When reconstruction is not achieved, a good esthetic outcome is rarely obtained. The aim of this retrospective study was to asses the outcome of implant-bone block movement, after segmental osteotomy in the maxillary aesthetic region in patients with vertical alveolar bone atrophy. Patients with vertical alveolar bone atrophy who underwent repositioning of the bone-implant block segment in the maxillary aesthetic region with a minimum of 6 months of follow-up were selected. Outcome measures were the success rate of the implant, complications after the surgical procedure, amount of vertical bone augmentation, papilla formation, keratinized mucosal band height and patient satisfaction analysis. Nine patients were included in the study, all with multiple implants, totaling 25 implants. The mean vertical bone augmentation was 4.9 mm (3.0-8.4 mm), and only 1 implant failure occurred. A statistically significant improvement in papilla formation was observed after surgery, leading to a good aesthetic result. More than 2 mm of height of keratinized mucosa was observed in 6 patients, and also a high satisfaction and acceptance to the treatment. After data evaluation, it was concluded that the technique presented can reconstruct vertical alveolar bone atrophy, with a high implant survival rate in a short period of follow-up, improving the aesthetic-functional result with a good acceptance by the Patients / Atrofia óssea alveolar vertical representa um desafio para a reconstrução e reabilitação oral, especialmente quando envolve a zona estética da maxila. Quando a reconstrução óssea não é realizada, um bom resultado estético dificilmente é obtido. O objetivo deste estudo retrospectivo foi avaliar o resultado do reposicionamento do bloco osso-implante, após a osteotomia segmentar em pacientes com atrofia óssea alveolar vertical na região estética de maxila. Para a realização deste trabalho, foram selecionados pacientes com atrofia óssea alveolar vertical que se submeteram à reposicionamento do segmento de bloco osso-implante na região estética maxilar com um mínimo de 6 meses de acompanhamento. As variáveis avaliadas foram o índice de sucesso do implante, complicações após o procedimento cirúrgico, quantidade de aumento ósseo vertical, formação da papila, altura da faixa de mucosa ceratinizada e a análise de satisfação do paciente. Nove pacientes foram incluídos no estudo, todos com implantes múltiplos, totalizando 25 implantes. A média de aumento ósseo vertical foi de 4,9 mm (3,0-8,4 mm), e apenas 1 falha de implante ocorreu. Uma melhora estatisticamente significativa na formação de papila foi observada após a cirurgia, levando a um bom resultado estético. Mais de 2 mm de altura de mucosa ceratinizada foi observado em 6 pacientes, e também uma alta satisfação e aceitação ao tratamento. Após a avaliação dos dados concluiu-se que, a técnica apresentada possui a capacidade de reconstruir atrofia óssea alveolar vertical, com uma elevada taxa de sobrevivência de implantes em período curto de acompanhamento, melhorando o resultado estético-funcional com uma boa aceitação por parte dos pacientes.

Distração osteogênica médio-sagital da mandíbula: avaliação em modelos de estudo

A amostra consistiu de 14 pacientes (5 do gênero masculino e 9 do gênero feminino) com idades variando de 13 a 19 anos e média de idade de 15,74 ± 1,89 anos no início do tratamento. Modelos de estudo foram obtidos de cada paciente no início do tratamento (T1), três meses após a DOMM (T2) e no momento da remoção dos aparelhos fixos (T3). Os dados foram submetidos à Análise de Variância e teste complementar de Duncan e demonstraram que todas as dimensões transversais aumentaram com a DOMM, o comprimento do arco não sofreu alteração estatisticamente significante, o perímetro do arco aumentou e o Índice de Irregularidade diminuiu consideravelmente, refletindo a correção do apinhamento ântero-inferior. Os dentes posteriores apresentaram maior expansão das cúspides do que das cervicais, indicando uma possível inclinação causada pelo uso de aparelho dento-suportado. Todas a medidas mostraram algum grau de recidiva, que variou de 2,8% a 8,84%. Os achados desta pesquisa permitiram concluir que a DOMM é uma alternativa eficaz para o tratamento do apinhamento associado à deficiência transversal de mandíbula. / The purpose of this study was to evaluate the effects of Mandibular Symphyseal Distraction Osteogenesis (MSDO) with a tooth-borne distraction device, analysed in dental casts. The sample included 14 patients (5 males and 9 females) between 13 and 19 years of age, with a mean age of 15,74 ± 1,89 years at the start of treatment. Dental casts were obtained at the start of treatment (T1), three months after distraction (T2) and at the end of treatment, when fixed appliances were removed (T3). The data were analysed statistically using ANOVA and Duncan’s test and showed that all transverse widths increased, arch length was maintained, arch perimeter increased significantly and Irregularity Index decreased, showing the correction of crowding. Molars and premolars had a disproportionate pattern of distraction, characterized by greater cusps expansion than cervical one. This pattern might be caused by the use of a tooth-borne distraction device. All the measurements demonstrated evidence of relapse, that varied from 2,8% to 8,84%. The findings of this study indicate that MSDO is an efficient treatment alternative for dental crowding and mandibular transverse deficiency.

Permite mensurar a densidade do osso alongado, por meio da avaliação dos tons de cinza. Desta forma, pode-se realizar uma análise quantitativa da densidade óssea, dimensionando a atenuação dos feixes de raios X pelos tecidos moles nas imagens. OBJETIVOS: Avaliar, por intermédio da TCCB, o osso mandibular neoformado em mandíbulas de coelhos submetidos à DO, sob efeito biomodulatório da irradiação por laser infravermelho AsGaAl (λ=830 nm, 40 mW), aplicado durante a fase de ativação do aparelho distrator. A área da distração é avaliada pelos níveis de cinza das imagens, observando também a influência dos tecidos moles na atenuação dos feixes de raios X. MATERIAIS E MÉTODOS: Dez coelhos foram utilizados (dois grupos experimentais e um grupo controle). Os grupos experimentais foram submetidos à DO e à terapia a laser de baixa potencia (LLLT). Os coelhos do grupo experimental A foram mortos após 11 dias do início da DO e os coelhos do grupo experimental B, mortos após 21 dias do início da DO. Os 10 coelhos foram submetidos a tomografia com tecidos moles e posteriormente dissecados e submetidos a tomografia sem tecidos moles. Assim, foi possível analisar a influência dos tecidos moles em coelhos submetidos à DO na atenuação dos feixes de raios X e, consequentemente, na diminuição dos valores de níveis de cinza observados na imagem tomográfica. RESULTADOS: A presença dos tecidos moles aumenta a atenuação dos feixes de raios X contribuindo na diminuição dos valores de níveis de cinza, observados na imagem do tecido ósseo em 3D, na mandíbula dos 10 coelhos submetidos a tomografia, antes e após a remoção dos tecidos moles. A área das mandíbulas submetidas à distração apresentaram valores superiores de cinza, nos coelhos mortos após 21 dias do início da DO. CONCLUSÕES: Com tomógrafos do tipo Cone-beam, foi possível avaliar a variação da densidade óssea em áreas submetidas à DO. A presença dos tecidos moles contribui na diminuição dos valores de níveis de cinza observados nas imagens do tecido ósseo dos 10 coelhos em 3D. Isto ocorre devido à absorção do feixe de raios X pelos tecidos moles submetidos a tomografia. Também observou-se que a densidade óssea na área da distração, visualizada na tomografia, aumenta proporcionalmente ao período de maturação óssea ocorrido após a fase de ativação da DO, conforme observação dos valores de cinza obtidos. / INTRODUCTION: Cone-beam computed tomography (CBTC) provides detailed information about the amount and quality of new bone formed during distraction osteogenesis (DO). The density of the lengthened bone is measured based on different levels of gray found in the images. The use of CBTC therefore allows to perform a quantitative analysis of bone density and, thus, to measure the level of X-ray attenuation by soft tissues. OBJECTIVES: To assess, via CBTC, newly formed bone in mandibles of rabbits treated with DO under the biomodulating effects of AsGaAl infrared laser radiation (λ=830 nm, 40 mW), applied during the activation phase. The distracted area was assessed based on the gray levels shown on images and observing the influence of soft tissues on X-ray attenuation. MATERIALS AND METHODS: Ten rabbits were used (two experimental groups and one control group). The two experimental groups were treated with DO and low-level laser therapy. Rabbits in experimental group A were killed 11 days after the beginning of DO, and those in experimental group B, after 21 days of DO. CBTC images were obtained from the mandibles of the ten rabbits with soft tissues and subsequently (after dissection) without soft tissues. This comparison allowed to analyze the influence of soft tissues on X-ray attenuation, and consequently on the decrease in gray levels observed in the tomographs of rabbits treated with DO. RESULTS: The presence of soft tissues increased X-ray attenuation, resulting in a decrease in the levels of gray observed in three-dimensional CBCT images of mandibular bone tissue of the rabbits before and after soft tissue removal. The area of the mandibles treated with DO presented higher levels of gray in rabbits killed at 21 days. CONCLUSIONS: CBCT allowed to assess bone density variation in areas treated with DO. The presence of soft tissues contributes to decrease the levels of gray in three-dimensional CBCT bone tissue images of the ten rabbits assessed. This occurs due to the absorption of X-rays by soft tissues studied by tomography. We found that bone density in the area treated with DO, as observed on tomography, increased in direct proportion to the bone maturation period after the activation phase in DO, based on the gray levels observed.

Avaliação dos efeitos do avanço maxilar com distração osteogênica, através de distrator externo rígido (RED), em pacientes com fissura labiopalatina / Evaluation of the effects of maxillary advancement with distraction osteogenesis using a rigid external distraction (RED) device, in patients with cleft lip and palate

Introdução: Os pacientes com fissura labiopalatina, com deficiências maxilares muito severas, geralmente são tratados com avanço maxilar por meio da osteotomia tipo Le Fort I. Entretanto, a distração osteogênica com o distrator externo rígido (RED) pode funcionar como uma alternativa terapêutica para a correção da discrepância esquelética. Proposição: O objetivo do presente estudo é avaliar os efeitos do avanço maxilar por meio da distração osteogênica com distrator externo rígido (RED), associada à osteotomia tipo Le Fort I, em pacientes com fissura transforame unilateral ou bilateral, quanto à quantidade de avanço maxilar e à sua estabilidade a médio e longo prazo. Materiais e Métodos: Para a realização deste estudo longitudinal e retrospectivo, foram usadas telerradiografias em norma lateral de 9 pacientes (6 do gênero masculino e 3 do gênero feminino), onde 4 apresentaram fissura transforame unilateral e 5 apresentaram fissura transforame bilateral, submetidos ao avanço maxilar por meio da distração osteogênica com distrator externo rígido (RED). Foram estabelecidos três tempos de avaliação: fase pré-distração (T1), fase pós-distração imediata (T2) e fase pós-distração controle, com o mínimo de 1 ano após a finalização da distração (T3). A demarcação dos pontos cefalométricos e a obtenção das medidas das variáveis cefalométricas foram realizadas através do software Dolphin Imaging®, versão 11.5. Para a análise dos resultados, o teste estatístico ANOVA de medidas repetidas foi utilizado, adotando-se o nível de significância de 5%. Resultados: No início da distração, a idade média foi de 14 anos e 4 meses (idade mínima de 9 anos, e máxima de 21 anos). O período médio de distração foi de 18 dias, com uma média de ativação no distrator de 1,0mm/dia. O avanço médio da maxila medido em LVR-A, em T2, foi de 15,6mm (p<0,001), com recidiva não estatisticamente significante de 21,79% (p=0,102), em T3. O aumento médio de SNA, em T2, foi de 14,8º (p<0,001), com recidiva não estatisticamente significante de 18,90% (p=0,130), em T3. Os valores médios das medidas SN.GoMe, 1.PP e IMPA não apresentaram variação estatisticamente significante (p>0,05) entre T1, T2 e T3. Conclusão: A terapia de distração osteogênica para avanço maxilar com o RED mostrou ser eficiente, com aumentos significantes das medidas cefalométricas lineares e angulares relacionadas ao avanço maxilar, demonstrando efeito predominantemente esquelético, e estabilidade no período pós-distração médio (T3) de 1 ano e 8 meses. / Introduction: Patients with unilateral and bilateral cleft lip and palate, with significant maxillary hypoplasia are commonly treated with maxillary advancement by Le Fort I osteotomy. However, distraction osteogenesis with a rigid external distraction (RED) device can function as an alternative option for treatment of the skeletal discrepancy. Purpose: The aim of this study is to assess the effects of maxillary advancement by distraction osteogenesis using a rigid external distraction (RED) device, associated with the Le Fort I osteotomy in patients with unilateral or bilateral cleft lip and palate, as the amount of maxillary advancement and their stability in the medium and long term. Materials and Methods: To perform this retrospective longitudinal study, lateral cephalograms of 9 patients (6 males and 3 females) were used, where 4 had unilateral cleft lip and palate and 5 had bilateral cleft lip and palate, who underwent maxilla advancement by distraction osteogenesis with RED device. Three stages of evaluation were established: pre-distraction (T1), immediate post-distraction (T2) post-distraction control, with a minimum of 1 year after completion of distraction (T3). The anatomic landmarks and measurements of cephalometric variables were performed by using the Dolphin Imaging® version 11.5 software. To evaluate the results, the ANOVA test for repeated measures was used, adopting a significance level of 5%. Results: At the start of distraction, mean age was 14 years and 4 months (minimum age 9 years old and maximum of 21 years old). The mean distraction period was 18 days, with a mean rate of distractor activation in 1.0 mm / day. The mean maxillary advancement in LVR-A, at T2, was 15.6 mm (p<0.001), with no statistically significant relapse of 21.79% (p=0.102) at T3. The SNA angle increase, at T2, was 14.8º (p<0.001), with no statistically significant relapse of 18.90% (p=0.130), at T3. The mean values of SN.GoMe, IMPA and 1.PP measures showed no statistically significant variation (p>0.05) between T1, T2 and T3. Conclusion: The therapy of distraction osteogenesis for maxillary advancement with RED is efficient, with significant increases in the linear and angular cephalometric measurements related to the maxilla advancement, demonstrating predominantly skeletal effect and stability in mean post-distraction period (T3) of 1 year and 8 months.

Desenvolvimento e uso de mola absorvível para expansão craniana em coelho

Introdução: O uso de molas metálicas para o tratamento das craniossinostoses têm ganho cada vez mais espaço no arsenal terapêutico do cirurgião Crânio-Maxilo-Facial, na intenção de minimizar procedimentos extensos e mórbidos. Apesar da simplificação cirúrgica promovida pela mesma ainda persiste a necessidade de sua remoção. Objetivos: Realizar expansão craniana cirúrgica em modelo animal, utilizando-se de um implante (mola) totalmente integrado, confeccionado em blenda polimérica bioabsorvível. Testar sua eficácia e realizar análise histológica. Material e Métodos: Estudo experimental, aberto e prospectivo, utilizando doze coelhos fêmeas da raça New Zealand (Oryctolagus cuniculus) com seis semanas de vida. Os animais foram randomizados em dois grupos: controle (G1) aonde foi realizada craniectomia linear, e estudo (G2) aonde além da craniectomia foi inserida uma mola confeccionada a partir de uma blenda de POLI ÁCIDO LÁCTICO-CO-GLICÓLICO/POLI ISOPRENO, com o objetivo de realizar expansão craniana no sentido transversal à ostectomia. A movimentação craniana foi mensurada radiologicamente nas 12 semanas seguintes, através de marcadores e ao final foi realizada análise histológica para avaliação de reação inflamatória. Resultados: As molas confeccionadas apresentaram uma força média de 4,2N. A expansão craniana no grupo estudo no nível do marcador frontal foi de 9,6mm a 11,67mm e foi significante em relação a grupo controle. A análise histológica demonstrou pequena reação inflamatória. Conclusão: É possível a realização de expansão craniana em modelo animal utilizando-se uma mola bioabsorvível através de craniectomia linear, com boa tolerabilidade dos tecidos circunjacentes ao implante. / Introduction: The use of metal springs for the treatment of craniosynostosis have gaining more and more posicion in the therapeutic armory of the Craniofacial surgeon in the intention to minimize morbid and extensive procedures. Objectives: Perform cranial expansion surgery in an animal model, using an implant elastic (spring) fully integrated, made of bio-absorbable polymeric material. Test its efficacy and histological analysis Methods: An experimental study, open, prospective, using twelve New Zealand female rabbits (Oryctolagus cuniculus) with six weeks. The animals were randomized into two groups: control (G1) where linear craniectomy was performed and study (G2) where besides the craniectomy a spring made from a blend of POLY LACTIC ACID-CO-GLYCOLIC / POLI ISOPRENE, in order to perform cranial expansion in the transverse osteotomy. The cranial movement was assessed radiographically at 12 weeks following the surgical procedure and the final histological analysis was performed to evaluate the inflammatory reaction. Results: The springs made showed a medium force of 4,2N. The cranial expansion in the study group in the level of the frontal marker was of 9,6mm to 11,67 and was significant in relation to the control group. The histological analysis showed a small inflammatory reaction. Conclusion: It is possible to perform cranial expansion in an animal model using a bio-absorbable spring through the linear craniectomy. There was good tolerability of the surrounding tissues to the implant.

Metodos: Foram avaliados 90 modelos de gesso digitalizados de 30 pacientes adultos,18 masculinos e 12 femininos submetidos a ERMAC utilizando aparelho expansor do tipo Hyrax. Os pacientes foram distribuidos em dois grupos: Grupo Sem Contencao (GSC) n= 15; e Grupo Com Contencao (GCC) n=15. No GCC foi instalado um Arco Transpalatino (ATP) como contencao logo apos a remocao do expansor. Os modelos de gesso foram realizados nos periodos: pre-operatorio (T1); aos quatro meses, quando o expansor foi removido (T2) e dez meses apos o termino da expansao (T3). Todos os modelos de gesso foram digitalizados com escaner a laser 3D Vivid 9i (Konica Minolta, Wayne, NJ). Foram avaliadas as medidas: distancias Inter-Cuspides de Pre-Molares e Molares (Inter-Cusp PM) (Inter-Cusp M), Inter- Colo de Pre-Molares e Molares (Inter-Colo PM) (Inter-Colo M), Inter- Borda WALA de Pre-Molares e Molares (Inter-BW PM) (Inter-BW M), altura do palato no molar, area e volume do palato nos tempos T1, T2 e T3. Resultados: Ocorreu um aumento medio significante de todas as variaveis aos quatro meses em relacao aos valores preoperatorios (p<0,05), exceto para a altura do palato que apresentou uma diminuicao significante nesse periodo (p<0,001). A unica variavel que apresentou comportamento diferente entre os grupos aos dez meses foi adistancia Inter-Cusp PM, sendo no GSC a recidiva de 1,84 mm (5,4%) (p<0,001) e esta nao foi observada no GCC (p>0,999). Para as demais variaveis nao houve diferenca entre os grupos sendo as recidivas medias apos dez meses (T3) em ambos os grupos de 0,95 mm (2,99%) na distancia Inter- Colo PM; 0,88 mm (1,75%) na distancia Inter-BW PM, 1,04 mm (2,35%) para distancia Inter- Colo PM, 1,04 mm (2,05%) Inter-BW PM 1,04mm(2,92%)Inter-Cusp M; 0,74 mm (1,9%) na distancia Inter-Colo M e 0,84 mm (1,34%) na Inter-BW M (p<0,05). As medidas da altura, area e volume do palato se mantiveram estaveis apos 10 meses em ambos os grupos (p>0,05). Conclusao: A contencao ortodontica impede a recidiva dental na regiao de pre-molares e nao e necessaria para evitar as recidivas dentais na regiao de molares e ossea na regiao de pre-molares e molares apos (ERMAC) / Introduction: Surgically Assisted Rapid Maxillary Expansion (SARME) is the procedure of choice for treatment of adults with transverse maxillary deficiency greater than 5 mm. The stability of the results after SARME is not yet well established, as well the need to use retainers after removal of the expander. Objective: To evaluate the effect of the orthodontic retainer on transverse maxillary dental and skeletal stability after SARME. Methods: Ninety digitized dental casts of 30 adult patients (18 males and 12 females) submitted to SARME using the Hyrax expansion appliance were evaluated. Patients were distributed into two groups: Group Without Retention (GSC) n = 15; and Group With Retention (GCC) n = 15. In the GCC group, a Transpalatal Arch (TPA) was installed for retention soon after removal of the expander. The dental casts were scanned using a Vivid 9i 3D laser scanner (Konica Minolta, Wayne, NJ). The plaster models were performed preoperatively (T1), 4 months after the expander was removed (T2), and 10 months after the end of the expansion (T3). The distances measured were Inter-Cusps of Premolars and Molars (Inter-Cusp PM) (Inter-Cusp M), Inter-Cervicals of Premolars and Molars (Inter-Cervical PM) (Inter-Cervical M), Inter-WALA edges of Premolars and Molars (PM Inter-BW) (BW Inter-M), palatal height at the molar, and palatal area and volume at T1, T2, and T3. Results: There was an significant average increase of all variables signi at four months compared to pre-operative values (p <0.05), except for the palatal height that showed a significant decrease in this period (p <0.001). The only variable that showed a different behavior between the groups at ten months was the distance InterCusp PM. The GSC mean relapse was 1.84 mm (5.4%) (p <0.001) and this was not observed in the GCC (p> 0.999). The other variables did not differ Abstract | 134 between the groups being the average relapse after ten months (T3) in both groups of 0.95 mm (2.99%) in the distance Inter-Colo PM, 0.88 mm (1.75 %) in the distance BW Inter-AM, 1.04 mm (2.35%) for inter-distance Cusp M, 0.74 mm (1.9%) in the Inter-Colo distance M and 0.84 mm (1 34%) in Inter-M BW (p <0.05). Measurements of height, area and volume of the palate remained stable after 10 months in both groups (p> 0.05). Desenvolvimento e uso de mola absorvível para expansão craniana em coelho

Introdução: O uso de molas metálicas para o tratamento das craniossinostoses têm ganho cada vez mais espaço no arsenal terapêutico do cirurgião Crânio-Maxilo-Facial, na intenção de minimizar procedimentos extensos e mórbidos. Apesar da simplificação cirúrgica promovida pela mesma ainda persiste a necessidade de sua remoção. Objetivos: Realizar expansão craniana cirúrgica em modelo animal, utilizando-se de um implante (mola) totalmente integrado, confeccionado em blenda polimérica bioabsorvível. Testar sua eficácia e realizar análise histológica. Material e Métodos: Estudo experimental, aberto e prospectivo, utilizando doze coelhos fêmeas da raça New Zealand (Oryctolagus cuniculus) com seis semanas de vida. Os animais foram randomizados em dois grupos: controle (G1) aonde foi realizada craniectomia linear, e estudo (G2) aonde além da craniectomia foi inserida uma mola confeccionada a partir de uma blenda de POLI ÁCIDO LÁCTICO-CO-GLICÓLICO/POLI ISOPRENO, com o objetivo de realizar expansão craniana no sentido transversal à ostectomia. A movimentação craniana foi mensurada radiologicamente nas 12 semanas seguintes, através de marcadores e ao final foi realizada análise histológica para avaliação de reação inflamatória. Resultados: As molas confeccionadas apresentaram uma força média de 4,2N. A expansão craniana no grupo estudo no nível do marcador frontal foi de 9,6mm a 11,67mm e foi significante em relação a grupo controle. A análise histológica demonstrou pequena reação inflamatória. Conclusão: É possível a realização de expansão craniana em modelo animal utilizando-se uma mola bioabsorvível através de craniectomia linear, com boa tolerabilidade dos tecidos circunjacentes ao implante. / Introduction: The use of metal springs for the treatment of craniosynostosis have gaining more and more posicion in the therapeutic armory of the Craniofacial surgeon in the intention to minimize morbid and extensive procedures. Objectives: Perform cranial expansion surgery in an animal model, using an implant elastic (spring) fully integrated, made of bio-absorbable polymeric material. Test its efficacy and histological analysis Methods: An experimental study, open, prospective, using twelve New Zealand female rabbits (Oryctolagus cuniculus) with six weeks. The animals were randomized into two groups: control (G1) where linear craniectomy was performed and study (G2) where besides the craniectomy a spring made from a blend of POLY LACTIC ACID-CO-GLYCOLIC / POLI ISOPRENE, in order to perform cranial expansion in the transverse osteotomy. The cranial movement was assessed radiographically at 12 weeks following the surgical procedure and the final histological analysis was performed to evaluate the inflammatory reaction. Results: The springs made showed a medium force of 4,2N. The cranial expansion in the study group in the level of the frontal marker was of 9,6mm to 11,67 and was significant in relation to the control group. The histological analysis showed a small inflammatory reaction. Conclusion: It is possible to perform cranial expansion in an animal model using a bio-absorbable spring through the linear craniectomy. There was good tolerability of the surrounding tissues to the implant.

Avaliação histológica, tomográfica e física do efeito da LLLT na distração osteogênica em mandíbulas de coelhos

In order to achieve the distracted bone, the following protocol was used latency period – three days, activation period-seven days 0.7mm/d, consolidation period- 10 days. The animals were irradiated with infrared laser (GaAl) As (λ=830nm, 40mW) with 10J/cm2 dose per spot applied directly to the distraction osteogenesis site during the consolidation stage at 48hy intervals. Histologically, the area of the new bone was higher on irradiated group (57.89%) than the control (46.75%), p=0.006. The tomographic analysis shows higher bone density, measured by Hounsfield Units (HU) on the broken bone irradiated with laser and better uniform repair. The mechanical properties were the same in the broken bone. On the distracted bone, the hardness was similar in broken groups, but the laser modified the elastic modulus in the these rabbits. / Esta pesquisa teve como objetivo avaliar, por métodos histológicos, tomográficos e físicos, o efeito do laser de baixa potência (LLLT – 830 nm) no osso neoformado em mandíbulas de coelhos submetidas à fratura, sem ou com distração osteogênica (DO). Foram utilizados 10 coelhos para as análises histológicas (quatro para o grupo controle e seis para o experimental) além de cinco coelhos para as análises tomográficas e físicas (um nãofraturado, não-distraído e sem irradiação laser; um fraturado, não-distraído e sem irradiação laser; um fraturado, não-distraído e com irradiação laser; um fraturado, distraído e sem irradiação laser; um fraturado, distraído e com irradiação laser). Para os coelhos que sofreram alongamento ósseo por DO foi adotado o seguinte protocolo: latência – três dias, ativação – sete dias 0,7mm/d, e consolidação – 10 dias. Para os animais irradiados, utilizou-se o laser infravermelho (GaAl)As (J=830 nm, 40 mW) com o seguinte protocolo: dose pontual de 10 J/cm2, diretamente sobre o sítio ósseo submetido à fratura e/ou DO, no período de consolidação óssea, respeitando-se intervalos de 48 horas. O percentual do osso neoformado foi maior no grupo com laser (57,89%) do que no controle (46,75%), p=0,006, conforme observado na análise histológica. A análise tomográfica revelou aumento da densidade, medida em Unidades Hounsfield (HU), dos ossos fraturados que foram irradiados com laser. Com relação às propriedades físicas, pareceu não haver grandes diferenças entre os grupos que sofreram fratura. Nos ossos submetidos à DO, o laser não promoveu alteração da nanodureza, mas modificou os valores do módulo de elasticidade, na amostra estudada.

Avaliação da dureza e do módulo de elasticidade do osso mandibular submetido à distração osteogênica e à LLLT: estudo preliminar em ovelhas

Objectives: To validate the use of nanoindentation tests in research protocols involving animals and to assess the quality of newly formed bone in sheep mandibles submitted to distraction osteogenesis and low-level laser therapy (LLLT) based on hardness and modulus of elasticity values. We also aim to determine the best moment for the application of laser therapy, i. e., during the latency/activation or consolidation periods, and to assess the dimensions of newly formed bone with the use of computed tomography. Background data: Nanobiotechnology is an emerging concept in the field of science and technology that has a potential to radically change the criteria currently used to assess the quality of bone formed after distraction osteogenesis. Methods: Extraoral distraction devices were placed in five sheep so as to achieve 1. 5 cm of lengthened bone in 60 days. Distraction devices were removed 50, 40, and 33 days after surgery. Four animals were also treated with eight applications of infra-red laser (830ŋm), each 48 hours, in three equidistant spots, with 5Jcm2 and 50mW of potence, at different times. One was used as control (no laser therapy). The newly formed bone was submitted to nanoindentation tests for the assessment of hardness and modulus of elasticity and a computed tomography to bone contraction evaluation. Results: When applied in the bone consolidation period, LLLT caused an increase in hardness and modulus of elasticity values. On the other hand, animals irradiated with LLLT during the activation period presented lower values. Computed tomography revealed that the animals using the devices for 33 days presented contraction of the newly formed bone. Conclusions: Nanoindentation tests were able to detect slight abnormalities in bone metabolism and proved to be important tools for the assessment of bone quality after distraction osteogenesis. LLLT provides increased benefits when applied during the bone consolidation period, once it promotes an increase in hardness and modulus of elasticity values. The bone consolidation period should be of at least 3 weeks, so as to prevent bone contraction. / Introdução: A distração osteogênica é um processo terapêutico para induzir neoformação tecidual entre superfícies ósseas vascularizadas, por meio de uma força lenta e progressiva de tração, através de aparelhos funcionais. Objetivos: Validar a utilização dos testes de nanoindentação em pesquisas com modelo experimental não humano, a fim de conhecer a qualidade do osso mandibular neoformado em ovelhas submetidas à distração osteogênica e à terapia a laser de baixa potência (LLLT), baseado nos valores de dureza e módulo de elasticidade. Complementarmente, objetiva-se conhecer o momento mais benéfico para irradiação do laser, se na fase de latência/ativação ou maturação. Ademais, foram realizadas tomografias computadorizadas para avaliar a dimensão de osso neoformado e verificar possíveis contrações desse alongamento. Contexto de Pesquisa: A nanobiotecnologia é um campo emergente da ciência e da tecnologia que tem o potencial para transformar radicalmente, por meio de novos conhecimentos, os critérios atualmente utilizados para conhecer a qualidade do osso formado por distração osteogênica. Materiais e Métodos: Cinco ovelhas foram utilizadas e nelas instalados distratores extra-bucais, com intuito de neoformar 1,5 cm de osso em 60 dias de observação, sendo que os distratores eram removidos aos 50, 40 e 33 dias de pósoperatório. Quatro animais foram irradiados com LLLT infravermelho (830 ŋm), tendo recebido oito aplicações, a cada 48 horas, com três pontos de 5J/cm2 e potência de 50mW, em diferentes períodos terapêuticos; o quinto foi o controle, sem tratamento LLLT. As peças ósseas foram submetidas a testes de nanoindentação para aferir módulo de elasticidade e dureza das amostras e a tomografias computadorizadas para avaliar a contração óssea. Resultados: A LLLT quando utilizada na fase de maturação do osso, promoveu aumento da dureza e do módulo de elasticidade. Por outro lado, os animais irradiados com LLLT durante o período de latência/ativação apresentaram menores valores de dureza e módulo de elasticidade. A análise tomográfica complementar demonstrou que, nos animais com manutenção do distrator por 33 dias, ocorreu contração do osso neoformado. Conclusões: A nanoindentação é um teste capaz de captar as mínimas alterações do metabolismo ósseo e, portanto, uma importante ferramenta na aferição da qualidade do osso em procedimentos de distração osteogênica. A LLLT é mais benéfica quando aplicada no período de maturação do osso, pois promove aumento da dureza e do módulo de elasticidade. O período de consolidação óssea com permanência do distrator deve ser de, no mínimo, três semanas, a fim de prevenir contração do tecido ósseo alongado.
