It is believed that several factors may influence adherence to treatments, but few studies address the issue. With the objective of evaluating the main factors that may influence the adherence of the elderly to drug treatments, this research was proposed. The methodology chosen was based on a cross-sectional, mixed-design study that evaluated the drug adherence of elderly people living in the city of São Paulo in the year 2016. A questionnaire was applied with open and multiple choice questions, which adressed sociodemographic and clinical aspects, the patient’s perception of their disease and treatment, the influence of the religion in adherence to the treatments and what they expect from the physician in relation to the prescription. It was found that among the 66 elderly people evaluated, the mean age was 72 years, with a predominance of females (72.7%). On average, it was observed that the elderly reported being followed by three doctors and used about four drugs. It was observed that forgetting to take the medication was directly associated with inappropriate medication use. Although 68.2% of the sample stated that they were adequately using the prescribed drugs, 30 elderly patients indicated that they stopped using medication on their own, demonstrating that they did not understand that these behaviors also meant inappropriate use of the medication. In the qualitative analysis, it was noted the strong presence of medicalization, being considered the use of drugs normal to aging. It is concluded that several factors can interfere in adherence to drug treatments among the elderly and that problems related to medications are not only the responsibility of the patient. There is a growing need for commitment from health professionals and government agencies in chronic care programs, making the elderly a participatory subject in their process of illness and treatment / O mundo está passando por uma transição demográfica e epidemiológica, com aumento do número de idosos e, concomitante, acréscimo de doenças crônicas não transmissíveis, o que acarreta um aumento significativo no consumo de medicamentos por essa faixa etária. Acredita-se que vários fatores podem influenciar a adesão aos tratamentos, mas poucos estudos abordam o tema. Com o objetivo de avaliar os principais fatores que podem influenciar a adesão dos idosos aos tratamentos medicamentosos, essa pesquisa foi proposta. A metodologia escolhida valeu-se de estudo transversal, de desenho misto, que avaliou a adesão medicamentosa de pessoas idosas, residentes no município de São Paulo no ano de 2016. Realizou-se aplicação de questionário com perguntas abertas e de múltipla escolha, onde foram abordados aspectos sociodemográficos e clínicos, a percepção que o paciente tem de sua doença e tratamento, influência da religião na adesão aos tratamentos e o que esperam do médico em relação à prescrição. Constatou-se que, dentre os 66 idosos avaliados, a média etária foi 72 anos, havendo predominância do sexo feminino (72.7%). Em média, observou-se que os idosos relatavam ser acompanhados por três médicos e utilizavam cerca de quatro fármacos. Observou-se que se esquecer de tomar a medicação esteve diretamente associado ao uso inadequado de medicamentos. Apesar de 68.2% da amostra afirmar utilizar adequadamente os medicamentos prescritos, 30 idosos indicaram interromper o uso medicamentoso por conta própria, demonstrando não terem o entendimento de que essas condutas também significam uso inadequado da medicação. Na análise qualitativa, notou-se a forte presença da medicalização, sendo considerado o uso de medicamentos normal ao envelhecimento. Conclui-se que vários fatores podem interferir na adesão aos tratamentos medicamentosos entre os idosos e que os problemas relacionados aos medicamentos não são apenas de responsabilidade do paciente. Existe a necessidade crescente de empenho dos profissionais de saúde e órgãos governamentais, nos programas de atenção aos doentes crônicos, tornando o idoso um sujeito participativo no seu processo de doença e tratamento
