Informedness about clinical trials of patients participating in placebo-controlled clinical trials in Lithuania / Pacientų, dalyvaujančių placebu kontroliuojamuose klinikiniuose vaistinio preparato tyrimuose Lietuvoje,informuotumas apie klinikinius tyrimus

The dissertation aims to analyse the problem of informed consent in clinical drug trials. Understanding of the provided information is one of the basic components of a person’s sound and informed decision to participate in a clinical trial. Our research evaluated informedness about clinical trials of patients participating in clinical drug trials in Lithuania, with emphasis on the informedness about key elements of clinical trial design (placebo-control, double-blindness, and randomisation). An anonymous survey of patients participating in clinical drug trials was conducted for the purposes of this study. The results of the study reveal that the legal framework sets the basis for adequate informedness about clinical trials of clinical trial participants, however, patients participating in placebo-controlled clinical trials are insufficiently informed about clinical trials. Patients participating in placebo-controlled clinical trials are better informed about the rights of clinical trial participants than about clinical trial design, however, informedness about design is a more important condition for overall informedness. The majority of placebo-controlled clinical trial participants do not understand at least one of the three key elements of clinical trials design and they tend to interpret the scientific methods used in clinical trials therapeutically. / Disertacijoje analizuojama informuoto asmens sutikimo įgyvendinimo klinikiniuose vaistinio preparato tyrimuose problema. Pateiktos informacijos supratimas yra viena svarbiausių asmens apsisprendimo dalyvauti klinikiniame tyrime sąlygų. Moksliname darbe vertinamas pacientų, dalyvaujančių placebu kontroliuojamuose klinikiniuose vaistinio preparato tyrimuose Lietuvoje (toliau – klinikinių tyrimų dalyviai), informuotumas apie klinikinius tyrimus, akcentuojant informuotumą apie klinikiniuose tyrimuose taikomus mokslinius metodus (placebo kontrolę, dvigubą aklumą, atsitiktinį tiriamųjų grupių sudarymą). Klinikinių tyrimų dalyvių informuotumas buvo tiriamas atliekant anoniminę apklausą. Mokslinio darbo rezultatai parodė, kad teisinis reglamentavimas sudaro prielaidas klinikinių tyrimų dalyvių informuotumui, tačiau jų informuotumas yra nepakankamas. Respondentai buvo geriau informuoti apie klinikinių tyrimų dalyvių teises nei apie klinikinių tyrimų metodologiją, tačiau informuotumas apie klinikinių tyrimų metodologiją yra svarbesnė prielaida bendram informuotumui. Dauguma klinikinių tyrimų dalyvių nesuprato vieno ar daugiau iš trijų pagrindinių klinikiniuose tyrimuose taikomų mokslinių metodų (placebo kontrolės, dvigubo aklumo, atsitiktinio tiriamųjų grupių sudarymo) ir buvo linkę suteikti jiems terapinę reikšmę. Tyrimo rezultatai sudaro prielaidas kryptingam klinikinių tyrimų dalyvių informuotumo gerinimui, informacijos apie klinikinius tyrimus viešinimui.

Dodržování práv pacientů ve vybraných nemocnicích České republiky. / Observing patients' rights in selected hospitals of the Czech Republic.

For everyone hospitalization represents a change accompanied by various restrictions. Anyway, this cannot mean a change in rights. Each patient is unique {--} they have specific needs, unchangeable principles, a value system of their own. A patient can be defined as a person under medical care, as an ill person undergoing treatment. The term {\clq}qpatient is derived from the Latin word {\clq}qpatiens`` {--} patient, suffering. The patients' rights are laid down in the Code of Patients' Rights which became effective in the Czech Republic in 1992. It is the knowledge of rights that is important before all for the strengthening of an equal relationship between the doctor and the patient The risks which represent a threat during the hospitalization can be lowered by consistent observation of the patients' rights as well as the patient's satisfaction with the care provided can be heightened. The basic individual rights of the patients include the right to silence to be preserved about the facts that the health worker became familiar with in connection with medical care or treatment, the right to being informed about their state of health, the right to gaining necessary information in order to be able to make a decision before a new diagnostic or therapeutical procedure is started up. The other rights include refusing a treatment and being informed about the consequences of the decision, the right to privacy during medical care or treatment and to a considerate, sensitive and worthy approach during the treatment. The thesis comprises the present state of problems in question, the aims of the thesis, determination of hypotheses, methodology, the results of the research, discussion, conclusion, the list of sources used, key words and supplements. The present state portrays the history of ethical codes, legal standards and the problems of medical law, the patients' rights, individual patients' rights, accreditation standards of the patients' rights. The aim of the thesis is to find out whether the patients are made acquainted with their rights after being taken to hospital, whether they are informed about the character of the disease, about suggested treatment and possible treatment alternatives. To map out whether there is the need of education being taken into account with each patient and whether the nurses keep notes in the documentation and find out the nurses' knowledge in the sphere of the patient's immobility. here have 7 hypotheses and 5 statistical hypotheses been specified in the thesis. Quantitative research approach was applied for the research. Questioning was used as a method, a questionnaire as a technique. The questionnaire for the patients was anonymous and included 26 questions. It focused on making the patient acquainted with their rights, on awareness of the state of health, the character of the disease and the suggested treatment, on the education of the patient by the nurse and on respecting the patient's intimacy. It included closed and semi-closed questions. At the end of the questionnaire the patients had a chance to express their possible comments, suggestions, impulses concerning the problems in question. The questionnaire for the nurses was anonymous and included 32 questions. It had its focus on observing the patients' rights, the informed patients' consent, their education, on the problems of restricting the patient on the bed and on respecting the patient's intimacy. The questionnaire included closed and semi-closed questions. The representative sample was made up of 218 patients and 205 nurses. The results were compiled in tabular and chart form, and subsequently processed by means of the SPSS statistical programme. The discussion includes a summary of the research knowledge in comparison with the sources used.

Children´s awareness of dealing with dogs starts to gain importance nowadays. More often we meet the problems of injuries caused in children by our best animal fellows.This situation needs to be solved. Problem cannot be seen only in accused animal. In most cases neither dog nor child is the deliquent. The main cause of a conflict between, in other way, harmonious coexistence of both partners seems to be children´s ignorance of behaviour, body speech and complex responsible attitude towards dogs. However,they cannot learn these basic rules themselves. Theoretical part of the thesis devotes attention to general outline of child {--} dog relation. Separate chapters are focused not only on psychological and social development of both arbiters, but also on their mutual impact, behaviour, care, communication and cooperation. At the same time, the thesis outlines the problems of dog bite-related injuries in children of our or foreign latitude. Last but not least it underlines the need of prevention and measures which could reduce or generally eliminate the rise of dramatic situations between a dog and a child. The main aim of this work was to chart awareness (informedness) of middle school- aged children of treating, care, behaviour and communication with dogs. Further presumption is to find out the knowledge differences in children who have dogs and those who haven´t. The output of practical part of the thesis are the results of quantitative research. There was used a method of questioning {--} technique standard questionnaire for children within the range of 3.-5. class of basic school. Research specimen represents pupils´ knowledge of four basic schools, in total 200 respondents. From the point of regional representation there were children of South Bohemia and Olomouc regions. According to the research there is evident absence in knowledge of dogs´ body speech and consequently the safety of mutual contact that represent the main problem in arising of risk situation between a child and a dog company. Therefore it is important to devote them more attention in future.

Kvalita ošetřovatelské péče z pohledu pacientů se stomií / The nursing care quality from the point of view of stoma patients

Person with stoma is any handicapped human with temporarily or permanently drawn hollow organ out onto body surface. The most common it concerns colon, small intestine or urinal system. Uncontrollable emptying of faecal matter or urine happens by innatural way with necessary use of stoma appliances. In stomas sphere the most important factor is informedness not only of the patient himself but of nurses as well. Sufficient education of a patient and ability of a nurse to treat stoma is the base of good nursing care. The aim of the thesis was to find out to what degree the stoma patients are informed on ways of stoma treatment and how they are satisfied with the quality of given nursing care. The work is divided on contemporary state and methodological procedures. Contemporary state includes especially pre-operation, post-operation, home and out-patient care, clients´ education, importance of stoma nurse and association of stoma patients, kinds of stoma appliances, specifics of stoma patients´ nutrition, social problems of stomas etc. For the research part six hypotheses were established. They concerned the quality of nursing care, problems of stomas treating, cooperation with nurse and club of stoma patients. It was found out that 81% addressed people are satisfied with the quality of given nursing care, 98% are familiar with problems of stomas treating, 99% were able after six months after setting of stoma to take care of stoma themselves,72% are members of the club and 90% cooperate with a stoma nurse. On the base of the research all hypotheses were confirmed and the aim of the thesis was fulfilled.In the thesis a method of quantitative research was chosen. The research proceeded in Hospital České Budějovice, Inc. in clubs of stoma patients in Tábor and Příbram. The technique of data collecting was anonymous questionnaire established for clients with a stoma. 100 respondents were addressed. The research proceeded on the base of personal contact and cooperation with stoma patients, therefore the return of questionnaires was 100%. Two questionnaires were from the reason of incompleteness taken out. The results are given in percentages and diagrammatized.

Spectrum of threat types and their combination is very extensive. The essence remained for chiliads unchanged, but during the development of mankind some patterns extinct or changed their shape, other reappear. Efficient managing of emergency situations requires beside this competent specialists, organizations, systems and legislative regulations and also ready and well-informed population. Principal part of knowledge and skills in the field of inhabitants protection and behavior in the emergency situation acquire the kids on the primary and secondary level of education. This object is included in the education on the primary and secondary schools according to Instruction of Ministry of Education, Youth and Sport about the integration of the subject of protection of man in a emergency situation into the education program, r. n. 13 586/03-22, and the related appendix to the educational documents "Protection of man in emergency situations", r. n. 13 586/03-22 of the 4. march 2003. Education proceeds to the extent of at least 6 lessons in every class, most often cross-sectional within the selected school subjects. Very important role in the education of inhabitants plays Fire Rescue Service of the Czech Republic. This master thesis focuses as one of its main goals on the inquiry of the knowledge and opinions of the selected age group of 15 to 20 years old related to the behavior in the emergency situation and to the knowledge of the integrated rescue system. Another goal was to find out, if the results of the inquiry are influenced by the fact, that the school and permanent home of the student is situated within or outside the emergency planning zone of the Dukovany nuclear power plant (EPZ Dukovany NPP). The theoretical part focuses mainly on the development of the education of children and youth in the field of protection of population after 1991 and on the contemporary system of education in the field of protection of man in emergency accident on professional and grammar schools. In the experimental part of the thesis two hypothesis were set. According to H1 should at least 75 % of the students correctly answer more than half of the questions. H2 was divided into two partial hypothesis, whereas it was supposed, that between the informedness of the students of grammar schools resident and studying within EPZ Dukovany NPP or outside EPZ Dukovany NPP and the students of the professional schools resident or studying in EPZ Dukovany NPP and outside EPZ Dukovany NPP these is no statistically significant difference. Presented data were acquired on the chosen schools within and outside the EPZ Dukovany NPP in the form of questionnaire survey. Questionnaire was set together from four questions of qualitative nature and 16 questions of quantitative nature. On the grounds of the measure data it was necessary to reject the hypothesis H1, since only 21,5 % of the students answered correctly more than a half of the answers. The average achievement in the questionnaire was 41,3 % of correct answers. In the testing of parametric hypothesis was by means of comparison of the selective statistical sets proved, that the informedness of the students of both types of secondary schools is on the same level and further that there is no significant statistical difference between the informedness of grammar school students living in and outside the EPZ Dukovany NPP. The hypothesis H2 was thereafter confirmed. The results imply, that the informedness of students in the field of the protection of population is insufficient. As a suitable solution of this problem appears the education of these topic within a separate subject focused on the protection of man.
