Para tanto, foi utilizada uma amostra de conveniência de 109 imagens de radiografia panorâmica e de TCFC, dos mesmos pacientes. Três observadores calibrados avaliaram um total de 1.875 ápices, que foram classificados de acordo com sua relação topográfica e mensurados quando o ápice se encontrava aquém ou além do assoalho do seio maxilar, em ambas as modalidades de imagens. Concordância entre as duas técnicas de imagens foram examinadas estatisticamente. Após um intervalo de 30 dias, a avaliação foi repetida em 25% da amostra para avaliar estatisticamente a concordância intraobservador; os valores encontrados para o Kappa ponderado na avaliação qualitativa e o coeficiente de correlação intraclasse para a avaliação quantitativa representaram correlação entre considerável-quase perfeito e excelente, respectivamente. Para observar a concordância entre as duas imagens, o Kappa ponderado na avaliação qualitativa mostrou uma leve concordância para as raízes palatinas dos primeiros molares do lado direito e dos segundo molares. Quando comparados os valores da avaliação quantitativa pelo coeficiente de correlação intraclasse, houve pobre concordância para as raízes palatinas do primeiro molar esquerdo e dos segundos molares, como também para as raízes mesiovestibulares dos segundos molares direitos e disto-vestibulares dos segundos molares esquerdos. Concluiu-se que há uma baixa concordância entre as duas modalidades de imagens quando as raízes se encontram em contato com o assoalho do seio maxilar ou quando foi observada uma projeção das raízes além do assoalho do mesmo na radiografia panorâmica; e uma alta concordância da radiografia panorâmica com a tomografia computadorizada de feixe cônico quando as raízes se encontram aquém do assoalho do seio maxilar / Abstract: The aim of this study was to assess what is the best image that offers better qualitative and quantitative information comparing the panoramic radiography with cone beam computed tomography to evaluate the relationship between the maxillary sinus and dental apices. For this, we used a convenience sample of 109 images of panoramic radiography and CBCT, from the same patients. Three calibrated observers evaluated a total of 1,875 apices, which were classified according to their topography relationship and measured when the apex was beyond/below the maxillary sinus floor in both imaging modalities. Agreement between the two imaging techniques was examined statistically. After an interval of 30 days, the assessment was repeated in 25% of the sample to statistically evaluate the intra-observer, the values found in the kappa test for qualitative assessment and the intraclass correlation coefficient for quantitative assessment represented a considerable - almost perfect correlation and excellent, respectively. To observe the correlation between the two images, the Kappa test for quality assessment showed a slight correlation to the palatal roots of the right first molars and second molars. When comparing the values of the quantitative assessment by intraclass correlation coefficient, there was poor agreement for the palatal roots of the left first molar and second molar, and also for the mesio-buccal roots of the right second molars and disto-buccal of the left second molars. It was concluded that there is a poor correlation between the two imaging modalities when the roots are in contact or when a projection of the roots was observed beyond the maxillary sinus floor in panoramic radiography, and a high concordance of panoramic radiography with cone beam computed tomography when the roots are below the maxillary sinus floor / Mestrado / Radiologia Odontologica / Mestra em Radiologia Odontológica

The Efficacy of Platelet-Rich Fibrin (PRF) on Healing Following Surgical Removal of Third Molar Teeth

Magister Scientiae Dentium - MSc(Dent) / Aim: A clinical trial to determine the post-operative outcomes of a PRF-treated socket versus a conventionally treated socket following surgical removal of third molar teeth. Introduction: Third molar surgery is a procedure many have to endure and which often results in prolonged healing time with consequential absence from work and school. This has motivated clinicians to seek methods to enhance the healing process and in effect, reduce the healing time. Research on the use of PRF in enhancing wound healing in maxillofacial and oral surgery have shown varying results. Therefore, this study was conceived to ascertain the effect PRF has on the post-operative sequelae of third molar teeth. Materials and Methods: This was a split mouth, prospective, single blinded, randomized control trial. The study sample was made up of 26 patients (N=26) who met the inclusion criteria. Four of the patients experienced neurosensory fallout of an associated nerve and were subsequently excluded from the sample. This resulted in the total sample size of twenty-two patients (n=22). Symmetrically impacted maxillary and mandibular third molars were removed under general anaesthesia. Patients were treated in a within subject design: when one side of a patient was treated with PRF, the other side was conventionally treated and acted as a control. The allocation of the side treated with PRF was 'random'. Patients were followed-up on Day 2 and Day 7, respectively. Pain scores were recorded on a visual analogue scale (VAS) using 0 to 10 pain score. Swelling, wound dehiscence, development of alveolar osteitis, wound infection and post-operative bleeding were compared between the intervention and control side. Presence of accessory canals in the furcation region of primary molars

The existence of accessory canals in the furcation region of primary molars has been the subject of extensive research. The objective of this study was to use a micro-computed tomography scanner to analyze accessory canals in the furcation region of extracted primary molars. Sixty-five primary molars which were extracted for purposes not including this study were collected and scanned at a resolution of 9.0 μm using a Skyscan 1176 Micro-CT scanner. For each extracted molar collected, the patients age at the time of extraction was recorded, as well as whether the tooth came from the maxillary or mandibular arch and if the tooth was a first or second molar. DataViewer v1.5.1 software was used to analyze each scanned primary molar for the presence or absence of canals, canal patency and number of canals present. Collected data was analyzed using SPSS 20.0 for Windows. Fisher’s exact and Kruskal-Wallis tests were used to identify association between variables. Canals were observed in the furcation area of 83% of molars scanned, and patent canals were found in the furcation area of 8% of the molars. The mean number of canals per tooth was found to be 2.6. The majority of these canals were found adjacent to the periodontal ligament on the outer surface of the furcation. Second molars were observed to have a higher number of canals than first molars (p<0.05), with maxillary second molars having a mean number of canals of 3.3, mandibular second molars having a mean number of canals of 3.5, maxillary first molars having a mean of 1.8, and mandibular first molars having a mean of 2.1 canals. No significant association was found between patient age and the number of canals present. Micro-Computed Tomography was an effective method to quantify accessory canals in the furcation region of extracted primary molars. / October 2016

O objetivo desse estudo foi avaliar, por meio das radiografias panorâmicas, as inclinações axiais mesiodistais, em pacientes tratados ortodonticamente com as extrações dos quatro primeiros pré-molares ao início, ao término e cinco anos pós-tratamento ortodôntico e verificar se as alterações ocorridas nas inclinações axiais mesiodistais dos dentes ântero-inferiores poderiam influenciar na recidiva do apinhamento desses dentes. A amostra constituiu-se de 30 pacientes, sendo 16 do gênero feminino e 14 do masculino, com idade média inicial de 13 anos e 1 mês, apresentando má oclusão de Classe I ou II de Angle, tratados ortodonticamente com a técnica edgewise simplificada. As radiografias panorâmicas e os modelos de gesso de cada paciente foram analisados nas fases iniciais (T1), finais (T2) e cinco anos pós-tratamento (T3). Mensurou-se o apinhamento ânteroinferior por meio do Índice de Irregularidade de Little. Os dados foram analisados estatisticamente pela análise de variância a um critério -ANOVA- para identificar se houve diferença entre as fases estudadas e posteriormente realizou-se o teste de Tukey para determinar entre quais fases ocorreu diferença estatisticamente significante. As médias das inclinações axiais mesiodistais nas fases T1, T2 e T3 foram comparadas com os valores médios normais pelo teste t de Student. Para verificar se as alterações das inclinações axiais mesiodistais entre as fases T2 e T3 influenciaram na recidiva do apinhamento ântero-inferior, realizou-se no grupo de pacientes que apresentaram a recidiva, a comparação das médias dos valores das inclinações axiais da fase T2 e T3. Os resultados demonstraram que as inclinações axiais obtidas ao início do tratamento foram diferentes daquelas observadas em uma oclusão normal em 85% dos dentes avaliados. Entretanto, 45% dos dentes ao final do tratamento, e 55% na fase cinco anos pós-tratamento mostraram valores similares às configurações anatômicas normais. Em relação à estabilidade da inclinação no período cinco anos pós-tratamento, 75% dos dentes mantiveram a inclinação obtida ao término do tratamento independente se essas eram ou não semelhantes aos valores normais. As alterações das inclinações axiais mesiodistais entre as fases T2 e T3 não influenciaram a recidiva do apinhamento ântero-inferior. / The objective of this study was to evaluate, through panoramic radiographs, the mesiodistal axial inclination, in orthodontically treated patients which had the four first premolars extracted, at the beginning, at the end and five years posttreatment, and to verify if the alterations in the mesiodistal axial inclination of the mandibular anterior teeth would present some influence in the relapse of their crowding. The experimental sample was comprised by 30 subjects (16 females and 14 males) with an initial mean age of 13 years and 01 month (10 years and 10 months - 18 years and 04 months), presenting either Class I or II malocclusions, treated orthodontically by standard edgewise appliance. The panoramic radiographic and dental casts of each patient were evaluated at the beginning (T1), at the end (T2) and five years posttreatment (T3). The mandibular anterior crowding was measured by Little irregularity index. The statistical analysis in which the data were processed included one-way analysis of variance (ANOVA) in order to identify the changes between the three phases related. As an adjunctive method, the Tukey test was conducted to indicate in which phases of the proposed study the statistically significant changes were occurred. The mean values of the mesiodistal axial inclination at T1, T2 and T3 were compared to the mesiodistal axial inclination of a normal control sample by means of the t test. In the group of patients that presented the relapse of mandibular anterior crowding (15 patients), the mean values of the mesiodistal axial inclination at T2 were compared with the mean value at T3 to verify if the alterations in the axial inclination could influence the relapse of mandibular anterior crowding. Results showed that the mesiodistal axial inclinations of the teeth at the beginning of treatment were different from those observed in a normal occlusion in 85% of the evaluated teeth. However, 45% of the teeth at the end, and 55% of the teeth five years posttreatment showed mean values similar to values of the normal occlusion. In relation of the stability of the mesiodistal axial inclination five years posttreatment, 75% of the teeth maintained the inclination obtained at the end of the treatment, independent if these inclinations were similar or not to the normal values. The alterations in the mesiodistal axial inclination between T2 and T3 did not influence the relapse of mandibular anterior crowding.
