Prädiktion der linksventrikulären Funktion nach Mitralklappenrekonstruktion unter Verwendung des präoperativen Tei Index

Die chirurgische Mitralklappenrekonstruktion (MKR) ist der konservativen Therapie bei signifikanter Mitralklappeninsuffizienz (MI) überlegen. Bisher fehlen sensitive präoperative Parameter zur Detektion latenter linksventrikulärer Funktionsstörungen. Aufgrund der pathophysiologisch bedingten Nachlastreduktion und Vorlaststeigerung bei MI bergen die konventionell verwendeten Ejektionsindices, Ejektionsfraktion (EF) und Fractional Area Change (FAC), die Gefahr der Überschätzung der effektiven Pumpfunktion des linken Ventrikels (LV). Der dopplersonographisch erhobene Tei Index gilt als ein Marker globaler myokardialer Funktion. Ziel der vorliegenden Studie war es, mit dem Tei Index (bzw. dem Myokardialen Performance Index, MPI) einen sensitiveren präoperativen Parameter zur Prädiktion der postoperativen linksventrikulären Funktion zu finden. Hierzu wurden im Rahmen einer prospektiven klinischen Studie 130 Patienten mit signifikanter MI am Herzzentrum Leipzig entsprechend den ASE/SCA Leitlinien vor und nach kardiopulmonaler Bypass-Operation mittels transösophagealer echokardiographischer (TEE) Bildgebung untersucht. Die Quantifizierung der MI erfolgte durch Messung der Vena contracta (VC). Die FAC wurde in der transgastrischen midpapillären kurzen Achse und die EF im midösophagealen Zwei- sowie Vier-Kammer-Blick erfasst. Die Zeitintervalle zur Berechnung des Tei Index wurden im tiefen transgastrischen und midösophagealen Vier-Kammer-Blick erfasst. Eine statistische Korrelation zwischen präoperativen Tei Index und postoperativer EF und FAC konnte zur Validierung unserer Hypothese nicht detektiert werden. Folgend kann der Tei Index nicht als Prädiktor der effektiven linksventrikulären Funktion vor MKR gewertet werden.

WUnicuspid Aortic Valve- An Uncommon Anomaly With a Common Presentation

Unicuspid aortic valve (UAV), which is a rare congenital anomaly, usually presents as aortic stenosis and/or aortic regurgitation. Here we present a case of UAV co-existent with an ascending aortic aneurysm. A 26-year-old male with no significant past medical history presented to the hospital after two episodes of syncope. Transthoracic echocardiogram showed an ejection fraction of 62%, severely stenotic aortic valve, and moderate aortic regurgitation. Computed tomography revealed calcification of the aortic valve, compatible with aortic stenosis and aneurysm of the ascending aorta measuring 4.3 cm in diameter. He underwent successful aortic valve replacement and repair of ascending aortic aneurysm. He recovered well without any complications. This case suggests that any young patient who presents with syncope, aortic stenosis would be a differential and further workup by any available non-invasive modality needs to be performed.

Submitted in fulfillment of the requirements for the Degree of Masters in Technology: Cardiology, Durban University of Technology, 2012. / Cardiomyopathies and their resultant heart failure (HF) remain a major cause of cardiovascular morbidity and mortality (Wood and Picard, 2004). Idiopathic dilated cardiomyopathy (IDCMO) is a primary myocardial disease of unknown cause, characterized by left ventricular (LV) or biventricular dilatation and impaired myocardial contractility. Dilated cardiomyopathy (DCMO), along with rheumatic heart disease and hypertension (HPT), is one of the leading causes of HF in Africa. In fact, in an epidemiology study of 884 patients in Soweto, IDCMO was the second major cause of HF. Thirty five percent of patients in the study, with HF, had IDCMO (Sliwa, Damasceno, Mayosi, 2005). Methodology: Patients referred to the cardiomyopathy (CMO) clinic at Chris Hani Baragwanath hospital, situated in the echocardiographic lab, were recruited, provided they satisfied the exclusion and inclusion criteria and were enrolled after obtaining voluntary informed consent. From May 2009 to September 2010, 70 patients with IDCMO were recruited for this trial. Patients with DCMO were identified by means of echocardiographic criteria which included a left ventricular ejection fraction (LVEF) of less than 45% and an end diastolic dimension (EDD) of greater than of 52 mm (2D in long parasternal axis). Results: In the present study the prevalence of left ventricular (LV) thrombus in patients with IDCMO was 18.6%. When using Univariate logistic regression, the only independent predictors of LV thrombus formation was LVEF and age. However, when multivariate logistic regression analysis was applied to the data, the only predictor with a significant association was age. The reason for this is not clear. It is postulated that perhaps younger patients have differences in the pathophysiology of their disease such as a greater smoldering inflammatory component which may therefore predispose them to thrombus formation. For example the presence of IL-6 may be important in the formation of LV clot in cases of LV dysfunction (Sosin, Bhatia, Davis, Lip, 2003). The association between LVEF and LV thrombus was borderline significant. Conclusion: The prevalence of LV thrombus formation in this cohort of patients with IDCMO was 18.6%. Echocardiographic parameters alone cannot predict which patients are more likely to develop thrombus formation. / National Research Foundation

Outcome of patients with severe aortic stenosis – A retrospective follow-up study

<p>Aortic stenosis is the most common valvular disease in the adult population. A significant aortic stenosis is a serious condition, and if a symptomatic patient is not operated on, it may in most cases cause death. We have examined how many aortic stenoses that were diagnosed during one year, and a follow-up of the patients was also performed. We found 77 patients with significant aortic stenosis with a mean age of 76±13 years. At the time of follow-up 30 (39%) patients, aged between 29-85 years, had been surgically treated with implantation of a valve prosthesis within 2-23 months after the initial examination. At this initial examination 14 of the 30 patients who later underwent surgery had no symptoms. A coronary bypass operation was also performed on seven patients. Postoperative complications were observed in six patients, but none of them was fatal. At the initial examinations there were 26 (34%) patients with a significant aortic stenosis and symptoms who were not treated surgically. The main reason why these patients were not operated was high age, unwillingness, or severe left ventricular dysfunction. This study indicates the importance of repeated clinical and echocardiograpic examinations in patients with aortic stenosis. Almost half of the patients, that later underwent surgery, had no symptoms at the initial examination, but later developed symptoms which made surgery necessary. In one third of the patients no surgery was performed in spite of clinical symptoms.</p>

Association of metabolic and hemodynamic variables during exercise in children.

Three investigations were conducted using pulsed Doppler echocardiography (PDE) and oxygen consumption to non-invasively determine the relationship between hemodynamic and metabolic variables from rest through submaximal and maximal exercise in early adolescent males. In the first study, interinvestigator and day-to-day variability of cardiac output measurements at rest and during exercise determined by PDE were analyzed in six junior high school age boys. Four Doppler-derived variables (cardiac output, cardiac index, stroke volume, and stroke index) were not different when calculated by independent investigators and showed the same interinvestigator variability from rest through submaximal and maximal exercise as has been reported at rest in previous validation studies. There was a slight but statistically significant increase in the absolute values of the above hemodynamic variables and simultaneously collected metabolic variables during a second identical test conducted approximately nine days later. A faster adjustment to increasing workrates in the second test could explain this latter finding since supine cycle ergometry is not as familiar an activity as more frequently used modes of exercise testing. It is concluded that hemodynamic measurements using PDE exhibit the same acceptable variability throughout exercise as has been previously demonstrated at rest. In the second study, PDE was used to determine rapid serial measurements of cardiac output during a rapid loading supine cycle ergometer exercise test in twenty-two junior high school age boys. These measurements were compared to simultaneoulsy determined measurements of oxygen consumption. Cardiac output adjusted faster to each new workrate than oxygen consumption. Further analysis of these responses revealed that increases in heart rate (as opposed to stroke volume) were responsible for this rapid adjustment. It is concluded that there is an uncoupling and recoupling of these normally closely related hemodynamic and metabolic variables during the transitional periods between increasing levels of steady-state supine exercise. In the final study, gradual loading and rapid loading supine cycle ergometer protocols were compared in fifteen junior high school age boys. Maximal metabolic measurements and heart rate from each test were not different. Maximal PDE-derived measurements of cardiac output, cardiac index, stroke volume, and stroke index were slightly, but significantly, higher in the gradual loading protocol. It is concluded that a more complete adjustment of cardiac output (reflected by stroke volume) to maximal supine exercise occurs in a more gradual loading protocol than in a rapid loading protocol.

Time-course changes in the echocardiographic parameters and NT-proBNP levels in patients with severe mitral regurgitation undergoing valve replacement.

Conventional echocardiographic parameters are currently used in determining the timing for surgery in patients with mitral regurgitation. Since brain natriuretic peptide (BNP) rises in response to ventricular muscle stretch, and is to detect early heart failure, we hypothesized that BNP would be activated in patients with regurgitant valvular heart disease and concomitant left ventricular dilatation. Aim/Objectives: We therefore studied the pattern of changes in NT-pro BNP in patients with chronic severe rheumatic mitral regurgitation who were undergoing mitral valve replacement and compared this with the newer modality of tissue Doppler imaging (TDI). Setting: Patients submitted to surgery were prospectively evaluated over 8 months at Inkosi Albert Luthuli Central Hospital, Department of Cardiology. Controls were obtained from the outpatients' follow-up clinic. Methods: Simultaneous quantification of the severity of mitral regurgitation (MR), left ventricular (LV) end systolic volume (ESV), left atrial (LA) volume and Doppler filling ratios (mitral (E)/annulus (Ea)) were performed at baseline in all patients and was repeated at 1-week and at the six-week follow-up visit in surgical patients. Results: Both groups were similar for age and gender and echo-Doppler parameters in all patients preoperatively except LA size (p< 0.01) and volume (p<0.004) which were more elevated in the surgical group. Mean NT-pro BNP levels were markedly elevated preoperatively (262 pmolll) in all surgical cases compared to controls (57 pmol/l; p=0.0001). NT-pro BNP levels increased further at one week post surgery (395 pmol/l) and subsided at the six week follow-up visit (94 pmol/I). These changes were accompanied by significant reduction in LA (p= 0.003) and LV chamber dimensions (EDD = 0.004) with an increase in the ejection fraction from 42% at one week to 52 % at six weeks. Four patients had abnormally elevated NT-pro BNP levels (>53pmol/l) at the 6-week follow-up visit. A ROC curve was constructed for all variables to separate surgical cases from controls. The area under the curve was highest for NT-pro BNP (sensitivity= 96%, specificity 45 %). Conclusion: 1. There was a significant difference in the left atrial chamber size and volume, as well as Em/Ea (TDI) and NT-proBNP levels preoperatively between the two groups. The lack of a significant difference in the LV parameters between surgical and control groups suggest an almost total reliance on symptoms in deciding the timing of surgery which was reflected by markedly elevated NT-pro BNP in all surgical patients. 2. Postoperatively, there was a significant reduction in LA and LV dimensions. 3. The high false positivity rate for NT-pro BNP suggests that the test is most likely reflecting early LV decompensation in the less symptomatic control patients who rightly need surgery. 4. Tissue Doppler indices had similar sensitivity but low specificity compared to NT-proBNP. 5. Mapping Myocardial Elasticity with Intracardiac Acoustic Radiation Force Impulse Methods

<p>Implemented on an intracardiac echocardiography transducer, acoustic radiation force methods may provide a useful means of characterizing the heart's elastic properties. Elasticity imaging may be of benefit for diagnosis and characterization of infarction and heart failure, as well as for guidance of ablation therapy for the treatment of arrhythmias. This thesis tests the hypothesis that with appropriately designed imaging sequences, intracardiac acoustic radiation force impulse (ARFI) imaging and shear wave elasticity imaging (SWEI) are viable tools for quantification of myocardial elasticity, both temporally and spatially. Multiple track location SWEI (MTL-SWEI) is used to show that, in healthy in vivo porcine ventricles, shear wave speeds follow the elasticity changes with contraction and relaxation of the myocardium, varying between 0.9 and 2.2 m/s in diastole and 2.6 and 5.1 m/s in systole. Infarcted tissue is less contractile following infarction, though not unilaterally stiffer. Single-track-location SWEI (STL-SWEI) is proven to provide suppression of speckle noise and enable improved resolution of structures smaller than 2 mm in diameter compared to ARFI and MTL-SWEI. Contrast to noise ratio and lateral edge resolution are shown to vary with selection of time step for ARFI and arrival time regression filter size for STL-SWEI and MTL-SWEI. </p><p>In 1.5 mm targets, STL-SWEI achieves alternately the tightest resolution (0.3 mm at CNR = 3.5 for a 0.17 mm filter) and highest CNR (8.5 with edge width = 0.7 mm for a 0.66 mm filter) of the modalities, followed by ARFI and then MTL-SWEI.</p><p>In larger, 6 mm targets, the CNR-resolution tradeoff curves for ARFI and STL-SWEI overlap for ARFI time steps up to 0.5 ms and kernels $\leq$1 mm for STL-SWEI. STL-SWEI can operate either with a 25 dB improvement over MTL-SWEI in CNR at the same resolution, or with edge widths 5$\times$ as narrow at equivalent CNR values, depending on the selection of regression filter size. Ex vivo ablations are used to demonstrate that ARFI, STL-SWEI and MTL-SWEI each resolve ablation lesions between 0.5 and 1 cm in diameter and gaps between lesions smaller than 5 mm in 3-D scans. Differences in contrast, noise, and resolution between the modalities are discussed. All three modalities are also shown to resolve ``x''-shaped ablations up to 22 mm in depth with good visual fidelity and correspondence to surface photographs, with STL-SWEI providing the highest quality images. Series of each type of image, registered using 3-D data from an electroanatomical mapping system, are used to build volumes that show ablations in in vivo canine atria. In vivo images are shown to be subject to increased noise due to tissue and transducer motion, and the challenges facing the proposed system are discussed. Ultimately, intracardiac acoustic radiation force methods are demonstrated to be promising tools for characterizing dynamic myocardial elasticity and imaging radiofrequency ablation lesions.</p> / Dissertation

Atrial function and loading conditions in athletes

Intensive training is associated with hemodynamic changes that typically induce an enlargement of cardiac chamber. Despite LA dilatation in athletes has been interpreted as a benign adaptation, little evidence is available. The aim of this thesis is to demonstrate that LA size changes in response to alterations in loading conditions and to analyse atrial myocardial function in athletes through the application of novel echocardiographic techniques. We found that top-level athletes exhibit a dynamic morphological and functional LA remodelling, induced by training, with an increase in reservoir and conduit volumes, but stable active volume. Training causes an increase in biatrial volumes which is accompanied by normal filling pressures and stiffness. These changes in atrial morphology are not associated with respective electrical changes. Extending the evidence from adult athletes to children, we found that training-induced atrial remodelling can occur in the early phases of the sports career and is associated with a preserved biatrial function. Finally, in a meta-analysis study of the available evidence we demonstrated that atrial function and size are not affected by aging. In conclusions, athlete’s heart is characterized by a physiological biatrial enlargement. This adaptation occurs in close association with LV cavity enlargement, is dynamic and reversible. This increase in biatrial size is not intrinsically an expression of atrial dysfunction. Indeed, in athletes the atria are characterized by a preserved reservoir function, normal myocardial stiffness, and dynamic changes in response to different loading conditions.

Etude de la mécanique ventriculaire en échographie : modélisation de l'asynchronisme mécanique / Myocardial deformation study by echocardiography

Introduction : La resynchronisation cardiaque (CRT) améliore les patients insuffisants cardiaques, symptomatiques malgré un traitement médical optimal et présentant un élargissement du QRS>120ms. Cependant, un tiers des patients ne répondent pas à cette thérapie. L'objet de cette thèse est d'identifier les mécanismes déterminant la réponse à la CRT. Méthode: Dans un premier temps, nous avons apprécié la précision des méthodes de quantification de la contraction myocardique utilisées pour caractériser l'asynchronisme. Ensuite, nous avons évalué les facteurs liés à l'asynchronisme et à la réponse à la CRT (fibrose, nécrose myocardique, réserve contractile). De plus, nous avons développé et validé un nouvel indice permettant d'évaluer les conséquences « énergétiques » de l'asynchronisme sur la contraction myocardique. Résultats: Nous avons démontré que le strain longitudinal en speckle tracking était supérieur au Doppler tissulaire pour évaluer la déformation et l'asynchronisme myocardique et qu'il était mieux corrélé au pronostic des patients insuffisants cardiaques. Ensuite nous avons mis en évidence que le retard de contraction mécanique n'était pas lié simplement à un bloc de conduction électrique mais qu'il était observé pour l'ensemble des segments myocardiques nécrosés. De plus ces zones de fibrose et de nécrose évaluées en échographie de stress influencent la réponse à la CRT. Ces résultats suggèrent l'importance de considérer le retard de contraction et la contractilité résiduelle pour prédire la réponse à la CRT. A partir de cette hypothèse, nous avons validé un indice unique associant l'asynchronisme et la contractilité résiduelle pour évaluer la perte d'énergie contractile liée au retard de contraction. Conclusion : Les travaux réalisés ont permis de développer des outils pour mieux apprécier les conséquences de l'asynchronisme myocardique. / Background: Randomized studies demonstrated that Cardiac Resynchronization Therapy (CRT) improves symptoms and survival in heart failure patients with wide QRS duration that remains symptomatic despite optimal medical treatments. However, up to 40% of patients did not response to CRT. The purpose of this work was to investigate the underlying mechanisms of mechanical cardiac dyssynchrony to optimize the identification of responder to CRT. Methods: The first part of our study was to identify the accurate echocardiography method for quantifying of myocardial deformation and dyssynchrony (Tissue Doppler Imaging and Speckle tracking analysis). Next, we studied factors (myocardial scar and contractile reserve) interacting with myocardial dyssynchrony and response to CRT. Then, we developed and validated a mathematical model (strain delay index) to assess the wasted energy related to myocardial dyssynchrony. Results: First we demonstrated that longitudinal strain computed from spec kle tracking analysis was superior to tissue Doppler imaging in assessing myocardial dyssynchrony and function with a better correlation with outcome in heart failure patients. Next, we showed that mechanical dyssynchrony was not specific of electrical delay but was prevalent in scar segments. In addition, using dobutamine stress echocardiography, we demonstrated that contractile reserve in delayed segments greatly impacts on response to CRT. Then, we proposed and validated a mathematical model, the strain delay index for assessing the wasted energy related to mechanical dyssynchrony. Conclusion: The mathematical model proposed in the present study to assess the impact of dyssynchrony on myocardial contractility allows a better identification of responder to CRT.
