Postula-se que, por ser puramente restritiva, a gastrectomia vertical apresente resultados inferiores à derivação gástrica em Y de Roux. OBJETIVOS: Avaliar o perfil lipídico de obesos submetidos a gastrectomia vertical e a derivação gástrica em Y de Roux; Comparar a efetividade do controle lipídico dos pacientes submetidos a gastrectomia vertical e a derivação gástrica em Y Roux. METODOLOGIA: Trata-se de um estudo tipo coorte retrospectiva, de análise prospectiva, em que foram avaliados 334 pacientes submetidos a gastrectomia vertical e 178 pacientes submetidos derivação gástrica em Y de Roux. Realizadas dosagens séricas de colesterol total, LDL, HDL e triglicerídeos no pré-operatório e com 3, 6, 12 e 24 meses de seguimento. RESULTADOS: No pré-operatório 80% dos pacientes tinham no mínimo uma anormalidade no perfil lipídico. Dois anos após a cirurgia houve melhora do colesterol total, LDL, HDL e triglicerídeos na grupo submetido a DGYR. No grupo submetido a GV, após 2 anos houve melhora dos níveis de colesterol total, HDL e triglicerídeos, apenas. CONCLUSÃO: Ambas as técnicas resultaram em melhorias no perfil lipídico, porém a derivação gástrica em Y de Roux foi mais efetiva. / INTRODUCTION: Lipid disorders are common in obese patients, being an important risk factor for coronary syndromes. Bariatric surgery can reduce this risk. Sleeve gastrectomy, a purely restrictive technique, seems to be worse than Roux-en-Y gastric bypass to improve lipid profile. OBJECTIVES: To evaluate lipid profile of obese who underwent sleeve gastrectomy and Roux-en-Y gastric bypass; To compare lipid improvement of patients who underwent sleeve gastrectomy and Roux-en-Y gastric bypass. METODOLOGY: 334 patients who underwent sleeve gastrectomy (SG) and 178 patients who underwent Roux-en-Y gastric bypass (RYGB) were evaluated in a retrospective cohort with prospective analysis. Serum levels of total cholesterol, LDL, HDL and triglycerides were measured preoperatively and at 3, 6, 12 and 24 month of follow-up. RESULTS: At baseline 80% of patients had at least one abnormality among the lipid measurements. Two years after surgery, total cholesterol, LDL, HDL and triglycerides levels improvement were seen in the group who underwent RYGB. In the group who underwent SG, total cholesterol, HDL and triglycerides levels improved within two years after surgery. CONCLUSION: Both techniques improved lipid profiles, but Roux-en-Y gastric bypass achieved better rates
