A sociological review and application of Illich's theory of iatrogenesis with specific reference to problems concerning the aged

Illich, a critic of the medical profession in industrial societies, researched the "damage done" by the medical establishment on three levels. This study presents an application of Illich's three-level theory of iatrogenesis to a sample of elderly people in old-age homes in and around Pretoria. Fifty in-depth interviews were conducted with old-age home residents. In the focus on clinical iatrogenesis, the aspect of defenceless patients was investigated. With regard to social iatrogenesis, focus was on the interpretation of attitudes revealing an increased medical dependency. Cultural iatrogenesis, which involves the influence of values and norms on thinking patterns, is investigated in terms of the impact of institutionalisation on the autonomy, independence and personal responsibility of residents. Illich's solution to the problem of medicalisation is to be found in the de-bureaucratisation and de-industrialisation of society. Based on the assumptions of critical theory, the emancipation of the individual is suggested as a basis of Illich's proposed structural societal changes / M.A. (Sociology)

La radiothérapie occupe une place majeure dans l’arsenal thérapeutique des cancers.Malgré des progrès technologiques importants depuis près de vingt ans, des tissus sains au voisinage ou à distance de la tumeur cible continuent à être inévitablement irradiés à des niveaux de doses très différents. Ces doses sont à l’origine d’effets secondaires précoces (Œdème, radionécrose, Dysphagie, Cystite) ou tardifs (rectorragies, télangiectasie, effets carcinogènes, les pathologie cérébrovasculaires).Il est donc primordial de quantifier et de prévenir ces effets secondaires afin d'améliorer la qualité de vie des patients pendant et après leur traitement.La modélisation du risque d'événements iatrogènes radio-induits repose sur la connaissance précise de la distribution de doses au tissu sain d'intérêt ainsi que sur un modèle de risque capable d'intégrer un maximum d'informations sur le profil d'irradiation et des autres facteurs de risques non dosimétriques. L'objectif de ce travail de thèse a été de développer des méthodes de modélisation capables de répondre à des questions spécifiques aux deux aspects, dosimétriques et statistiques, intervenant dans la modélisation du risque de survenue d'événements iatrogènes radio-induits.Nous nous sommes intéressé dans un premier temps au développement d'un modèle de calcul permettant de déterminer avec précision la dose à distance due au rayonnements de diffusion et de fuite lors d'un traitement par radiothérapie externe et ce, pour différentes tailles des champs et à différentes distances de l'axe du faisceau. Ensuite, nous avons utilisé des méthodes d'analyse de données fonctionnelles pour développer un modèle de risque de toxicité rectales après irradiation de la loge prostatique. Le modèle proposé a montré des performances supérieures aux modèles de risque existants particulièrement pour décrire le risque de toxicités rectales de grade 3. Dans le contexte d'une régression de Cox flexible sur données réelles, nous avons proposé une application originale des méthodes de statistique fonctionnelle permettant d'améliorer les performances d'une modélisation via fonctions B-splines de la relation dose-effet entre la dose de radiation à la thyroïde.Nous avons également proposé dans le domaine de la radiobiologie une méthodes basée sur l’analyse en composantes principales multiniveau pour quantifier la part de la variabilité expérimentale dans la variabilité des courbes de fluorescence mesurées. / Radiotherapy plays a major role in the therapeutic arsenal against cancer. Despite significant advances in technology for nearly twenty years, healthy tissues near or away from the target tumor remain inevitably irradiated at very different levels of doses. These doses are at the origin of early side effects (edema, radiation necrosis, dysphagia, cystitis) or late (rectal bleeding, telangiectasia, carcinogenic, cerebrovascular diseases). It is therefore essential to quantify and prevent these side effects to improve the patient quality of life after their cancer treatment.The objective of this thesis was to propose modelling methods able to answer specific questions asked in both aspects, dosimetry and statistics, involved in the modeling risk of developing radiation-induced iatrogenic pathologies.Our purpose was firstly to assess the out-of-field dose component related to head scatter radiation in high-energy photon therapy beams and then derive a multisource model for this dose component. For measured doses under out-of-field conditions, the average local difference between the calculated and measured photon dose is 10%, including doses as low as 0.01% of the maximum dose on the beam axis. We secondly described a novel method to explore radiation dose-volume effects. Functional data analysis is used to investigate the information contained in differential dose-volume histograms. The method is applied to the normal tissue complication probability modeling of rectal bleeding for In the flexible Cox model context, we proposed a new dimension reduction technique based on a functional principal component analysis to estimate a dose-response relationship. A two-stage knots selection scheme was performed: a potential set of knots is chosen based on information from the rotated functional principal components and the final knots selection is then based on statistical model selection. Finally, a multilevel functional principal component analysis was applied to radiobiological data in order to quantify the experimental Variability for replicate measurements of fluorescence signals of telomere length.

Ultrasound-Guided Percutaneous Thrombin Injection for Femoral Artery Pseudoaneurysms

We reviewed 13 cases of ultrasound-guided thrombin injection of femoral pseudoaneurysms. All cases occurred within a 17-month period from January 1998 through May 1999 and were complications of femoral artery puncture. Immediate total thrombosis occurred in nine of 13 patients. Twenty-four-hour follow-up ultrasound in seven patients revealed no recurrence of pseudoaneurysm. Two of 13 patients required operative repair. One pseudoaneurysm thrombosed with 15 minutes of compression after injection and one case required a second injection. No cases of arterial thrombosis were noted. Ultrasound-guided thrombin injection for femoral artery pseudoaneurysm represents a safe and effective alternative to operative repair.

Eventos adversos (EAs), definidos como complicações não intencionais decorrentes do cuidado prestado, são reconhecidos como um dos maiores problemas na área da saúde. Embora a maior parte dos eventos acarrete incapacitações leves, uma proporção considerável está relacionada à morte de pacientes. O atendimento de urgência é considerado importante fator de risco para o desencadeamento destas complicações. No Brasil, estudos relacionados a este tema não foram publicados até o momento. O presente estudo objetivou identificar a ocorrência de EAs em pacientes admitidos por acidente vascular cerebral (AVC) ao Pronto-Socorro de Clínica Médica (PSM) do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP) e determinar as categorias de EAs associadas a óbito. Este estudo caso-controle pareado envolveu 468 pacientes admitidos por AVC ao PSM-HCFMUSP no período de março de 1996 a setembro de 1999. O grupo-caso compreendeu 234 óbitos hospitalares consecutivos e o grupo-controle 234 pacientes que receberam alta, pareados pelo diagnóstico provisório e pela época de internação. Eventos adversos, detectados por revisão de prontuários, foram classificados segundo sua gravidade, causas imediatas, sistemas acometidos e categorias profissionais envolvidas no cuidado aos pacientes. A associação com óbito foi analisada por regressão logística multivariada condicional, incluindo variáveis relacionadas a aspectos demográficos, gravidade do quadro inicial e características da assistência. Nos 468 pacientes foram identificados 1.218 EAs: 932 EAs (76,5%) em 170 casos e 286 EAs (23,5%) em 125 controles. Eventos adversos major corresponderam a 54,1% do total de eventos, com 659 episódios: 538 eventos em 143 casos e 121 em 65 controles. Os procedimentos diagnósticos e terapêuticos e os cuidados de enfermagem foram responsáveis em conjunto por 55,2% do total de eventos. Em relação ao tipo de sistema afetado, 46,0% dos EAs identificados ocasionaram manifestações gerais. Eventos adversos relacionados à enfermagem e EAs médicos representaram as categorias profissionais de EAs mais freqüentes (38,4% e 31,0% do total de eventos). Uma associação significante com óbito foi encontrada em relação a EAs major, EAs médicos e infecções hospitalares, com valores de OR ajustado estimados em 3,72 (IC 95% = 1,63-8,48), 3,69 (IC 95% = 1,60-8,50) e 3,20 (IC 95% = 1,20-8,51), respectivamente. Em resumo, eventos adversos, na sua maioria graves, foram freqüentes em casos e controles, determinando predominantemente manifestações gerais. Os procedimentos diagnósticos e terapêuticos e os cuidados de enfermagem corresponderam às principais causas imediatas de EAs. Em relação à categoria profissional envolvida, os EAs relacionados à enfermagem e os eventos médicos predominaram. This paired case-control study enrolled 468 patients admitted for stroke to the HCFMUSP medical emergency department from March 1996 to September 1999. The cases comprised 234 consecutive deaths and the controls 234 discharged patients, matched for primary diagnosis and admission period. AEs, detected by retrospective chart review, were classified with regard to their severity, immediate causes, affected systems and professional involved in patient care. The association with death was analyzed by multivariate conditional regression including variables related to demographic aspects, clinical severity on admission and care characteristics. A total of 1,218 AEs were identified in 468 patients: 932 AEs (76.5%) in 170 cases and 286 AEs (23.5%) in 125 controls. Major AEs corresponded to 54.1% of all AEs, with 659 episodes: 538 events in 143 cases and 121 in 65 controls. Diagnostic and therapeutic procedures and nursing activities accounted together for 55.2% of all events. Concerning the affected system, 46.0% of the identified AEs lead to general manifestations. Nursing and medical AEs represented the most frequent professional categories involved (38.4% and 31.0% of all events). A significant association with death was found regarding major AEs, medical AEs and nosocomial infections, with adjusted OR estimates of 3.72 (95% IC = 1.63-8.48), 3.69 (95% IC = 1.60-8.50) and 3.20 (95% IC = 1.20-8.51), respectively. Vergiftungsbedingte Todesfälle - Retrospektive Analyse von 316 letalen Intoxikationen des Giftinformationszentrums-Nord / Intoxication related fatalitities - Retrospective study of 316 lethal poisonings of GIZ-Nord Poisons Centre Göttingen

Schätzungen über die Gesamtzahl der Vergiftungen in Deutschland belaufen sich auf 150 - 200.000 pro Jahr. In Deutschland machen Vergiftungsfälle 10% der in Notaufnahmen und auf Intensivstationen behandelten Patienten aus. Bis dato gibt es in Deutschland nur wenige strukturiert erfasste epidemiologische Daten von vergiftungsbedingten Todesfällen. In der vorliegenden Arbeit wurden die vergiftungsbedingten Todesfälle aus dem Giftinformationszentrum-Nord (GIZ-Nord) für den Zeitraum vom 01.01.1996 bis einschließlich 31.12.2009 retrospektiv analysiert. Insbesondere vor dem Hintergrund der Toxikovigilanz konnten folgende Fragen zu Risikokonstellationen für letale Vergiftungen durch diese Arbeit beantwortet werden: 1. Gibt es bei den vergiftungsbedingten Todesfällen jahreszeitliche Schwankungen? Ja, die vergiftungsbedingten bzw. suizidalen letalen Intoxikationen zeigen jahreszeitliche Schwankungen mit einem maximalen Anteil im Frühling von knapp 30% und dem geringsten Anteil im Herbst mit etwa 20%. 2. Bergen die iatrogen bedingten Vergiftungen ein besonderes Mortalitätsrisiko? Ja, die iatrogenen Vergiftungen weisen die höchste Mortalitätsrate mit knapp 5 ‰ aller iatrogenen Expositionen auf, vor allem bei parenteraler Arzneimittelapplikation. 3. Stellen Adoleszente (Heranwachsende und Jugendliche) und Senioren über 70 Jahre besonders gefährdete Altersgruppen für letale Intoxikationen dar? Ja, beide Populationen stellen besondere Risikogruppen für vergiftungsbedingte Todesfälle dar. Vor allem männliche Heranwachsende repräsentieren eine Risikogruppe für letale Intoxikationen durch Abusus. 30% aller vergiftungbedingten Todesfälle durch Abusus sind bei männlichen Jugendlichen aufgetreten. Senioren (Männer und Frauen) über 70 Jahre sind eine im hohen Maß gefährdete Altersgruppe für letale Vergiftungen. Einerseits ist die relative Mortalitätsrate bezogen auf alle Expositionen dieser Altersgruppe mit knapp 6 ‰ am höchsten. Andererseits zeigen die suizidalen Intoxikationen bei den Senioren eine zunehmende Tendenz. Außerdem bilden Senioren mit einem Anteil von 24% die Gruppe mit dem größten Anteil an den letalen nicht suizidalen Intoxikationen. Mehr als 70% der akzidentellen letalen Intoxikationen und 50% der letalen Vergiftungen durch Verwechslungen betreffen Senioren, dabei sind Frauen mehr als 3fach häufiger betroffen als Männer. Darüber hinaus sind Senioren durch Aspiration schäumender, prinzipiell niedrig toxischer Kosmetika und tensidhaltiger Chemikalien besonders gefährdet. 4. Gibt es bei den gewerblichen Expositionen besondere Risikokonstellationen? Ja, die gewerblichen Expositionen, die mit einer hohen Männerlastigkeit auftreten, bergen eine hohe letale Intoxikationsgefahr. Vor allem bei den gewerblichen inhalativen Expositionen von toxischen Reizgasen besteht eine Gefährdung für die betroffenen Beschäftigten, aber auch für die nicht selbstgeschützten Ersthelfer: 6% der inhalativen gewerblichen Intoxikationen durch Schwefelwasserstoff endeten letal.
