Patient satisfaction is one of the indicators of the quality of care being given to the users of a service. It can also be used as benchmarks for ensuring the delivery of quality physiotherapy services in health facilities. Physiotherapists have been involved in treatment of persons suffering from low back pain for decades. Treatment approaches are varied, but all have the common goals of pain relief, rehabilitation, and prevention of recurrence of low back pain. The purpose of this study was to investigate the satisfaction of low back pain sufferers with the physiotherapy services they receive. The study was carried out in selected public hospitals in Nairobi and the Central Province in Kenya.

The Harambee Movement and its Effects on Kenya's Economic Development

This paper examines the Harambee Movement and how it affected Kenya's economic development. Before 1963 Kenya was a British colony and economic development favored the colonial government. At Independence the new government tried to restructure the economy for the Kenyan people. Recognizing the lack of adequate capital and a rural development program, the movement became a means by which people could provide services for themselves. These services included schools, technical training, water supply and health facilities. Sources of data were books, articles, government publications and papers published by the Institute of Development Studies, Nairobi University. The Harambee Movement flourished in the provision of educational opportunities, clean water supply and health facilities. Problems included planning, coordinating and duplication of services.

Psychosocial support and wellbeing of orphans and vulnerable children (OVC) in the context of HIV/AIDS: a case study of early childhood development and education (ECDE) centres in Nairobi, Kenya

However, great concerns still persist due to a multiplicity of factors that affect the children’s quality of life. This study examines experiences of psychosocial support and wellbeing of these children in Nairobi, Kenya. While there are few existing studies on the effects of HIV and AIDS on OVC in Kenya, most focus on OVC in primary schools and high schools leaving out infants and preschool aged OVC, as is the case in sub-Saharan Africa. Thus, information that details psychosocial experiences and wellbeing of OVC in preschools in Kenya is lacking. This study focused on OVC aged between 5 and 7 years old within a resilience theoretical perspective; investigated their lived experiences, risk exposure, support structures and other adaptive experiences. A total of 57 participants (45 were children and 12 teachers) from ECDE centres in Nairobi County were purposively sampled for the study. Multiple methods utilised included: visual stimulus illustrations, face-to-face and conversational interviews, thematic drawings, focus group discussions questionnaires and infrastructural and supportive resource assessment. Thematic content analysis (TCA) which was informed by phenomenological hermeneutic approach was used to analyse data. Findings consistently showed that, OVC were not only susceptible to numerous risk experiences due to environmental and care factors at home, but also on their way to and from school, and within ECDE centres. Nonetheless, ECDE centres exposed them to cognitive skills, social networks, safety, alternative care, primary health services, nutrition and resilience nurturance. These findings indicate the crucial role of having a collective action in reduction of adverse experiences and improving resources to empower the OVC in urban ECDE centres. Directions for further research could focus on expanded population-based studies examining poverty, malnutrition, resilience and coping resources among preschool children living with, and affected by HIV and AIDS, using creative multiple qualitative approaches. A more critical understanding on variations in sexes is equally important in intervention strategies for this group of OVC. / MT2018

An analysis of social relationships at a development site in Kenya

Both development and post-structuralist anthropologists have critiqued development. Development anthropologists are concerned that development does not take adequate account of the social and cultural factors of developing nations, while post structuralists question the ontology of development and assert that domination over developing nations is inherent in the concept of development. To examine the social effects of development projects I conducted ethnographic research for a nine-week period on social relations at Sagana Fish Farm, located two kilometers from Sagana Township in the Kirinyaga District of the Central Province of Kenya. (The population of Sagana Township is approximately 5,885 people.) I obtained thirty-one semi-structured interviews, also relying on participant observation and informal interviews with civil servants (permanent employees) and casual laborers (temporary employees). I analyzed information obtained during interviews and participant observation within the context of power and resistance theory. Civil servants tended to value the presence of development projects because projects made structural enhancements and renovations to the fish farm. Development projects presented civil servants the possibility for skills development and additional income to fish farm employees and created temporary employment for individuals from neighboring communities. Civil servants stated that an inexpensive source of protein was available in the form of fish, chicken, and milk as a result of the work of development organizations. However, they also expressed concerns regarding the sustainability of development projects due to tension between expatriate development project workers and fish farm employees, inadequate information sharing and technology transfer, and financial and human resource mismanagement in the Kenya civil service. Casual laborers discussed the possibility of skills development through their work with development projects at the fish farm and expressed concern about their employment conditions. They were concerned about low wages in contrast with the intensity of their labor; insecure terms of employment; an absence of protective equipment at their work site; health and medical issues; and the availability of treated drinking water. Other concerns were associated with dignity, tribalism, and a fear of power of expatriate development workers and Kenyan civil service officers. Based on the above findings I made the following recommendations were made: implement cultural sensitivity training for expatriate development workers; develop project plans that foster a sense of investment in project operations; provide discounted fish to casual laborers; improve the work conditions of casual laborers; establish a health clinic; and provide informal loans to facilitate technology transfer. / Graduation date: 1999

Attitudes of teachers and pupils toward use of corporal punishment in Nakuru municipal primary schools, Kenya.

The use of corporal punishment in schools is a highly topical and much debated subject worldwide. With the advent of humanitarian principles, corporal punishment was classified by the United Nations (UN) as violence and child abuse and this led to the banning of its use in schools by many countries especially in the western world. Kenya is not an exception as it recently followed suit by banning the use of corporal punishment in schools, through the enactment of Education Act Number 30 of 2001. Despite the ban, information from press reports and other literature seem to indicate that the practice of corporal punishment continued unabated in Kenyan schools. However, there is little information to indicate: (1) whether infact corporal punishment was being practiced in the selected schools or the extent/prevalence and nature of its practice; (2) what maintains corporal punishment in schools; and (3) strategies for eliminating it. Moreover, the extent to which teachers and pupils are aware of the ban is not known. Three main goals of this study, therefore, were to establish: whether corporal punishment was being practiced in Nakuru Municipal primary schools in Kenya; the attitudes that the primary school teachers and pupils hold towards the use of corporal punishment in the selected schools and the impact, if any, that their attitudes had on the practice of this punishment in those schools. The study applied a Survey research design. Questionnaires, with both open-ended and closed-ended items were used to collect data from the respondents. The respondents' attitudes were expressed in terms of their beliefs, feelings and views about the use of corporal punishment in school. The attitudes were measured in terms of the respondents' degree of agreement and disagreement with the attitudinal statements measured along a Likert scale which gauged their beliefs, feelings and views about the use of corporal punishment in the selected schools. The target population consisted of teachers and pupils in four primary schools in Nakuru Municipality. A sample of 72 teachers and 160 pupils was selected. Descriptive and inferential statistics were used to analyse data using the SPSS software package. The results obtained indicated that corporal punishment was being practiced in the selected schools and the teachers and pupils this practice was supported. Corporal punishment was being practiced by teachers in Nakuru municipal primary schools despite their knowledge of the legal prohibitions and negative consequences of this practice. It was also established that the attitudes of the teachers and pupils towards the use of corporal punishment in school had a significant relationship with the practice of corporal punishment in the four selected schools. Where the teachers and pupils had a favourable or positive attitude towards the use of corporal punishment in school, the level of the practice of corporal punishment was very high but where the teachers and pupils has a negative attitude towards the use of corporal punishment in school, the level of the practice of corporal punishment was extremely low. The teachers and pupils justified the practice of corporal punishment in the selected schools under the belief that it had merits. It (corporal punishment) was believed to be a chastisement for children, a catalyst for good academic performance and the most effective deterrent for bad behavior, laziness and a corrective measure. The conclusion drawn was that the teachers’ and pupils’ attitudes towards the use of corporal punishment in school had a big impact on the practice of corporal punishment in the four Nakuru municipal primary schools. The study concludes with a set of recommendations for curtailing the use of corporal punishment in schools in Kenya. (1.) The Swahili architecture of Lamu, Kenya : oral tradition and space

This dissertation is about the architecture of the Swahili peoples living along the eastern coast of Africa. Specifically, it explores the links and relationships between oral traditions, rituals and the built environment of the Waswahili (sing. Mswahili) or the 'people of the coast'. The 'ambiguous' and 'anomalous' identity of the Waswahili raises important questions on the definition and the understanding of Swahili architecture. To understand Swahili architecture, one must, first, understand the language and identity of the Waswahili. This dissertation makes use of new sources for the interpretation of the built environment of the Waswahili as depicted in the standing 18th century buildings in Lamu town, the oldest living town on the eastern coast of Kenya. Designated on UNESCO's World Heritage List, Lamu has a unique architecture that has often been misinterpreted and misunderstood, and such studies often lack authenticity. This dissertation is an attempt to bridge the gap between the identity and the built environment of the Waswahili and to portray Swahili architecture through oral discourse.

Gender and property among sedentarized pastoralists of Northern Kenya

In the context of growing poverty and sedentarization, the socio-economic status of pastoral women is an important indicator of how pastoralists in northern Kenya respond to social change. Accordingly, this study examines women's position in three communities in distinct settings of sedentarization. One is located in a semi-arid region dedicated to pastoralism and conservation, while the other two are in a moister mountain area where rain-fed and irrigation agriculture is combined with animal production. Analyses of quantitative and qualitative data, gathered from women and men during 2002--2003, indicate that women have largely responded to social change by using two strategies to secure the well-being of themselves and their children. First, despite the cultural assumption that married women are supported by their husbands, they have strengthened their relationships with natal kin, solidifying a support network to carry them through times of difficulty. Secondly, given decline in returns from subsistence pastoralism, women have seized numerous opportunities to diversify their economic pursuits in order to generate steady income. / Essentially, findings illustrate that, in addition to gaining access to various resources through their marital homes and their own efforts, the majority of women receive socio-economic support from their natal kin, especially brothers. In two communities where land is being privatized, most women have been excluded from the land registration process because of traditional and national policies. However, many fathers are awarding their daughters permanent usufruct rights to family land to ensure they do not become landless because of the death of a spouse or negligence on the part of husbands. Although this does not eliminate the discrimination many women face from being excluded from the registration process, it is a move towards the betterment of women's long-term food and financial security. / Besides contributing to domestic activities, women engage in very different forms of income-generating activities; in Archer's Post, they obtain earnings from craft sales or tourist-related services, in Parkishon/Karare they gain income from milk-marketing, while in Songa, women pursue cultivation for subsistence and market sale. It is a positive finding that most women generate steady income over which they have managed to maintain control.

Health education needs among individuals with low back pain.

The prevalence of low back pain has assumed an upsurge trend in the last five decades despite the many interventional strategies. One interventional strategy that has been unsuccessful has been patient education. Lack of positive results from many of the existing patient education programmes is probably due to the type of health information that has been presented and the method that has been used. Many of the health education programmes have been planned according to what the medical professionals assumed the individuals needed to know, assumptions that could have ignored some crucial aspects. This study explored the perceived health education needs of individuals with low back pain at the Nairobi Hospital Rehabilitation Unit in Kenya, the method used to educate the individuals, the appropriateness of the method according to the individuals in addition to identifying the source of the health education that the individuals had.

Ethnography of production practices in Kenyan television entertainment programmes: imagining audiences.

How television entertainment programmes producers in Kenya conceptualise audiences is the primary objective of this study. It begins with a brief examination of how the operations of broadcast media institutions in Kenya have been historically linked to government and commerce. Throughout the history of television in Kenya, producers have conceptualised audiences in line with the political, economic and socio-cultural factors that were paramount in the instituting of broadcasting in this society. This historical background continues to shape the character of television entertainment programmes, and therefore how producers conceptualise audiences for these programmes. During their production practices, producers are also influenced by particular communication dynamics within which television programmes are produced and viewed. The dynamics of 'being the audience of television' include that the 'active audience' is autonomous in its various relationships with programmes content, yet the subjectivity of viewers to the institutional systems within which broadcasting happens constrains the audience‘s freedom in how it relates to entertainment programmes. Programme content hails and guides the audience into 'attending' to given shows in specific ways. This study reveals that the audience multi-facetedly relates with entertainment programmes, but the degree to which the audience can exercise its 'will' over the television text is limited. This is because television programmes are constructed meanings, framed and constricted by the elements that constitute them. Also, structures of culture constrain the plurality of the resources audiences have at their disposal as tools for 'reading‘ the programmes. The research-participant producers conceptualised the audience from a 'value-based‘ socio-cultural perspective. Therefore, they attached a kind of magnanimity to television as an institution for influencing in specific ways the segments of society they imagined watched it. Hence, producers of the particular entertainment programmes considered in this case study intended them to represent quality socio-cultural values for the social development of Kenyan society. In agreement with the producers, the audience respondents cited in this study appeared to consider entertainment programmes as important narratives capable of helping them better understand the social world they live in. They saw entertainment programmes as stories that authenticate their world by reflecting that world back to them. Overall, the findings of this case study established that Kenyan producers of television entertainment programmes technically operated within the political economic conventions of television production. However, a strong philosophical, moral-value code appeared to guide the producers‘ sense of purpose and duty to their audience. Apparently, the producers‘ resolve to embed in programmes meanings that propagated particular socio-cultural ideals was as prominent as the institutional political economic objectives for which they were hired to fulfil. This 'extra‘ sense of purpose catalysed the producers‘ unique regard for entertainment programmes as functional narratives, whose primary objective it should be to elevate society‘s moral fabric. Conclusively, the research-participant producers employed an old-fashioned approach to conceptualizing the audience.
