Introdução: Os pacientes com fissura labiopalatina, com deficiências maxilares muito severas, geralmente são tratados com avanço maxilar por meio da osteotomia tipo Le Fort I. Entretanto, a distração osteogênica com o distrator externo rígido (RED) pode funcionar como uma alternativa terapêutica para a correção da discrepância esquelética. Proposição: O objetivo do presente estudo é avaliar os efeitos do avanço maxilar por meio da distração osteogênica com distrator externo rígido (RED), associada à osteotomia tipo Le Fort I, em pacientes com fissura transforame unilateral ou bilateral, quanto à quantidade de avanço maxilar e à sua estabilidade a médio e longo prazo. Materiais e Métodos: Para a realização deste estudo longitudinal e retrospectivo, foram usadas telerradiografias em norma lateral de 9 pacientes (6 do gênero masculino e 3 do gênero feminino), onde 4 apresentaram fissura transforame unilateral e 5 apresentaram fissura transforame bilateral, submetidos ao avanço maxilar por meio da distração osteogênica com distrator externo rígido (RED). Foram estabelecidos três tempos de avaliação: fase pré-distração (T1), fase pós-distração imediata (T2) e fase pós-distração controle, com o mínimo de 1 ano após a finalização da distração (T3). A demarcação dos pontos cefalométricos e a obtenção das medidas das variáveis cefalométricas foram realizadas através do software Dolphin Imaging®, versão 11.5. Para a análise dos resultados, o teste estatístico ANOVA de medidas repetidas foi utilizado, adotando-se o nível de significância de 5%. Resultados: No início da distração, a idade média foi de 14 anos e 4 meses (idade mínima de 9 anos, e máxima de 21 anos). O período médio de distração foi de 18 dias, com uma média de ativação no distrator de 1,0mm/dia. O avanço médio da maxila medido em LVR-A, em T2, foi de 15,6mm (p<0,001), com recidiva não estatisticamente significante de 21,79% (p=0,102), em T3. O aumento médio de SNA, em T2, foi de 14,8º (p<0,001), com recidiva não estatisticamente significante de 18,90% (p=0,130), em T3. Os valores médios das medidas SN.GoMe, 1.PP e IMPA não apresentaram variação estatisticamente significante (p>0,05) entre T1, T2 e T3. Conclusão: A terapia de distração osteogênica para avanço maxilar com o RED mostrou ser eficiente, com aumentos significantes das medidas cefalométricas lineares e angulares relacionadas ao avanço maxilar, demonstrando efeito predominantemente esquelético, e estabilidade no período pós-distração médio (T3) de 1 ano e 8 meses. / Introduction: Patients with unilateral and bilateral cleft lip and palate, with significant maxillary hypoplasia are commonly treated with maxillary advancement by Le Fort I osteotomy. However, distraction osteogenesis with a rigid external distraction (RED) device can function as an alternative option for treatment of the skeletal discrepancy. Purpose: The aim of this study is to assess the effects of maxillary advancement by distraction osteogenesis using a rigid external distraction (RED) device, associated with the Le Fort I osteotomy in patients with unilateral or bilateral cleft lip and palate, as the amount of maxillary advancement and their stability in the medium and long term. Materials and Methods: To perform this retrospective longitudinal study, lateral cephalograms of 9 patients (6 males and 3 females) were used, where 4 had unilateral cleft lip and palate and 5 had bilateral cleft lip and palate, who underwent maxilla advancement by distraction osteogenesis with RED device. Results: At the start of distraction, mean age was 14 years and 4 months (minimum age 9 years old and maximum of 21 years old). The mean distraction period was 18 days, with a mean rate of distractor activation in 1.0 mm / day. The mean maxillary advancement in LVR-A, at T2, was 15.6 mm (p<0.001), with no statistically significant relapse of 21.79% (p=0.102) at T3. The SNA angle increase, at T2, was 14.8º (p<0.001), with no statistically significant relapse of 18.90% (p=0.130), at T3. The mean values of SN.GoMe, IMPA and 1.PP measures showed no statistically significant variation (p>0.05) between T1, T2 and T3. Conclusion: The therapy of distraction osteogenesis for maxillary advancement with RED is efficient, with significant increases in the linear and angular cephalometric measurements related to the maxilla advancement, demonstrating predominantly skeletal effect and stability in mean post-distraction period (T3) of 1 year and 8 months.

Objetivo: O objetivo é determinar qual dos dois métodos é mais efetivo. Métodos: Oitenta pacientes foram incluídos neste ensaio clínico randomizado. O desfecho primário foi o questionário Disabilities of the Arm, Shoulder and Hand (DASH) e a mensuração da dor (EVA). A avaliação final deu-se com 12 meses de pós-operatório. Resultados: Na avaliação final não houve diferença entre os grupos nas avaliações do questionário DASH (3,71 para o grupo PVB e 2,72 para o grupo FE, p=0,58), e da dor pela EVA (0,84 para o grupo PVB e 0,53 para o grupo FE, p=0,39). O tratamento com PVB foi mais efetivo que aquele com FE na avaliação precoce com oito semanas para questionário DASH (21,82 para o grupo PVB e 39,88 para o grupo FE, p=0,0012). No grupo tratado com PVB, houve 11,7% de complicações e, no grupo tratado com fixador externo, 26,3%. Houve três falhas de tratamento no grupo tratado com PVB e nenhuma no outro grupo. Conclusão: Não se aferiram diferenças entre os grupos nas avaliações finais do questionário DASH e da dor pela EVA. Na avalição precoce com oito semanas, houve diferença favorável ao método de PVB. / Introduction: In the treatment of unstable fractures of the distal radius there is no conclusive evidence about the greater effectiveness of the reduction and fixation methods: bloodless external fixation (BEF) or open locked volar plate (LVP). The goal is to determine which of the two methods is most effective. Methods: Eighty patients were enrolled in this randomized clinical trial. The primary endpoint was the DASH questionnaire and measurement of pain (VAS). The final assessment was given at 12 months postoperatively. Results: In the final evaluation there was no difference between groups in assessments of the DASH questionnaire (3.71 for the LVP group and 2.72 for the BEF group, p = 0.58), and pain by VAS (0,84 to LVP group and 0.53 for the BEF group, p = 0.39). Treatment with LVP was more effective than one with BEF in early evaluation with eight weeks to DASH questionnaire (21.82 for the LVP group and 39.88 for the BEF group, p = 0.0012). In the group treated with LVP, there were 11.7% of complications and, in the group treated with external fixator, 26.3%. There were three treatment failures in the group treated with LVP and none in the other group. Conclusions: There were not assessed differences between groups in the final evaluations of the DASH questionnaire and the pain by VAS. Avaliação de pontos cefalométricos no alongamento ósseo do terço médio da face com a utilização de dispositivo externo rígido em portadores craniossinostose sindrômica / Evaluation of cephalometric points in the midface bone lengthening with the use of rigid external device in syndromic craniosynostosis patients

A distração osteogênica tem sido extensamente empregada na correção da grave hipoplasia do terço médio da face de portadores de craniossinostose sindrômica. Contudo, poucos estudos têm apresentado os resultados da distração do terço médio da face através de avaliação cefalométrica. O objetivo do presente estudo foi o de avaliar os resultados obtidos com o avanço ósseo do terço médio da face após osteotomia tipo Le Fort III ou frontofacial em monobloco seguida da utilização de dispositivo rígido externo de distração (RED), em portadores de craniossinostose sindrômica, em termos de quantidade de alongamento ósseo, estabilidade esquelética e crescimento facial. Onze pacientes submetidos aos procedimentos de distração, de fevereiro de 2002 a janeiro de 2006, na unidade de cirurgia plástica do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, foram avaliados retrospectivamente. Seis pacientes eram portadores da síndrome de Crouzon, quatro da síndrome de Apert, e um da síndrome de Saethre-Chotzen. Onze pacientes foram submetidos ao procedimento de alongamento ósseo do terço médio da face através do uso de dispositivo externo rígido (RED), após osteotomias tipo Le Fort III (N = 4) ou frontofacial monobloco (N = 7). Foram avaliadas retrospectivamente três telerradiografias de face em norma lateral de cada paciente (T1- préoperatório; T2 - pós-operatório recente, logo após a remoção do distrator; T3 - pós-operatório tardio, obtida com um intervalo mínimo de 12 meses após cirurgia). Três cefalogramas foram obtidos de cada paciente, através da direta sobreposição das três telerradiografias, tomado como referência reparos anatômicos do crânio e da porção anterior da fossa craniana. Desta forma, os três traçados cefalométricos foram obtidos no mesmo papel acetato, o qual foi digitalizado. Utilizando o programa de computação gráfica de domínio público Image J, várias mensurações foram realizadas com a intenção de determinar a extensão do avanço sofrido pelos segmentos ósseos na direção do vetor do movimento esquelético, assim como de seus componentes horizontal e vertical, tomando como referência pontos A e orbitário. Pacientes foram ainda divididos em dois grupos (G 1 - pacientes submetidos à osteotomia tipo Le Fort III; G 2 - pacientes submetidos à osteotomia frontofacial em monobloco), e os dados obtidos a partir mensurações de ambos os grupos foram comparados. Avanço significativo do terço médio da face foi obtido com os procedimentos. O componente horizontal do movimento esquelético predominou ao componente vertical. A taxa de reposicionamento posterior horizontal tardio (perda de resultado) foi mínima. Quando comparados os dois procedimentos, foi observada uma diferença significativa entre os grupos Le Fort III e monobloco. A quantidade de avanço obtido foi maior no grupo monobloco que no grupo Le Fort III. Em termos perda de resultado, o grupo Le Fort III foi mais estável que o grupo monobloco. Foi observada uma evidente alteração vertical no posicionamento dos pontos de referência no pós-operatório tardio, se comparado ao pós-operatório recente, evidenciando crescimento na vertical da face, ao contrário do que ocorre na direção horizontal, onde existiu um pequeno reposicionamento posterior e nenhuma evidência de crescimento. / Distraction osteogenesis has been applied extensively to correct the severe midface hipoplasia in syndromic craniosynostosis patients. However few studies have reported midface distraction outcomes through cephalometric evaluation. The purpose of the present study was to evaluate outcomes with midface distraction after Le Fort III and frontofacial monobloc osteotomy using a rigid external device (RED) in patients with syndromic craniosynostosis, in terms of quantity of bone lengthening, skeletal stability and facial growth. Eleven patients underwent to midface distraction from February of 2002 to January of 2006 at the plastic surgery unit of The \'Hospital das Clínicas\' of the Medical School of The University of São Paulo were retrospectively evaluated. Six patients had Crouzon, four had Apert, and one had Saethre-Chotzen syndrome. The patients were submit to bone lengthening procedure of the midface using a rigid external device (RED) after osteotomy type Le Fort III (n=4) and frontofacial monobloc osteotomy (n=7). Three teleradiography were retrospectively evaluated of each patient (T1 - before surgery; T2- after surgery, rigth after distractor removal; T3 - after surgery, obtained with a minimal interval of 12 months after surgery). The three lateral cephalograms were obtained from each patient by direct teleradiography superimposition taken as references the anatomic repairs in the cranium and anterior skull base. This way the three cephalometric tracings were obtained in the same acetate paper which was digitalized. Utilizing a public domain program Image J, various mensurations were accomplished with intension of determine the extent of advancement suffered by the bone segments in the direction of vector skeletal movement and its horizontal and vertical components as well, as taken as references point A and orbitale. Patients still were divided between two groups (G 1- patients submitted to an osteotomy type Le Fort III; G 2- patients submitted to monobloc frontofacial osteotomy), and the data obtained from mensurations from both groups were compared. Significant midface advancement was achieved with the procedures. The horizontal component of the movement was predominant if compared to vertical. The rate of horizontal relapse was minimal. When compared the two procedures was noted a significant difference between Le Fort III and monobloc groups. The advancement rate was greater in monobloc than Le Fort III group. In terms of relapse Le Fort III group was more stable than monobloc group. In vertical direction was noted an evident altered position of the reference points at late postoperative period if compared with recent postoperative period given evidence of facial vertical direction growth, contrary from what occurred in horizontal direction where existed a small relapse and no growth.

Eleven patients underwent to midface distraction from February of 2002 to January of 2006 at the plastic surgery unit of The \'Hospital das Clínicas\' of the Medical School of The University of São Paulo were retrospectively evaluated. Six patients had Crouzon, four had Apert, and one had Saethre-Chotzen syndrome. The patients were submit to bone lengthening procedure of the midface using a rigid external device (RED) after osteotomy type Le Fort III (n=4) and frontofacial monobloc osteotomy (n=7). Three teleradiography were retrospectively evaluated of each patient (T1 - before surgery; T2- after surgery, rigth after distractor removal; T3 - after surgery, obtained with a minimal interval of 12 months after surgery). The three lateral cephalograms were obtained from each patient by direct teleradiography superimposition taken as references the anatomic repairs in the cranium and anterior skull base. This way the three cephalometric tracings were obtained in the same acetate paper which was digitalized. Utilizing a public domain program Image J, various mensurations were accomplished with intension of determine the extent of advancement suffered by the bone segments in the direction of vector skeletal movement and its horizontal and vertical components as well, as taken as references point A and orbitale. Patients still were divided between two groups (G 1- patients submitted to an osteotomy type Le Fort III; G 2- patients submitted to monobloc frontofacial osteotomy), and the data obtained from mensurations from both groups were compared. Significant midface advancement was achieved with the procedures. The horizontal component of the movement was predominant if compared to vertical. The rate of horizontal relapse was minimal. When compared the two procedures was noted a significant difference between Le Fort III and monobloc groups. The advancement rate was greater in monobloc than Le Fort III group. In terms of relapse Le Fort III group was more stable than monobloc group. In vertical direction was noted an evident altered position of the reference points at late postoperative period if compared with recent postoperative period given evidence of facial vertical direction growth, contrary from what occurred in horizontal direction where existed a small relapse and no growth.

Uso do fixador externo articulado no tratamento cirúrgico da instabilidade multiligamentar crônica do joelho / Articuled external fixator for treatment of the chronic multiple ligament-injured knee

INTRODUÇÃO: A instabilidade multiligamentar do joelho, normalmente, é provocada por um trauma que determina sua luxação, um evento pouco frequente, mas, que pode trazer sequelas devastadoras. Mesmo com o tratamento cirúrgico preconizado é alto o índice de complicações. A mobilização precoce no pósoperatório provoca afrouxamento dos ligamentos reconstruídos, o que leva à instabilidade residual. A imobilização melhora a estabilidade, mas provoca dor e rigidez. Este trabalho tem o objetivo de avaliar se o uso do fixador externo articulado proporciona melhora na mobilidade, estabilidade e na função subjetiva de pacientes submetidos à reconstrução ligamentar. MÉTODOS: Neste ensaio clínico randomizado com grupos paralelos, 33 pacientes do ambulatório do Instituto de Ortopedia e Traumatologia do Hospital das Clínicas da Universidade de São Paulo com mais que 3 semanas de lesão dos ligamentos cruzados anterior e posterior associado à lesão de ligamento colateral fibular e/ ou ligamento colateral tibial foram submetidos à cirurgia de reconstrução multiligamentar, após alocação cega aleatória ao grupo 0 - controle (18 pacientes), com órtese rígida ou ao grupo 1 - fixador externo articulado por 6 semanas (15 pacientes), no período entre novembro de 2010 e novembro de 2013. Após seguimento mínimo de um ano de pós-operatório, a estabilidade dos ligamentos reconstruídos foi avaliada ao exame físico, foram mensurados os déficits de extensão e de flexão residual em relação ao joelho contralateral não acometido e foi aplicado o questionário específico para sintomas do joelho de Lysholm. RESULTADOS: Não houve diferença na avaliação da estabilidade articular nem na extensão entre os grupos. No grupo 1, o déficit de flexão passiva foi menor (4,8º +- 5,4º contra 18,2 +- 14,8º, p < 0,05) com mais pacientes, obtendo déficit de flexão considerado normal, de até 5º (64% contra 18%, p < 0,05) e com classificação excelente ou boa no questionário de Lysholm (73% contra 35%, p < 0,05). CONCLUSÕES: Comparado ao protocolo de reabilitação com órtese rígida removível, o uso do fixador externo articulado no tratamento cirúrgico das lesões multiligamentares do joelho proporciona os mesmos resultados de estabilidade ligamentar, aumenta a amplitude de movimento final, com manutenção da extensão e redução do déficit de flexão e proporciona melhores resultados funcionais / INTRODUCTION: Multiligament knee instability is normally caused by a trauma which results in its dislocation, an infrequent event, but one which can have devastating aftereffects. Even with the recommended surgical treatment the rate of complications is high. Early post-operative mobilization provokes loosening of the reconstructed ligaments, which leads to residual instability. Immobilization improves the stability, but causes pain and stiffness. This study aims to assess whether the use of an articulated external fixator provides improvements in the mobility, stability and the subjective function of the patients submitted to ligament reconstruction. METHODS: In this randomized clinical trial with parallel groups, 33 patients of the outpatient clinic of the Institute of Orthopaedics and Traumatology of the Hospital das Clínicas of the University of Sao Paulo with more than 3 weeks of injury to the anterior cruciate ligaments and the posterior cruciate ligaments associated with injury to the fibular collateral ligament and/or medial collateral ligament, were submitted to multiligament reconstruction surgery, after blind random allocation to either: Group 0 - control (18 patients), with rigid bracing, or to Group 1 - articulated external fixator for 6 weeks (15 patients), in the period from November, 2010 to November 2013. The stability of the reconstructed ligaments were assessed after 1 year postoperatively by physical examination, the deficit of residual extension and flexion was measured in relation to the unaffected contralateral knee and the Lysholm knee scoring scale questionnaire was applied. RESULTS: There was no difference in the assessment of joint stability or of the extension between the groups. In group 1 the passive flexion deficit was less (4.8º +- 5.4º versus 18.2 +- 14.8º, p<0.05) with more patients obtaining a flexion deficit considered normal, or up to 5º (64% versus 18%, p<0.05) and with excellent or good classification in the Lysholm knee scoring scale questionnaire (73% versus 35%, p<0.05). CONCLUSIONS: Compared to the rehabilitation protocol with removable rigid bracing, the use of an articulated external fixator in the treatment of multiligament knee injuries provides equal results for ligament stability, increases the amplitude of the Range of Motion, maintains the extension and reduces the flexion deficit, and provides better functional result
