Genetic investigation of ocular inflammatory disease-uveitis.

Despite considerable progress in treatment, safe and effective management is still a clinical challenge. Uveitis can be anatomically classified as anterior, intermediate, posterior, and panuveitis, anterior uveitis (AU) is the most common form. Based on etiology, uveitis can be also categorized as infectious and noninfectious subtypes. Uveitis is generally accepted as an inflammatory condition and regulated by various endogenous immunological mechanisms. Moreover, uveitis can occur in individuals with genetic predisposition coupled with environmental factors. / Based on our understanding of the critical checkpoints in the uveitis process, two major pathways, T-cell response and complement system, appear to be most related to uveitis in the molecular level. We therefore target on several molecular factors involved in the two major pathways to evaluate the genetic impact on susceptibility to uveitis, reveal potential disease mechanisms, and discover diagnostic markers. / We recruited a total of 501 unrelated Chinese individuals at the Hong Kong Eye Hospital and the Prince of Wales Hospital in Hong Kong, including 98 AU patients, 95 patients with noninfectious intermediate and posterior uveitis (NIPU), and 308 control subjects aged ≥ 50 years without major eye diseases or any systemic immune-related disorders. Clinical information and demographic conditions of the patients were documented. We moved on to depict the disease profile and estimate the contribution of each complement activation pathway in uveitis process. We also conducted interaction analysis among the complement factor genes to reveal the putative clinical markers for uveitis. Genotype-phenotype correlations were performed to explore their relationships with the disease severity and progression. / Study 1: In a systemic review to explore recent genetic findings in uveitis susceptibility, we found several genes persistently associated with uveitis and involved in various pathways. They are genes expressing interleukin, chemokine, tumor necrosis factor, and genes involved in complement and oxidation pathways. Genetic polymorphisms were selected based on the immunological and inflammatory properties of uveitis. (1) Interleukin and CFH genes, involving in the T-cell response and complement system respectively. (2) CFB, as a competitor of CFH, involved in the alternative pathway of complement cascade together. (3) Investigation of C1INH (SERPING1) in uveitis, with a view to elucidating the involvement of the classic pathway of complement cascade in uveitis development. (4) Evaluation of C3 and C5 genes in uveitis, due to their respective role of center component and downstream factor in the complement cascade. / Study 2: Genetic variations in the CFH, KIAA1109 and IL27 genes were examined. Our results showed an association between AU and CFH polymorphisms (rs800292 and rs1065489). The frequency of the CFH-rs800292 184G allele and GG homozygosity was higher in female patients than in controls. CFH-rs1065489 TT genotype was identified as a clinical marker associated with higher uveitis recurrence frequency. Interactions with HLA-B27 status in AU patients and different gender susceptibility were observed. In the second part of this study, these three candidate genes were examined in the other uveitic entity, NIPU, in our study cohort. CFH gene polymorphisms (rs1065489 and rs800292) were associated with NIPU patients. Specific association between KIAA1109-rs4505848 polymorphism and Behçet’s disease was identified. There was also gender specific genetic difference. The dominant genotype of KIAA1109-rs4505848 in male NIPU patients was significantly more frequent than in male controls (GG/AG vs. AA). / Study 3: CFB, a competitor of CFH and participated in the same complement alternative pathway, was investigated. SNP rs1048709 in the C2/CFB region was associated with AU, and this genetic influence was affected by HLA-B27 status. Furthermore, one haplotype block across CFB (AATA) significantly predisposed AU with an increased risk of 1.97 (95% CI: 1.41-2.76; Pcorr=0.0005). Joint effects of CFB-rs1048709 with (CFH-rs800292 and CFH-rs1065489) were identified to be at a risk of 7.48 and 7.0 respectively. In addition, patients carrying rs1048709 (AG) were predicted to develop a higher degree of anterior chamber cells and higher proportion of keratic precipitate (KP) during AU course. For NIPU, association with CFB was detected for a different SNP, rs4151657, in female patients only. / Studies 4, 5, and 6: Three candidate genes (SERPING1, C3 and C5) across the complement cascade were orderly evaluated in the whole study cohort of AU, NIPU and controls. They did not show any significant associations with both two uveitis entities, although multiple in-depth analyses have been performed. / Collectively, our results provide evidence for the involvement of the complement system in the disease pathogenesis of uveitis. CFH and CFB, involved in the complement alternative pathway, are identified as genetic risk factors for uveitis. Other complement pathway genes, SERPING1 (C1INH), C3 and C5, as well as IL2_21 region and IL27 involving in the T-cell response, confer either no or limited risk for the development of uveitis. Future genetic and immunologic investigations in uveitis should therefore be focused on the complement system and the alternative pathway. / Detailed summary in vernacular field only. / Detailed The watery eye

The notion surfaces that, as fish evolved, left the sea and colonised the land, they then needed not only a supply of tears for their ocular surface to function, but also a means by which tears could be eliminated. The fascinating study of abnormalities of this system, which are common, frequently debilitating, and can be challenging to manage, is the purpose of this thesis. An attempt has been made initially to assess our current state of knowledge in the Introduction. In subsequent chapters, new concepts including plerolacrima, temporal plical shift and videoreflective dacryomeniscometry are introduced; some of these should prove useful clinically. Quality of life in affected preoperative and postoperative cohorts is then assessed. Original publications have been achieved in all but one of the following subsections. Conjunctivochalasis in watery eye patients has been examined for the first time in a sizeable study clinically and histopathologically. The lateral tarsal strip procedure has been described as a new method of dealing with the punctal apposition syndrome. A new assessment for success in dacryocystorhinostomy (DCR) surgery, the Valsalva bubble test, has been carried out. Tear osmolality has been studied in watery eye cohorts and compared with normal controls; this study suggests that tear production decreases significantly with obstructed lacrimal outflow. Videoreflective dacryomeniscometry has shown that watery eye patients have a higher tear meniscus than normal controls, and that this normalises following DCR surgery. Lower eyelid tensometry has been shown in patients to be unchanged compared with normal controls. Computerised tomography of the lacrimal drainage region has been used to study patients with nasolacrimal duct obstruction. The detailed clinical mechanics of syringing the canaliculus have been described. Transcaruncular medial orbitotomy surgery for laxity of the medial canthal tendon has been tested in human cadavers, and evaluated in adult living humans. In summary, this thesis may be regarded as a tentative approach to a re-evaluation of the clinical management of the watery eye.

<p>SammanfattningFör att veta vad läsarna tittar på i en tidning har det gjorts flera studier med så kallade eye-tracking-metoder. Eye-tracking innebär att man mäter hur ögonen rör sig på en redigerad tidningssida för att se var blicken fastnar. En läsare spenderare ungefär tjugo minuter av sin tid till att gå igenom sin tidning och endast ett fåtal läser allt. I stället fastnar läsarnas ögon för olika beståndsdelar, till exempel bilder och grafik, som är de två som lockar mest till läsning tätt följt av rubriker.. Färg är en annan komponent som lockar ögat, oavsett om den är en del av texten eller grafiken. Utöver dessa beståndsdelar som lockar till läsning finns det vissa områden, så kallade hot-spots, på ett tidningsuppslag som tenderar att dra till sig mer uppmärksamhet.Syftet med den här undersökningen har varit att ta reda på vad Västernorrlands två största tidningar, Sundsvalls Tidning och Tidningen Ångermanland placerar på sina mest lästa områden, om det är bilder, grafik eller text samt vad texterna handlar om i stora drag. I den här undersökningen har vi också försökt ta reda på hur tankarna går kring redigeringen och placeringen av det redaktionella materialet på tidningens nyhetssidor.För att ta reda på detta har vi dels gjort kvantitativa innehållsanalyser av det material som placeras i de mest lästa områdena, hot-spotsen, i varje tidning samt gjort intervjuer med nyhetschef, redigeringschef och redigerare på båda tidningarna.Resultatet visar att Tidningen Ångermanland har fler och större bilder, mer färg och grafik i de undersökta områdena än Sundsvalls Tidning som i stället har fler rubriker, notiser och brödtext. När det gäller textinnehållet är kommunal- och samhällsinformation den dominerande genren i Tidningen Ångermanland medan Sundsvalls Tidnings texter mest handlar om brott, en genre som knappt förekommer i Tidningen Ångermanland. Sundsvalls Tidning har också fler texter som handlar om politik, ekonomi och olyckor.Resultatet visar att Tidningen Ångermanland har mer fokus på färg och form, något som kallas ytans formgivning medan Sundsvalls Tidnings fokus är på nyhetsvärderingen där det mestadels är innehållet som styr redigeringen, även kallat innehållets formgivning. Båda tidningarna styrs i första hand av sitt format men utöver det är estetiken den enskilda faktor som får styra mest i Tidningen Ångermanland medan Sundsvalls Tidning styrs av utrymmet och policyn. Etiken påverkar båda tidningarna men den är inte en avgörande faktor för redigeringen och placeringen av materialet.</p>

SammanfattningFör att veta vad läsarna tittar på i en tidning har det gjorts flera studier med så kallade eye-tracking-metoder. Eye-tracking innebär att man mäter hur ögonen rör sig på en redigerad tidningssida för att se var blicken fastnar. En läsare spenderare ungefär tjugo minuter av sin tid till att gå igenom sin tidning och endast ett fåtal läser allt. I stället fastnar läsarnas ögon för olika beståndsdelar, till exempel bilder och grafik, som är de två som lockar mest till läsning tätt följt av rubriker.. Färg är en annan komponent som lockar ögat, oavsett om den är en del av texten eller grafiken. Målmedvetenhet i butiker : Hur konkreta och abstrakta uppdrag påverkar konsumenters köpbeteende

Syftet med uppsatsen är att ta reda på hur konkreta och abstrakta uppdrag påverkar konsumenters köpbeteende i livsmedelsbutiker. Genom en kvantitativ studie på Ica Maxi i Karlstad undersöktes hur uppdragets karaktär påverkar faktorer som antal observationer, spenderad tid i butiken samt antal produkter i varukorgen. Resultatet visade på en skillnad i hur konkreta och abstrakta uppgifter påverkar konsumenters beteende. Respondenter med ett konkret uppdrag gjorde fler observationer samt tog längre tid på sig för att ta sitt beslut. Studien fann även att konsumenters beteende från ett uppdrag inte påverkade beteendet i kommande uppdrag. Ur ett företagsperspektiv spelar kunskapen om vad konsumenten observerar en betydande roll i marknadsföring och placering av produkter i butiken. / The purpose of this thesis is to find out how distinct and abstract tasks affects consumers purchasing behavior in grocery stores. Through a quantitative study at Ica Maxi in Karlstad the authors examined how the nature of a mission affects factors such as number of observations, time spent in the store and the quantity of products in the basket. The results showed a difference in how distinct and abstract goals affects consumer behavior. The respondents with a distinct task made ​​more observations and spent more time to make their decisions. The study also found that consumer behavior from one task did not affect the behavior in an upcoming task. The findings could be useful from a business perspective where the knowledge of what the consumer observes plays a significant role in marketing and positioning of products in the store.
