, diminuindo, significativamente, os riscos de rejeição inerentes à este tipo de procedimento. No entanto, seus efeitos colaterais sobre os núcleos hipotalâmicos envolvidos na regulação da ingestão de alimentos e o efeito no excessivo ganho de peso e suas comorbidades associadas são desconhecidos. Neste sentido, esta pesquisa teve como objetivo desenvolver a análise estereológica dos núcleos hipotalâmicos relacionados com a ingestão de alimentos em ratos Wistar submetidos ao tratamento com os imunossupressores tacrolimus (TAC) ou micofenolato mofetil (MMF), comparar a densidade neuronal e número total de neurônios hipotalâmicos entre ratos Wistar tratados com imunossupressores e com isso observar possíveis alterações nos núcleos hipotalâmicos paraventricular, área hipotalâmica lateral, dorsomedial, ventromedial e arqueado. O estudo experimental foi realizado em ratos Wistar machos submetidos ao tratamento das drogas imunossupressoras TAC e MMF, via oral, durante o período de 14 semanas. Os animais foram randomicamente categorizados em quatro grupos: Controle (sem administração da droga); Sham (administração de soro fisiológico (SF) 0,9%, via gavagem); Tacrolimus (administração de 1mg/kg ao dia de tacrolimus diluídos em SF 0,9%, via gavagem) e Micofenolato (administração de 30mg/kg ao dia de micofenolato mofetil diluídos em SF 0,9%, via gavagem). Após o período de tratamento imunossupressor, os animais foram eutanasiados e seus encéfalos fixados para posterior confecção de lâminas histológicas utilizando a coloração de Nissl. Essas lâminas foram fotodocumentadas e as imagens obtidas receberam "máscaras" delimitantes para posterior implantação do dissector físico para quantificação e análise estereológica dos corpos celulares dos neurônios hipotalâmicos. Não houve diferenças significativas nas densidades neuronais e nos números de neurônios hipotalâmicos dos núcleos paraventricular, área hipotalâmica lateral, dorsomedial, ventromedial e arqueado entre os grupos Controle, Sham, Tacrolimus e Micofenolato. Neste estudo, o tratamento de animais com imunossupressores TAC e MMF por 14 semanas não induziu alterações no número dos corpos celulares dos neurônios hipotalâmicos e não evidenciaram efeitos neurotóxicos dos imunossupressores TAC e MMF. Amostragem Aleatória e Sistemática. / Abstract: Immunosuppressive drugs are indispensable to post-transplant patients for significantly reducing the inherent risks of graft rejection related to the procedure. However, the side effects on the hypothalamic nuclei responsible for regulation of food intake and the effect on excessive weight gain and the related complications are unknown. Therefore, this research aimed to perform stereological analysis of the hypothalamic nuclei related to food intake in Wistar rats submitted to immunosupressive treatment with tacrolimus (TAC) or mycophenolate mofetil (MMF) in order to compare neuronal density and total number of neurons in the hypothalamic paraventricular, lateral hypothalamic area, dorsomedial, ventromedial and arched. The experimental study was performed in male Wistar rats submitted to immunosuppressive treatment with TAC or MMF, orally, during a 14-week period. The animals were randomly categorized into four groups: Control (no drug administration); Sham (0.9% saline solution administration, by gavage); Tacrolimus (administration of 1mg/kg/day of tacrolimus diluted in 0.9% saline solution by gavage) and Mycophenolate (administration of 30mg/kg/day of mycophenolate mofetil diluted in 0.9% saline solution by gavage). After the immunosuppressive treatment, animals were euthanized and their brains fixed for subsequent histological slides using Nissl staining. These slides were photodocumented and the images obtained received delimiting "masks" for later implantation of the physical dissector to quantify and stereologically analyze the cellular bodies of the hypothalamic neurons. There were no significant differences in neuronal densities and numbers of hypothalamic neurons of the paraventricular nuclei, lateral hypothalamic area, dorsomedial, ventromedial and arched between the Control, Sham, Tacrolimus and Mycophenolate groups. In this study, the experimental treatment with immunosuppressive CT or MMF for 14 weeks did not induce changes in the number of cell bodies of hypothalamic neurons and did not show neurotoxic effects of the used immunosuppressant agents. Recuperação da informação de textos livres não estruturados em prontuários eletrônicos de otorrinolaringologia

Defesa: Curitiba, 08/12/2017 / Inclui referências : f.63-71 / Resumo: Introdução: O uso de prontuários eletrônicos na prática médica é uma realidade e os textos livres não estruturadas, contêm uma grande quantidade de dados que podem ser utilizados para a formação de conhecimento. Utilizar técnicas de recuperação da informação, contribui para a descoberta dos termos mais utilizados e para compará-los a protocolos existentes. O preenchimento automatizado de coleta pode auxiliar o aumento das amostras em pesquisas. Objetivos: Extrair palavras, de textos livres não estruturados, em prontuário eletrônico e comparar sua frequência com um protocolo já existente de termos, das doenças de maior prevalência, em uma base de dados. Criar uma interface de ligação para o preenchimento automático dos sintomas e doenças, entre o Sistema CLINIC? e o Sistema SINPE©. Material e Métodos: O estudo foi realizado com base em um big data, de 375.095 prontuários eletrônicos. Destes, foram selecionados os que possuíam CID e campo histórico preenchido, com no mínimo cinco caracteres e excluídos os pós-operatórios e reconsultas inferiores a 30 dias. Do resultado, foram analisados os CID's mais prevalentes e analisados os textos livres não estruturados, com a ferramenta word cloud e, após, comparados com termos do sistema de protocolos. Foi elaborada uma interface, entre o sistema de prontuários e o Sistema SINPE©, visando uma coleta de dados automatizada. Resultados: Foram analisados 125.973 prontuários, contendo textos livres não estruturados. A amostra apresentou 57,24% de pacientes do sexo feminino e 42,76% do sexo masculino, com idade média de 38,53 anos. Os 19 CID's de maior prevalência representaram 70,11% dos casos. Foram desenvolvidas word cloud's para os textos livres, nas doenças de maior prevalência. Compararam-se as palavras inseridas no prontuário eletrônico, com os termos do protocolo de otorrinolaringologia, do Sistema SINPE© sendo identificadas as palavras mais frequentes, nas doenças de maior prevalência e construídos intervalos de confiança (IC=95%). A interface desenvolvida, mostrou ser capaz de identificar, nos textos livres, os termos do protocolo e automatizar a coleta de dados. Conclusões: Foram extraídas, de textos livres não estruturados, as palavras de maior frequência, e comparadas com um protocolo de termos já existente, nas doenças de maior prevalência. Foi desenvolvida uma interface entre os sistemas CLINIC? e SINPE©, capaz de preencher de forma automática os itens do protocolo mestre, tanto em sintomas como em diagnósticos. Using information retrieval techniques contributes for the discovery of the most used terms and the comparison of such terms to existing protocols. Automated data collection may help in increasing samples in research. Objectives: Extract words from free unstructured text out of electronic medical records and compare their frequency to an existing protocol of terms for the most prevalent diseases from a database. Create a connecting interface for the automatic filling in of symptoms and diseases between the CLINIC? and the SINPE© systems. Material and Methods: The study was performed based on a big data made up of 375,095 electronic medical records. Out of these, those which contained the ICD and medical history fields filled in and which had at least five characters were selected, and post-ops and return visits within less than thirty days were excluded. From the results, the most prevalent ICDs and free unstructured text were analyzed with the word cloud tool, and later compared to the terms in the protocol system. An interface was created between the SINPE© system and the medical records system, aiming at automated data collection. Results: 125,973 medical records were analyzed which had free unstructured text. The sample was made up of 57.24% of female patients and 42.76% male patients, with an average age of 38.53 years old. The 19 most prevalent ICDs represented 70.11% of cases. Word clouds were developed for the free text in the most prevalent diseases. The words inserted in the electronic medical records were compared to the terms in the otolaryngology protocol in the SINPE© system. The most frequent words were identified in the most prevalent diseases and confidence intervals were built (CI 95%). The interface developed was able to identify the terms of the protocol in free text and automate the data collection. Conclusions: The most frequent words were extracted from free unstructured text and compared to an existing protocol of terms for the most prevalent diseases. An interface between the CLINIC? and the SINPE© systems was developed which is able to automatically fill in the items of the master protocol, relative to both symptoms and diagnosis. Estudo do bloqueio pleural bilateral em analgesia pós-operatória de cirurgias abdominais de urgência por via mediana

Com o objetivo de determinar uma alternativa aos opióides, avaliou-se a analgesia pós-operatória (APO) em laparotomias de urgência com incisão mediana, pela anestesia regional (AR). Seu representante foi o bloqueio pleural bilateral (BPB) com a bupivacaína® (bp), um anestésico local (AL) de ação prolongada. Durante o estudo, realizado em 25 pacientes da enfermaria cirúrgica, 4 destes foram excluídos por intercorrências durante a avaliação. Os restantes 21 pacientes foram efetivamente estudados com 79 BPBs num período de pouco mais de 48 horas, durante o pós-operatório imediato (POI). Os sete primeiros pacientes foram estudados exclusivamente com a bp, através de 24 BPBs. Quanto aos restantes 14 pacientes, a analgesia de 38 BPBs com a bp foi comparada com 17 BPBs com soro fisiológico (SF), mediante teste duplo cego, com o propósito de realçar, ou não, o efeito do AL.Sabe-se, por outro lado, que incisões torácicas e abdominais altas manifestam-se bastante dolorosas durante a respiração. Tendo em vista essa realidade, a analgesia do BP também foi testada, indiretamente, através das alterações das funções pulmonares (FP) antes e depois da administração da bp, com o Enhance VMI®, um miniexpirômetro da Clement Clarke International Ltd, Edinburg. A analgesia obtida conferiu valores mais elevados das funções pulmonares, em relação àqueles aferidos antes do bloqueio, notadamente com o Volume Expiratório Forçado de um segundo (VEF1) e com a Capacidade Vital Forçada (CVF) pelo teste pareado de Wilcoxon (p<0,05). A análise estatística revelou que todos os 62 BPBs com a bp determinaram boa analgesia pelo teste exato de Fischer (p<0,001), enquanto que dos 17 BPBs com o SF, apenas cinco resultaram em pouca analgesia. O BPB com a bp mostrou ser um método proporcionador de APO em laparotomias de urgência por via mediana. / With the objective of determining an alternative to the opioids, we study the postoperative analgesia (POA) in urgency laparotomies made by median incision, with regional anesthesia (RA). His representative one was the bilateral pleural blockade (BPB) with bupivacaine® (bp), a long action local anesthetic (LA). During the study accomplished in 25 patient, 4 of these were excluded during the evaluation. The remaining 21 patients were studied with 79 BPBs in a period of little more than 48 hours during the immediate postoperative (IPO) period. The first seven patiens were studied exclusively with the bp through 24 BPBs. The remaining 14 patients, analgesia of 38 BPBs with bp was compared with 17 BPBs with saline solution (SS) by double blind test, with the purpose of enhancing or not, the effect of the LA. On the other hand, it is known, that thoracic and high abdominal incisions are very painful during the breathing. Tends in view this reality, analgesia of BPB was also tested, indirectly, through alterations of the lung functions (FP) before and after the administration of bp with the aid of the Enhance VMl a Mini-Spirometer, a product of Clement Clarke lnternational ltd (Edinburgh). The obtained analgesia checked higher values of the lung functions in relation to those confronted before the blockade, especially with the Forced Expired Volume in 1 second (FEV1) and with the Forced Vital Capacity (FVC) analyzed by the Wilcoxon paired test (p<0,05). Statistical analysis revealed that, all the 62 BPBs with bp resulted in good analgesia by the Fisher’s exact test (p<0,001) while the 17 BPBs with SF, only five resulted in little analgesia. The BPB with bp showed to be a method that provide POA in urgency laparotomies made by median incision.
