Effect of low-cost housing on household and environmental health of residents in Phumlani Village, City of Cape Town

Magister Public Health - MPH / Many poor households in South Africa find themselves living in informal housing and only become proprietors of formal housing via the government subsidy scheme for core low-cost housing, thereby also realizing their constitutional right to housing. The subsidy is however limited and it largely determines materials, and construction methods used. Obtaining a formal low-cost dwelling means that basic services such as electricity, sanitation, water and waste collection, is available to the home owner. Formal low-cost housing settlements are commonly located in poor areas and recipients of the housing subsidy are commonly unemployed or have low-income jobs, and frequently originate from informal settlements where services, albeit limited and often communal, were provided at no cost. This study sought to assess the combined effect of relocating from an informal dwelling to a formal low-cost dwelling and receiving individual house-based basic services of electricity, water, sanitation and waste collection, on environmental- and household health. An ecological study design was used whereby data was collected at "baseline" while households were living in the informal settlement, and again at "2 years relocated" i.e. 2 years after moving in to the formal low-cost dwelling. The study population included all households residing in the Phumlani- and Pelican Park- Zeekoevlei Informal area in the year 2000, who were on the waiting list to receive low-cost core housing units in Phumlani Village and were due to be relocated there. Due to the rapid pace at which construction of new homes occurred not all households could be captured whilst living in the informal settlement, i.e. at "baseline". The actual sample subsequently consisted of 53 households at "baseline", and all, i.e. 124 households at "2 years relocated". Data was collected via a structured interview, whereby one respondent per household was interviewed by a trained fieldworker. Positive health improvements were reported by households in terms of personal and household health. Significant (p<0.05) positive improvements were found for households in formal lowcost housing at "2 years relocated" for exposures to: overcrowded living conditions (PR=1.159, 95%CI=1.153 – 3.328); indoor air pollution due to cooking and heating (PR=2.185, 95%CI=1.655 – 2.885); improper household waste management (PR=7.381, 95%CI=4.313 – 12.633 and inadequate sanitation (PR=0.365, 95%CI=0.255 – 0.523). The incidence of childhood diarrhoea episodes decreased significantly (PR=5.588, 95%CI=1.284 – 24.315) at "2 years relocated". Water access, availability and use also increased significantly (PR=0.212, 95%CI=0.125 – 0.358) 2 years after relocation. Factors that did not improve include levels of employment for which households were found to be worse off, with 16% of households having no person employed at "2 years relocated" as opposed to only 2% at ‘baseline”. Other factors remaining unchanged included incidences of respiratory, skin and eye infections amongst children ≤ 6 years old. Although exposure levels to indoor air pollution decreased for some households, this remained present for others as electricity in combination with bio-mass fuels are still being used for heating and cooking. Environmental health conditions for a variety of factors remained unchanged and there was a reversion back to living conditions and habits of the informal settlement. Littering, dumping of waste within the neighbourhood and a high pest presence, remained unchanged. Subsidised formal housing and associated basic services does have a positive impact on health. However, the amount of free basic services, specifically electricity, provided, in lieu of household energy requirements, does not satisfactorily cover all household needs. Factors such as unemployment and low-incomes hamper the household’s ability to maintain the electricity supply as is needed and for this reason alternatives to reliance on electricity should be included in the design and construction of the low-cost house. The manifestation of poor environmental health conditions indicates that provision of low-cost housing by itself is not sufficient to ensure good environmental health. Therefore hygiene promotion should be included as part of the total beneficiary package.

Towards a pro-poor service-centred public service: The case of delivery to indigents in two Western Cape municipalities: attitudes, practices and policies among municipal officials

Masters in Public Administration - MPA / The problems of the predispositions of municipal public officials and the professionalization of the public service have in the recent past come into the spotlight in service delivery protests involving poor people and unemployed youth. The criticism levelled at bureaucrats within the public service relates to administrative systems, bad policies as well as unresponsive attitudes among frontline staff. The existing perception is that South African street-level bureaucrats are lazy, uncaring, self-serving, unethical, and conceive of the poor in derogatory terms. They are generally only in public service because it is a means of employment or enrichment. Most recent literature on municipal water and free basic services focused on exposing the cost recovery drive of the state and its associated forms of oppressive neoliberal surveillance of the poor. The research sought to primarily understand the attitudes of street-level bureaucrats (SLB’s) within two B-category municipalities in the Western Cape, South Africa. This was done by identifying what motivates them to work in the public sector, how they see and interact with identified poor members of the public (usually defined as municipal indigents), what their value orientations are, and if and how principles of Batho Pele are understood by frontline workers. It explored how these principles are applied when interacting with indigent citizens in their everyday work environment. Additionally, the research explored how poor citizens view their experience of interacting with the state. A qualitative study, using semi-structured questionnaires, was conducted in the Cape Agulhas municipality which is the most southern municipality and the Matzikama municipality which is the most northern municipality of the WC on the west coast. Interviews with frontline municipal employees, senior bureaucrats and residents were conducted. The interviews were conducted, to obtain three different views on the problem, with a total of 71 participants. The participants comprised of 15 street-level bureaucrats, 8 senior staff, with a minimum of 5 years’ tenure, and a total of 43 indigent citizens. Given the sample size, composition and the demographics relating to these municipalities, the research is not generalizable. The public workers in the case studies, I found, cared deeply for the community and the community in turn are very appreciative of this. I also found that indigent citizens were not ashamed of their indigent status although there were those who outright denied being indigent and therefore were not interviewed.

Assessing the effective management of the allocated funding for the provision of free basic services in Polokwane Local Municipality

Most households from rural areas did not have access to basic municipal services prior to the advent of democracy in 1994. The most affected were those from the former homelands and deep rural areas from the former South Africa. Hence, since 1994, the South African government has inherited huge infrastructure backlogs which affect effective delivery of basic municipal services, particularly Free Basic Services (FBS), to alleviate the plight of the rural and poor communities. Municipalities have to be self-sustainable for effective service delivery, even though their revenue base is such that they cannot generate enough income to maintain, extend and sustain basic services. Yet, they are mandated by the Constitution of the Republic of South Africa, 1996 to deliver services to all communities within their areas of jurisdiction, irrespective of whether the beneficiaries of the services contribute to their revenue. For this reason, the national government has committed in the Constitution to allocate funding to municipalities through the equitable share allocations to subsidise the costs of providing FBS to indigent households. However, the impact of the FBS programme on indigent households in the Polokwane Local Municipality has not been significant, given the effective management of the programme. Hence, this study was conducted at the Polokwane Local Municipality to establish the effectiveness of the management of the funding allocated for the provision of FBS. The primary aim of the study was to determine the extent to which the Polokwane Local Municipality effectively manages the provision of FBS and the related funding to establish deficiencies in the system, with a view to improve the effectiveness of FBS delivery to indigent households. The data comprised responses to a questionnaire, semi-structured interviews, documents and studies conducted at the Polokwane Local Municipality. The study highlighted that the Polokwane Local Municipality has developed an indigent policy for the implementation of the FBS programme; however, it is only applied in urban or established areas, thus only covering Polokwane city, Seshego and Westernburg out of a possible 265 settlements. Proper planning is also only done around these three areas, while estimations are made for rural areas or villages in terms of targeted households and budgets. Furthermore, National Treasury provided more than three times the funding budgeted by the municipality to fund the FBS programme. This suggests that the rest of the funds allocated for FBS delivery were used for municipal operations rather than for benefitting indigent households. Therefore, it can be concluded that the Polokwane Local Municipality does not manage the funding allocated for FBS effectively, given the limitations in the registration of indigent households as well as the planning, budgeting and implementation of the FBS programme. Finally, the study could not establish whether the municipality is making any impact on the lives of the indigent households as the municipality has not assessed the impact of the FBS programme on the indigent households since the inception of the programme in 2000. The study will enable the development of proposals and recommendations which can assist in improving the effectiveness of the Polokwane Local Municipality’s management of funding allocated for the provision of FBS. / Public Administration and Management / M.
