Análise dos resultados do tratamento da mordida cruzada posterior funcional com o expansor fixo Hyrax

Materiais e Métodos: Oito pacientes com média de idade de 7 anos e 1 mês, portadores de mordida cruzada posterior unilateral funcional foram submetidos a um tratamento ortodôntico por meio de um aparelho expansor fixo, seguindo um protocolo clínico com ativações do parafuso de dois quartos de volta por dia até atingir a sobrecorreção das mordidas cruzadas. Foram obtidas tomografias pré-expansão (T1) e pós-contenção (T2), caracterizado por um período de 5 meses pós-expansão. Sobre estas tomografias foram realizadas medidas da base maxilar, da distância entre as cristas alveolares e da distância intermolares vestibulares e palatinas. Resultados: Os resultados mostraram um aumento percentual em todas as dimensões lineares transversais, com uma diminuição da magnitude dos efeitos dentários (6%) para o osso basal (2,1%). Conclusão: A expansão da maxila foi obtida como efeito geral pós-tratamento, sendo a expansão predominante no aspecto dentário / Purpose: The present controlled clinical trial aimed to investigate the effects of maxillary expansion with modified Hyrax device on the transversal dimension of maxilla in the skeletal, dento-alveolar and dental levels by using a cone beam computerized tomography (CT). Materials and Methods: Eight patients (mean age of 7 years and 1 month) with functional unilateral posterior crossbite were subjected to orthodontic treatment with fixed expansion appliance. The activation of the screw was two-quarter turns per day. CT scan was conducted in the pre (T1) and pos (T2) treatment. The maxillary base, the distance between the alveolar ridges, and the intermolar buccal and palatine distances were measured. Results: An increase of all linear transverse dimensions, decreasing in magnitude from dental (6%) to basal bone (2.1%). Conclusion: Maxillary expansion was observed after treatment in which the dental aspect was the most predominant expanded

Para tal estudo, foram utilizados nove indivíduos jovens de ambos os sexos, com idades variando entre 17 e 35 anos, sem histórico de tratamento ortodôntico e/ou portadores de algum tipo de alteração da articulação temporomandibular (A. T .M.). Para a captação dos sinais eletromiográficos, foram utilizados eletrodos de superfície do tipo Beckman, e um eletromiógrafo TECA TE4 de duplo canal. Os dados obtidos foram submetidos à análise estatística não-paramétrica, empregando-se o teste de Friedman, sendo retirados, a partir de então, os resultados mais expressivos, considerando-se as atividades geradas pelo músculo massetei de ambos os lados. Observou-se uma expressiva atividade nos movimentos de lateralidade com contato oclusal esquerdo, propulsão livre, propulsão com contato oclusal, mastigação incisiva, mastigação molar direita e oclusão cêntrica forçada. Concluiu-se, portanto, que à medida que o balanço oclusal vai sendo alterado, as maloclusões passam a predominar e o músculo masseter sofre modificações estrutural e funcional / Abstract: The objective of this work was to study the action of the masseter muscle through electromyography and to analyze the difference of action potential generated by several pre-established jaw movements in individuaIs with Class II ANGLE malocclusion. We have studied nine young individuaIs from both sexes, with ages ranging from 17 to 35, with no history of orthodontic treatment and/or having some kind of alteration in the temporomandibular joint (T.M.J.). In order to collect the ectromyographic signs we have TECA TE-4 used surface electrodes type Beckman. The data were submitted to non-parametric statistical analysis employing the Friedman's test. From these data with left occlusive contact, free propulsion, propulsion with occlusive contact, incisor mastication, right molar mastication and forced central occlusion were the most, significant movements. O objetivo deste estudo, composto por dois capítulos, foi avaliar fatores morfológicos, funcionais e fisiológicos em crianças e adolescentes de 7 a 17 anos com sinais e sintomas de BS. O primeiro capítulo, refere-se a um estudo do tipo caso-controle, intitulado "Is sleeping bruxism related to temporomandibular disorders, malocclusion and orofacial dysfunction?", que comparou a presença de disfunção temporomandibular (DTM), maloclusão e disfunção orofacial em indivíduos com e sem BS. Foram avaliados 316 indivíduos, dos quais 52 apresentaram BS, de acordo com os critérios de diagnóstico estabelecidos pela Classificação Internacional de Distúrbios do Sono. O grupo controle foi formado por 104 indivíduos pareados de acordo com a idade, gênero e variáveis corporais. Deste modo, a amostra considerada para este estudo foi composta por 156 indivíduos, com idade média de 10,86±2,32 anos. A DTM foi avaliada pelo Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) (Eixo I). Para a maloclusão utilizou-se o Índice de Necessidade de Tratamento Ortodôntico (IOTN) e para as disfunções orofaciais o instrumento The Nordic Orofacial Test Screnning (NOT-s). Os dados foram submetidos à estatística descritiva, teste de Shapiro- Wilks, testes t de Student para amostras independentes, Mahn-Whitney, Chi-quadrado ou exato de Fisher, quando indicados, com nível de significância de ?=0,05. Os resultados mostraram que a prevalência de BS foi maior no gênero masculino. A presença maloclusão e DTM foi similar em ambos os grupos. No entanto, os escores de NOT-S foram significativamente maiores no Grupo Caso e no gênero feminino. Hábitos, problemas na mastigação, na deglutição e nos músculos mastigatórios foram as alterações mais prevalentes da disfunção orofacial em indivíduos com BS. Concluiu-se que a disfunção orofacial foi mais severa e prevalente em crianças e adolescentes com BS; maloclusão e DTM não apresentaram relação com BS. O segundo estudo, intitulado "Evaluation of sleep bruxism, temporomandibular disorder, anxiety and salivary cortisol levels in children and adolescents," teve como objetivo verificar se havia associação entre BS e fatores emocionais, subjetivamente pela avaliação de sintomas de ansiedade e objetivamente pela quantificação dos níveis de cortisol salivar. Verificou-se a influência de características antropométricas, gênero e DTM. O estudo envolveu 316 indivíduos com idade de 10,64±2,24 anos. Os sintomas de ansiedade foram avaliados com a Escala Multidimensional de Ansiedade para Crianças (MASC). Para avaliação do cortisol salivar coletou-se amostras de saliva estimulada em dois dias alternados, ao acordar, 30 minutos e uma hora após e antes de dormir. A quantificação do cortisol foi realizada pela técnica de enzimaimunoensaio. Para as características antropométricas determinou-se o índice de massa muscular (IMC=peso/altura2). A DTM foi avaliada pelo RDC/TMD. Os resultados foram analisados por meio de estatística descritiva, análise de variância e regressão logística múltipla. Não houve associação significativa entre BS e idade, IMC, DTM, ansiedade e níveis de cortisol salivar. O gênero masculino apresentou maior probabilidade de apresentar BS. Os escores de ansiedade foram significativamente mais altos tanto para o gênero feminino quanto para indivíduos com DTM. Conclui-se que crianças e adolescentes com DTM apresentaram maiores escores de ansiedade em relação aquelas com bruxismo e do grupo controle. Escores de ansiedade foram maiores em meninas, porém meninos foram mais propensos a apresentar BS. O BS não apresentou associação com ansiedade, níveis de cortisol salivar, IMC e DTM / Abstract: Sleeping bruxism (SB) is a parafunction characterized by grinding or clenching of teeth that can cause tooth wear and pain, shows high prevalent in children, and is considered a physical response to stress. The aim of this study, a composite of two chapters, was to evaluate morphologic, functional, and physiological factors in children and adolescents age 7 to 17 years with signs and symptoms of SB. The first chapter refers to a case-control study titled ?Is sleeping bruxism related to temporomandibular disorders, malocclusion, and orofacial dysfunction?? It compared the presence temporomandibular disorders (TMD), malocclusion, and orofacial dysfunction in individuals with and without SB. Three hundred and sixteen subjects were evaluated, of whom 52 presented BS, according to the diagnostic criteria established by The International Classification of Sleep Disorders. The control group was composed of 104 individuals matched by gender, age, and body variables. Thus, the considered sample consisted of 156 subjects, with mean age of 10.86±2.32 years. The TMD was assessed by the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) (Axis I). For malocclusion the study used The Dental Health Component of the Index of Orthodontic Treatment Need (IOTN), and for orofacial dysfunction The Nordic Orofacial Test Screening (NOT-S) was employed. Data were submitted to descriptive statistics, Shapiro-Wilks, Student t test for independent variables, Mahn-Whitney, Chi-square, and Fisher's exact tests, when indicated, with a significance level of ?=0.05. The results showed that the prevalence of SB was higher in the male gender. The presence of malocclusion and TMD was similar in both groups. However, the NOT-S scores were significantly higher for the case group than the control groups, as well as for females. Habits, impairment in chewing, swallowing, and masticatory muscles were the most frequent conditions of orofacial dysfunction in subjects with bruxism. It was concluded that the orofacial dysfunction was more severe and prevalent in children and adolescents with SB; malocclusion and TMD had no relation to SB. The second chapter, ?Evaluation of sleep bruxism, temporomandibular disorders, anxiety, and salivary cortisol levels in children and adolescents,? aimed to verify if there was an association between SB and emotional factors, both subjectively, by evaluating anxiety symptoms, and objectively, by determining the salivary cortisol levels. The influence of the anthropometric characteristic, gender, and TMD was verified. The study involved 316 individual with mean age of 10.64±2.24 years. Anxiety symptoms were assessed with the Multidimensional Anxiety Scale for Children (MASC). For assessment of salivary cortisol, salivary samples were collected on two alternate days: upon waking, 30 minutes and 1 hour after waking, and before sleep. Quantification of cortisol was performed by enzyme immunoassay. For anthropometric characteristics, the body mass index was calculated (BMI=weight/heght2). The TMD was evaluated by the RDC/TMD. The results were analyzed using descriptive statistics, analysis of variance, and multiple logistic regression. No significant relation was found between bruxism, age, BMI, TMD, anxiety, and salivary cortisol levels. Males had a higher probability of having SB. Anxiety scores were higher in females and subjects with TMD. It was concluded that children and adolescents with TMD had higher anxiety scores compared to those in the control group and those with bruxism. Anxiety scores were higher in girls, but boys were more prone to BS. Tomou-se uma amostra de 46 indivíduos caucasóides brasileiros, de ambos os sexos, nascidos na região de Piracicaba, SP, portadores de maloclusão classe II, divisão 1 e com idades compreendidas entre 11 e 15 anos inclusive. A amostra foi dividida em 2 grupos, de acordo com a movimentação sofrida pelos incisivos centrais superiores e inferiores com o tratamento: o grupo 1 constou de 25 indivíduos, em cujos cefalogramas finais foi observada redução nos ângulos l.NA e 1.NB, quando comparados com os cefalogramas iniciais; o grupo 2 constou de 21 indivíduos em cujos cefalogramas finais foi observada redução no ângulo 1.NA e aumento ângulo 1.NB, ao se comparar com os cefalogramas iniciais. No Os objetivos do trabalho foram os seguintes: 1. verificar o comportamento dos ângulos SNA, SNB e ANB, segundo as correções cefalométricas, definidas por: a) redução do ângulo 1.NA e redução do ângulo 1.NB; b) redução do ângulo 1.NA e aumento do ângulo 1.NB; 2. verificar se o posicionamento do ponto A é afetado por possíveis movimentações dos incisivos centrais superiores ou por possíveis rotações da maxila; 3. verificar se o posicionamento do ponto B é afetado por possíveis movimentações dos incisivos centrais inferiores ou por possíveis rotações da mandíbula. Os dados foram submetidos à análise estatística, chegando-se às seguintes conclusões: 1. houve redução significativo nos ângulos SNA e ANB, em função do tratamento ortodôntico, independente das variações sofridas pelos ângulos 1.NA e 1.NB; 2. houve aumento significativo no ângulo SNB, no grupo onde os ângulos 1.NA e 1NB sofreram redução; 3. no grupo onde o ângulo .NA foi reduzido e o ângulo 1.NB aumentado, não houve alteração no ângulo SNB; 4. No grupo 1, onde houve redução em ambos os ângulos 1.NA e1.NB, os ângulos SNA e SNB tiveram a mesma influência na redução do ângulo ANB; 5. No grupo 2, onde houve redução no ângulo 1.NA e aumento no ângulo 1.NB, apenas o ângulo SNA exerceu marcada influência na redução apresentada pelo ângulo ANB; 6. houve rotação da maxila no sentido anti-horário, em função do tratamento ortodôntico; 7. o posicionamento ântero-posterior do ponto A não manteve relação com as movimentações sofridas pela maxila e incisivos centrais superiores; 8. não se observou rotação da mandíbula em função do tratamento ortodôntico; 9. O valor do ângulo SNB mostrou-se inversa e altamente relacionado com o do ângulo SNGoMe, que representa o posiciona¬mento da mandíbula em relação à base anterior do crânio; 10. O posicionamento ântero-posterior do ponto B manteve pouco significante com as movimentações sofridas pelos incisivos centrais inferiores / Abstract: The behaviour of points A and B (Downs) and related structures was roentgenographicaly studied in 46 Brazilian caucasian children with Class II, division malocclusion. These children of both sexes and aged from 11 to 15 years, were observed before and after edgewise orthodontic treatment. This malocclusion sample was divided into two different groups, according to the kind of orthodontic correctian imposed to the central maxillary and mandibular incisors: The group 1 had 25 pacients in whose final cephalograms were observed reductions of the 1.NA and 1.NB angles when compared with the initial oneSi The group 2, with 21 pacients which presented 1.NA reductian and 1.NB increase when respectively compared with the original situation. The following points were considered the main objectives of the work: 1. To check the behaviour of the SNA, SNB and ANB angles , according to the following cephalometric corrections: 1.1) concomitant reduction of 1.NA and 1.NB angles. 1.2) reduction of 1.NA angle and increase of 1.NB angle. 2. To verify if the point A position is or not influenced by any orthodontic movement of maxillary central incisors or by a possible maxillary rotation. 3. To verify if the point B position is or not influenced by any orthodontic movement of the mandibular central incisors or by a possible mandibulaar rotation. The found data were than submitted to statistical analysis that made possible the following conclusions: 1. As a result of orthodontic treatment there was asignificant reduction of the SNA and ANB angles, independent changes occurred in 1.NA and 1.NB angles.Of the 2. There was significant increase of the SNB angle in thegroup of pacients whose 1.NA and 1.NB angles were reduced by treatment. 3. No change was detected in the SNB angle. in the group pacients whose 1.NA angle was reduced and 1.NB angle of was increased. 4. In the pacients of group 1, where there was a reduction of both angles 1.NA and 1. NB, the angles SNA and SNB had showed the same paper in the reduction of the ANB angle. 5. In the group 2, where there was a reduction of the 1.NA angle and an increase of the 1.NB angle, it was noted that the reduction on ANB was only affected by changes of SNA. 6. There was an anti-clockwise maxillary rotation as a result of orthodontic treatment. 7. The antero-posterior position of point A had no relationship with the movements of either the maxilla itself or with the maxillary central incisors. 8. It was observed no mandibular rotation as a result of orthodontic treatment. 9. The measure of the SNB angle showed to be inverse andhighly related with the SN.Go.Me angle which represents the relative position of mandible to the anterior cranial basis. 10. Estudo da recidiva do apinhamento ântero-inferior de casos tratados ortodonticamente sem extrações no arco inferior e mecânica Edgewise, na fase pós-contenção

O tratamento ortodôntico possui diversos objetivos, sendo que um dos mais importantes é a estabilidade das correções alcançadas, entretanto, após várias décadas de estudos, evidenciou-se que a estabilidade do alinhamento dos dentes é altamente variável e amplamente imprevisível. Considerando que é de grande valia para os ortodontistas a possibilidade de previsão das prováveis alterações no período pós-tratamento, decidiu-se investigar, neste estudo, a recidiva do apinhamento ântero-inferior. A amostra utilizada consistiu-se de 40 pacientes leucodermas, de ambos os gêneros, com más oclusões de Classe I ou II, tratados com mecânica Edgewise e sem extrações no arco inferior. Avaliaram-se os modelos de estudo e as telerradiografias laterais dos pacientes nas fases inicial e final de tratamento e póscontenção (em média 5 anos pós-tratamento). Avaliou-se, então, a presença de associações entre a recidiva do apinhamento ântero-inferior e os seguintes fatores: o aumento das distâncias intercaninos e intermolares, o aumento do comprimento do arco, o gênero dos pacientes, a realização de extrações no arco superior, a realização de desgastes interproximais no segmento ântero-inferior, o tipo de má oclusão inicial, o padrão de crescimento craniofacial predominante, o grau de apinhamento inicial, o aumento ou não da protrusão dos incisivos inferiores devido ao tratamento, a posição final dos incisivos inferiores, os tempos de tratamento e de contenção. Os dados foram comparados pelo teste estatístico de Mann-Whitney209, e mediu-se o apinhamento ântero-inferior pelo índice de irregularidade de Little. Como resultados, encontrou-se que a recidiva do apinhamento ânteroinferior ocorreu na maioria dos pacientes, porém a porcentagem média desta recidiva foi relativamente pequena (26,54%). O gênero masculino apresentou, em média, maior apinhamento nas fases inicial e póscontenção do que o gênero feminino. A porcentagem média de recidiva foi maior no gênero masculino do que no feminino, mas não estatisticamente significante. O grupo com má oclusão de Classe II e o grupo com padrão de crescimento craniofacial redominantemente vertical apresentaram maior recidiva, porém não significantes estatisticamente. O grupo com apinhamento inicial suave apresentou maior recidiva que o grupo com apinhamento inicial severo, com valores estatisticamente significantes, portanto, pode-se concluir que o grau de apinhamento inicial influenciou e se mostrou associado à recidiva do apinhamento ântero-inferior. O grupo com maiores tempos de contenção apresentou menor recidiva, porém, não significante estatisticamente. Com relação aos demais fatores estudados, aumento das distâncias intercaninos e intermolares, aumento do comprimento do arco, realização de extrações no arco superior, realização de desgastes interproximais, aumento da protrusão dos incisivos inferiores devido ao tratamento, posição final dos incisivos inferiores e tempo de tratamento, nenhum deles se mostrou significantemente associado à recidiva do apinhamento ântero-inferior. / One of the most important objectives of Orthodontics is treatment stability. However, after many decades of research there is a consensus that stability of the aligned teeth is variable and largely unpredictable. Considering the great importance that clinical orthodontists place on the ability to predict when posttreatment changes are prone to occur, it was decided to evaluate the relapse of mandibular anterior crowding. The experimental sample consisted of 40 patients, of both gender, presenting with either Class I or II malocclusions, treated without extractions in the mandibular arch with Edgewise mechanics. The lateral cephalogram and dental casts of each patient were evaluated at three different stages: pretreament, posttreatment and after a mean postretention period of 5 years. The relapse of mandibular anterior crowding was assessed and associations between this relapse and the following factors were investigated: increase in intercanine and intermolar distances, increase in arch length, gender, extractions in the upper arch, interproximal stripping, type of malocclusion, craniofacial growth pattern, degree of initial mandibular anterior crowding, protrusion of the lower incisors consequent to treatment, final position of the lower incisors, treatment time and retention time. The data were evaluated by the Mann-Whitney test, and the mandibular anterior crowding was measured by the Little irregularity index. Results demonstrated that the relapse of mandibular anterior crowding occurred in most of the patients. However, the mean relapse amount was relatively small (26,54%). The male patients showed more mandibular anterior crowding in the pretreatment and postretention periods. The mean relapse amount was greater in the male patients, but not statistically significant. The group with Class II malocclusion and the group with vertical craniofacial growth pattern presented more relapse than the other groups, but not statistically significant. The group with the smaller amount of initial crowding demonstrated a statistically significant greater relapse than the other groups. Therefore, it was concluded that the degree of initial crowding was associated with the relapse of mandibular anterior crowding. The group with greater retention time presented less relapse, but not statistically significant. The other evaluated factors did not present any association with the relapse of mandibular anterior crowding.

Estudo da recidiva do apinhamento ântero-inferior de casos tratados ortodonticamente sem extrações no arco inferior e mecânica Edgewise, na fase pós-contenção

O tratamento ortodôntico possui diversos objetivos, sendo que um dos mais importantes é a estabilidade das correções alcançadas, entretanto, após várias décadas de estudos, evidenciou-se que a estabilidade do alinhamento dos dentes é altamente variável e amplamente imprevisível. Considerando que é de grande valia para os ortodontistas a possibilidade de previsão das prováveis alterações no período pós-tratamento, decidiu-se investigar, neste estudo, a recidiva do apinhamento ântero-inferior. A amostra utilizada consistiu-se de 40 pacientes leucodermas, de ambos os gêneros, com más oclusões de Classe I ou II, tratados com mecânica Edgewise e sem extrações no arco inferior. Avaliaram-se os modelos de estudo e as telerradiografias laterais dos pacientes nas fases inicial e final de tratamento e póscontenção (em média 5 anos pós-tratamento). Avaliou-se, então, a presença de associações entre a recidiva do apinhamento ântero-inferior e os seguintes fatores: o aumento das distâncias intercaninos e intermolares, o aumento do comprimento do arco, o gênero dos pacientes, a realização de extrações no arco superior, a realização de desgastes interproximais no segmento ântero-inferior, o tipo de má oclusão inicial, o padrão de crescimento craniofacial predominante, o grau de apinhamento inicial, o aumento ou não da protrusão dos incisivos inferiores devido ao tratamento, a posição final dos incisivos inferiores, os tempos de tratamento e de contenção. Os dados foram comparados pelo teste estatístico de Mann-Whitney209, e mediu-se o apinhamento ântero-inferior pelo índice de irregularidade de Little. Como resultados, encontrou-se que a recidiva do apinhamento ânteroinferior ocorreu na maioria dos pacientes, porém a porcentagem média desta recidiva foi relativamente pequena (26,54%). O gênero masculino apresentou, em média, maior apinhamento nas fases inicial e póscontenção do que o gênero feminino. A porcentagem média de recidiva foi maior no gênero masculino do que no feminino, mas não estatisticamente significante. O grupo com má oclusão de Classe II e o grupo com padrão de crescimento craniofacial redominantemente vertical apresentaram maior recidiva, porém não significantes estatisticamente. O grupo com apinhamento inicial suave apresentou maior recidiva que o grupo com apinhamento inicial severo, com valores estatisticamente significantes, portanto, pode-se concluir que o grau de apinhamento inicial influenciou e se mostrou associado à recidiva do apinhamento ântero-inferior. O grupo com maiores tempos de contenção apresentou menor recidiva, porém, não significante estatisticamente. Com relação aos demais fatores estudados, aumento das distâncias intercaninos e intermolares, aumento do comprimento do arco, realização de extrações no arco superior, realização de desgastes interproximais, aumento da protrusão dos incisivos inferiores devido ao tratamento, posição final dos incisivos inferiores e tempo de tratamento, nenhum deles se mostrou significantemente associado à recidiva do apinhamento ântero-inferior. / One of the most important objectives of Orthodontics is treatment stability. However, after many decades of research there is a consensus that stability of the aligned teeth is variable and largely unpredictable. Considering the great importance that clinical orthodontists place on the ability to predict when posttreatment changes are prone to occur, it was decided to evaluate the relapse of mandibular anterior crowding. The experimental sample consisted of 40 patients, of both gender, presenting with either Class I or II malocclusions, treated without extractions in the mandibular arch with Edgewise mechanics. The lateral cephalogram and dental casts of each patient were evaluated at three different stages: pretreament, posttreatment and after a mean postretention period of 5 years. Concluiu-se que as proposiçoes apresentadas pelas análises de STEINER 68 (1959) e de TWEED 80 (1966) diferiram significantemente dos resultados obtidos com o tratamento ortodôntico, com exceção das variáveis 1-NA ângulo e FMA. Observou-se que a relaçao 1:1 de HOLDAWAiO 30(1959), referente ao posicionamento do incisivo inferior à linha NB e"esta ao pogônio, foi verdadeira no final do tratamento. Por outro lado, a relaçao 1:1, angulo e distancia, apresentada por STEINER 68 (1959), em sua tabela de "compromissos aceitáveis", para o incisivo superior, não foi verdadeira, enquanto a relação 1:4 do incisivo inferior se apresentou correta. Os "compromissos aceitáveis" da análise de STEINER 68 (1959), não devem ser aplicados a brasileiros portadores de maloclusao Classe I de Angle, entretanto os valores normais preconizados pelo autor, são compativeis com os resultados obtidos com o tratamento ortodôntico / Abstract: A comparative study was made between the propositions presen ted by the cephalometric analysis of STEINER 68 (1959) and TWEED 80 (1966), and the results obtained by orthodontic treatment done on twenty-four individuals who were Brazilian, Caucasian, and carriers of Angle's Glass I malocclusion, and whose treatment was effected with out considering the eventual scheduling of the extraction of four bicuspid teeth. It was concluded that the propositions presented in the ana lysis of STEINER 68 (1959) and TWEED 80 (1966) were significantly different from the results obtained by the orthodontic treatment, with the exception of the variables 1-NA angle and FIMA. It was observed that the 1:1 relation of HOLDAWA 30 (1959) refering to the positioning of the lower incisor to the and the relation to the pogonion was true in the final NB line treatment. On the other hand, the 1:1 relation, angle, and distance presented by STEINER 68 (1959) in his table of acceptable arrangements for the upper incisor, was not true although the 1:4 relation of the lower incisor was shown to be correct. Estudo ortopantomográfico longitudinal das inclinações axiais mesiodistais em pacientes tratados ortodonticamente com extrações dos quatro primeiros pré-molares / Longitudinal ortopantomograph study of mesiodistal axial inclinations in orthodontically treated patients with first premolars extraction

O objetivo desse estudo foi avaliar, por meio das radiografias panorâmicas, as inclinações axiais mesiodistais, em pacientes tratados ortodonticamente com as extrações dos quatro primeiros pré-molares ao início, ao término e cinco anos pós-tratamento ortodôntico e verificar se as alterações ocorridas nas inclinações axiais mesiodistais dos dentes ântero-inferiores poderiam influenciar na recidiva do apinhamento desses dentes. A amostra constituiu-se de 30 pacientes, sendo 16 do gênero feminino e 14 do masculino, com idade média inicial de 13 anos e 1 mês, apresentando má oclusão de Classe I ou II de Angle, tratados ortodonticamente com a técnica edgewise simplificada. As radiografias panorâmicas e os modelos de gesso de cada paciente foram analisados nas fases iniciais (T1), finais (T2) e cinco anos pós-tratamento (T3). Mensurou-se o apinhamento ânteroinferior por meio do Índice de Irregularidade de Little. Os dados foram analisados estatisticamente pela análise de variância a um critério -ANOVA- para identificar se houve diferença entre as fases estudadas e posteriormente realizou-se o teste de Tukey para determinar entre quais fases ocorreu diferença estatisticamente significante. As médias das inclinações axiais mesiodistais nas fases T1, T2 e T3 foram comparadas com os valores médios normais pelo teste t de Student. Para verificar se as alterações das inclinações axiais mesiodistais entre as fases T2 e T3 influenciaram na recidiva do apinhamento ântero-inferior, realizou-se no grupo de pacientes que apresentaram a recidiva, a comparação das médias dos valores das inclinações axiais da fase T2 e T3. Os resultados demonstraram que as inclinações axiais obtidas ao início do tratamento foram diferentes daquelas observadas em uma oclusão normal em 85% dos dentes avaliados. Entretanto, 45% dos dentes ao final do tratamento, e 55% na fase cinco anos pós-tratamento mostraram valores similares às configurações anatômicas normais. Em relação à estabilidade da inclinação no período cinco anos pós-tratamento, 75% dos dentes mantiveram a inclinação obtida ao término do tratamento independente se essas eram ou não semelhantes aos valores normais. As alterações das inclinações axiais mesiodistais entre as fases T2 e T3 não influenciaram a recidiva do apinhamento ântero-inferior. / The objective of this study was to evaluate, through panoramic radiographs, the mesiodistal axial inclination, in orthodontically treated patients which had the four first premolars extracted, at the beginning, at the end and five years posttreatment, and to verify if the alterations in the mesiodistal axial inclination of the mandibular anterior teeth would present some influence in the relapse of their crowding. The experimental sample was comprised by 30 subjects (16 females and 14 males) with an initial mean age of 13 years and 01 month (10 years and 10 months - 18 years and 04 months), presenting either Class I or II malocclusions, treated orthodontically by standard edgewise appliance. The panoramic radiographic and dental casts of each patient were evaluated at the beginning (T1), at the end (T2) and five years posttreatment (T3). The mandibular anterior crowding was measured by Little irregularity index. The statistical analysis in which the data were processed included one-way analysis of variance (ANOVA) in order to identify the changes between the three phases related. As an adjunctive method, the Tukey test was conducted to indicate in which phases of the proposed study the statistically significant changes were occurred. The mean values of the mesiodistal axial inclination at T1, T2 and T3 were compared to the mesiodistal axial inclination of a normal control sample by means of the t test. In the group of patients that presented the relapse of mandibular anterior crowding (15 patients), the mean values of the mesiodistal axial inclination at T2 were compared with the mean value at T3 to verify if the alterations in the axial inclination could influence the relapse of mandibular anterior crowding. Results showed that the mesiodistal axial inclinations of the teeth at the beginning of treatment were different from those observed in a normal occlusion in 85% of the evaluated teeth. However, 45% of the teeth at the end, and 55% of the teeth five years posttreatment showed mean values similar to values of the normal occlusion. In relation of the stability of the mesiodistal axial inclination five years posttreatment, 75% of the teeth maintained the inclination obtained at the end of the treatment, independent if these inclinations were similar or not to the normal values. The alterations in the mesiodistal axial inclination between T2 and T3 did not influence the relapse of mandibular anterior crowding.
