Die infektiöse Endokarditis und ihre Erreger – Eine monozentrische Studie / Infectious endocarditis and its pathogens - a single center study

Es existieren mehrere Arbeiten, welche die spezifischen Charakteristika der infektiösen Endokarditis unterschiedlicher Erreger untersucht haben, jedoch hat bisher kein direkter Vergleich der drei großen, für die infektiöse Endokarditis in der westlichen Welt hauptsächlich verantwortlichen Erregergruppen (Staphylokokkus aureus, andere Erreger und fehlender Keimnachweis) stattgefunden. In der folgenden Arbeit wurde anhand eines repräsentativen Kollektivs von 103 Patienten retrospektiv der Frage nachgegangen, ob sich zwischen den genannten drei Gruppen signifikante Unterschiede im Krankheitsgeschehen herausarbeiten lassen. Dies könnte einen ersten Schritt der Therapieoptimierung darstellen, indem die Heterogenität der Erkrankung durch das Bilden von Untergruppen reduziert und entsprechende klinische Handlungsstrategien etabliert werden könnten. Im Hinblick darauf wurden die Epidemiologie, der Krankheitsverlauf und das Outcome der Gruppen analysiert. In einem zweiten Schritt wurde mit Hilfe statistischer Analysen untersucht, ob Unterschiede in der Aussagekraft zwischen den gängigen, in der klinischen Situation eingesetzten Nachweismethoden für die jeweiligen Keime bestehen. / Several studies have already examined the specific characteristics of infectious endocarditis (IE) induced by different pathogens. So far, direct comparison of the three large groups of IE-related pathogens in the western world (S. aureus, other pathogens and sterile endocarditis) are still missing. In the following work, a representative cohort of 103 patients was used to retrospectively investigate potential differences in disease progression between the three groups mentioned above. This could represent a first step in the optimization of therapy via reduction of disease heterogeneity by creation of subgroups and establishment of appropriate clinical treatment strategies. With this in mind, epidemiology, course of disease and outcome was analysed. In a second step, statistical analyses were used to investigate potential differences between current detection methods used in clinical reality for respective germs.

Aortic root reinforcement in aortic valve endocarditis with annular abscess: The Calamari procedure

Treatment of infective endocarditis can often prove challenging due to its wide range of anatomical presentations. When complicated by an aortic root abscess, patients may require extensive root surgery, which on its own leads to a worse outcome. We present our experience with a surgical technique for reinforcing the aortic annulus with a ring from a Dacron aortic prosthesis placed in the left ventricular outflow tract to avoid the need for root replacement procedures or patch closures of the defect. The technique described in this paper provides a viable alternative to the standard techniques used for the treatment of annular abscesses in aortic valve endocarditis. Due to the relative simplicity and ease of use, this approach may present a means of reducing operation time and possibly postoperative complications of this severe condition.
