Treatment and disposal of medical waste in rural and urban clinics within Polokwane municipality of South Africa

Published Article / The objective of the study was to assess the medical waste management practices used by clinics located in the Polokwane Municipality, South Africa. The clinic personnel were not informed prior to the visit. A walkthrough survey was conducted on-site where potential health and safety risks were identified and noted. The health care personnel in charge of specific clinics completed the questionnaires providing general and detailed information about the treatment and disposal of medical waste. Frequencies and percentages were calculated from the data obtained. Data obtained indicate that (i) medical waste was not segregated from general waste in most of the clinics, (ii) rural clinics were burning medical waste with general waste inside the clinic yard (iii) there was no sufficient collection and transport of medical waste in clinics, (iv) sharps were disposed at various hospital incinerators and (v) most of the clinics did not have storage facilities for medical waste.

The Study of PCDD/Fs Emitted from Flue Stacks and Open Burnings in Southern Taiwan

This work investigated the characteristics of polychlorinated dibenzo-p-dioxins anddibenzofurans (PCDD/F) in stack-flue gases from six stationary emission sources in five types ofincinerators: industrial waste incinerator (IWI), small-scale municipal solid waste incinerator(MSWI), medical waste incinerator (MWI), cement kilns (CK), and crematories (CR). These characteristics were further investigated using factor analysis and cluster analysis. Experimental results reveal that PCDDs dominate MSWI and CR, and PCDFs dominate IWIa, IWIb, CK and MWIs. The factor analysis results showed that CR and MSWI have similar fingerprints, as do IWIb and MWI3. The cluster analysis showed that if a vertical line is cut at a rescaled distance of four, then the PCDD/F congener profiles fall into four groups. The indicators of PCDD/Fs are OCDD, 1,2,3,4,6,7,8-HpCDF, 2,3,4,7,8-PeCDF, and 1,2,3,4,6,7,8-HpCDD. The emission factors of PCDD/Fs herein were from 0.0433

嫌惡性設施對生活環境品質影響之研究-以台北市內湖、木柵、士林三個垃圾焚化廠為例

嚴重的廢棄物處理問題,是現今施政的一大課題,而由於垃圾焚化相較於其他處理方式更能達到減量化、衛生化、安定化及資源化之目標,故成為未來垃圾處理方式之主流,垃圾焚化廠亦將陸續興建完工。但不可避免地,垃圾焚化廠亦會產生一些負面影響,往往被視為嫌惡性設施之一種,而形成設置上之阻力。   本研究之目的主要在藉由居民對焚化廠之認知、評價、接受程度、造成影響之感受,以及居民對焚化廠回饋地方措施之期望及其成效如何等相關課題之探討,以了解焚化廠設置形成阻力之原因所在,此外,並輔以相關文獻、專家意見及相關法規等資訊之整合,嘗試對降低焚化廠嫌惡性之可行方向提出建議,冀能對減輕日後興建焚化廠之阻力的有所助益,並期能確保民眾之生活環境品質。   本研究之重要結論與建議如下:   一、結論    (一)民眾對於國內興建焚化廠之品質及二次污染控制能力仍缺乏信心,且居民預期焚化廠可能造成之影響通常會較焚化廠實際產生之影響大。    (二)在居民願意接受焚化廠設置於其住家附近之條件中,以風險減輕方案最為居民所重視。    (三)民眾對焚化廠回饋措施之認知程度愈高者,其對焚化廠接受程度愈高。   二、建議    (一)在焚化廠風險減輕之策略方面,主管單位除了污染防治工作外,還必須了解公眾對風險的感受及反應,縮短政府與民眾在認知上之差距。    (二)回饋措施(經濟誘因)有助於降低焚化廠之嫌惡性,故主管單位應加強焚化廠回饋措施方面之配置與宣導,以爭取民眾之支持。    (三)適當的資訊回饋,可避免民眾對焚化廠之風險太過高估而增添不必要之憂慮,進而減輕其設置之阻力,故主管單位應適時提供民眾所需之環境資訊。 / The serious solid waste disposal problem is one of the challenges to the government policies. Landfilling is not a suitable solid waste disposal alternative in Taiwan, because of the high population density and the difficulty of acquiring land, At the same time, incinerators can help achieve waste reduction, sanitary disposal, stabilty, and resource recovery. Therefore, incineration will play an increasingly important role in the government's solid waste disposal plans.   Although incineration is one of the better approaches to dealing with solid waste disposal, it still has certain negative impacts on the quality of life. Therefore, it has been treated as a NIMBY (Not-In-My-Back-Yard), To some extent, NIMBYs are necessary to the community. The issue to be dealt with, then, becomes"how to find an equitable way to solve this dilemma."   This thesis focuses on three incinerators in Neihu, Wenshan, and Shihlin, Taipei. From the results of literature review, field surveys, data collection, questionnaires, and interviews, this study tries to explore the relationships between incinerators and the quality of life. This study also examines the effectiveness of compensation packages of these NIMBYs and people's attitudes toward incinerators.   The important findings of this thesis are as follows:   1.conclusions    (1).Although most people have positive attitudes toward MSW incinerators, they do not feel confident about the quality of MSW incinerators and the administration's ability to control the second pollution.    (2).People are more concerned with risk reduction strategies, rather than economic incentives.    (3).The degree to which residents accept MSW incinerators will be affected by residents' knowledge about the compensation packages of Neihu incinerator area.   2.Policy implications    (1).From the analyses we can see that people are more willing to accept risk reduction solutions rather than economic incentives. Therefore, if the government tries to increase the acceptance of MSW incinerators among residents, it should focus more on risk reduction plans.    (2).Because residents are still lack of confidence about the quality of MSW incinerators and the ability of the administration to control the second pollution, the government should provide more accurate information to the public and guarantee the quality of MSW incinerators and the ability to control the second pollution.    (3).If the government can provide some economic incentives to reduce the equity issues, the residents may be more willing to live closer to LULU facilities. From the analysis , we can see that economic incentives do reduce the NIMBY syndrome to some extents (even though not so much as risk reduction programs). Therefore, in addition to the pollution prevention and risk reduction programs, the government should also reinforce and provide more information about the compensation packages.

Se estudió las emisiones de material particulado (PM), vapores y gases de chimenea del incinerador del Instituto Nacional de Enfermedades Neoplásicas y se registró la micrometeorolo' gía. Los gases de chimenea fueron recolectados con un muestreador isocinético; los sólidos totales suspendidos, por muestreo pasivo. Los análisis muestran elevados niveles de PM 10, que contiene Pb, Al, Zn y Ca, así como altas concentraciones de benceno y naftaleno en el gas de chimenea. Se observó que la dispersión del PM tiene escala local. / Toxic substances in gaseous emissions from hospital incineratorsThe Nationallnstitute for Neoplastic Diseases incinerator was studied: emissions were sampled and analyzed for particulate matter (PM), stack gases and vapors and the micrometeorology was also registered. Gases were collected with an isokinetic sampler, total suspended solids by passive sampling. Values and concerns in decision-making about a waste-reduction incinerator at Stormsriver, Tsitsikamma : a case study in applied ethics

Thesis (MPhil)--Stellenbosch University, 2003. / Full text to be digitised and attached to bibliographic record. / ENGLISH ABSTRACT: The Tsitsikamma Forest, in the most southern part of the Eastern Cape, is part of the Cape Floral Kingdom, one of six such Kingdoms in the world. It has the richest area of plant bio-diversity and fauna in the world which are rare and endemic to the area. The deep gorges, temperate forests, un spoilt oceans and various other features such as the famous Bloukrantz River Bridge, where the world's highest bungi jumps take place are factors that have become the main source of job-creation and income for tourism and its inter-related activities. However, this ecosystem is also home to about 480 families who live in low-income housing at Stormsriver. This settlement which was relocated from the shack area of Coesa, nearby, has an unemployment index of 67% and a similar poverty index. In addition, the Stormsriver Village nearby has a population of about 300 families, mainly older retired white folks. The area is administered by the Koukamma Municipality which is an amalgam of several smaller municipalities viz., Clarkeson, Kareedouw, Stormsriver, Woodlands, Eerste River, ]oubertina and Bloukrantz. Waste reduction and the disposal thereof has always been a major concern to local authorities and the Koukamma Municipality is obviously faced with the same challenge. The authorities have resorted to the concept of landfill sites, which has proven to be the cheapest solution to date. But, with this option comes the problems of health risks to the poorer people who converge on the dump sites in search of food, the harm to children searching through rubbish heaps and the various legal repercussions emanating out of this practice. The option of open-burning and its related effects on human life and forest fires rules it out as an option. Transporting waste out of the area is also very costly. It was then resolved by the Koukamma Municipalities that the possibility of installing an incinerator at Stormsriver would be an option to be investigated. The reasoning behind the selection of this site was the concern of high unemployment in the Stormsriver area and extreme poverty and this venture would in some small measure create jobs, put food on the table and break the poverty cycle. The process of incineration gives rise to emissions such as dioxins and furans which is known to cause cancer, asthma and tuberculosis, if subjected in high doses over a long period of time. It is also regulated by the Atmospheric Pollution Act 45 of 1965. The important moral debate at the core of this problem is the whole question of humans rights and should these be violated solely because the subjects do not belong to an affluent class and should be shown less respect and have their health jeopardised by the toxic air from incineration. Or, should the people's poverty index rise further due to lack of employment. The question that also begs here is whether the well-being of the entire ecosystem, especially the rare and endangered plants and animals take preference over the well-being of the people of Stormsriver, given that the emissions from the incinerator will impact on the animal, bird and plant life, including that of water, soil and air. This would also put into jeopardy the income generated by Tourism and the jobs inherent in it for the locals and would threaten the area's sustainability. The various moral views of philosophers have been canvassed in order to come to a holistic understanding. I have in this vein looked at the views of ethicists such as Tibor Machan, who believes that animals and other organisms do not have any rights and are there purely for human consumption. Then the views of Peter Singer and Tom Regan have also been included, in which consideration is shown to animals. Others such as AIda Leopold, J. Baird Callicott and Paul Taylor whose thinking extends to that of not just showing respect for a single entity but allows for equal respect for all members of the biotic community, have also been. considered. Various solutions have thereafter been considered and amongst those are the relocation of the incinerator out of the Stormsriver, moving the plant to the industrial zone at Kareedouw, transporting the waste out to Port Elizabeth, dumping and finally the 'Do Nothing' option. Using Paul Taylor's "respect for nature" ethics as a guide, I have come to the conclusion that the installation of a waste-reduction incinerator at Stormsriver, Tsitsikamma, may be supported subject to a set of serious riders and conditions in that firstly, the health of the immediate and surrounding inhabitants should not be affected in any way whatsoever. Secondly, the well-being of the non-human environment which must include air, soil, water and plant life, including the animal and bird life must not be harmed in any way. In this way all members of the biotic community will be shown equal respect, thus creating a viable and sustainable community / AFRIKAANSE OPSOMMING: Die Tsitsikamma Woude, in die mees suidelike deel van the Oos-Kaap, is deel van die "Cape Floral Kingdom", een van ses sulke woudgebiede in die wêreld. Dit beskerm die rykste area van plant bio-verskeidenheid asook fauna wat skaars is in die wêreld en slegs daar gevind word. Die verskeie standpunte van filosowe is na gekyk ten einde tot 'n holistiese verstaan van die situasie te kom. Langs hierdie weg het ek dan na die etiese standpunt van Tibor Machan gekyk. 現有的媒體-抗爭互動機制研究,其結論多是主流媒體傾向非法化、瑣碎化社會運動。但針對威權語境中的媒體-抗爭互動現象,這一論點不再適用。本研究通過2009年廣州番禺反建垃圾焚燒廠事件,探究地方媒體與草根環境運動在與政府協商公共議題時,形成何種互動機制。本文以公共性概念為理論起點,以參與式公共性與可視性公共性這兩種模式作為分析工具,來理解媒體與運動的實踐及其互動。 / 本研究使用多種方法考察事件發展過程,同時試圖建構出媒體-運動的互動機制。主要研究材料包括媒體文本、新聞從業者與抗爭居民的口頭和書面陳述。本文用內容分析方法考察媒體如何再現抗爭居民與當地政府的話語鬥爭,以及新聞框架如何發展演變。研究者與20位新聞從業者與抗爭居民進行深度訪談,以瞭解其行動的目的、策略與束縛,並探索雙方如何理解與評價自身、對方與社會語境。 / 本文的主要貢獻,是通過考察威權語境中的媒體-運動的互動實踐現象來豐富原有的公共性概念。研究發現雙方形成了深度互動、平等、共存的關係。首先,媒體的報道使與抗爭相關的公共論述得以生成、發展、充實。多角度的媒體報道涵蓋了環保與民主參與等話題,由此挑戰了官方對抗爭的打壓式論述。其次,在運動的架構過程中,抗爭居民創造出媒介互動策略,具體表現為抗爭者認識到傳統媒體記者所受的束縛,並與其合作突破這種束縛。他們同時也使用新媒體平台直接動員公眾展開集體行動。最後,新聞從業者發展出抗爭新聞劇目的生產機制,使得抗爭事件停留於報端,令抗爭者成為公共論述的參與者。新聞從業者在追求職業認同與獨立時還借用抗爭的概念,將其用作由下至上新聞改革的資源。 / 以上發現表明,媒體與社會運動作為社會行動者,其互動機制中體現出一種具有建構性與矛盾性的公共性。通過這一媒介化的抗爭事件,雙方發展出針對官方論述的話語抗爭,使得原本屬於越軌範疇的抗爭話語得以進入可公開爭議的合法範疇,由此得以重新界定公開話語空間中的意識形態邊界。 / Existing studies of media-movement interaction have argued that mainstream media tends to illegitimize and trivialize social movements. However, when applied to the newly emerged media-movement interactions in the context of an authoritarian regime, such argument becomes untenable. This study takes an anti-incinerator event taken place in Panyu, Guangzhou in 2009 as its foci of investigation. It examines how local media and the grassroots environmental movement cooperatively negotiated public issues with the government. The concept of publicity and two models of publicity (participatory publicity and visibility publicity) will be introduced. These two models will be used as analytical tools to understand the practice of and the interaction between the media and the movement. / Methodologically, this study adopts multiple methods to trace the process of the event and model the media-movement interaction mechanism. This research constructs the case by analyzing the media texts, together with both oral and written accounts of the journalists and the activists. Content analysis is employed to measure how the media represent the discursive struggles between local residents and local government, as well as how the media frames have evolved during the event. In-depth interviews have been conducted with 20 activists and journalists to explore their intentions, strategies and constraints. Questions were also asked about how they understand and evaluate themselves, each other and the social context. / The primary significance of this study is to explore how the notion of publicity is enriched by the dynamics of the media-movement interaction mechanism within an authoritarian context. The findings demonstrate that both parties have developed an increasingly interactive, equal and co-dependent relationship with one another. First, public discourses surrounding the movement have emerged and survived through the cooperation between journalists and activists. The media has developed diversified reporting angles covering not only environmental issues but also notions of democratic participation that challenged the suppressive official discourse. Second, during the framing process of the movement, the activists have invented an interactive media strategy that helped to break the constraints of journalistic practice within traditional media organizations. Meanwhile, they also adopted new media platforms to directly mobilize for collective action. Third, the journalists have developed a “journalistic repertoire of contention by which they successfully kept the story staying in the newspaper and incorporated the protesters’ voice into the public discussion. Moreover, while in search of their professional identity, the journalists also borrowed the idea of protest as a resource for the bottom-up media transformation. / The above findings suggest that the notion of publicity constructed by the media-movement interaction in this study could be described as constructive and inconsistent. Against the official discourse, journalists and activists have collaborated on the discursive contention by means of the mediated event.
