Enabling the Reuse of Electronic Health Record Data through Data Quality Assessment and Transparency

With the increasing adoption of health information technology and the growth in the resulting electronic repositories of clinical data, the secondary use of electronic health record data has become one of the most promising approaches to enabling and speeding clinical research. Unfortunately, electronic health record data are known to suffer from significant data quality problems. Awareness of the problem of electronic health record data quality is growing, but methods for measuring data quality remain ad hoc. Clinical researchers must handle this complicated problem without systematic or validated methods. The lack of appropriate or trustworthy electronic health record data quality assessment methodology limits the validity of research performed with electronic health record data. This dissertation documents the development of a data quality assessment framework and guideline for clinical researchers engaged in the secondary use of electronic health record data for retrospective research. Through a systematic literature review and interviews with key stakeholders, we identified core constructs of data quality, as well as priorities for future approaches to electronic health record data quality assessment. We used a data-driven approach to demonstrate that data quality is task-dependent, indicating that appropriate data quality measures must be selected, applied, and interpreted within the context of a specific study. On the basis of these results, we developed and evaluated a dynamic guideline for data quality measures in order to help researchers choose data quality measures and methods appropriately within the context of reusing electronic health record data for research.

Cloud computing : COBIT-mapped benefits, risks and controls for consumer enterprises

Accordingly, many information technology service offerings are now termed as cloud offerings. Cloud computing has attracted, and continues to attract, extensive technical research attention. However, little guidance is given to prospective consumers of the cloud computing services who may not possess technical knowledge, or be interested in the in-depth technical aspects aimed at information technology specialists. Yet these consumers need to make sense of the possible advantages that may be gained from utilising cloud services, as well as the possible incremental risks it may expose an enterprise to. The aim of this study is to inform enterprise managers, who possess business knowledge and may also be knowledgeable on the main aspects of COBIT, on the topic of cloud computing. The study focuses on the significant benefits which the utilisation of cloud computing services may bring to a prospective consumer enterprise, as well as the significant incremental risks this new technological advancement may expose the enterprise to. Proposals of possible controls that the prospective consumer enterprise can implement to mitigate the incremental risks of cloud computing are also presented. / AFRIKAANSE OPSOMMING: “Cloud computing” (wolkbewerking) het na vore getree as een van die mees opspraakwekkende inligtingstegnologieverwante onderwerpe van die dekade. Gevolglik word talle inligtingstegnologie-dienste nou as “cloud”-dienste aangebied. Uitgebreide aandag in terme van tegnologiese navorsing is en word steeds deur “cloud computing” ontlok. Weinig aandag word egter geskenk aan leiding vir voornemende verbruikers van “cloud”-dienste, wie moontlik nie tegniese kennis besit nie, of nie belangstel in die diepgrondige tegniese aspekte wat op inligtingstegnologie-spesialiste gemik is nie. Tog moet hierdie verbruikers sin maak van die moontlike voordele wat die gebruik van “cloud”-dienste mag bied, asook die moontlike inkrementele risiko’s waaraan die onderneming blootgestel mag word. Die doel van hierdie studie is om die bestuurders van ondernemings, wie besigheidskennis besit en moontlik ook kundig is oor die hoof aspekte van COBIT, in te lig oor wat “cloud computing” is. Die studie fokus op die beduidende voordele wat die benutting van “cloud computing”-dienste aan die voornemende verbruikersonderneming mag bied, asook die beduidende inkrementele risiko’s waaraan die onderneming blootgestel mag word as gevolg van hierdie tegnologiese vooruitgang. Voorstelle van moontlike beheermaatreëls wat die voornemende verbruikersonderneming kan implementeer ten einde die inkrementele risiko’s van “cloud computing” teë te werk word ook aangebied.

The main purpose is to describe this IT _productivity paradox. The paper explores ideas that align organizations' business strategies and information technology as a key to achieving improved productivity. Which are possible to properly measure in terms of the financial results? The main aims and objectives are to find out why managers invest in IT; whether there is a phenomenon such as an IT productivity paradox; whether IT pays off as expected; and how IT impacts on organizations. The study will investigate options for proper management of information technology and data structures. It is necessary to ascertain whether ensuring proper IT implementation has a positive impact on productivity, leading to increased innovation and performance. The research design builds on research done on the use of IT in organizations, usmg a qualitative research method. This research paper looks at organizational issues such as IT management styles, political and ethical issues, and work settings. The paper looks at organizations across the service and manufacturing sectors to determine their production, innovation, and profits into their existing organizational processes and how technology is interpreted. A group of IT users, IT managers and analysts were used as a sample to study the way IT managers and knowledge workers encounter information technology in organizations. The research method used in this research paper is called the informant approach, to take points of entry IT users would provide. This means that the interviewee, in this case the IT user was questioned on the use of information technology tools to gather information. In this study the aim was to conduct interviews with IT users and those they work with about their experiences. The sampling population was selected on the basis that they use this technology. In the data collection method a second interview was used to gather first-hand responses from the respondents to help me consolidate the information gathered to validate and ensure that it is reliable. The validity and reliability aspect of this research paper are based on the main sources of data and interpretation and adopts coding as the main technique of analysis. The internal reliability of this research methodology concern itself with the research methods that were used within this research paper. Measures to be taken in the paper are to obtain internal reliability in systematic gathering of data. The last part of this paper presents the conclusions and recommendations for changes to be made by managers and those investing in IT. IT managers should plan strategically when dealing with sales and marketers in order to put business needs before the needs of IT or systems. Technology should fit business needs rather than the business adjusted to fit the technology. IT should not cost an organization any additional profit it generates. / AFRIKAANSE OPSOMMING: Belegging in Informasietegnologie skyn nie so betalend te wees as wat aanvanklik verwag is nie. Die laaste deel van hierdie dokument bied die gevoltrekkings aan en ook voorstelle vir veranderings wat gemaak kan word deur bestuurders en diegene wat belê in IT. IT bestuurders behoort strategies te beplan wanneer hulle in aanraking kom met handelaars, om doelgerig die benodighede van die maatskappy te stel voor die benodigdhede van die IT en gepaardgaande sisteme. Tegnologie behoort in te pas by die benodigdhede van die maatskappyeerder dat die maatskappy hoef aan te pas by die tegnologie. IT behoort nie die maatskappy meer uit die sak te jaag as wat dit aan wins genereer nie.

Tricky technology, troubled tribes: a video ethnographic study of the impact of information technology on health care professionals??? practices and relationships.

Whilst technology use has always been a part of the practice of health care delivery, more recently, information technology has been applied to aspects of clinical work concerned with documentation. This thesis presents an analysis of the ways that two professional groups, one clinical and one ancillary, at a single hospital cooperatively engage in a work practice that has recently been computerised. It investigates the way that a clinical group???s approach to and actual use of the system creates problems for the ancillary group. It understands these problems to arise from the contrasting ways that the groups position their use of documentation technology in their local definitions of professional status. The data on which analysis of these practices is based includes 16 hours of video recordings of the work practices of the two groups as they engage with the technology in their local work settings as well as video recordings of a reflexive viewing session conducted with participants from the ancillary group. Also included in the analysis are observational field notes, interviews and documentary analysis. The analysis aimed to produce a set of themes grounded in the specifics of the data, and drew on TLSTranscription?? software for the management and classification of video data. This thesis seeks to contribute to three research fields: health informatics, sociology of professions and social science research methodology. In terms of health informatics, this thesis argues for the necessity for health care information technology design to understand and incorporate the work practices of all professional groups who will be involved in using the technology system or whose work will be affected by its introduction. In terms of the sociology of professions, this thesis finds doctors and scientists to belong to two distinct occupational communities that each utilise documentation technology to different extents in their displays of professional competence. Thirdly, in terms of social science research methodology, this thesis speculates about the possibility for viewing the engagement of the groups with the research process as indicative of their reactions to future sources of outside perturbance to their work.
