Laser light interaction and their influence on DNA and cell under in vitro conditions

Laser photobiomodulation (PBM) or low-level laser therapy (LLLT) is a form of phototherapy recognized worldwide for its expansive use in medicine. PBM/LLLT has the ability to enhance enzymatic activity and mitochondrial transmembrane potential that increase the availability of energy and the signal transduction, which promotes cell proliferation. This study primarily investigates the appropriateness of a range of treatment parameters, including light wavelength, irradiance (doses) and exposure time of PBM/LLLT in proliferation of cultured human monocytic leukaemia cell line THP-1(Tamm-Horsfall Protein 1), as well as DNA under in vitro conditions. A secondary objective was to exploit the beneficial effect of PBM/LLLT to reduce the harmful impacts caused by exposure to ultraviolet (UV) radiation. Cells were irradiated with near infrared (NIR) diode laser at 850 nm with doses ranging from 0 – 26.8 J/cm2. After irradiation, cells were incubated for 12 hr and 24 hr to allow time for proliferation. Comet assay was conducted to evaluate genotoxicity of the irradiated cells. Trypan blue exclusion test and MTT assay (3-4-5-dimethy-2.5 thiazol-2.5 diphenyl tetrazolium bromide) were used to estimate cytotoxicity, Exposure to NIR diode laser revealed the beneficial effect of PBM/LLLT at low doses (< 5 J/cm2), which increased the viability and proliferation of THP-1 cells, which conformed by increasing ATP synthesis and the activity of mitochondria. Comet assay showed no significant difference between irradiated and non-irradiated cells at low doses and showed no DNA damage. The photobiomodulation effect of low doses, in particular 0.6 J/cm2 and 1.2 J/cm2, managed to reduce the damage to THP-1 cells after exposure to UV radiation by decreasing cell apoptosis, accelerate DNA damage repair and increase cell survival. While, the exposure for high doses (> 5 J/cm2), showed different results involved decrease in the number of THP-1 cells and viability, in addition to a considerable increase in DNA damage, which emphasised by decrease in the level of ATP synthesis and reducing the mitochondrial activity. These findings demonstrated the photobiomodulation effect of NIR diode laser through modulating various pathways such as ATP synthesis and mitochondrial activity in monocyte cells and DNA.

Efeito dos laseres de baixa intensidade na proliferação e diferenciação de osteoblastos humanos / Low level laser effects in proliferation and differentiation of human osteoblasts

Dentre os vários compostos utilizados na pesquisa e na terapia de doenças osteo-degenerativas, a fototerapia com laseres de baixa potência (LLLT) e os diodos emissores de luz (LEDs) vem sendo investigada com o intuito de avaliar seus efeitos no metabolismo ósseo. Estes, que possuem comprimentos de ondas específicos, atuam na biomodulação das células, funcionando como um agente terapêutico, reequilibrando e normalizando a sua atividade. No entanto, pouco se sabe sobre o efeito dos diferentes espectros na proliferação e diferenciação de osteoblastos humanos, bem como seus efeitos no metabolismo celular como a síntese e a ativação de proteínas sinalizadoras envolvidas nesses processos. Diante disso, o objetivo deste trabalho foi avaliar, comparativamente, a influência da fototerapia com LLLT e LED na proliferação e diferenciação de osteoblastos humanos. Além disso, investigamos o envolvimento da ativação da via de sinalização ERK1,2 nestas respostas, utilizando o seu inibidor específico e/ou avaliando a sua ativação durante a proliferação e após fototerapia. Para esse estudo, osteoblastos humanos (HOAL) foram cultivados em meio de cultura DMEM suplementado com 10% de soro fetal bovino (SFB) e incubados em estufa de CO2. As células foram irradiadas pontualmente com os laseres vermelho (660nm), infravermelho (780nm) e LED (637nm), nas doses de 10, 20 e 50 J/cm2 na potência de 40mW, após adesão celular. Após 24, 48, e 72 horas foram realizados os ensaios de redução do MTT (brometo de 3-(4,5-dimetiltiazol-2-yl)-2,5- difeniltetrazólio) e cristal violeta (CV) para avaliar a viabilidade das células e após 72 horas foi realizada a análise da proliferação por citometria de fluxo nos quais os resultados sugerem aumento de células viáveis ou proliferação quando estimuladas pelos diferentes espectros. Após a verificação do efeito positivo dos laseres e LED na viabilidade e/ou proliferação, foi realizada a análise da ativação da proteína intracelular ERK por western blotting usando anticorpo específico após 10 minutos da irradiação pontual. Mostramos que a irradiação das células HOAL com LLLT, na dose de 10 J/cm2, aumentou significativamente a fosforilação da ERK1/2 em relação ao controle. Para os ensaios de diferenciação, as células receberam pontualmente o estimulo a cada 6 dias e após os períodos de 07, 14, 21 e 28 dias foram realizados os ensaios da atividade de fosfatase alcalina (ALP), expressão gênica do colágeno I (COL1A1) e SPARC (osteonectina) por RT-PCR em tempo real e ensaio de mineralização com vermelho de alizarina. De um modo geral, não foram observados diferenças na atividade da ALP entre os grupos tratados em relação ao controle. A expressão gênica do COL1A1 e SPARC foi aumentada, no qual o LED em ambas doses apresentou maior eficácia em aumentar a expressão desses genes. No que se refere à mineralização, foram observadas pequenas diferenças quanto a deposição de cálcio. Dessa forma, a LLLT e LED, nesse regime de aplicação modulou positivamente o metabolismo dos osteoblastos humanos em relação à viabilidade, porém, no processo de mineralização foram observadas poucas diferenças. / Among the various compounds used in research and bone degenerative diseases therapy, phototherapy with low level laser (LLLT) and light emitting diodes (LEDs) has been investigated in order to evaluate its effects on bone metabolism. Those, who have specific wavelengths, act in biomodulation cells functioning as a therapeutic agent, rebalancing and normalizing their activity. However, little is known about the effect of the different spectra in the proliferation and differentiation of human osteoblasts and their effects on cellular metabolism as well as the synthesis and activation of signaling proteins involved in these processes. Therefore, the aim of this study was to compare the influence of LLLT and LED phototherapy in the proliferation and differentiation of human osteoblasts. In addition, we investigated the involvement of activation of ERK1,2 signaling pathway these responses using its specific inhibitor and/or evaluating their activation during the proliferation and after phototherapy. For this study, human osteoblasts (HOAL) were cultured in DMEM culture medium supplemented with 10 % fetal bovine serum (FBS) and incubated in CO2 incubator . Cells were irradiated with punctual red lasers (660nm), infrared (780nm) and LED (637nm) at doses of 10, 20 and 50 J/cm2 in power 40mW, after cell adhesion. After 24, 48, and 72 hours, MTT assay (- (4,5- dimethylthiazol-2- yl) -2,5 - diphenyltetrazolium bromide 3 ) and violet crystal (CV) were performed to assess the viability of cells and after 72 hours, was performed of proliferation analysis by flow cytometry. The results suggest an increase in viable and proliferation of cells when stimulated by different spectra. After checking the positive effect of lasers and LED viability and/or proliferation, analysis of ERK activation of intracellular protein by western blotting using a specific antibody was performed 10 minutes after the spot irradiation. We show that irradiation of HOAL cells with LLLT at a dose of 10 J/cm2, significantly increased the phosphorylation of ERK1/2 compared to control. For differentiation assays, cells promptly received stimulation every 6 days and after periods of 07, 14, 21 and 28 days testing the activity of alkaline phosphatase (ALP), gene expression of type I collagen (COL1A1) and SPARC (osteonectin) were performed by Real Time RT-PCR and mineralization experiment with alizarine red. In general, no differences in the activity of ALP between the treated groups compared to the control were observed. The gene expression was increased, and SPARC COL1A1, where in the LED in both doses showed greater effectiveness in increasing the expression of these genes. With respect to mineralization, small differences in the deposition of calcium were observed. Thus, LLLT and LED, this enforcement regime, positively modulated the metabolism of human osteoblasts during the cell viability, but in the process of mineralization few differences were observed.

Near infrared laser propagation and absorption analysis in tissues using forward and inverse Monte Carlo methods

For understanding the mechanisms of low level laser/light therapy (LLLT), accurate knowledge of light interaction with tissue is necessary. In order to have a successful therapy, laser energy needs to be delivered effectively to the target location which depending on the application can be within various layers of skin or deeper. The energy deposition is controlled by input parameters such as wavelength, beam profile and laser power, which should be selected appropriately. This thesis reports a numerical study that investigates the laser penetration through the human skin and also provides a scale for selection of wavelength, beam profile and laser power for therapeutic applications. First, human skin is modeled as a three-layer participating medium, namely epidermis, dermis, and subcutaneous, where its geometrical and optical properties were obtained from the literature. Both refraction and reflection are taken into account at the boundaries according to Snell’s law and Fresnel relations. Then, a three dimensional multi-layer reduced-variance Monte Carlo tool was implemented to simulate the laser penetration and absorption through the skin. Local profiles of light penetration and volumetric absorption densities were simulated for uniform as well as Gaussian profile beams with different spreads at 155 mW average power over the spectral range from 1000 nm to 1900 nm. The results showed that lasers within this wavelength range could be used to effectively and safely deliver energy to specific skin layers as well as to achieve large penetration depths for treating deep tissues, without causing any skin damage. In addition, by changing the beam profile from uniform to Gaussian, the local volumetric dosage could be increased as much as three times for otherwise similar lasers. In the second part of this thesis, a three-dimensional single-layer reduced-variance inverse Monte Carlo method was developed to find the optical properties of the skin using the experimental values of transmittance and reflectance. The results showed that both transmittance and reflectance scale well with transport optical thickness. Moreover, it was also shown that penetration depth is highly sensitive to the laser wavelength and varied within the range from 1.7 mm to 4.5 mm. / text

No. of bitstreams: 0 Previous issue date: 2018-02-18 / A Laserterapia de Baixa Potência (Low Level Laser Therapy – LLLT) apresenta-se como uma terapia não invasiva reconhecida por acelerar processos reparativos tanto de tecidos moles quanto de tecidos duros devido aos seus efeitos biomoduladores nas células e nos tecidos. Quando há associação de uma fonte de luz de comprimento de onda conhecido a um corante fotossensível de baixa toxicidade, temos a, também conhecida, Terapia Fotodinâmica (Photodynamic Therapy - PDT) que, ainda pouco se sabe sobre sua ação no processo de consolidação óssea. O objetivo deste trabalho foi avaliar o processo de reparo ósseo em ratos submetidos à LLLT associada ou não ao azul de metileno a 0,01%, utilizando-se para isso a avaliação radiográfica e histológica. Defeitos ósseos foram realizados e distribuídos em quatro grupos: não tratados (Grupo controle); preenchidos por azul de metileno e não tratados (Grupo azul de metileno); tratados com laser 660nm (Grupo LLLT); preenchidos por azul de metileno e tratados com laser 660nm (Grupo LLLT + PDT). As radiografias foram realizadas nos dias 0, 7 e 14. Metade dos animais, irradiados ou não, foram eutanasiados após sete dias e a outra metade após 14 dias, sendo os fêmures removidos para análise histológica. Os resultados sugerem que, radiograficamente não houve consolidação óssea em nenhum dos grupos tratados, porém os defeitos ósseos do grupo LLLT + PDT apresentaram maior grau de radiopacidade; histologicamente houve maior aumento da área de neoformação óssea nos grupos LLLT e LLLT + PDT, com destaque para este último em ambos os períodos avaliados. Podemos desta forma, concluir que além da associação da PDT à LLLT ter sido eficaz, a combinação das duas técnicas promoveu indução da osteogênese de maneira precoce. / Low Level Laser Therapy (LLLT) is a recognized noninvasive therapy for accelerating repair processes of both soft and hard tissues due to their biomodulating effects on cells and tissues. When there is an association of a light source of known wavelength with a low toxicity photosensitive dye, we also have the known Photodynamic Therapy (PDT), which is still little known about its action in the process of bone healing. The objective of this work was to evaluate the bone repair process in rats submitted to LLLT, associated or not with 0.01% methylene blue, using radiographic and histological evaluation. Bone defects were performed and distributed in five groups: untreated (control group); filled with methylene blue and untreated (methylene blue group); treated with laser 660nm (LLLT Group); filled with methylene blue and treated with laser 660nm (LLLT + PDT Group). Radiographs were performed on days 0, 7 and 14. Half of the animals, irradiated or not, were euthanized after seven days and the other half after 14 days, and the femurs were removed for histological analysis. The results suggest that radiographically there was no bone consolidation in any of the treated groups, but the bone defects of the LLLT + PDT group presented a higher degree of radiopacity; histologically, there was a greater increase in the area of bone neoformation in the LLLT and LLLT + PDT groups, especially in the two evaluated periods. We can therefore conclude that besides the association of PDT with LLLT was effective, the combination of the two techniques promoted the induction of osteogenesis in an early manner.

Laser de baixa potência no tratamento da artrite induzida por zymosan

No. of bitstreams: 0 Previous issue date: 2018-06-29 / A Artrite Reumatóide (AR) é uma doença inflamatória crônica, caracterizada por inflamação das articulações e associada à incapacidade motora e laboral dos pacientes. Tendo em vista que as estratégias atuais para o tratamento da AR podem apresentar sérios efeitos colaterais e nem sempre resultam em melhora clínica, a terapia com laser de baixa potência (LLLT) surgiu como alternativa para o tratamento da artrite, devido às suas propriedades anti-inflamatórias e de regeneração tecidual. Entretanto, os mecanismos anti-inflamatórios da LLLT nos tecidos biológicos ainda não são completamente conhecidos. Assim, o presente estudo teve por objetivo avaliar os mecanismos imunológicos e a participação das metaloproteinases de matriz (MMP) e seu inibidor (TIMP2), na resolução do processo inflamatório articular após o tratamento com LLLT. Para isso, um processo inflamatório foi induzido nas articulações talocrural e subtalar dos membros posteriores de camundongos C57BL/6, através da administração de zymosan na região periarticular. Os animais foram divididos em 4 grupos (n = 8): (ZY) artrite induzida por zymosan e não tratado, (ZY + 3Jcm-2) artrite induzida por zymosan + LLLT em 3Jcm-2, (ZY + 30Jcm-2) artrite induzida por zymosan + LLLT em 30Jcm-2 e (ZY + DEXA) artrite induzida por zymosan e tratado com dexametasona. As condições da LLLT foram: 830nm, 10mW e densidades energéticas de 3Jcm-2 e 30Jcm-2, no modo contínuo de emissão. A irradiação foi realizada durante 4 dias consecutivos, iniciando 5 horas após a indução da inflamação. Os animais foram eutanasiados 24h após a última aplicação da LLLT e as seguintes análises foram realizadas no tornozelo: morfológica, níveis relativos de mRNA de quimiocinas, citocinas, MMPs e TIMP2 por RT-qPCR, quantificação de citocinas por ELISA e quantificação da expressão tecidual de MMP13 e TIMP2 por imunohistoquímica. No linfonodo poplíteo foi realizada análise de citometria de fluxo para identificação e quantificação das populações de leucócitos. As análises morfológicas revelaram a presença de infiltrado celular no tecido conjuntivo adjacente à região periarticular, após a administração de zymosan. Foi observada diminuição dos níveis teciduais de citocinas após LLLT e alteração nos níveis de mRNA de citocinas e quimiocinas com importância na fisiopatologia da AR, diferentemente, dependendo da densidade de energia utilizada. O grupo ZY + 3Jcm-2 demonstrou aumento na maioria das populações de leucócitos analisadas, bem como na expressão de proteínas co-estimulatórias em macrófagos e células dendríticas do linfonodo proximal à inflamação. Adicionalmente, foi observado maior população de células Treg nos grupos LLLT, e no grupo ZY + 30Jcm-2, mais células Treg CD8+ expressando níveis elevados de CD25. Embora LLLT tenha diminuído os níveis de mRNA da maioria das MMPs analisadas e aumentado de TIMP2, foi observado aumento da expressão tecidual de MMP13 e TIMP2 na região inflamatória do grupo ZY + 3Jcm-2. Tendo em vista a totalidade dos resultados, o LLLT em ambas as densidades de energia, apresentou efeitos anti-inflamatórios positivos para o tratamento da AR, uma vez que diminuiu os níveis da maioria das citocinas, quimiocinas e MMPs nos tecidos inflamados, além de modificar o quantitativo e o fenótipo dos leucócitos linfonodais, especialmente de células Treg. / Rheumatoid Arthritis (AR) is a chronic inflammatory disease, characterized by joint inflammation and associated with motor and labor incapacity. Since treatment strategies could present a serious side effects and not always achieved clinic improvement, the Low Level Laser Therapy (LLLT) have being considered as an alternative to arthritis treatment, due to its anti-inflammatory and healing abilities. However, LLLT anti-inflammatory mechanisms in biological tissues is not completely understood. Then, this study aimed to evaluate immunological mechanisms and contribution of matrix metaloproteinases (MMPs) and its inhibitor (TIMP) on inflammation resolution process induced by LLLT. Arthritis was induced in C57BL/6 mice by zymosan injection into periarticular region. Experimental animals were divided in 4 groups (n=8): (ZY) arthritis induced by zymosan and untreated; (ZY + 3Jcm-2) arthritis induced by zymosan and treated with LLLT at 3Jcm-2; (ZY + 30Jcm-2) arthritis induced by zymosan and treated with LLLT at 30Jcm-2; (ZY + DEXA) arthritis induced by zymosan and treated with dexamethasone. LLLT parameters were: 830nm, 10mW energy densities at 3 Jcm-2 e 30 Jcm-2, in continuous wave emission mode. Irradiation sections and dexamethasone treatment were carried out 4 days consecutively, starting 5 hours after arthritis induction. Animals were euthanized 24h after the last treatment section and the following analysis were procedure at ankle: morphological, cytokine mRNA relative levels, chemokines, MMPs and TIMP2 by RT-qPCR, quantification of cytokines tissue expression by ELISA and MMP13 and TIMP2 by imunohistochemistry. In popliteal lymph node, leukocytes populations were identificated and quantificated by flow cytometry. Morphological analysis showed inflammatory infiltration at adjacent connective tissue of joints, after zymosan injection. After LLLT, it was observed cytokines tissue expression decrease and mRNA levels alteration of important cytokines and chemokines in pathophisiology of AR, depending of energy density used. The ZY + 3Jcm-2 group showed an increase leukocytes populations, as well as, higher expression of costimulatory proteins in macrophages and dendritic cells of popliteal lymph node. Additionally, it was observed higher Treg population in LLLT groups than ZY group. More, ZY + 30Jcm-2 showed higher Treg CD8+ population presenting CD25high. LLLT decreased mRNA levels of almost MMPs analyzed and increased of TIMP2, except for ZY + 3Jcm-2 group in which was observed MMP13 and TIMP2 tissue expression increase. Taken togheter, all results showed that LLLT, at both energy densities used, presente positive anti-inflammatory effects for RA treatment, since it decreases the levels of cytokines, chemokines and MMPs at inflammed tissues. LLLT could also alter number and phenotype leukocytes in lymph node, especially Treg cells.

Fotobiomodulação comparativa entre o laser e LED de baixa intensidade na angiogênese de feridas cutâneas de ratos / Comparative fotobiomodulation between the LED and low intensity laser in the angiogenisis of skin wounds in rats

Os diodos emissores de luz – Light Emitting Diodes – (LEDs) são uma fonte de luz que estão sendo introduzidas comercialmente, mas com discreta base científica nesta modalidade fototerapêutica. O presente estudo comparou os efeitos angiogênicos da luz laser coerente e colimada à luz LED ausente de coerência e colimação em feridas induzidas em ratos, com fluências diferentes. O modelo experimental consistia da indução de uma ferida circular no quadríceps de 120 ratos, utilizando um “punch" com 15 mm de diâmetro. Os animais foram divididos aleatoriamente em 5 grupos: laser (660 nm) e LED (635 nm), sendo cada um ajustado a 5 J/'CM POT.2' e 20 J/'CM POT.2', além do controle. Após 6 horas da indução das feridas, os grupos tratados recebiam aplicação pontual de contato, e irradiados a cada 24 horas. A angiogênese foi analisada por meio da histomorfometria (H.E), e a contração das feridas pelo software de planimetria, sendo estudados os resultados no 3º, 7º, 14º e 21º dias pós-lesão. Os achados da análise histológica no 3º dia foram determinantes para induzir uma grande eficiência na proliferação de vasos sanguíneos dos grupos tratados em relação ao controle, demonstrando uma taxa próxima da constante no 7º dia, e com discreto aumento no 14º dia, porém com destaque para a tecnologia LED a 5 J/'CM POT.2' (p '< OU =' 0,05). No 21º dia, os grupos fototratados com fluência de 5 J/'CM POT.2' apresentavam valores estatísticos com maior eficiência na angiogênese quando comparados com o grupo laser a 20 J/'CM POT.2, sugerindo que fluências elevadas podem induzir um processo do tipo saturação. Não ocorreu diferença da neovascularização no centro e nas margens da ferida (p '< OU =' 0,05), sugerindo que ocorria uma fotobiomodulação de toda a lesão, favorecendo o crescimento homogêneo dos vasos sanguíneos. Tanto o laser quanto o LED não apresentaram resultados significativos na redução da área das feridas. As fototerapias a laser e LED de baixa intensidade sugeriram que a coerência e a colimação não eram fatores decisivos para induzir alterações nas funções celulares, e sim a banda de absorção do espectro eletromagnético. The experimental model consisted of the induction of a circular wound on the quadriceps of 120 rats, using a “punch" with 15 mm diameter. Animals were divided randomyzed in 5 groups: laser and LED each device with a dosage of 5 J/'CM POT.2' and 20 J/'CM POT.2', and control. After 6 hours of the induction of the wounds, the treated groups received contact of punctual application, and irradiated every 24 hours. The angiogenesis was studied through the histomorphometric (H.E) and the wounds contraction was photographed and analyzed in the planimetrical software in the 3rd, 7th, 14th and 21st lesion induced days. The discoveries of the histological analysis in the 3rd day revealed larger efficiency in the proliferation of the blood vessels in all irradiated groups in comparison to controls, being a rate near of the constant in the 7th day, following to discreet improves in the 14th, even so with prominence for the LED with 5 J/'CM POT.2'. The Effect of Photobiomodulation Therapy on Exercise-Induced Muscle Damage

Purpose: To explore the difference between continuous and pulsed photobiomodulation (PBMT) versus a placebo treatment when using a red-blue light combination over multiple treatment sessions to decrease the symptoms of muscle damage in the quadriceps muscle after a bout of muscle damaging exercise. Methods: Thirty-six healthy, nonactive male and female participants were randomly assigned to one of three groups: continuous PBMT, pulsed PBMT, and placebo treatment. Participants were assessed for muscle damage with knee extension maximal isometric and isokinetic contractions, as well as Visual Analog Scale (VAS) and Lower Extremity Functional Scale (LEFS) scores. Blood creatine kinase (CK) was also analyzed. Participants were given treatment immediately prior to undergoing a bout of damaging eccentric exercise. Participants were treated with PBMT for the next 4 consecutive days for a total of 5 treatments. Results: The continuous treatment group lost significantly less isokinetic average peak torque than the placebo treatment when averaged across all time points postexercise. However, for isometric testing, the continuous group had more reduction in force compared to the placebo group. Between the treatment groups, the continuous treatment group had significantly more muscle soreness measured by the VAS and had significantly less function in daily tasks reported on the LEFS patient-oriented outcome scale. There was no significant difference in level of creatine kinase between the treatment groups. Conclusion: Pulsed photobiomodulation treatments had no significant effect when compared to the placebo group. Continuous photobiomodulation helped to reduce isokinetic force loss, yet exacerbated all other muscle damage markers following exercise relative to the placebo condition.

A periodontite apical, muito freqÃente como seqÃela da cÃrie dentÃria, ainda à considerada como um processo patolÃgico de difÃcil controle e resoluÃÃo, tendo o sistema imunolÃgico como o principal fator de desenvolvimento e perpetuaÃÃo da mesma. A terapia a laser de baixa potÃncia (LLLT) jà vem sendo estudada como mÃtodo terapÃutico em vÃrias Ãreas da Odontologia, porÃm seu mecanismo de aÃÃo e conseqÃentes efeitos sobre as periodontites apicais nÃo sÃo ainda bem compreendidos. Esse estudo teve os objetivos de avaliar: I. a conseqÃÃncia da aplicaÃÃo da LLLT sobre a evoluÃÃo da inflamaÃÃo periodontal apical induzida em ratos, e II. o efeito da utilizaÃÃo da LLLT como adjuvante à terapia convencional no tratamento da periodontite apical estabelecida, nesse mesmo modelo experimental. O estudo foi realizado em duas fases: na primeira, 40 ratos Wistar machos adultos foram utilizados, sendo 4 deles como grupo controle (GC) nÃo-operados (sham-operated), e os demais 36 ratos operados divididos em 2 grupos de 18 ratos cada (G1 e G2) e submetidos à induÃÃo de periodontite apical atravÃs da abertura coronÃria e da exposiÃÃo das cÃmaras pulpares do primeiro molar inferior esquerdo. Os animais do grupo G1 sofreram apenas a induÃÃo da lesÃo periapical, enquanto os do G2 receberam tambÃm a LLLT, com comprimento de onda de 830 nm e potÃncia de saÃda de 100 mW na regiÃo periapical dos dentes, nos dias 0, 2, 4, 6 e 8 da induÃÃo da lesÃo (dose total = 28 J/cm2, a 5,6 J/cm2 por aplicaÃÃo). Os animais sofreram eutanÃsia na seguinte sistemÃtica: um dos controles (GC) em cada um dos dias 0, 10, 20 e 30, e 6 animais do G1 e 6 do G2 aos dias 10, 20 e 30. Na segunda fase do estudo, 12 animais foram divididos nos seguintes 3 grupos experimentais de 4 animais cada: G3 - periodontite apical induzida e sacrificados 21 dias apÃs; G4 â periodontite apical induzida + tratamento com Calen no 21o dia apÃs a induÃÃo da doenÃa,sofrendo eutanÃsia 21 dias apÃs; e G5 â periodontite apical induzida + tratamento com Calen + aplicaÃÃo de LLLT nos mesmos moldes do grupo G2 , sofrendo eutanÃsia 21 dias depois. Estudos histolÃgicos realizados nas mandÃbulas do lado esquerdo dos animais revelaram que os ratos controles (GC) nÃo apresentaram alteraÃÃes dos tecidos periapicais. Os ratos do grupo G1 revelaram lesÃes inflamatÃrias periapicais, ao passo que os animais que receberam LLLT desde a induÃÃo da lesÃo periapical (G2) apresentaram lesÃes muito mais intensas, com uma maior perda Ãssea na regiÃo periapical, do que nos ratos operados e nÃo irradiados do G1. Estudos histolÃgicos nos grupos da segunda fase (G3, G4 e G5) evidenciaram que os animais tratados somente com Calen (G4) e com CalenÂ+LLLT (G5), apresentaram lesÃes menos intensas no 42o dia da induÃÃo, em comparaÃÃo aos animais que nÃo receberam nenhum tratamento (G3) no 21o dia da doenÃa. Os animais tratados com CalenÂ+LLLT (G5) revelaram lesÃes periapicais menos intensas do que ratos tratados apenas com Calen (G4). Esses dados indicam que a irradiaÃÃo com laser de baixa potÃncia, como Ãnico tratamento da periodontite apical, poderà apresentar o risco de agravamento do processo inflamatÃrio em curso, ao passo que o uso da mesma como tratamento adjuvante à terapia endodÃntica convencional parece contribuir para o melhor controle do processo inflamatÃrio da periodontite apical. Essas observaÃÃes preliminares, aparentemente contraditÃrias, parecem sugerir que a LLLT nÃo possui de fato o efeito antiinflamatÃrio a ela atribuÃdo em estudos ocasionais, mas exerce algum efeito estimulador sobre as cÃlulas imunes engajadas na formaÃÃo do processo inflamatÃrio, ou nas envolvidas na involuÃÃo do processo patolÃgico iniciado pelas medidas terapÃuticas convencionais. / Apical periodontitis, most frequent as a consequence of dental caries, is considered a pathological process of difficult control and cure, due to the involvement of immune system in its evolution and maintenance. The low-level laser therapy (LLLT) is lately being used as a therapeutic measure in some areas of Dentistry; however its mode of action and the consequent effects on apical periodontitis are not yet well understood. The objectives of the present study was to show a histological evaluation of the consequence of application of LLLT during the evolution of experimentally induced inflammatory apical lesion in rats, and evaluate the effect of use of LLLT as an auxiliary therapy to conventional endodontic treatment of established apical periodontitis, in the same experimental model. The study was performed in two stages: in the first phase, 40 adults, male Wistar rats were utilized, with four animals used as âsham-operatedâ control group (GC), and the other 36 animals divided into two groups of 18 each (G1 and G2), and subjected to induction of apical periodontitis by opening the crown of the left front molar and leaving the root pulp open to the oral cavity. The animals of G1 suffered only the induction of lesion, while those of G2 also received LLLT, at 830nm wave length and exit potency of 100 mW in the dental apical region, on days 0, 10, 20 and 30 (total irradiation dose of 28 J/cm2, at 5.6 J/cm2 per application). The animals were sacrificed in the following manner: one animal of the control group (GC) on each of the days 0, 10, 20 and 30, and six animals from each of the groups G1 and G2, on days 10, 20 and 30. In the second phase of the study, 12 animals constituted the following three experimental groups of four each (G3, G4 and G5): G3 â apical periodontitis induced and sacrificed 21 days later; G4 â apical periodontitis induced + treated with Calen on day 21 of induction of lesion, and sacrificed 21 days later; and G5 â apical periodontitis + treated with Calen + LLLT applied as in G2, and sacrificed 21 days later. Histological evaluations performed on the left mandibles revealed that animals of the control group (GC) did not have alterations in the periapical tissues. All the animals of G1 developed apical periodontitis, while the G2 animals treated with LLLT from day 0 of lesion induction revealed markedly more intense inflammatory lesions and greater alveolar bone loss in the periapical region, as compared to those of G1 which did not receive laser irradiation. Histological studies on the second phase groups G3, G4 and G5, demonstrated that animals treated with Calen alone (G4), and with Calen + LLLT (G5) presented less severe apical periodontitis on day 42 of induction of disease, when compared with those of G3 at day 21 of disease induction. More significantly, animals of G5, treated also with LLLT, revealed less intense apical periodontitis, than those of G4 which received only Calen treatment. These evidences indicate that low-level laser irradiation, when employed as the only treatment for apical periodontitis, could present the risk of aggravating an ongoing inflammatory process; while its use as an adjunct therapy to conventional endodontic treatment seems to contribute to a better control of the periapical inflammatory disease. These preliminary, and apparently contradictory, results seem to suggest that LLLT may not, in fact, possess an anti-inflammatory potential attributed to it in occasional reports, but it likely exerts some stimulatory influence on the activity of immune cells engaged in the inflammatory disease process, or on those involved in the resolution of the inflammatory disease initiated by the conventional endodontic therapeutic measures.
