Impact of Automated Dispensing Technology on Medication Safety and Costs at an Inpatient Pharmacy

Class of 2014 Abstract / Specific Aims: To compare two groups of automated dispensing technology and their impact on medication safety and costs at an inpatient pharmacy. Methods: A total of 784 medications were audited for Pyxis refill errors, 352 prior to and 432 post implementation of Boxpicker and the ATP High Speed Tablet Packager. Data were collected by obtaining refill reports for automated dispensing. Every other medication on the refill report was audited for errors in the corresponding location of the automated dispensing cabinet. The rate of reported errors was obtained from a self-reported error program, Patient Safety Net (PSN). Analysis related to costs included automated dispensing cabinet related inventory and costs associated with bulk repackaging. All data associated with costs were obtained from pharmacy financial records. Main Results: There was no significant difference in the Pyxis refill error rate between Pyxis PARx and Boxpicker (0.00284% versus 0.00231%, respectively, p =0.88). The total number of automated dispensing cabinet problems reported through Patient Safety Net transiently increased during and after implementation of new automated technology. Value of pharmacy inventory costs associated with automation was $674,460 prior to and $594,789 post implementation of technology. Bulk repackaging with the ATP High Speed Automatic Tablet Packager resulted in an estimated cost savings of $203,400 annually. Conclusion: Implementation of Boxpicker and ATP High Speed Tablet Packager resulted in no significant change in Pyxis refill error rates, a transient increase in reported automated dispensing cabinet problems, a decrease in inventory costs, and savings associated with bulk repackaging.

Terrain suitability analysis for the proposed rapid-rail link between Pretoria, Johannesburg and Johannesburg International Airport

M.A. / The proposal to build the Gautrain rapid-rail link between Pretoria, Johannesburg and Johannesburg International Airport was approved in June 2000 by the Gauteng Provincial Government. It was noted that this particular development could have serious environmental implications for the area involved. The aim of this research is therefore to identify the most suitable terrain in order to optimise the rail route alignment with minimal environmental impact. This will be accomplished by undertaking a terrain suitability analysis. The different high-speed railway types are discussed and their impact on the environment is considered, culminating in an evaluation of the background and status of the proposed Gautrain rapid-rail link. The different approaches to terrain evaluation are discussed in order to draw conclusions relative to the methodology used in this research. Subsequently the activity approach as described by Mitchell (1991) and Hugo et al (1997) was chosen as the methodology to use for this research and the terrain within the study area was classified according to its suitability for the development of the Gautrain rapid-rail link. A Geographic Information System (GIS) was utilised to aid in the terrain suitability analysis. As a result of the analysis an optimised route is proposed and compared to the rail routes already proposed for the Gautrain rapid-rail link. It is imperative that a terrain suitability analysis should form part of the preliminary phase of any environmental management cycle as portrayed in Fuggle & Rabie (1998), in order to identify suitable terrain for the development in question; the rapid-rail link route should be no exception.

An investigation of machining induced residual stresses on Grade 4 and 5 titanium alloys

M.Ing. (Mechanical Engineering) / Titanium and its alloys have the potential to serve as a strategic economic driver of the South African economy. The manufacture and use of high strength, lightweight materials such as titanium alloys have become of great importance in the aerospace and biomedical industries over the past few decades. The manufacturing costs of titanium alloy components however, are considered high due to the poor machinability of the material. Furthermore, as with all metals during machining, surface residual stresses are induced into the material. These are of particular interest in the aerospace industry as they can be either detrimental or beneficial to the performance and fatigue life of materials. The aim of this investigation is therefore to examine the effect that machining parameters have on the magnitude, sign and distribution of residual stresses induced in Grade 4 and 5 titanium alloys during high performance machining (turning). The effect of these machining parameters is investigated by residual stress measurements conducted with X-ray diffraction and grain structure analysis of the machined surfaces by optical microscopy. Results show that cutting speed and depth of cut have a significant effect on the residual stresses. At low cutting speeds, the surface residual stresses are largely compressive, becoming more tensile with an increase in cutting speed. An increase in depth of cut also introduces more compressive residual stresses into the material. The microstructural analysis of the alloys shows that grain deformation decreases with an increase in cutting speed and cutting depth.
