Effekter på smärta och hälsa av akupunkturbehandling hos personer med fibromyalgi : En litteraturstudie

I världen lever cirka 3-5 procent av befolkningen med det långvariga smärtsyndromet fibromyalgi. Det är svårt att finna behandlingar mot fibromyalgi då det inte finns någon vetenskap om vad som orsakar syndromet. Syftet med föreliggande litteraturstudie var att beskriva effekterna på smärta och hälsa av akupunkturbehandling hos personer med fibromyalgi samt beskriva kvaliteten av de valda artiklarnas metod. Studien utformades med en deskriptiv design där 11 vetenskapliga artiklar inkluderades och sammanställdes. Litteratursökningarna utfördes i databaserna Medline (PubMed) samt Cinahl. Resultatet visade att akupunktur gav positiva kortsiktiga effekter på smärta och hälsa i kombination med farmakologisk behandling hos personer med fibromyalgi. Däremot var långtidseffekten av akupunktur inte betydande effektiv. Placebo visade sig även ge en god effekt. Akupunktur gav betydande minskning av smärta genom en förhöjd aktivering av endogena opioidreceptorer samt genom ökat blodflöde i hud och muskler. Slutsatsen av studien var att akupunktur med fördel kan användas tillsammans med farmakologisk behandling till personer med fibromyalgi. För att kunna ge en god omvårdnad är det viktigt att få kunskap och förståelse om akupunktur som behandlingsmetod till denna population. För att dra några slutsatser om akupunktur kan ge några långsiktiga effekter behövs mer forskning inom området.

Perceptions of exercise among individuals with fibromyalgia : identifying the benefits and barriers

This qualitative study investigated the perceptions of exercise among individuals with fibromyalgia. The goal of this research was not to prove that exercise is good for people with fibromyalgia, but rather to gain an understanding of how health practitioners can help fibromyalgia patients adhere to regular physical activity. The research was conducted using three focus groups with people who are treated for fibromyalgia at Medical Consultants in Muncie, Indiana. The groups consisted of current exercisers and non-exercisers. Four themes emerged from the groups that the participants discussed. These themes were, reasons for exercising, barriers to exercise, desired characteristics of an exercise program, and key support personnel helping participants deal with their condition. The information contained within these themes can be used by health practitioners to develop exercise programs that entice fibromyalgia patients to adhere to a regular exercise program. / Fisher Institute for Wellness and Gerontology

Ce travail questionne la place du féminin dans l’expérience subjective des personnes atteintes de fibromyalgie, maladie touchant majoritairement des femmes et dont la douleur et la fatigue chroniques occupent le devant de la scène. Notre hypothèse principale nous conduit à investiguer l’existence d’un trouble primaire du féminin chez les personnes qui en sont atteintes. Ce trouble serait lié à un échec de l’accès à la voie passive. Le modèle identificatoire serait celui de la mère des premiers soins, mal différenciée, mais représentant un idéal de toute-puissance narcissique. Dans ce contexte, les éprouvés douloureux de la passivité primaire laisseraient la place à une difficulté majeure face à la position passive, comprise comme une défense contre l’emprise maternelle. Des travaux de recherche proposent de considérer cette maladie comme étant une manifestation moderne de l’hystérie de conversion. Nous mettons cette hypothèse à l’épreuve à travers les cas cliniques étudiés. Nous investiguons également la question de la dépression en suivant l’idée que la fatigue chez les fibromyalgiques pourrait être interprétée comme une forme de dépression, non réactionnelle, préexistante à la maladie et caractérisée par une susceptibilité mélancolique. Les douleurs éprouvées constitueraient ainsi la composante mélancolique de cette dépression. Notre travail s’appuie sur le cadre théorique de la psychopathologie psychanalytique et sur des outils d’investigation qui sont les entretiens cliniques de recherche et les épreuves projectives (Rorschach et TAT). / This work questions the place of the feminine in the subjective experience of people suffering from fibromyalgia, a disease that mostly affects women and which chronic pain and fatigue occupy center stage. Our main hypothesis leads us to investigate the existence of a primary disorder of the feminine in people suffering from this disease. This disorder would be linked to a failure of reaching the passive way. The role model would be the first care mother, poorly differentiated, but which represents an omnipotent narcissistic ideal. In this context, the pain felt in primary passivity would leave room for a major difficulty regarding the passive position which is seen as a defense against the mother’s control. Some research intends to consider this disease as a modern expression of conversion hysteria. We challenge this hypothesis through the clinical cases that we studied. We also investigate the question of depression by following the idea that the fatigue of fibromyalgic people could be interpreted as a form of depression, non-reactive, pre-existing from the disease and characterized by a melancholic susceptibility. Then, the pain would consist in the melancholic component of the depression. Our work is based on the theoretical framework of psychoanalytical psychology and on investigation tools which are clinical research interviews and projective tests (Rorschach and TAT).

Fibromyalgi : En litteraturstudie om fibromyalgins möjliga riskfaktorer / Fibromyalgia : A litteraturstudy on fibromyalgias possible risk factors

Bakgrund: Fibromyalgi är ett syndrom som kännetecknas av utbredd smärta, överkänslighet vid beröring men även att övriga delar av kroppen påverkas. Metod: En litteraturstudie har genomförts där 10 stycken vetenskapliga artiklar har analyserats och utgjort resultatet. Syfte: Huvudsyftet är att undersöka möjliga riskfaktorer som kan påverka utvecklandet av fibromyalgi. Resultat: Studiens resultat har tagit upp fyra teman med tänkabara riskfaktorer. Traumatiska barndomshändelser, där både fysisk och verbal misshandel och sexuellt utnyttjande var riskfaktorer. Att bli utsatt för trauma som vuxen, där fysisk misshandel, operation och arbetsrelaterad skada ingick, var också en riskfaktor. Stress och ett högt BMI-värde var också riskfaktorer relaterade till utvecklandet av fibromyalgi. Diskussion: Studiens resultat visade på att det är flera riskfaktorer som påverkar utvecklandet av detta syndrom. Alla som utsätts för en av dessa riskfaktorer utvecklar dock inte fibromyalgi, utan med största sannolikhet så är det en kombination av flera riskfaktorer som gör att man utvecklar fibromyalgi. Slutsats: Det behövs mer forskning för att studera sambandet mellan fysiska, psykiska och sociala riskfaktorer. / Background: Fibromyalgia is a syndrome characterized by widespread pain, hypersensitivity to the touch but other body areas are also affected. Method: A literature study was conducted in which 10 scientific articles were analyzed and discussed. Purpose: The main purpose was to investigate possible risk factors that may affect the development of fibromyalgia. Results: The results of this study raised four themes describing risk factors. Traumatic childhood events, including both physical and verbal abuse and sexual abuse, were risk factors. Becoming a victim of trauma as an adult, where physical abuse, operation and work related injuries were included, was a risk factor as was stress and a high BMI. Discussion: Study results showed that there are many risk factors that affect the development if this syndrome. All persons exposed to one of these risk factors however does not develop fibromyalgia, so most likely it is a combination of several risk factors that makes the development of fibromyalgia possible. Conclusion: More research is needed to study the connection between physical, psychological and social risk factors.
