Estudo epidemiológico das gestantes adolescentes de Niterói / Epidemiologic study of the pregnant adolescents at Niterói

Trata de um estudo descritivo, que teve como fonte de dados o Sistema de Informaçöes de Nascidos Vivos (SINASC). Foi realizada a distribuiçäo de freqüências de variáveis ali constante, por grupos etários e construiu-se uma variável dicotômica para o recém-nascido denominada "comprometido" (baixo peso ao nascer e/ou prematuridade) visando relacionar a idade da mäe com as condiçöes de nascimento das crianças. Em seguida, comparou-se a distribuiçäo das variáveis entre as adolescentes de 11 a 19 anos com as mulheres de 20 a 49, e posteriormente, entre as adolescentes da primeira etapa (11 a 17 anos) com as da segunda etapa (18 a 19 anos). Como resultados encontrou-se 16 por cento de grávidas adolescentes. As adolescentes, quando comparadas às mulheres adultas, têm chances estatisticamente significativas de ter menor escolaridade e piores condiçöes de nascimento expressas por maior chance de ocorrência de bebês prematuros (OR=1,53), com baixo peso (OR=1,57) e mais deprimidos no 1§ e 5§ minuto de vida (OR1,23 e 1,49 respectivamente). Detectou-se maior proporçäo (14,9 por cento) de recém-natos "comprometidos" entre as mäes adolescentes. Quando comparadas as jovens entre si, verificou-se uma associaçäo estatisticamente significativa entre as adolescentes da primeira fase e as variáveis: parto normal (OR=1,24), Apgar no 5§ minuto (OR=1,72) e näo ter filhos nascidos vivos (OR=3,07). Observou-se maior proporçäo (17,2 por cento) de recém-nascidos "comprometidos" nas jovens de 11 a 17 anos. A escolaridade mostrou-se como fator de confundimento, ao comparar-se as adolescentes às mulheres adultas, nas variáveis: parto normal (OR=1,90); näo ter filhos nascidos vivos (OR=7,64); baixo peso (OR=1,39) e natimortos (OR=3,29). Além disso, verificou-se interaçäo nas duas primeiras variáveis. Entre as adolescentes, a escolaridade foi fator de confusäo para a variável näo ter filhos nascidos vivos (OR=3,44) com interaçäo entre os estratos. Estes resultados reforçam a necessidade de medidas de saúde e de educaçäo específicas voltadas para este grupo etário. / The purpose of this work is to describe the pregnant women’s epidemiologic profile that searched for Niterói’s maternity hospitals during 1994. It’s about a descriptive study, that had as source of data the Sistema de Informações de Nascidos Vivos (SINASC). It was carried out the variable frequency distribution that there is there by age groups and it was built a dichotomic variable to the newborn child denominated “compromised” (low weight when born and/or prematurity) aiming for relate the mother’s age to the children’s birth conditions. Soon afterwards, it was compared the variable distribution among the adolescents from 11 to 19 years old with the women from 20 to 49 years old, and later among the first stage adolescents (11 to 17 years) with the ones from the second stage (18 to 19 years). As results it was found 16% of adolescents pregnants. The adolescents when compared to the adult women, have chances statistically significant to have lower school frequency and worse birth conditions expressed by greater chance of premature babies occurrence (OR=1,53), with low weight (OR=1,57) and more depressed on the 1st and 5th minute of life (OR=1,23 and 1,49 respectively). It was detected greated proportion (14,9%) of “compromised” newborn children among the adolescent mothers. When compared the adolescents amongst themselves, it was verified an statistically significant association among the first stage adolescents and the variables: normal childbirth (OR=1,24), Apgar on the 5th minute (OR=1,72) and not have children born alive (OR=3,07). It was observed greater proportion (17,2%) of “compromised” newborn children on the adolescents from 11 to 17 years old. The school frequency demonstrated as confounding factor when comparing the adolescents to the adult women on the variables: normal childbirth (OR=1,90); not have children born alive (OR=7,64); low weight (OR=1,39) and born dead (OR=3,29). Moreover, it was observed interaction on the two first variables. Among the adolescents the school frequency was confusion factor to the variable of not have children born alive (OR=3,44) with interaction among the layers. These results reinforce the necessity of especific health and education steps turned to this age groups.
