Objectives: To analyze the profile of prescribing cholinesterase inhibitors and other medications in elderly patients with Dementia of Alzheimer's disease (DAD) met in Center of attention for the elderly in the city of Bauru/SP. Methods: Study of transverse, descriptive and analytical design, conducted with elderly patients with Dementia of Alzheimer's disease (DAD), in the Municipal Program of Attention to the older persons (PROMAI), from April to August 2015. Participants: 81 elderly (66 women and 15 men) who were monitoring the Municipal program of Attention to the Elderly (PROMAI). Results: Of the elderly 81.5% belonged to the female, 92.6% were 75 years or older, 64.2% were unmarried, 58% had up to 4 years of schooling, 60.5% had incomes up to 1 minimum salary, 60.5% had as main caregiver daughters and 90.1% lived with a family member. Among the most prevalent comorbid associated with dementias found Hypertension (29.6%), Diabetes mellitus (13.4%), Hypothyroidism (8.4%) and dyslipidemias (7.8%). Cognitive ability expressed by the MMSE was an average of 14.5 points. The polypharmacy was present in 63% of the elderly and among the elderly 77.8% made use of some MPI. The frequency of prescription of (the) ICH was: 54.9% rivastigmine, 33.3% Donepezil and 11.8% galantamina. Memantine had a prevalence of prescription of 2.5%. The reasons of the non-use of treatment for DAD, the undesirable side effects accounted for 28% of the cases. The elderly studied 91% had access to treatment with the ICH trough medication program CEAF. The CDR (Clinical Dementia Rating) was not found in all the analyzed records. Conclusion: Polypharmacy was present in 63% of the elderly studied and 77.8% made use of some MPI. The polypharmacy had statistically positive association with the number of comorbid (p<0.01), education (p < 0.01), hypertension (p<0.01) and melittus diabetes (p<0.01) and the use of MPI presented statistically positive association with polypharmacy (p < 0.01). Of the medications to treatment DAD the rivastigmine appears with the higher prevalence of use, 34.6% and 30.9% did not use any treatment for DAD. Undesirable side effects and advanced-stage disease, 28%, appear as the main reasons for non-use of medicines

Podíl logopeda na komplexní péči o osoby se syndromem demence / Participation of the speech therapist on comprehensive care of people with the syndrome of dementia

Zábranská, Nicola January 2016 (has links)
The dissertation deals with issues of the dementia syndrome and the speech therapy as a part of its therapy. It describes basic types of dementia, risk factors affecting its formation, diagnostics including the most famous diagnostic tests, the general treatment of dementia, and institutions, where the care for people with dementia could be realized. Subsequently it namely focuses on the speech intervention for this group of people. As a part of this dissertation a workbook with the plan for the treatment of impaired communication abilities in dementia was created. Tests were carried out with four individuals with dementia caused by Alzheimer's disease. The results, even evaluated just subjectively, show that this plan might be appropriate as a cornerstone of speech intervention for people with dementia, which in our country is not very broadly spread in our country. Keywords: dementia, cognitive abilities, speech therapy, Alzheimer's disease, communication, homes with special regime

Bayesian belief networks for dementia diagnosis and other applications : a comparison of hand-crafting and construction using a novel data driven technique

Oteniya, Lloyd January 2008 (has links)
The Bayesian network (BN) formalism is a powerful representation for encoding domains characterised by uncertainty. However, before it can be used it must first be constructed, which is a major challenge for any real-life problem. There are two broad approaches, namely the hand-crafted approach, which relies on a human expert, and the data-driven approach, which relies on data. The former approach is useful, however issues such as human bias can introduce errors into the model. We have conducted a literature review of the expert-driven approach, and we have cherry-picked a number of common methods, and engineered a framework to assist non-BN experts with expert-driven construction of BNs. The latter construction approach uses algorithms to construct the model from a data set. However, construction from data is provably NP-hard. To solve this problem, approximate, heuristic algorithms have been proposed; in particular, algorithms that assume an order between the nodes, therefore reducing the search space. However, traditionally, this approach relies on an expert providing the order among the variables --- an expert may not always be available, or may be unable to provide the order. Nevertheless, if a good order is available, these order-based algorithms have demonstrated good performance. More recent approaches attempt to ''learn'' a good order then use the order-based algorithm to discover the structure. To eliminate the need for order information during construction, we propose a search in the entire space of Bayesian network structures --- we present a novel approach for carrying out this task, and we demonstrate its performance against existing algorithms that search in the entire space and the space of orders. Finally, we employ the hand-crafting framework to construct models for the task of diagnosis in a ''real-life'' medical domain, dementia diagnosis. We collect real dementia data from clinical practice, and we apply the data-driven algorithms developed to assess the concordance between the reference models developed by hand and the models derived from real clinical data.

Vybrané metody sociální práce s nesoběstačnými klienty ve zdravotnických zařízeních / The selected methods of social work with clients self - non sufficient in health care facilities

Kiliánová, Hana January 2016 (has links)
The main goal of the thesis entitled "The selected methods of social work with clients self-non sufficient in health care facilities" was to describe and to analyze methods used in activation of the elderly suffering from dementia. It draws a comparison of different settings and explores workers's attitude to the methods of activation. The text of the thesis was mapped out into five chapters. The first chapter was set into the context of old age and ageing. The second chapter is concerned with the issues of self-sufficiency and dependency of old age and other related issues. The topic of the third chapter is old age in relation with memory. It defines memory and memory disorders in the elderly suffering from dementia. The fourth chapter is crucial and gives an idea of non-pharmacological and other possible activities which are eligible for application in practice and are intended to reduce cognitive disorders of cognitive functions and behavior disorders. The theoretical part enters into research carried out in four facilities which care for clients with dementia. The research section is represented by a qualitative research where methods of a half structured interview and observation were opted for. The interviews were realized with eight practicing experts. The outcomes gathered from the...
