No. of bitstreams: 1 2014_CassiaValeriaDeCastro.pdf: 2725809 bytes, checksum: cacc31b5ff40c8ab9c9031bb4901f0fd (MD5) / A partir do censo brasileiro dos Hospitais de Custódia e Tratamento Psiquiátrico (HCTP), realizado em 2011, foi feito um recorte dos HCTP onde havia mulheres esquizofrênicas internadas a fim de realizar um estudo descritivo das condições físicas e do cuidado em atenção psicossocial nesses HCTP, bem como comparar os resultados obtidos com diretrizes nacionais (Ministério da Saúde) e internacionais (Schizophrenia Patient Outcome Research Team (PORT)) para o tratamento de pacientes esquizofrênicos. Para a coleta de dados, foi construído um questionário baseado no instrumento WHO QualityRights da Organização Mundial da Saúde (OMS). O instrumento foi aplicado em 13 HCTP, sendo respondido por seus diretores ou representantes. O principal resultado foi a confirmação do caráter ambíguo desses dispositivos, que ficam na fronteira entre o cuidado e a reclusão, mantendo características de espaços de tratamento e de prisão. Ao mesmo tempo em que há a tentativa de seguir algumas recomendações das diretrizes nacionais e internacionais, como a presença de equipes multidisciplinares e a oferta de atividades diversificadas às pacientes, há, em alguns desses HCTP, a presença de algemas, escolta, restrição do espaço de circulação e celas fortes. Este estudo tem como principal fragilidade o fato de não ter verificado as informações dadas pelos entrevistados, baseando-se, portanto, no depoimento de profissionais ligados à direção dos HCTP estudados. Também não foram ouvidas as mulheres a quem esses cuidados se destinam. As informações apontam para questões sérias ligadas ao tratamento de reclusão a pacientes psiquiátricos, ainda que estejam cumprindo medida de segurança. ______________________________________________________________________________ ABSTRACT / Based upon the Brazilian census of Custodial and Psychiatric Treatment Hospitals (CPTH) conducted in 2011, a sample of CPTH with schizophrenic women patients was selected in order to describe the physical conditions and the psychosocial attention care in these institutions, as well as to compare the results with the national guidelines set by the Brazilian Health Ministry and also the international guidelines set by Schizophrenia Patient Outcomes Research Team (PORT). The data was collected using a questionnaire based on the instrument WHO QualityRights conceived by the World Health Organization (WHO). The questionnaire was completed by 13 hospital directors of the CPTH. The main results include the confirmation of the ambiguous nature of these devices which reside on a fuzzy border between care and confinement, while maintaining characteristics of treatment and imprisonment spaces. On the other hand, there are some attempts to follow recommendations of national and international guidelines such as the presence of multidisciplinary teams and offering diversified activities for patients. In some of these CPTH, we can observe the use of handcuffs, security escort, restricted space movement and strong cells. The main weakness of this study has to do with the fact that the information given by the respondents were not verified, therefore relying exclusively on the information given by hospital directors of CPTH. The female patients who are the users of the care service also weren’t heard. The data collected points to serious issues relating to the treatment of psychiatric patients in confinement, even if security measures are being followed.
