Hepatobiliary diseases in small animals: a comparison of ultrasonography and multidetector-row computed tomography

Ultrasonography (US) is an essential imaging tool for identifying abnormalities of the liver parenchyma, biliary tract and vascular system. US has replaced radiography as the initial imaging procedure in screening for liver disease in small animals. There are few reports of the use of conventional and helical computed tomography (CT) to assess canine or feline parenchymal and neoplastic liver disease and biliary disorders. In human medicine the development of multidetector- row helical computed tomography (MDCT), with its superior spatial and temporal resolution, has resulted in improved detection and characterization of diffuse and focal liver lesions. The increased availability of MDCT in veterinary practice provides incentive to develop MDCT protocols for liver imaging in small animals. The purpose of this study is to assess the rule of MDCT in the characterization of hepatobiliary diseases in small animals; and to compare this method with conventional US. Candidates for this prospective study were 175 consecutive patients (dogs and cats) referred for evaluation of hepatobiliary disease. The patients underwent liver US and MDCT. Percutaneous needle biopsy was performed on all liver lesions or alterations encountered. As for gallbladder, histopatological evaluation was obtained from cholecystectomy specimens. Ultrasonographic findings in this study agreed well with those of previous reports. A protocol for dual-phase liver MDCT in small animals has been described. MDCT findings in parenchymal disorders of the liver, hepatic neoplasia and biliary disorders are here first described in dogs and cats and compared with the corresponding features in human medicine. The ability of MDCT in detection and characterization of hepatobiliary diseases in small animals is overall superior to conventional US. Ultrasonography and MDCT scanning, however, play complementary rules in the evaluation of these diseases. Many conditions have distinctive imaging features that may permit diagnosis. In most instances biopsy is required for definitive diagnosis.

La leishmaniosi canina (LCan) causata da Leishmania infantum rappresenta un’importante zoonosi in molte aree del mondo ed il cane rappresenta il principale reservoir del parassita per l’uomo. Il tipo di risposta immunitaria che i soggetti colpiti mettono in atto condiziona fortemente la progressione della malattia: animali che non sviluppano un’adeguata risposta immunitaria cellulo-mediata mostrano la sintomatologia clinica nonostante abbiano una forte ma inefficace risposta umorale che contribuisce al peggioramento della sintomatologia clinica. L’obbiettivo dello studio è stato quello valutare da un punto di vista descrittivo il segnalamento, i segni clinici e clinicopatologici dei pazienti affetti da leishmaniosi portati in visita presso il Dipartimento di Scienze Mediche Veterinarie nel periodo compreso da Gennaio 2002 a Marzo 2012 con particolare attenzione sull’impatto della patologia renale e dell’anemia nel quadro clinico della LCan. In base ai risultati ottenuti è stato possibile affermare che la leishmaniosi canina è una patologia relativamente frequente nella nostra realtà clinica universitaria e che presenta caratteristiche cliniche e clinicopatologiche simili a quelle riportate in letteratura. I nostri risultati preliminari suggeriscono che in questa malattia il coinvolgimento renale e le conseguenze sistemiche che ne derivano possono essere predominanti a livello clinico e laboratoristico. La gravità del quadro clinico appare associata in maniera significativa all’entità della risposta umorale e del successivo coinvolgimento glomerulare nel contesto di una risposta infiammatoria sistemica cronica. Successivamente, sono state misurate le concentrazioni di IgG ed IgM in corso di follow-up in alcuni dei soggetti inclusi nello studio e sottoposti a differenti trattamenti anti-leishmania. Dai risultati preliminari ottenuti nel nostro lavoro è stato possibile affermare che in corso di trattamento le concentrazioni di tali immunoglobuline subiscono una riduzione progressiva confermando pertanto l’efficacia del trattamento anti-leishmania non solo nella remissione della sintomatologia clinica ma anche nel ripristino della normale risposta umorale. / Canine leishmaniasis (LCan) caused by Leishmania infantum is an important zoonosis in many parts of the world and the dog is the main reservoir of the parasite to humans. The type of immune response that those affected put in place strongly influences the progression of the disease: the animals do not develop adequate cell-immune mediated response show clinical symptoms despite having a strong but ineffective humoral response that contributes to worsening the clinical symptoms . The objective of this study was to evaluate a descriptive standpoint, signaling, clinical and clinicopathological signs of leishmaniasis patients taken to visit at the Department of Veterinary Medical Sciences during the period from January 2003 to March 2012 with particular attention to the impact of kidney disease and anemia in the clinical picture of LCan. Based on the results obtained it was possible to say that leishmaniasis is a relatively common disease in our clinical practice and that the clinical features and clinicopathological features are similar to those reported in the literature. Our preliminary results suggest that in this disease, renal involvement and systemic consequences can be predominant in the clinical and laboratory findings. The severity of the clinical picture is significantly associated with the extent of the humoral response and subsequent glomerular involvement in the context of a chronic systemic inflammatory response. Subsequently, we measured the concentrations of IgG and IgM in the course of follow-up in some of the subjects included in the study and subjected to different anti-Leishmania treatment. By preliminary results obtained in our work it has been possible to state that in the course of treatment, the concentrations of these immunoglobulins undergo a progressive reduction thus confirming the effectiveness of anti-Leishmania treatment not only in the remission of clinical symptoms but also in the restoration of normal humoral response.

Anatomic study of the feline bronchial and vascular structures with a 64 detector-row computed tomography

Multidetector row computed tomography over the last decade is commonly used in veterinary medicine. This new technology has an increased spatial and temporal resolution, could evaluate wider scanning range in shorter scanning time, providing an advanced imaging modality. Computed tomography angiographic studies are commonly used in veterinary medicine in order to evaluate vascular structures of the abdomen and the thorax. Pulmonary pathology in feline patients is a very common condition and usually is further evaluating with computed tomography. Up to date few references of the normal computed tomographic aspects of the feline thorax are reported. In this study a computed tomographic pulmonary angiography (CTPA) protocol is reported in normal cats and is compared with the up to date anatomical references. A CTPA protocol using a 64 MDCT in our study achieved high resolution images of the pulmonary arteries, pulmonary veins and bronchial lumen till the level of minor segmental branches. Feline pulmonary bronchial parenchyma demonstrates an architecture of mixed type with a monopedial model observed in the most anatomical parts and the dichotomic aspect is seen at the accessory lobe. The arterial and venous architecture is similar to the bronchial. Statistical analysis demonstrates the linear correlation of tracheal diameter to the felines weight. Vascular variations were noticed. The pulmonary venous system enters into the left atrium through three ostia (left cranial ostia: consisted of the anastomosis of the cranial and caudal portion of the left cranial pulmonary vein; right ostia: consisted of the anastomosis of the right cranial and middle pulmonary vein; and the caudal ostia: consisted of the anastomosis of the right and left caudal pulmonary vein). In conclusion CTPA is applicable in feline patients and provides an excellent imaging of the pulmonary arterial, venous and bronchial system till the level of minor segmental branches.

Utilzzo dei parametri ecocardiografici tradizionali ed a Doppler tissutale nella valutazione della disfunzione atrio-ventricolare in cani affetti da malattia mitralica cronica / Use of standard and tissue Doppler echocardiographic parameters for the evaluation of atrioventricular dysfunction in dogs with chronic mitral valve disease

In questo lavoro ci siamo posti come obiettivo lo studio della disfunzione atrio-ventricolare mediante tecniche ecocardiografiche avanzate (come il Tissue Doppler Imaging - TDI) in cani affetti da malattia mitralica cronica (MVD). Una prima parte è volta alla valutazione della funzionalità diastolica del ventricolo destro. Ci siamo proposti di analizzare la funzione del ventricolo destro in cani affetti da malattia del cuore sinistro per comprendere se quest’ultima possa condizionare direttamente la performance del settore cardiaco controlaterale. I risultati più importanti che abbiamo riscontrato sono: l’assenza di differenze significative nella disfunzione sisto-diastolica del ventricolo destro in cani con MVD a diverso stadio; la diretta correlazione tra le variabili TDI di funzionalità del ventricolo destro con il grado di disfunzione del ventricolo sinistro, come indicatori di interdipendenza ventricolare; ed infine il riscontro di una maggior tendenza ad alterazioni diastoliche del ventricolo sinistro in cani con ipertensione polmonare. A quest’ultimo proposito, per quanto riguarda le variabili TDI, il rapporto E/e’ dell’anulus mitralico laterale e settale è risultato avere una differenza significativa tra i cani con ipertensione polmonare e quelli privi di ipertensione polmonare (P<0,01). Nel secondo studio abbiamo applicato il TDI per l’analisi della funzione sisto-diastolica dell’atrio sinistro. Il lavoro è stato articolato in una parte di validazione della metodica su cani normali ed una su animali affetti da MDV. I risultati ottenuti mostrano che la valutazione ecocardiografica delle proprietà di deformazione dell’atrio sinistro basata sul TDI è attuabile e riproducibile nel cane. Abbiamo fornito dei valori di normalità per questa specie e confrontato questi dati con quelli ricavati in cani portatori di MVD. Le differenza tra le varie classi di malattia, nei diversi gradi di dilatazione atriale, sono risultate limitate, ma abbiamo individuato delle correlazioni tra i parametri TDI ed alcune variabili di funzionalità atriale. / The aim of this study is to evaluate the atrioventricular function using brand new echocardiographic techniques, such as Tissue Doppler Imaging (TDI) in dogs affected by chronic mitral valve disease (MVD). The first part of the project is focused on the analysis of right ventricular function in dogs affected by left sided heart failure, in order to better understand if the dysfunction of one ventricle might interfere with the function of the contralateral one. The main results were: absence of significance sisto-diastolic right ventricular dysfunction in dogs with different stages of MVD; direct relationship between right ventricular TDI variables and left ventricular functional parameters; lastly, the presence of more severe left ventricular diastolic abnormalities in patients with different degrees of pulmonary hypertension. In particular, the TDI variable mitral annulus E/e’ ratio was directly correlated to pulmonary artery pressures and was a useful indicator in predicting presence of this condition. In the second study we used TDI to analyze global and regional function of the left atrium in normal dogs and in dogs with MVD and different degrees of left atrial dilation. The results showed that left atrial TDI is feasible and reproducible in dogs and we proposed some ranges of normality for this specie. We found a few differences in TDI left atrial variables between normal and diseased animals, however we identified some correlations between TDI parameters and the traditional echocardiographic left atrial functional variables (such as left atrial diameter, transmitral waves flow velocity). In conclusion we verified that new TDI based echocardiographic techniques are useful in the noninvasive evaluation of segmental cardiac function in normal and diseased dogs.

Ecografia bidimensionale e con mezzo di contrasto nello studio delle patologie gastro-intestinali del gatto / Use of conventional and contrast-enhanced ultrasonography for the evaluation of gastro-intestinal disorders in the cat

L’ecografia del tratto gastroenterico è una delle metodiche d’elezione nella valutazione diagnostica delle patologie gastrointestinali nel gatto. In questa tesi dottorale sono presentati i risultati di tre studi in cui l’ecografia convenzionale e con mezzo di contrasto è stata impiegata in gatti sani o con patologie gastroenteriche. Lo scopo del primo studio, prospettico, è stato quello di determinare lo spessore ecografico dei singoli strati di parete nell’intestino tenue in una popolazione di gatti sani. Lo strato mucoso è risultato significativamente più spesso nel duodeno e nel digiuno, per la maggiore grandezza dei villi in queste porzioni dell’intestino tenue. A livello dell’ileo, gli strati di maggior spessore sono risultati quello sottomucoso, per l’abbondante presenza di aggregati linfoidi, e quello muscolare, a causa delle caratteristiche anatomo-funzionali di sfintere che questo tratto intestinale svolge. Il secondo progetto, retrospettivo, nasce dalla collaborazione tra due centri universitari, uno italiano e uno americano, con l’obiettivo di confrontare lo spessore della tonaca muscolare intestinale in gatti affetti da Inflammatory Bowel disease (IBD) o da neoplasie intestinali. In questo studio, l’ipertrofia della tonaca muscolare (ITM) è stato maggiormente osservato in gatti con IBD rispetto a gatti con neoplasie intestinali, ma non sono state evidenziate differenze di spessore della tonaca muscolare tali da poter differenziare le due patologie. Lo scopo del terzo progetto, prospettico, è stato quello di descrivere il pattern di perfusione parietale del piccolo intestino, valutato mediante uso di mezzo di contrasto ecografico, in gatti con ITM associata a IBD. In tutti gli animali studiati, l’ITM si è associato a una modesta assunzione del mezzo di contrasto rispetto agli altri strati della parete intestinale. Questi risultati confermano che l’ITM che si osserva in gatti con IBD non è associato a significativi aumenti della vascolarizzazione di tale strato parietale. / Diagnostic ultrasound is one of the preferred diagnostic tool for the evaluation of gastro-intestinal disorders in cats. In this doctoral thesis, the results of three different studies carried out using conventional and contrast enhanced ultrasonography for the evaluation of the gastro-intestinal tract of normal cats and cats affected by gastrointestinal disorders. The goal of the first study was to measure the thickness of the different intestinal layers in clinically normal cats. The thickness of the mucosal layer was significantly greater in the duodenum and jejunum, because of the greater size of intestinal villi. The submucosal and muscular layers were significantly thicker in the ileum, because of the abundant lymphoid aggregates and the physiological role of anatomical barrier between the small and large intestine, respectively. The second study was carried out as a collaboration between one Italian and one American center. The aim of the study was to measure ultrasonographically the thickness of the intestinal muscular layer in cats with inflammatory bowel disease (IBD) and in cats with intestinal neoplasia. A thickened intestinal muscular layer was more frequently observed in cats with IBD but the thickness of this layer was not helpful in distinguishing cats affected by IBD or intestinal neoplasia. The aim of the third study was to describe the pattern of ultrasonographic contrast enhancement in cats with thickened intestinal muscular layer associated with IBD. In all cats with IBD, the increased thickness of the intestinal muscular layer was poorly associated with contrast enhancement compared to the other intestinal layers. Results confirm that thickening of the intestinal muscular layer in cats with IBD is not associated with significant increased vascularization of this intestinal layer.
