Nessa época, BOLDYREFF acreditava que esse refluxo poderia ser benéfico, pois, neutralizando acidez gastrica em excesso, evitaria a formação de úlcera peptica. No entanto, alguns autores desde àquela época acreditavam na ação deletéria de tal refluxo. Nos primeiros 50 anos deste século, inúmeros estudos experimentais comprovaram a ação nociva do refluxo alcalino para o estômago. In that time, BOLDYREFF believed this reflux could be benefic once it neutralized the excessive gastric acidity, and would avoid the peptic ulcer.Therefore, some authors have since that time believed in the deleterious action of such reflux. Gastrite alcalina de refluxo : estudo experimental sobre a etiopatogenia

No começo foi considerado como benéfico, mas desde logo verificou-se que o refluxo causava inflamação da mucosa gástrica. Mais tarde, observou-se gastrite de refluxo em pacientes submetidos a gastrojejunostomia ou gastrectomia. Desde então vários estudos clínicos e experimentais foram realizados para estabelecer um melhor conhecimento sobre essa patologia. A maioria dos autores acredita que desde 1947 a gastrite alcalina de refluxo tornou-se entidade clinica bem estabelecida e aceita como uma síndrome pós-gastrectomia diferente das outras. No presente trabalho, pretendeu-se verificar se o direcionamento da bile para o antro ou o corpo gástricos produzia gastrite de freqüência e intensidade diferentes. Além disso, se a vagotomia troncular seria capaz de proteger a mucosa gástrica contra os efeitos nocivos da bile e suco gástrico. Com esse objetivo foram operados 40 cães nos quais realizamos uma anastomose entre a vesícula biliar e o estômago,depois de ligadura dupla do colédoco. Num grupo de 20 animais, em 10 deles essa anastomose foi feita no antro gástrico e, nos outros 10, no corpo. Em outro grupo de cães, as mesmas cirurgias foram feitas em 20 cães, porém acrescidas de uma vagotomia troncular bilateral. Em 10 deles a anastomose foi feita no antro e, nos outros 10, no corpo gástrico. Todos os animais foram sacrificados 180 dias depois das cirurgias. O estudo macroscópico e microscópifco das peças revelou que a gastrite foi menos freqüente e menos intensa nos cães que foram submetidos à vagotomia. Assim, a vagotomia foi capaz de proteger à mucosa gástrica dos efeitos deletérios da bi1e e do ácido clorídrico e pepsina do suco gástrico. Notou-se também que o processo infamatório foi mais intenso quando a anastomose foi feita no antro do que no corpo, em todos os animais submetidos ou não à vagotomia. Outro fato observado no presente trabalho foi o de que a gastrite, em alguns cães, foi mais intensa ao redor da anastomose. / Abstract: The duodenogastric reflux is a phenomenon know around 100 years. In the beggining it was considered verified that the reflux caused inflamation on the gastric mucosa. Later the reflux gastritis was observed in patients submitted to gastrojejunostomy or gastrectomy. Since then, several clinical and experimental studies were realized to reach better knowledge about this pathology. Most of the authors believe that since 1947 the alkaline reflux gastritis became a well estabilished clinical entity and accepted as a pos-gastrectomy syndrome different from the others. In the present paper we intended to verify if leading the bile to the gastric antrum or gastric corpus, it caused gastritis in different frequency and intensity. Moreover, if the truncal vagotomy would be able to protect the gastric mucosa from the noxious effects of bile and gastric juice. With this objective we operated 40 dogs in which was performed an anastomosis between the gallblader and the stomach, after ligadure double of the choledocus. In a group of twenty animals, in ten of them this anastomosis was done in the antrum gastric, and in the ten others in the corpus. In another group of dogs the same surgeries were made in twenty dogs, but added of a bilateral truncal vagotomy. In ten of them the anastomosis was done in antrum and in the other ten in the gastric corpus. All the animals were sacrificed 180 days after the surgeries. The macroscopic and microscopic studies of the specimes revealed the gastritis was less frequent and less intense in the dogs that were submitted to the vagotomy. So the vagotomy was able to protect the gastric mucosa from the deleterious effects of the bile and chloridric acid and pepsin of the gastric juice. We noted also the inflamatory process was more intense when the anastomosis was made in the antrum than the corpus in all the animals, submitted or not the vagotomy. Other aspect observed in this paper was that the gastritis, in some dogs, was more intense aroud the anastomosis. / Doutorado / Cirurgia Geral / Doutor em Medicina
