Associação entre jantar muito tarde e não tomar o café da manhã com o prognóstico de pacientes com infarto agudo do miocárdio com supradesnivelamento do segmento S-T

Orientador: Marcos Ferreira Minicucci / Resumo: Introdução: O infarto agudo do miocárdio com supradesnivelamento do segmento ST (IAMCST) é a apresentação clínica de maior gravidade dentre as síndromes coronarianas agudas, com alta morbidade e mortalidade e responsável por grande número de hospitalizações cardiológicas no mundo. A literatura evidencia que o padrão alimentar interfere no prognóstico dos pacientes vítimas de síndromes coronarianas agudas. Recentes estudos associaram hábitos alimentares como jantar tarde e não tomar o café da manhã com maior risco de eventos cardiovasculares. Hipótese: Foi elaborada a hipótese de que jantar tarde e não tomar o café da manhã está relacionada a pior prognóstico em pacientes admitidos com IAMCST. Objetivo: O objetivo deste estudo foi avaliar a associação entre jantar tarde e não tomar o café da manhã com o desfecho combinado de mortalidade e arritmias malignas (TV/FV) nos pacientes com IAMCST durante a internação e 30 dias após a alta hospitalar. Casuística e Métodos: Esse estudo é prospectivo e observacional, realizado com pacientes admitidos com o diagnóstico de IAMCST na Unidade de Terapia Intensiva Coronariana (UTI-UCO) da nossa Instituição, com idade maior ou igual a 18 anos, durante 13 meses consecutivos. Durante a internação os doentes responderam sobre os seus hábitos em relação à ingestão do café da manhã e os seus horários de jantar previamente às suas internações. (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Introduction: ST-Elevation Myocardial Infarction (STEMI) is the worst type of acute myocardial infarction, which represents a large amount of hospitalizations, morbidity and mortality around the world. Literature has shown a correlation between dietary pattern and prognosis of patients victim of Acute Coronary Syndromes. Recent studies have associated eating habits as late-night-dinner and breakfast skipping with higher cardiovascular risk. Hypothesis: Association between late-night-dinner and breakfast skipping with a worst prognosis in patients with STEMI. Objective: Evaluate the association between breakfast skipping and late-night-dinner with the primary end point, mortality and malignant arrhythmias (VT/VF), in patients with STEMI during hospitalization and 30 days after hospital discharge. Methods: This is a prospective and observational study that included patients admitted with diagnosis of STEMI, in Coronary Intensive Care Unit of our institution, aged 18 or older, for 13 months. During the hospitalization the patients answered a food frequency questionnaire about breakfast skipping and dinner time, before admission. Uni and multivariate analyses were performed. Výjimečné šváby (Blattidae) z cenomanského Myanmarského jantaru / Unusual cockroaches (Blattidae) from Cenomanian Myanmar amber

The diploma thesis presents an overview of the extinct representatives of Blattidae, the still living cockroach family. Six already described species, belonging to three genera, have been confirmed. All of them lived during Cenozoic. Eight new morphotypes from Myanmar amber were recognized. Cenomanian Burmite with its exceptional state of preservation, allows a detailed study of the morphology of the inclusions. Six morphototypes belong to the family Blattidae. One morphotype belongs to the derived family Olidae studied herein. One female morphotype belongs to the still living family Ectobiidae, based on the presence of an attached ootheca. The work specifies the previous molecular analysis (J2-K1) and determines the divergence of Blattidae during Early Cretaceous. Any evidence and interpolations before 127Ma is not substantiated. Phylogenetic analysis confirmed Blattidae being parafyletic in respect to the family Olidae. The work emphasizes the need for changes in systematics of the Mesoblattinidae, the stem of the Blattidae. Biologické inkluze zachované v eocénním jantaru z lokality Študlov v Bílých Karpatech / The biological inclusions in Eocene amber from Študlov locality in White Carpathian Mts.

The tudlov amber has been the only known fossil resin from the Czech Republic, in which the biological inclusions are preserved, and at the same time the only Czech fossil resin occurring not in the area of the Bohemian Massif, but in outer arch of the Outer Western Carpathians. So far, only single findings of fossil Hymenoptera and Diptera have been published. In contrast to mostly Cretaceous ambers of the Bohemian Massif, the age of tudlov amber is usually estimated from the late Paleocene to mid Eocene. This diploma thesis provides an overview of the fossil organisms found in the tudlov amber in the past two decades and it is also the most comprehensive text dedicated to the palaeodiversity of embedded organisms from this remarkable fossil resin. The presence of various microscopic fungi is proven, as well as the presence of angiosperm trichomes, mites and insects (prevailed by Hymenoptera and Diptera). The discovery of a hymenopteran family Stigmaphronidae (Apocrita) is the first Cenozoic evidence of this family, formerly considered as extinct by the end of Cretaceous. Finding of an isolated gymnosperm twig with wood anatomical characters close to genus Glyptostrobus allows to outline some conclusions about the paleoecosystem. Amber in Bohemia in the Bronze Age - Abstract Amber found in archeaological contexts like any sensitive instrument can show the total span of meanings of different historic, economic, cultural, cult, and social traditions and systems of values in the prehistoric Europe. The work synthetically analyzes and evaluates the presence of amber finds in Bohemia during the Bronze Age. Attention is devoted relations to sites and amber artefacts demonstrating the probable course of "amber route". On the path leading from the Baltic coast to the Balkans and Italy and further into the eastern Mediterranean seen as a string, or rather a network of relations, most malleable and flexible, distributed among communities, which is also subject while reflecting internal and external influences, changes and developments. It reflects new amber artefacts and new results of IR spectroscopy. Its results shows, that amber did not came to Bohemia only from the north - from Baltic region; its results proved also rumanit amber. Work is a comprehensive evaluation and interpretation of participation Bohemia in the distribution network of leading amber in the wider European area. Mudanças alimentares: uma análise do hábito de jantar pelo relato dos mais vividos do SESC Uberlândia/ MG

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior / Nous avons étudié le groupe « Os mais vividos do SESC », Uberlândia MG, où nous faisons l\'analyse des transformations dans l\'habitude de dîner. Pour le faire, nous partons des rapports des personnes âgées qui vivent ces transformations et qui sont donc des témoins de celles-ci. Nous avons pour but d\'analyser les changements de cette habitude et, en considérant que cela prend en compte des changements alimentaires et sociaux plus vastes, nous avons cherché à observer les habitudes, significations et liens sociaux établis dans ce repas. En travaillant avec des personnes âgées, nous avons entendu aussi remarquer les spécificités concernant leur condition et leur alimentation. Il s\'agit d\'une amplification des recherches que nous faisons, en prenant en compte l\'importance de ce repas dans les relations dans la famille et le contexte actuel où ce repas n\'est plus pris, étant remplacé par des snacks individuels. Nous remarquons qu\'un repas considéré essentiel perd son importance, ce qui arrive en fonction du besoin d\'aliments forts quand l\'on travaille ou fait des activités fortes et aliments légers quand l\'activité est légère ou l\'on ne travaille plus. Dans son vieillissement, le groupe étudié incorpore l\'idéal d être actif, en bonne santé et indépendant et pour le réussir restreint leur alimentation à ce qui est considéré comme léger et sain. Le dîner est vu comme lourd, quelque chose qui fait grossir et est mauvais et qui est laissé de côté par la plupart. Néanmoins, l\'option de ne pas réaliser ce repas n\'est pas pris que pour cette étape de la vie. Il y a, dans la jeunesse et à la vie adulte, d\'autres aspects qui sont déterminants, comme le déménagement à la ville, des nouvelles activités, des nouveaux horaires, car en travaillant jusqu\'à plus tard, le dîner devient contre indiqué. Quand il s\'agit des personnes âgées, la plupart sont veuves et habitent seules, différents sentiments et facteurs prennent en compte leur alimentation et la font changer. Il y a la solitude quotidienne de chez soi et l\'individualisation des habitudes alimentaires. Par rapport à comment ces personnes voient la nourriture, nous pouvons dire que le passé et le présent sont mis en opposition : soit l\'un est vu tel que plus sain, soit l\'autre, ce que prennent en considération des facteurs comme la production naturelle, des poisons et des hormones et le danger lié à la graisse. L\'hygiène de vie et la disposition deviennent un objectif constant dans les changements alimentaires. Les personnes âgées cherchent à être des mangeurs compétents dans un contexte de cacophonie alimentaire et gastro-anomie, où l\'on reformule la connaissance commune et traditionnelle, en incorporant des savoirs scientifiques de façon à orienter ses pratiques. / Neste trabalho realizamos um estudo de caso com o grupo dos Mais vividos do SESC , de Uberlândia-MG, no qual analisamos as transformações pelas quais tem passado o hábito de jantar. Para isso partimos do relato de pessoas idosas que as têm vivenciado e que são, portanto, reveladoras delas. Tendo como objetivo analisar as mudanças desse hábito e considerando que envolve alterações alimentares e sociais mais amplas, procuramos observar costumes, significações e relações sociais estabelecidos nessa refeição. Ao trabalhar com pessoas idosas, buscamos também notar especificidades relativas a sua condição e a sua alimentação. Trata-se de uma ampliação das pesquisas que temos realizado considerando a importância dessa refeição nas relações familiares e o atual contexto, em que deixa de ser feita, substituída por lanches individuais. Vemos que uma refeição antes tida como essencial se torna dispensável, mudança que se relacionaà necessidade de alimentos fortes quando se trabalha ou pratica atividades pesadas e alimentos leves quando a atividade é leve ou não se trabalha mais.O grupo estudado incorpora o ideal da terceira idade: ser ativo, saudável e independente e para tal restringe sua alimentação ao que é tido como saudável e leve. O jantar,considerado pesado, algo que engorda e faz mal, deixa de ser feito pela maioria. No entanto, a opção por não fazer essa refeição não se dá apenas nessa fase da vida.Também na juventude e vida adulta outros aspectos são determinantes, como a mudança para a cidade, as novas atividades, os novos horários, pois,trabalhando até mais tarde, o jantar se torna contraindicado. Tratando-se de pessoas idosas, das quais a maioria é viúva e mora só, diferentes sentimentos e fatores envolvem sua alimentação e a modificam. Há solidão doméstica cotidiana e individualização dos hábitos alimentares.Sobre as visões que têm do alimento, podemos dizer que passado e presente são postos em oposição, ora um é tido como mais saudável, ora outro, o que leva em consideração fatores como produção natural, venenos e hormônios e o perigo devido à gordura.A saúde e a disposição se tornam uma busca fundamental nas mudanças alimentares. Procuram se tornar comedores competentes em um contexto de cacofonia alimentar e gastroanomia, reformulando o conhecimento comum e tradicional, incorporando saberes científicos de modo a orientar suas práticas. / Mestre em Ciências Sociais
