Aphakic iris-claw (Artisan®/Verisyse) lens implantation in low-income African population.

Purpose: To test the viability of implanting the Artisan®/VerisyseTM lens in a low-income monocular aphakic African population with insufficient capsular support where contact lens wear is expensive and spectacle correction is not possible, by assessing the postoperative visual functions. To further assess whether adverse effects (e.g.: pigment dispersion with secondary glaucoma, prolonged uveitis) in patients with highly pigmented irises would be as low as with patients in European and American trials. Methods: A prospective, randomised, controlled clinical trial comparing outcomes in two groups of unilateral aphakic patients. The patients in the first group received an Artisan intra-ocular iris-claw lens as a secondary procedure while the second group remained aphakic (the current treatment status quo for public patients in KwaZulu-Natal province in South Africa). Follow-up was done for 1 year. Results: The study was terminated early due to ethical and statistical reasons. Nine treated and five control patients were included. Monocular uncorrected vision was significantly higher in the treatment group (P=0,012) and patient satisfaction was higher (p=0,002). Changes in other variables (intraocular pressure, angle pigmentation, change in cup-to-disc ratio, iris pigment changes and best spectacle corrected vision) were not significantly different between the two groups. Conclusion: The Artisan®/VerisyseTM lens is a feasible option for aphakic African patients with regard to visual outcome, safety and patient satisfaction. This form of refractive correction should be the standard for patients with no capsular support and where other options are too expensive or carry greater risk. / Thesis (M.Med.)-University of KwaZulu-Natal, Durban, 2007.

Psychophysics-Based Electrode Selection for Cochlear Implant Listeners

<p>Cochlear implant listeners are presented with a time and frequency-quantized version of speech signals. In the frequency domain, resolution is limited by the number of electrodes in each listener's array. Current cochlear implant speech processing strategies implicitly assume that the information presented to each one of these electrodes is perceived as unique and independent. However, previous research suggests that stimuli presented on different electrodes can be indiscriminable (e.g. Zwolan et al., 1997; Throckmorton and Collins, 1999; Henry et al., 2000) . Additional studies suggest that stimuli presented on one electrode can influence the perception of stimuli on neighboring electrodes (e.g. Shannon, 1990; Chatterjee and Shannon, 1998; Boëx et al., 2003). Removing this redundant or occluded information could cause more distinct or perceivable information to be presented to the listener and possibly result in improved speech recognition.</p><p>Previous studies have used psychophysical data to identify the electrodes with the highest potential to confound speech recognition (Zwolan et al., 1997, Boëx et al., 2003, and Garadat et al., 2012). In order to minimize electrode interactions and maximize the amount of perceivable information, each of these studies used a single psychophysical metric to deactivate the electrodes across all time windows of the speech processing strategy. For some listeners, these reduced electrode sets resulted in improved speech recognition over using the of the electrodes in their array. These studies did not compare the results of using different psychophysical metrics to exclude electrodes for a group of listeners nor did they investigate speech recognition performance as a function of the number of electrodes excluded from the array.</p><p>In this work, three different psychophysical metrics were used to obtain a multidimensional estimate of the potential "usefulness'' of each electrode. These results were then used to inform two different methods of psychophysics-motivated electrode selection. The first method incorporated individual data into each listener's energy-driven speech processing strategy. For each time window, the electrodes with the highest energy that were also most likely to be perceived, according to the psychophysical data, were selected for stimulation. The second method sequentially excluded the electrodes with the highest potential to confound from the array across all time windows, resulting in a group of psychophysics-motivated electrode sets for each metric. Evaluating each of these electrode sets exhaustively would require a prohibitive amount of experimental time. To mitigate this problem, an adaptive procedure was developed to estimate performance as a function of cochlear implant parameters in a time-efficient manner. For each metric, the procedure estimated the set with the highest estimated probability of correct phoneme identification. Listeners' speech recognition performance using this electrode set was then compared to their performance using their full electrode array. For both electrode selection methods, listeners' speech recognition scores were generally comparable to those obtained in the clinical condition. This finding supports the hypothesis that listeners were not perceiving all the information presented to them using their clinical speech processing strategy and their complete set of electrodes. Additionally, these results suggest that improvements to the proposed electrode selection strategies should be in investigated in order to increase the amount of perceivable information presented to cochlear implant listeners.</p> / Dissertation

Corrosion and Fretting Corrosion Studies of medical grade CoCrMo implant material in a more clinically relevant simulated body environment.

In modular hip implants, micro-motion, which leads to fretting corrosion at the head/neck and neck/stem interfaces, has been identified as a major cause of early revision in hip implants, particularly those with heads larger than 32mm. It has been found that the type of fluid used to simulate the fretting corrosion of biomedical materials is crucial for the reliability of laboratory tests. Therefore, to properly understand and effectively design against fretting corrosion damage in modular hips, there is the need to replicate the human body environment as closely as possible during in-vitro testing and validation. In this work, corrosion behavior of CoCrMo in 0.14 M NaCl, phosphate buffered saline (PBS) and clinically relevant simulated body fluid (sbf) is carried out. Also, fretting corrosion studies of the CoCrMo alloy in a clinically relevant novel simulated body fluid (sbf) environment is studied. The presence of phosphate ions in PBS accounted for the higher corrosion rate when compared with 0.14 M NaCl and sbf environment. Despite the low and comparable corrosion rates in 0.14 M NaCl and sbf, the nature of the protective passive film formed in sbf shows the suitability of the novel sbf for future corrosion and fretting corrosion analysis. Finally, the influence of micro-motion at the modular head/neck and neck/stem interfaces on the concentration of metallic ions that goes into the synovial fluid and surrounding tissues is reported.

Long-term Complications Associated with Implant-supported Complete Fixed Dental Prosthesis

Rehabilitation of edentulous patients with Implant-supported Complete Fixed Dental Prosthesis (ICFDP) is a well-documented treatment option. This dissertation assessed the relation between the rate of biological/mechanical complications and the type of metal framework alloy, length of cantilever extension. The results showed that long-term clinical outcomes of ICFDP were favorable. While 30% of patients experienced biological complications, 66.6% of the prostheses needed to be repaired during follow-up period. The risk of prosthesis failure and mechanical complications was significantly higher in silver-palladium frameworks as compared with palladium-silver or type IV gold alloys. The length of cantilever was not correlated with the amount of marginal bone loss. The rate of marginal bone loss around anterior implants was higher than that of posterior implants associated with cantilever segments. The treatment improved the patients’ quality of life and 96% of patients would undergo the same treatment again if required.

The efficacy of biodegradable mesh as a fixation device for support of autogenous onlay bone grafts : a radiographic and histomorphometric analysis

Objective. The objective of this study is to test the efficacy of the biodegradable mesh as an alternative fixation device to the titanium mesh for support of onlay particulate, bone and cancellous, marrow (PBCM) grafts. Two parameters were evaluated: (1) Bone graft volume maintenance; and (2) Calcified tissue surface area and distribution. Methods. 12 New Zealand white rabbits were used and divided equally into two groups. In both groups, endochondral, cancellous bone graft was harvested from the anterior ilium. The bone graft was then compressed and placed as an onlay onto the lateral aspect of the mandible using two chambers for each animal. In the control group, the chambers were made of titanium, however in the test group the chambers were made of poly L-lactide, polyglycolide and trimethylene carbonate mesh (Inion GTR(TM) Finland), and thereafter the chambers were fixed to the mandible. The animals were then sacrificed after eight weeks postoperatively, and then Micro-CT imaging was performed for the entire sample in order to determine the total volume of calcified tissue present under each chamber. In addition, Histologic sections were obtained from each chamber, and thereafter stained using Toluidine blue and Von Kossa for the purpose of histomorphometric analysis in order to determine the calcified tissue surface area. Results. When comparing the data from both the titanium (Ti) and the biodegradable mesh (PLA) groups, Micro CT analysis showed no significant statistical difference (P-value = 0.546) with regard to the percentage of bone found under the chambers (Ti 15.0% and PLA 13.83%). Interestingly, neither did the histomorphometric analysis show any significant statistical deference (P-value = 0.8272) with regard to the percentage of calcified tissue surface area (Ti 16.86% and PLA 16.17%). Moreover, this calcified tissue was also found to be evenly distributed in both groups. Conclusion. Biodegradable mesh made of poly L-lactic and polyglycolic acid copolymers appears to be an appropriate alternative to the Ti mesh for support of PBCM bone grafts. However, further clinical trails should be conducted to confirm these findings.

The effects of vocal loudness and speaking rate on voice-onset time in typically developing children and children with cochlear implants

This study explores the effects of manipulating vocal loudness and speech rate on voice onset time (VOT) in normal hearing children and two children with cochlear implants (CIs). 15 normal hearing participants and two participants with CIs produced all six stop consonants in the phrase “It’s a Cod again” while speaking normally, softly, loudly, slowly, and quickly. Consonants were grouped into voiced and voiceless categories for comparison. Results indicated that the group of normal hearing children produced longer VOTs for voiceless stops than voiced across all conditions. When speaking loudly or quickly, VOT values were shorter than at normal levels. When speaking softly or slowly, VOT values were longer than at normal levels. The two children with CIs performed in a similar manner to the normal hearing group; however, VOTs produced by the six-year old participant were consistently longer than those of the normal hearing group across all conditions. / Speech-Language Pathology
