The contribution of smallholder agriculture to the nutrition of rural households in a semi-arid environment in South Africa

The contribution of own food production to the nutrition of households in two neighbouring, rural, semi-arid settlements was investigated. A survey of a 10% probability sample (n=131) of households in Sekuruwe and Ga-Molekane in the Mokgalakwena Local Municipality, Waterberg District Municipality, Limpopo Province, South Africa, conducted in 2001, provided data on household composition, income (cash and kind), poverty status, expenditure and agriculture, including a detailed account of the types and quantities of food that were purchased during the month preceding the date of the interview. For each household the food obtained from the different types of agriculture they practised was quantified. Protein, iron and Vitamins A and C were selected as indicators to assess the contribution of purchased and own produced food to the food intake of households. Food composition tables were used to estimate the nutrient content of the different foods. To assess the contribution of irrigated home gardening to food intake of households, Drum & Drip micro-irrigation systems which enabled irrigated vegetable production on an area of 36 m2 were installed on the residential sites of 10 volunteer households in the study area. The results confirmed that income is the most important determinant of household food security in rural South Africa. However, food obtained from various types of dry-land agriculture contributed significantly to household nutrition and without farming the food security of households would be reduced, especially among the ultra-poor. Small-scale irrigated vegetable production was shown to have the potential to substantially raise the amount of the Vitamins A and C available to households but did not address the lack of protein in the diet of ultra-poor households and the lack of iron in the diet of all households.

Promoting household food security and reducing malnutrition rates of a growing population with the same amount of water is a challenge facing South African nutritionists and agriculturalists alike. Apart from non-food related effects of agriculture in general, the crop and livestock production practices of the South African smallholder farmer may have nutritional implications, primarily when practised on residential land and resulting in home consumption. Yet, few studies have systematically investigated the impact thereof. It appears that crop diversification, gender issues and nutrition education are among the important factors that strengthen the link between agriculture and nutrition. Since food production is the most water-intensive activity in society, nutritional water productivity (i.e. nutrition per volume water) of foods and the nutritional water footprint of diets should be investigated in order to achieve a sustainable solution. This implies that both the demand for a diet consisting predominantly of water-productive plant products, as well as the supply thereof, be addressed.

The association between household food security and mortality in children under-five years of age in Agincourt, Limpopo Province, in 2004

Background: When children experience food insecurity, in addition to poverty, their resultant inadequate food intake and disease often leads to the development of proteinenergy malnutrition and ultimately to death. In South Africa, where three out of every four children live in poverty, food insecurity and its multiple negative effects are consequently among the most urgent social issues affecting households and their children. Since household food insecurity is thought to be associated with increased child mortality, it is important to study any such associations amongst South African children to determine additional risk factors for child mortality. Objectives: The main objective of this study was to establish the relationship between household food security and mortality in children under the age of five years in the Agincourt field site, Limpopo Province, in 2004. Methods: An analytical cross-sectional study of secondary data obtained from the 2004 census questionnaire and food security module of the Agincourt Health and Demographic Surveillance System in rural Limpopo Province was conducted, involving a total of 7,790 black children under the age of five years. Certain exposure variables were selected for use as indicators of food security and these were analysed with respect to child mortality using univariate and multivariate logistic regression. Results: Based on the outcome indicators of food consumption, 37% of the study population were found to have experienced household food insecurity in 2004, reporting insufficient food for the entire household in the previous month and year. The limited dietary diversity and insufficient quantities of food experienced by the majority of the population were supplemented by the local growth of food crops and the gathering of food from the bush. Of the 79 children (1%) under the age of five years who died in 2004, the majority (24%) died of HIV-related diseases, in addition to deaths caused by diarrhoea, respiratory infections, and malnutrition. Child mortality was found to be associated with the reporting of “unknown” for several indicators of food security. Additionally, expecting the food availability of the household in the coming year to be less than that of the current year (that is, the prediction of future household food insecurity) was significantly associated with an increased risk of under-five child mortality compared to the expectation of the same amount of food the following year (adjusted odds ratio (OR) 2.0), and with a greatly increased risk of mortality compared to the prediction of more food (future household food security) (adjusted OR 4.4). The latter association was age-specific to infants under the age of one year (adjusted OR 5.6) and cause-specific to HIV deaths (adjusted OR 5.9). Conclusions: Following a significant trend in this study in the rural north-east of South Africa, future household food security was inversely related to, and hence protective over, childhood mortality in 2004, even after controlling for confounding factors. Further research on the associations between household food security and under-five child mortality, conducted following the development of a standard nation-wide food security measurement tool specific to South African household conditions, would confirm household food insecurity as a significant risk factor for under-five child mortality and, consequently, as a target for future policies in the reduction of child mortality in this country.

Linking Household Food Inventories with Dietary Recalls to Examine the Association between Nutrient Availability and Dietary Intake among Mexican-origin Children who Reside in Texas Border Region Colonias

The purpose of this study was to determine the relationship between household food inventories (HFI) and dietary recalls among Mexican-origin children (ages 6-11 years old) who reside in Texas border region colonias. Household food availability is a known influence upon children's diets; however, this population faces unique influences upon children's dietary intake and household food availability may not exhibit the same influence upon children's dietary intake that it does in other areas of the United States. This study utilized promotoras (community healthcare workers native to the Texas border region) to collect data from participants due to their rapport with residents of the community. These promotoras collected a series of surveys, HFI's and 24-hour dietary recalls with each mother-child dyad participant. Data from the HFI-s and 24-hour recalls were entered into the Nutrition Data System for Research (NDS-R) from the University of Minnesota and the nutrient profiles for each were analyzed. The HFI data was then adjusted for household composition to obtain a more accurate representation of what nutrients are available to each specific member of the household. After analysis, participant children reported living in households with 5.7 adults and children (range 3-10). All children participated in school breakfast and lunch programs. Age- and gender-specific body mass index (BMI) percentiles indicated that 42% were considered overweight or obese. Lower food security was associated with greater energy, total sugar, and added sugar intakes. The largest correlations between HFI's and 24-hour intakes were for total protein, total sugar, sodium, and added sugar. Sodium was the only nutrient with a significant correlation between household availability and children's dietary intake. HFI was independently associated with greater intake of sodium and lower intakes of total sugar and added sugar. Results show a relationship between household food availability and children's dietary intake. However, at the nutrient level, this association is only statistically significant for sodium and almost significant for total sugar. Colonia children are likely eating many meals outside of the home; the traditional HFI might not be useful in determining what foods are available for these children to eat.

Using Multiple Household Food Inventories to Measure Food Availability in the Home

The purpose of this study was to determine the feasibility of conducting multiple household food inventories over the course of 30 days to examine weekly food variability. Household food availability influences the foods individuals choose to consume; therefore, by assessing the home food environment a better understanding of what people are eating can be obtained. Methods of measuring home food availability have been developed and tested in recent years; however most of these methods assess food availability on one occasion only. This study aimed to capture "usual" availability by using multiple assessments. After the development and pre-testing of the 171-item home observation guide to determine the presence and amount of food items in the home (refrigerator, freezer, pantry, elsewhere), two trained researchers recruited a convenience sample of 9 households (44.4% minority), administered a baseline questionnaire (personal info, shopping habits, food resources, and food security), and conducted 5 in-home assessments (5-7 day interval) over a 30-day period. Each in-home assessment included shopping and fast food activities since the last assessment and an observational survey of types and amounts of foods present. The final in-home assessment included an audio recorded interview on food habits and beliefs. Complete data were collected from all 9 women (32.8 y +/- 6.0; 3 married; 4 +/- 1.6 adults/children in household; 4 SNAP; 6 food insecure) and their households. Weekly grocery purchases (place, amount, and purpose) use (frequency) varied from once (n=1) to every week (n=5); 4 used fast food 2-3 times/wk for 4 weeks. Quantity and types of fresh and processed fruits and vegetables varied by week and by family. The feasibility of conducting multiple in-home assessments was confirmed with 100% retention from all participants. This methodology is important in that it provided detailed information on intra-monthly variation in food availability. The findings suggest the inadequacy of a single measure to assess food availability in the home.

Fast track land reform programmes and household food security : case of Mutare district (Zimbabwe)

The research attempted to demystify the Zimbabwean land reform that was spear headed by war veterans’ in Zimbabwe. This research investigated the impact of the Fast Track Land Reform Programme (FTLRP) in 2000 on Household Food Security. It was generally assumed that the programme did not improve Household Food Security. To verify that assertion the research used questionnaires in a survey research design. The questionnaires were administered to 322 household heads that had been selected by the random stratified sampling method in Mutare District. The results established that Household Food Security in Mutare District improved after the implementation of the FTLRP. The national grain storage however was depleted because the new farmers reduced the production levels set by the former white farmers. The research therefore recommends an orderly and sustainable transition of Land Reform in future programmes to enhance national grain reserves. This also further improves the Household Food Security.

The role of land consolidation programme in household food security in Rwanda : a case study of household farmers of Gisenyi Village of Bugesera District.

The aim of this study is to investigate the outcomes of the Land Consolidation Programme (LCP) in household food productivity. The implementation of the 2004 Rwandan National Land Policy (which incorporates LCP) has been one of the Rwandan government strategic attempts to improve the livelihood of the Rwandans. In this study we look at some of the social-economic factorsbenefited by household farmers through the LCP since its implementation in 2007. In Sub-Saharan Africa many people depend on land for their livelihood and consequently, one of the obvious negative impacts has been the fragmentation of land. Historically the customary land management, in which inheritance is the major mode of land acquisition, has been the main way of allocating land in African societies. This communal tenure is viewed as unstable and leads to detrimental implications, in the form of mismanagement and overexploitation of the available land. The demographic pressure has also aggravated the issue of land scarcity and land fragmentation. The latter has consequences on agricultural productivity since it makes harder the efficient use of land. In this study the researcher explores the outcomes of the LCP in Rwanda as a type of land reform that aims at preventing fragmentation of land and enhancing the livelihood of household farmers. For achieving this objective, the study used a case study of household farmers from Gisenyi village of Bugesera district (in Rwanda) who are involved in the LCP since its implementation. Empirical data was obtained through in-depth interviews with 20 household farmers and 8 key informants. The emphasis in the study was put on investigating the state of household food productivity in Gisenyi. The study was guided by the property right theory and its basic conceptual assumption of enhancing the income through credit access. The findings of the study demonstrate that household farmers in Gisenyi village have benefited from the LCP. Household farmers confirmed that agricultural productivity has increased due to the new farming techniques brought by the programme. The study concludes that once the programme is properly and fully implemented, the LCP will highly enhance food self-sufficiency situation in Rwanda, improving also the livelihood of rural areas through other benefits such as infrastructure development.

This thesis examines the economic and time barriers to dietary adherence for T2D patients living in Edmonton by using utility theory, household production theory and the concept of health capital. Socio-demographic, food consumption, food purchase and time use information was obtained by administering a questionnaire and a food record; collecting grocery receipts and a blood sample; conducting a telephone interview, and taking measurements. Multivariate regression analysis and correlations showed a negative association between fruit and vegetable expenditure and A1c. Diet quality was negatively associated with A1c and total food expenditure but had an inverted U-shaped association with income. While working time was negatively correlated with diet quality and positively correlated with A1c, regression analysis showed a negative association between working time and diet quality only among higher income participants. Budget constraints and time constraints appear to be the barriers to dietary adherence among low-income and high-income patients, respectively. / Agricultural and Resource Economics
