O presente estudo objetiva mostrar ao Cirurgião Dentista, os quesitos necessários para se formar um Odontólogo apto a praticar a sedação inalatória, bem como os itens de segurança e de respaldo legal que deverão compor o seu ambiente de trabalho ao se utiliza desta técnica. O estudo foi fundamentado a partir de revisão de literatura sobre o tema, tendo como metodologia o estilo de Vancouver. A analgesia inalatória pela mistura gasosa de óxido nitroso e oxigênio constitui um excelente instrumento no controle da dor e da ansiedade dos pacientes durante o tratamento odontológico. É importante observar que a técnica de analgesia inalatória pela mistura gasosa de óxido nitroso e oxigênio deve ser realizada por meio de normatização técnica para garantir a eficiência da técnica e evitar transtornos. O roteiro proposto no trabalho visa apresentar as características de segurança e detalhamento das especificações técnicas dos estabelecimentos de assistência odontológica para o emprego da técnica de analgesia inalatória pela mistura gasosa de óxido nitroso e oxigênio. / This study has as objective to suggest a number of propositions in order to prepare and develop a dental office to perform conscious sedation by inhalatory analgesia using nitrous oxide/oxygen mixture. It was based on the revision of literature about the subject, having as methodology the Vancouver style. The conscientious sedation constitutes an excellent instrument in the control of the patient?s anxiety during dental treatment. It also will show to the Dentist the necessary requirements to the training of his personal and himself. It is important to observe that the technique itself must follow normative rules to guarantee its efficiency. The scope considered in the work aims present characteristics of security and detailed specifications to establish dental assistance for the use of conscious sedation by nitrous oxide/oxygen mixture.

The management of a safe and cost effective conscious sedation unit

Philosophiae Doctor - PhD / Conscious sedation or moderate sedation and analgesia is an effective and popular alternative option for procedures outside the operating theater. If conscious sedation is a viable alternative to general anaesthesia then we as sedation practitioners must use safe sedation techniques in facilities that meet all the requirements for safe practice. Three studies were done to determine the safety and efficacy of conscious sedation outside the operating theatre. In the first study post sedation satisfaction in one hundred children aged 3-9 years was evaluated. It was extremely important to determine whether the combination of midazolam, ketamine and propofol, called an advanced sedation technique (SASA, 2015), can be safely used for paediatric sedation outside the operating theatre. The incidence of side-effects after conscious sedation using multiple drugs were documented. It is clear that intravenous sedation with midazolam, ketamine and propofol is safe and effective to use. There may be side effects but they are not long lasting and usually not life-threatening. In the second study intravenous sedation was administered to 447 adults (aged 18 years and older) using fentanyl (sublimazeR), ketamine (ketalar), midazolam (dormicum) and propofol (Diprivan) (FKMP) called an advanced sedation technique. Post sedation satisfaction, post sedation recovery on arrival home, and the relationship between side effects and different dental procedures were evaluated. The results of the study show that side effects are possible, and can be expected, when we use sedative and analgesic drugs for sedation. However, we report a low incidence of side effects when we compare it with other studies in literature as mentioned. It is known that the use of combinations of drugs may cause unforeseen synergistic pharmacological effects which can be lifethreatening. Our results show that the drugs used can be safely used for advanced sedation techniques. In trying to demonstrate the safety of sedative and analgesic agents used during sedation we looked at the haemodynamic parameters, duration of sedation, pulse rate and systolic blood pressure, in the third study. The sedation records of 335 patients for dental surgery were assessed for the period 2010 – 2011. Our results show the mean Duration of sedation is substantially and statistically significantly greater with combination FKMP than with the other combinations. The mean duration of sedation is not significantly different between ketamine and propofol (KP) and fentanyl, ketamine and propofol (FKP) (Figure 10). The use of polypharmacy regarding the combination of drugs, specifically FKMP, will cause a longer duration of sedation. This has implications for safety, as well as the side effect profile during and after sedation. When we use combinations of drugs patients were more comfortable which shows that we do not yet have a single drug that has all the characteristics of an ideal drug for sedation. Different combinations of drugs are used by other practitioners with a higher incidence of side effects. It is difficult to explain the higher values of blood pressures when all four drugs were used. It may have been a ketamine effect, although one would not expect this when using propofol with ketamine. In clinical terms the higher blood pressures are no reason for concern as all our patients were classified as ASA I and II. Our research study support the view that ketamine can be used safely outside the operating theatre with exciting possibilities for Third World countries for procedures outside the operating theatre. Sedation can be considered a reasonable alternative to general anaesthesia for certain surgical procedures in the Third World. Sedation will be an attractive option not only as far as costs are involved but also the availability of sedation providers. The important lesson from all the results is that sedation providers must be trained in procedural sedation as defined by all international sedation guidelines. We proved in this research study that sedation can be done safely, however we need to make a contribution to train sedation providers. Sedation will become an attractive alternative to general anaesthesia because of the low side-effect profile and high patient satisfaction. It is interesting that few studies are available that looked at this aspect of sedation. It is clear that a high side-effect profile can contribute to an unsafe sedation technique. Severe nausea and vomiting can cause numerous haemodynamic disturbances and dehydration. Our research study support the findings of the study by Lapere et al., (2015) that there is a high rate of patient satisfaction, and a low side-effect profile during and after sedation. This is an extremely important research study and the results are crucial as far as an option for healthcare in developing countries. Sub-Saharan Africa is a densely populated and resource poor subcontinent that provides unique challenges in patient care. These challenges include a lack of facilities and staff for the performance of operative as well as non-operative procedures. In conclusion, we feel that we are part of Sub-Saharan Africa with all the problems mentioned as far as provision of healthcare is concerned. This research study can make a crucial contribution to safe and cost-effective management of healthcare in Africa for procedures outside the operating theatre.

Conscious Sedation of the pediatric dental patient: a comparison of meperidine versus butorphanol

Indiana University-Purdue University Indianapolis (IUPUI) / Treating pediatric dental patients four years old and younger can be difficult at times due to patient behavior. Conscious sedation has been employed as a means to control pediatric dental patients for several years. Butorphanol tartrate has been used safely for pain control in pediatric patients for several years, but has never been used for sedating pediatric dental patients. The purpose of this study is to compare the behavioral and physiologic effects of conscious sedation on pediatric dental patients using intramuscular meperidine and an equipotent dosage of intramuscular butorphanol. Forty conscious sedations of ASA I pediatric dental patients between the ages of 13 and 60 months were accomplished using either 2.0 mg/kg of intramuscular meperidine or 0.03 mg/kg of intramuscular butorphanol. Each sedation was videotaped and three viewers viewed the videotapes rating them with a computer program (ACS) involving a four-code behavior rating scale. The tlrree viewers rated patient behavior for each sedation also with a form with global rating, categorical, and dichotomous scales. Physiologic signs of oxygen saturation, blood pressure, heart rate, and respiration rate were monitored at baseline and every 5 minutes during treatment. The operator also rated the sedation patient behavior with a form that had pre-treatment Frankl, post-treatment Frankl, global rating categorical, dichotomous, and sedation success rating scales. The two groups demographic data, physiologic data, ACS data, the three viewer's behavior rating form, and the operator's behavior rating form were analyzed for any statistically significant differences between the groups. The statistical analysis of the demographic data revealed a statistically significant trend in the butorphanol group toward extractions. The meperidine group had a statistically significant higher mean oxygen saturation during treatment (99.63 percent) than the butorphanol group (99.20 percent). The butorphanol group spent significantly more time in the annoyed ACS behavior rating code and showed a trend toward less time spent in the quiet ACS behavior rating code. There were no statistically significant differences in the three viewers ratings of global rating, categorical, and dichotomous scales. The operators' ratings showed the meperidine group had a statistically significant better global rating than the butorphanol group. Overall butorphanol appears to be equal clinically to meperidine in physiologic effects and patient behavior effects. No adverse effects occurred with either medication. Butorphanol may be offered as an alternative sedative agent to other narcotic sedative agents with more side effects.

O medo ou a ansiedade são emoções frequentemente presentes nos pacientes que nos procuram na clínica odontológica para tratamento odontológico. Estes pacientes são denominados pacientes odontofóbicos. O objetivo deste estudo é avaliar o efeito ansiolítico do Maleato de midazolam e/ou Óxido Nitroso usado para promover sedação consciente nestes pacientes para se observar posteriormente a percepção dos usuários . Foram colhidas suas narrativas descrevendo qual a contribuição do sedativo usado no alívio da ansiedade. O presente estudo foi realizado como pesquisa de campo pautada na técnica da representação social ou discurso do sujeito coletivo proposto por Lefevre et al. (2000). Foram entrevistados de 10 pacientes diagnosticados como odontofóbicos, suas respostas catalogadas e transformadas em resultados qualitativos. De todos os sujeitos da pesquisa, todos aprovaram a utilização de ansiolíticos durantes procedimentos cirúrgicos. Sendo assim, dentro desta amostra, concluímos que a sedação consciente por meio de sedativos é uma excelente forma de se controlar a ansiedade frente ao tratamento odontológico / Fear and anxiety are emotions often present in patients who come to us in the dental clinic for treatment. These patients suffer of dental anxiety. The objective of this study is to evaluate the anxiolytic effects of midazolam maleate and / or Nitrous Oxide used to promote conscious sedation in these patients. We collected the narratives describing the contribution of the sedative used to relieve their anxiety. This study was conducted as a field research based in the technique of representing social or collective subject discourse proposed by Lefevre et al. (2000). We interviewed 10 patients with a dental fear background, their narratives processed in qualitative results. After the surgery all patients described that the procedures were softer than usual, and by this results we can conclude that the use of sedatives in oral surgery should be encouraged.

Cetamina intranasal para sedoanalgesia na punção venosaperiférica em pacientes pediátricos : estudo randomizado, duplo cego e placebo controlado

Objetivos: Verificar eficácia da cetamina intranasal na sedação de crianças para punção venosa. Métodos: Estudo randomizado, duplo-cego, placebo controlado realizado no Hospital de Clínicas de Porto Alegre entre janeiro e agosto de 2016. Estudo aprovado pela comissão de ética em pesquisa da instituição. Incluídas crianças que necessitasse de punção venosa, sendo randomizadas a receber cetamina IN 4mg/Kg ou solução fisiológica no grupo Placebo. Os grupos foram comparados quanto: tempo de punção, facilidade do Enfermeiro para realizar o procedimento, eventos adversos, alterações dos sinais vitais e percepção do acompanhante. Resultados: Foram incluídas 39 crianças (21 Intervenção vs 18 Placebo) sem diferenças quanto à idade, sexo, peso, motivo da internação e experiência profissional. A mediana da idade foi 19,8 vs 15,8 meses (Intervenção vs. Placebo) e a do peso foi 10 vs. 11,3Kg. A Cetamina reduziu o tempo de punção (23,0 vs 67,5 segundos; p=0,01), deu maior facilidade ao Enfermeiro para realizar o procedimento (p=0,00009). A cetamina induziu uma maior sonolência 15 minutos após (p=0,003) e reduziu o número de pessoas para contenção da criança (p=0,025). Sem diferença entre os grupos nas alterações dos sinais vitais e eventos adversos. Evento adverso observou-se em 29% das crianças do grupo cetamina e 17% do grupo placebo, sendo irritabilidade o mais comum em ambos. Em 81% do grupo Intervenção, o acompanhante afirmou que a criança ficou mais calma (p=0,0003). Conclusões: Cetamina intranasal (4mg/Kg) reduz o tempo de punção venosa, facilitando o procedimento para o enfermeiro, diminuindo o número de pessoas envolvidas e permitindo um ambiente tranquilo. / Objectives: To verify the efficacy of intranasal ketamine as sedative agent for venous access in children. There is little information about the ability of a variety of scales in properly evaluate this behavior. The aim of this study was to analyze the characteristics of three scales,e,g,. Houpt Scale, Ohio State University Behavioral Rating Scale (OSUBRS) and Venham Behavior Rating Scale, to measure the behavior of preschool children during dental procedures under sedation. This study was an observational study nested to a randomized clinical trial (NCT02284204), which included 27 children between 4 and 6 years of age with early childhood caries and negative behavior in previous consultation. Participants underwent dental treatment under moderate sedation with midazolam and ketamine with or without addition of sevoflurane. The sessions were videotaped and five trained and calibrated observers watched the movies to record the behavior of children according to each scale. The data showed non-normal distribution (Shapiro-Wilk, P> 0.05); descriptive and correlation analyses were made (Spearman test). A total of 1,209 minutes of videos were observed and analyzed. Global scales (Houpt overall behavior and Venham) had closer scores of good behavior. The most frequent scores for Houpt were “no crying” and “no movement”, and for OSUBRS, “quiet”. Statistically significant correlations (P <0.05) occurred between Houpt overall behavior and Venham; Houpt overall behavior with Houpt categories movement (scores 2 and 4) and cry (scores 1, 2 and 4); Venham with Houpt movement (score 4) and cry (scores 2 and 4). OSUBRS scores 1 and 4 had high correlation coefficients with Houpt overall behavior and Venham. The Houpt overall behavior and the Venham Behavior Scale were highly correlated to measure behavior of children in dental treatment under sedation. OSUBRS showed better correlation with global scales compared to the categories of crying and movement of Houpt scale. / Para a avaliação da eficácia de um agente sedativo para tratamento odontológico de crianças, uma das medidas utilizadas é o comportamento, e isso é feito através de escalas observacionais. No entanto, há poucas informações sobre a capacidade de avaliar corretamente esse comportamento. O objetivo deste estudo foi analisar as características de três escalas - Escala comportamental de Houpt, Ohio State University Behavioral Rating Scale (OSUBRS) e Escala comportamental de Venham - em mensurar o comportamento de crianças pré-escolares durante procedimento odontológico sob sedação. Esse trabalho foi um estudo observacional aninhado a um ensaio clínico randomizado (NCT02284204) que incluiu 27 crianças entre 4 e 6 anos de idade com cárie dentária e comportamento resistente ao tratamento odontológico. Os participantes foram submetidos a tratamento odontológico sob sedação moderada com uso de midazolam e cetamina com adição ou não de sevoflurano. Os atendimentos foram gravados em vídeo e cinco observadores treinados e calibrados assistiram para registrar o comportamento das crianças segundo cada escala. Os dados apresentaram distribuição não-normal (Shapiro-Wilk, P>0,05); foram feitas análises descritivas e de correlação (teste de Spearman). Um total de 1.209 minutos de vídeos foram observados e analisados. Escalas globais (Houpt comportamento geral e Venham) apresentaram escores mais próximos de bom comportamento. Os escores mais frequentes para Houpt foram de pouco choro e movimento e, para OSUBRS, comportamento quieto. Correlações estatisticamente significantes (P<0,05) ocorreram entre Houpt comportamento geral e Venham (rho= -,87); Houpt comportamento geral com Houpt categorias movimento (escore 2 e 4) e choro (escores 1, 2 e 4); Venham com Houpt movimento (escore 4) e choro (escores 2 e 4). OSUBRS escores 1 e 4 tiveram altos coeficientes de correlação com Houpt comportamento geral e Venham. Concluiu-se que a escala de Comportamento Geral de Houpt e a Escala Comportamental de Venham foram altamente correlacionadas para aferir comportamento de crianças em tratamento odontológico sob sedação. OSUBRS apresentou melhor correlação com as escalas globais, se comparado com as categorias de choro e movimento de Houpt.
